Is Pancapsular Release More Effective than Selective Capsular Release for the Treatment of Adhesive Capsulitis?

  • Background: We assessed the effectiveness of arthroscopic capsular release for the treatment of adhesive capsulitis. Further, we tried to ascertain the clinical benefits, if any, of pancapsular release over selective capsular release, where the two differ by performing or not performing a posterior capsular release, respectively. Methods: Thirty-five consecutive patients with either primary or secondary adhesive capsulitis who failed conservative treatment for more than 6 months were enrolled in the study. A total of 16 patients allocated in group 1 received a pancapsular release that comprises the release of the rotator interval, anteroinferior capsular, and the posterior capsular release, whereas 19 patients in group 2 received a selective capsular release that comprises only the release of the rotator interval release and anteroinferior capsular release. The clinical outcomes, visual analogue scale (VAS) score, Constant score, and range of motion, were assessed preoperative and postoperatively. Results: In both groups, the preoperative VAS score, Constant score, and ROM showed a significant improvement by the 6-month follow-up. We found that the immediate postoperative internal rotation was significantly higher in group 1 than group 2. Despite significant differences seen between the two groups at the initial postoperative period, there were no significant differences in Constant score, VAS score, and the ROM at all the subsequent follow-ups between the two groups. Conclusions: Arthroscopic capsular release for the treatment of adhesive capsulitis is very effective. However, pancapsular release did not show any advantage over selective capsular release in terms of overall clinical outcome.

완효성비료 시비가 온시디움 생육과 개화에 미치는 영향 (Effect of Controlled Release Fertilizer on the Growth and Flowering of Oncidium 'Sweet Sugar')

  • 본 연구는 합리적이고 경제적인 시비량을 확립하기 위하여 온시디움 생육에 적절한 완효성 비료 시비량을 밝히기 위하여 수행하였다. 생육은 완효성 비료 2g, 3g 처리구에서 엽장과 위구경장이 Hyponex 및 완효성 비료 1g 처리에 비해 컸으며, 엽폭 및 엽수에서는 차이가 없었고, 식물체 무게는 완효성 비료 2g, 3g 처리에서 무거웠다. 개화기는 10월 상순으로 비료시용에 따른 차이는 없었다. 회수는 완효성비료 3g 처리에서 62.4로 가장 많았으나 처리간 차이는 없었고, 회경장, 화폭도 비슷한 경향을 보였다. 분지수는 완효성비료 2g, 3g 처리에서 0.2개 증가하였고, 지상부 식물체의 무기성분은 완효성 비료 1g처리 보다는 2, 3g 처리구에서 대부분 함량이 높았으며 특히 칼륨함량은 질소나 인산함량보다는 높은 경향을 보였다. 따라서 온시디움의 생장에 가장 적절한 완효성 비료량은 식물생장과 건물중 생산이 많은 2g이라고 판단되었다.

Effects of NMDA, AMPA and Kainate on the Release of Acetylcholine in Rat Hippocampal and Striatal Slices

  • This study examined the effects of N-methyl-D-aspartate (NMDA), ${\alpha}-amino$-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA) and kainate on basal and electrically-evoked release of acetylcholine (ACh) from the rat hippocampal and striatal slices which were preincubated with $[^3H]choline$. Unexpectedly, the basal and evoked ACh release were not affected at all by the treatment with NMDA $(3{\sim}100{\mu}M)$, AMPA $(1{\sim}100{\mu}M)$ or kainate $(1{\sim}100{\mu}M)$ in hippocampal slices. However, in striatal slices, under the $Mg^{2+}-free$ medium, $30{\mu}M$ NMDA increased the basal ACh release with significant decrease of the electrically-evoked releases. The treatment with $1{\mu}M MK-801 not only reversed the $30{\mu}M$ NMDA-induced decrease of the evoked ACh release, but also attenuated the facilitatory effect of $30\;{\mu}M$ NMDA on the basal ACh release. The treatment with either $30\;{\mu}M$ AMPA or $100\;{\mu}M$ kainate increased the basal ACh release without any effects on the evoked release. The treatment with $10{\mu}M$ NBQX abolished the AMPA- or kainate-induced increase of the basal ACh release. Interestingly, NBQX significantly attenuated the evoked release when it was treated with AMPA, although it did not affect the evoked release alone without AMPA. These observations demonstrate that in hippocampal slices, ionotropic glutamate receptors do not modulate the ACh release in cholinergic terminals, whereas in striatal slices, activations of ionotropic glutamate receptors increase the basal ACh release though NMDA may decrease the electrically-evoked ACh release.

