슬관절의 관절경적 변연절제술 (Arthroscopic Debridement of the Knee Joint)

  • We reviewed the cases of 35 knees of 34 patients who had arthroscopic debridement for degenerative osteoarthritis of the knee. The patients were followed up for an average of 29 months (range: 13 months to 45 months). The mean age of the patients at operation was 54 years (range: 44 years to 75 years). At the time of follow up, the patients were evaluated using the nine-point scale by Baumgaertner et al. We divided the knee into 4 stages according to the classification system by Jackson et al. Satisfactory results were achieved in 80%(stage I), 75%(stage II), 55.6%(stage III) and 25%(stage IV) of the patients. Arthroscopic debridement is of significant value in the earlier stages of degenerative osteoarthritis (stage I and II) for the symptom relief as a simple procedure with rare complication as well as in the later stages(stage III and IV) as a time buying procedure before the total knee arthroplasty.

Patient Expectations of Visiting Department of Oral Medicine for Burning Mouth Syndrome: Relationship between Expectations and Clinical and Psychological Characteristics

  • Purpose: The aims of this study were to investigate the expectation of patients with burning mouth syndrome (BMS) and assess the relationship between patient expectations and clinical and psychological characteristics. Methods: Eligibility was retrospectively assessed on 93 patients with BMS. A total of five questionnaires on patients' expectation for a visit, pain, sleep quality (Pittsburgh Sleep Quality Index), cognition (Pain Catastrophizing Scale) and psychological distress (Symptom Checklist-90 revised) were measured. Results: Sixty-five patients were included in this study. The top 3 priorities of expectation for a treatment visit to the Department of Oral Medicine were as following; "I want my pain to be free"; "I want to understand why it hurts"; "I want to communicate better with doctors about pain". Patients with priority of pain relief showed poor sleep quality than patients who are more interested in the disease related information. Conclusions: To improve treatment outcomes of BMS, clinicians should improve their own understanding of patients who are suffering from BMS. Inquiring about the patient expectations may be one of them, and it would contribute to the enhancement of patients' overall well-being.

  • Neuroablation should be performed cautiously because neuropathic pain can occur following denervation of a somatic nerve. A 34-year-old man presented with severe penile pain and allodynia following a selective neurectomy of the sensory nerve that innervated the glans penis for treatment of his premature ejaculation. He was treated with various nerve blocks, including continuous epidural infusion, lumbar sympathetic block and sacral selective transforaminal epidural blocks, as well as intravenous ketamine therapy. However, all of the treatments had little effect on the relief of his pain. We performed spinal cord stimulation as the next therapy. After this therapy, the patient has currently been satisfied for 3 months.

  • 우측 암면부, 특히 안구주위에 심한 통증을 호소하고 안검하수와 더불어 우측안구의 완전마비를 동반한 선천성 난장이 환자를 경험하였던 바 이는 신경방사선과학적소견, 검사실 검사소견, 전신적 corticosteroid치료에 대한 반응이 전형적인 Tolosa-Hunt Syndrome과는 다른 비전형적인 Tolosa-Hunt Syndrome이었기에 문헌적 고찰과 함께 보고하는 바이다.

요추추간판 탈충증에 대한 상체견인(V-trac)의 치료효과 연구 (Treatment Effects of Upper Body Traction (V-trac) for the HIVD)

  • The purpose of this study was to analyze the effects of V-trac on the clinical characteristics of patients who suffered from HIVD. The data were collected from 500 cases who had lumbar disc problems, from August, 1993 to July, 1995. The results of study were as follows; 1. Total patients statistics; mean age is 38 years, average duration of symptom is 5.4 years. 2. Symptomatic effects; HNP is 89.5 %, spinal stenosis is 55.1 %, and mean effects of total symptom is 80.0 %. 3. Pain relief effects; 1st session is 85.0 %, 7 th sessions are 89.3 %. 4. Back muscle improved effects; 10 sessions are 25.6 % rather than 1st session patients. 5. Sciatic scoliosis correction effect; Corrected scoliosis is improved 86.6 % by the 14 sessions V-trac. 6. Psychological effects; mental vigour is 51.2 %, emotional stability is 57.4 %, well-being is 72.5 %, comfortable sleeping is 60.2 %. 7. Follow-up states after V-trac therapy; Good conditions are 72.0 %, less conditions are 28.0 % after 1 month.

청대(淸代) 의정사(醫政史)에 관한 연구 (A Study of the History of Medical Administration in Qing(淸) Dynasty)

  • Basic summary of the medical administration : Due to Qing dynasty being the last of the dynastic era, it revealed overall extreme political traits in politics, economy, phenomenon, and cultural aspects. Few emperors of the early Qing dynasty adopted appeasement policy that mitigated ironies to a certain extent and showed growth in various business related fields. Even the medical administration had freshness during that period. United medical administrative system was generally formed, chicken pox was effectively prevented, shamanistic treatment was banned, medical journals were complied by the government, medical relief was more intensely done. However, actions on restoring Ming dynasty and against Qing dynasty as well as the reform power grew against Qing government threatening it. The drastically grown forces from the western region dan1aged Qing dynasty that the governors had to adopt despotic measures in politics, economy and culture. Social chaos began to arise, economy stagnated and weakened that the medical field also dwindled to the point where it could not be restored to the original point. The era of Qing dynasty was the period that had scientific culture at its fast growing pace, but for Chinese medicine, by contrary, due to autocracy and other factors, was faced with barriers in the medical development.

