Efficacy Analysis of Simplified Intensity-modulated Radiotherapy with High or Conventional Dose and Concurrent Chemotherapy for Patients with Neck and Upper Thoracic Esophageal Carcinoma

  • For patients with neck and upper thoracic esophageal carcinoma, it is difficult to control lymph node metastases with conventional dose therapy. In this study, we assessed the feasibility of simplified intensity-modulated radiotherapy (sIMRT) and concurrent chemotherapy for 44 patients and boosted high-dose to metastatic lymph nodes. Three radiation treatment volumes were defined: PGTVnd, with which 68.1Gy was delivered in high dose group (hsIMRT group), and 60Gy in the conventional dose group (csIMRT group); PTV1, featuring 63.9Gy in the hsIMRT group and 60Gy in the csIMRT group; PTV2, with 54Gy given to both groups. The sIMRT plan included 5 equi-angular coplanar beams. All patients received the cisplatin and 5-FU regimen concurrently with radiotherapy. The treatment was completed within six weeks and one case with grade three acute bronchitis was observed in hsIMRT group. For esophageal lesions, 80% complete response (CR) and 20% partial response (PR) rates were found in the hsIMRT group, and 79.2% CR, with 20.8% PR, in the csIMRT group; for lymph node lesions, 75% CR and 25% PR rates were observed in the hsIMRT group, with 45.8% and 37.5% respectively in the csIMRT group (P<0.05). The differences in 1-, 2- and 3-year relapse-free survival rates were all statistically significant (P<0.05). The major toxicity observed in both groups was Grade I~II leucopenia. sIMRT can generate a desirable dose distribution in treatment of neck and upper thoracic esophageal carcinoma with a better short-term efficacy. Boosted high dosing to metastatic lymph nodes can increase the relapse-free survival rate.

  • The recogition that the vast majority of patients with small cell lung cancer have distant metastatic disease at the time of diagnosis lead to the use of systemic chemotherapy and consequent major improvement in survival, but recently evaulated treatment strategies, integration of large field chest irradiation with chemotherapy lead to the improved the local control and relapse free survival in limited SCLC. Therefore, it is logical to combine the two modalities in an effort to maximize the therapeutic effect. Authors performed the combination chemotherapy of CAV (Cyclophosphamide, Adriamycin, an6 Vincristine) and radiotherapy of primary tumor and regional lymphatics with prophylatic cranial irradiation in 42 patients of limited SCLC, from Mar. 1978 to Dec. 1982 Seoul National University Hospital. The results are as follows : 1. CR and PR after 2 cycles chemotherapy is $7\%$ and $43\%$, respectively however, subsequent response to radiotherapy is $38\%$ and $43\%$ in CR and PR. 2. Overall median survival peroid is 8.6 months. 3. 1 year and 2 years survival rate is $46.3\%$ and $20.5\%$, respectively. 4. Incidence of proven brain metastasis is $5\%$.

자궁경부암의 수술후 방사선치료 결과 (Therapeutic Results of Postoperative Radiation Therapy for Uterine Cervical Cancer)

  • This is a retrospective analysis of 64 patients who was treated with postoperative radiation therapy after radical hysterectomy and bilateral pelvic lymphadenectomy (53 patients) or total abdominal hysterectomy(11 patients) for uterine cervix cancer between May 1980 and September 1991 at the Department of Radiation Oncology, Kyung Hee University Hospital. Most patients were FIGO IB (31 Patients) and IIA (25 patients), and median period of follow-up was 5.1 years. Of these patients,24 received adjuvant whole pelvis irradiation of 6000 cGy and 40 received 5000-5500 cGy whole pelvis irradiation and/or intracavitary radiation (7 Patients). The actuarial overall and relapse free 5 year survival rate were $71.0\%$, $68.3\%$ respectively. The survival rates by stage were $79.1\%$ in stage I, and $61.2\%$ in stage II. Treatment failure was noted in 18 of 64 patients ($28.1\%$), Iocoregional failure in 8 ($12.5\%$), distant metastasis in 8 ($12.5\%$), paraaortic node metastasis in 1 and one patient and concurrent locoregional and distant metastasis. The univariate analysis of prognostic factors affecting to overall survival rate represented lymph node status, the number and site of metastatic lymph node, parametrial invasion, the thichness of cervical wall invasion, and size of cancer mass. Histology, vessel invasion, endometrial extension, hemoglobin level. resection margin status, age, radiation dose were not significant prognostic factors. Complication relating to operation and postoperative radiation were variable according to radiation therapy method: 6000 cGy RT group 8/24($33.3\%$), 5000-5500 cGy+ICR 3/7 ($42.9\%$), 5000-5500 cGy external RT only group 3/33 ($9.1\%$). In conclusion, the results suggest that postoperative radiotherapy is necessary in high risk patients for locoregional control and improving survival rate, and higher dose does not improve results but only increases complication.