Managing Red Oak (Quercus rubra L.) Reduces Sensitivity to Climatic Stress

  • This study was conducted in a long-term experimental forest in the central hardwoods region of southwestern Michigan to retrospectively examine the role of past forest management practices and climate on red oak (Quercus rubra L.) productivity. Initially, in 1971, plots within the experimental forest were treated separately with a clearcut and shelterwood regeneration harvest in an attempt to increase red oak regeneration. From 1987-1989, a new study was initiated within a portion of the clearcut and shelterwood plots to evaluate the effectiveness of additional oak crop tree release using mechanical and chemical applications. Cumulative diameter and mortality rates of 719 red oaks were monitored across the four silvicultural treatments: Clearcut-A (clearcut without additional release treatment), Clearcut-B (clearcut with additional release treatment), Shelterwood-A (shelterwood without additional release), and Shelterwood-B (shelterwood with additional release) plus an untreated control. Increment cores were obtained from red oak trees and neighboring competitor species. Tree-ring analyses (dendrochronology) were applied to examine the effect of these silvicultural treatments and climatic factors (temperature and precipitation) on red oak productivity. The results indicated that crop tree release following a clearcut or shelterwood harvest reduced mortality rates and thus increased survival of red oak. Red oak in control plots or plots only receiving the initial regeneration harvesting treatment and no additional competition release were negatively affected by climatic stress, which included summer moisture stress. In contrast, red oak in plots that received the competition release treatment from shade tolerant tree species not only had higher tree level productivity (i.e., tree basal area) and lower mortality rates, but were also relatively more resilient to climatic stress by showing limited or no associations between climate and growth.

Does Aging Matter? The Efficacy of Carpal Tunnel Release in the Elderly

  • Open release remains the gold standard in the treatment of carpal tunnel syndrome in cases where conservative treatment fails. However, the efficacy of carpal tunnel release in the elderly has been debated in the literature throughout the years. This review aims to review the current evidence pertaining to the efficacy of carpal tunnel release in the elderly. Based on the current evidence, the outcome of carpal tunnel release is unpredictable in the elderly. Elderly patients are also less satisfied with the operation compared to younger patients. The authors recommend that these messages be conveyed to elderly patients before surgery. Moreover, open carpal tunnel release should be offered in the early stages of treatment whenever operative management is indicated.

스쿼트 동작 시 정적 스트레칭과 근막이완기법이 하지 관절의 운동학적 요인에 미치는 영향 (Effect of Static Stretching and Myofascial Release Techniques on Kinematic Factors of Lower Extremity Joints during Squat )

  • Objective: The purpose of this study was to analyze the differences in kinematic factors according to stretching treatment, myofascial release treatment, and static stretching treatment conditions during squat. Method: Twelve males with resistance training experience participated in this study. Participants performed squats without treatment (Pre-Test), and performed squats after treatment with the myofascial release technique (MRT) and static stretching (SS) on different days (post-test). Squat movements were captured using eight motion capture cameras (sampling rate: 250 Hz), and the peak joint angles of the ankle, knee, hip, and pelvis were calculated for each direction. One-way repeated ANOVA and Bonferroni post hoc analyses using SPSS 27 (IBM Corp. Armonk NY, USA) were used to compare the peak joint angle of the lower extremity joints and pelvis among the normal condition (squat without treatment), MRT condition (squat after MRT treatment) and SS condition (squat after static stretching). The statistical significance level was set at .05. Results: It was observed that the maximum ankle joint flexion angle during squats was statistically reduced under conditions of myofascial release and static stretching (p<.05), in comparison to the scenario where no stretching was performed. Furthermore, static stretching was found to enhance the maximum hip flexion angle during squat (p<.05), whereas the myofascial release stretching technique resulted in the minimal posterior pelvic tilt angle (p<.05). Conclusion: Employing myofascial release stretching as a preparatory exercise proved to be more efficacious in maintaining body stability throughout the execution of high-intensity squat movements by effectively managing the posterior tilt of the pelvis, as opposed to foregoing stretching or engaging in static stretching.