High-Level Cervical Spinal Cord Stimulation Used to Treat Intractable Pain Arising from Transverse Myelitis Caused by Schistosomiasis

  • The efficacy of spinal cord stimulation (SCS) for treatment of various chronic painful conditions is well established. Very few reports have documented the use of SCS for treatment of chronic pain after spinal cord injury. We present a case showing a good outcome after such treatment, and suggest that high cervical stimulation may be efficacious. A 53-year-old male underwent SCS on the C1-3 level for treatment of intractable neuropathic pain below the T3 level, and in the upper extremities, arising from spinal cord injury resulting from transverse myelitis caused by schistosomiasis. High cervical SCS significantly improved the pain in the upper extremities and at the T3-T10 dermatome level. The patient continues to report excellent pain relief 9 months later. The present case suggests that high cervical stimulation may improve chronic pain in the upper extremities and the T3-T10 dermatome level arising from spinal cord injury.

  • Since IMF relief financing, domestic economy has been rapidly cooled down. At this point that we predict an overall decrease of production and sale of industries, cooperative marketing activities of the whole dairy industry are required to promote milk consumption. As everyone knows, advertisement is only one of the various factors that affect sales. Other factors include price, distribution, package, promotion, and consumer's needs and taste. The objectives of the cooperative advertising campaign should be trial purchase of new consumers, maintenance of brand loyalty of existing consumers, more positive experience of using the product, and increase of frequency of purchase. Long term analysis is essential to making these kinds of behaviour changes. Also the objectives of the communication program should be a shift of awareness of milk from 'a basic health food for children' to a substitute to the beverage even for adults. To share database of consumers among companies, we need to find ways to use interactive on-line services, the internet, D.M. events and so on as well as the mass media. It should be a precondition that the main body of the cooperative advertisement includes the milk processing board, dairy companies, livestock cooperations, farmers and the Government. To assure continuous, activities to promote milk consumption, the Government should take the lead in legislating 'An enforcement ordinance for milk promotion and research', and seek complementary measures and support to establish the ordinance.

  • Ginseng has been traditionally used for several millennia in Asian countries, including Korea, China, and Japan, not only as a nourishing and tonifying agent but also as a therapeutic agent for a variety of diseases. In recent years, the various effects of red ginseng including immunity improvement, fatigue relief, memory improvement, blood circulation improvement, antioxidation, mitigation of menopausal women's symptoms, and anticancer an effect have been reported in clinical as well as basic research. Around the world, there is a trend of the rising consumption of health functional foods on the level of disease prevention along with increased interest in maintaining health because of population aging and the awareness of lifestyle diseases and chronic diseases. Red ginseng occupies an important position as a health functional food. But till now, international ginseng monographs including those of the World Health Organization have been based on data on white ginseng and have mentioned red ginseng only partly. Therefore, the red ginseng monograph is needed for component of red ginseng, functionality certified as a health functional food in the Korea Food and Drug Administration, major efficacy, action mechanism, and safety. The present red ginseng monograph will contribute to providing accurate information on red ginseng to agencies, businesses, and consumers both in South Korea and abroad.

Diagnosis and treatment of abnormal dental pain

  • Most dental pain is caused by an organic problem such as dental caries, periodontitis, pulpitis, or trauma. Diagnosis and treatment of these symptoms are relatively straightforward. However, patients often also complain of abnormal dental pain that has a non-dental origin, whose diagnosis is challenging. Such abnormal dental pain can be categorized on the basis of its cause as referred pain, neuromodulatory pain, and neuropathic pain. When it is difficult to diagnose a patient's dental pain, these potential alternate causes should be considered. In this clinical review, we have presented a case of referred pain from the digastric muscle (Patient 1), of pulpectomized (Patient 2), and of pulpectomized pain (Patient 3) to illustrate referred, neuromodulatory, and neuropathic pain, respectively. The Patient 1 was advised muscle stretching and gentle massage of the trigger points, as well as pain relief using a nonsteroidal anti-inflammatory and the tricyclic antidepressant amitriptyline. The pain in Patient 2 was relieved completely by the tricyclic antidepressant amitriptyline. In Patient 3, the pain was controlled using either a continuous drip infusion of adenosine triphosphate or intravenous Mg2+ and lidocaine administered every 2 weeks. In each case of abnormal dental pain, the patient's diagnostic chart was used (Fig.2 and 3). Pain was satisfactorily relieved in all cases.