자세이완치료를 병용한 침구치료가 급성기 족관절 염좌의 치료에 미치는 영향에 대한 임상적 연구 (A Clinical Study on Effectiveness of Acupuncture Treatment of Acute Ankle Sprain Combined with Positional Release Therapy)

  • Objectives: The purpose of this study is to evaluate the therapeutic effect of acupuncture treatment of acute ankle sprain combined with positional release therapy. Methods: A prospective randomized single blind study between positional release group and knee flexed supine position group was conducted. Patients with ankle sprain within 48 hours were evaluated by Ankle injury grade chart(AIGC). In group A, positional release therapy was combined with acupuncture, whereas in group B, acupuncture was conducted in knee flexed supine position. The treatment was planned for a duration of 1 week, 3times a week. In AIGC scores, VAS, weight bearing time and weight bearing time in blind were followed up and compared. Results and Conclusion: The VAS score decreased in both group. Weight bearing time increased in group B, weight bearing time in blind increased in group A. Comparing the therapeutic effect of each group, group A had significant effectiveness in weight bearing time in blind. So we may conclude that Acupuncture treatment combined with positional release therapy is effective and recommandable at early stage of ankle sprain.

관절가동기법과 근막이완기법이 경부에 미치는 영향 (The Effectiveness of Joint Mobilization and Myofascial Release on the Neck)

  • Purpose : to investigate the effectiveness of joint mobilization and myofascial release on the neck pain and to provide the effective treatment. Methods : Twenty-two subjects with neck pain participated in the experiment. All subjects were randomly assigned to a joint mobilization group(n=11) or a myofascial release group(n=11). Both groups received treatment for 15 minutes four times during 2 weeks. Cervical range of motion(CROM) instrument was used to measure range of neck motion, and Algometer was used to measure tenderness. All measurements of the subjects were measured at pre-treatment and post-treatment. Results : 1. The range of neck right side-bending motion of the myofascial release group was significantly increased(P<0.05), and the range of neck extension, right side-bending, left side-bending, right rotation motion of the joint mobilization group was significantly increased(P<0.05). 2. There was no significant improvement of tenderness in both groups(p>0.05). Conclusion : These data suggest that joint mobilization is more effective against increasing the range of motion than myofascial release, but myofascial release is more beneficial to tenderness than joint mobilization although it does not have a significant difference in the tenderness because there was a little improvement.

Consistent and Specific Suppression of Mucin Release from Cultured Hamster Tracheal Surface Epithelial Cells by Poly-L-Lysine

  • Poly-L-lysine (PLL) was reported to suppress mucin release from airway goblet cells during 30 min treatment period. In this study, we investigated whether PLL consistently suppresses mucin release from cultured airway goblet cells during 24 h after 30 min treatment and also specifically suppresses the release of mucin without any effects on the other releasable glycoproteins. Confluent primary hamster tracheal surface epithelial (HTSE) cells were metabolically radiolabeled with $^3H$-glucosamine for 24 h and chased for 30 min in the presence of varying concentrations of PLL to assess the effects on $^3H$-mucin release and on the total elution profile of the treated culture medium. The total mucin content during 24 h after 30 min treatment of PLL was assesed to investigate the consistency of effects. PLL did not affect the release of the other releasable glycoproteins whose molecular weights were less than mucin, and decreased the total mucin content during 24 h after 30 min treatment. We conclude that PLL can specifically suppress mucin release from cultured airway goblet cells and the suppression on mucin release is consistent. This finding suggests that PLL might be used as a specific airway mucin-regulating agent by directly acting on airway mucin-secreting cells.

  • 상기 증례에서 저자는 극상근건 부분 파열 환자에게 한방치료 및 ART 치료를 적용하여 견관절 ROM, VAS 평가를 통해 다음과 같은 결론을 얻었다. 1. 극상근건 부분 파열로 견통 및 견관절 ROM 제한 발생한 환자에게 한방치료 및 ART 치료를 적용하여 견관절 ROM, VAS에서 현저한 개선 보인 것을 확인할 수 있었다. 2. 비교적 증례가 많지 않고 한방적 치료 과정이 정립되어 있지 않은 극상근건 부분 파열에 한방 치료와 ART 치료를 병행하는 것이 효과가 있음 을 알 수 있었다.