Outcome of Surgery and Post-Operative Radiotherapy for Major Salivary Gland Carcinoma: Ten Year Experience from a Single Institute

  • Aims: To determine the clinical characteristics, pathological features, local and distant failure patterns in patients with carcinoma of major salivary glands treated with surgery and postoperative radiotherapy (PORT). Materials and Methods: We retrospectively reviewed 106 cases of major salivary gland tumor seen at our centre (1998-2008). Sixty five cases of major salivary gland carcinoma were selected for analysis (exclusions: benign, palliative, non-carcinomas). The patient population treated by surgery and PORT was divided into two groups: 1) Patients who underwent surgery and immediate PORT (Primary PORT); 2) Patients with recurrent carcinoma who underwent at least two surgeries and received PORT in the immediate post-operative period of the last performed surgery (Recurrent PORT). Recurrence free survival (RFS) was assessed using the Kaplan-Meier method. Results: Median age was 35 years with a male: female ratio of 1.3:1. The majority of cancers were located in the parotid gland (86.2%) and the most common histology was mucoepidermoid carcinoma (43%). Thirty nine cases (60%) were primary while 26 (40%) were recurrent. Optimal surgery was performed in 59/65 patients (90.8%). 43 patients (66.2%) underwent neck dissection, of which 14 (32.5%) had nodal metastasis. Overall, 61 (93.8%) patients complied with the prescribed radiotherapy. Median dose of PORT was 60 Gy. Median follow-up was 13.1 months (range 2-70). Relapse free survival was 50.4% at 60 months. Some 12 cases (18.5%) recurred with a median time to recurrence of 16.9 months. Conclusions: Surgery and PORT is an effective treatment for major salivary gland carcinoma with over 90% compliance and <20% recurrence. Early treatment with postoperative radiotherapy may increase the survival rate in major salivary gland carcinoma patients.

Endometrial Adenocarcinoma: Clinicopathologic and Survival Characteristics in Yazd, Iran

  • Background: Endometrial adenocarcinoma is the most common gynecological cancer in the Western world and its incidence appears to be rising. However, population-based studies on endometrial cancer providing survival estimates by age, histology, and stage in Asia have been sparse. The aim of this study was to evaluate the clinicopathological data and survival for patients with endometrial adenocarcinoma treated at three institutions in Yazd, Iran. Materials and Methods: Medical and anatomicopathological records at the Department of Pathology and Radiotherapy of the Shahid Sadoughi University of Medical Sciences and Madar private hospital, between 2005 and 2012 were reviewed. All cases of endometrial adenocarcinoma were included. The Kaplan-Maier method was used for survival analysis and Cox proportional hazards model for multiple regression analysis. Results: The study included 84 patients. Stages I, II, III, and IV were identified in 65.4%, 21.5%, 11.9% and 1.2%, respectively. Disease-free survival rate was $73.9{\pm}3.77$ months (95% confidence interval, 64.51-83.22 months) and relapse occurred in 12.3% of the patients. The overall survival rate was $78.2{\pm}3.65$ months (95% confidence interval, 71.0-85.3 months). A multivariate analysis revealed that stage and grade were associated with overall survival. Conclusions: In this survival analysis of patients with endometrial cancer, we found that the prognosis of endometrial cancer was fair but strongly varied by stage and grade, and moderately varied by histology and age.

Lymphovascular invasion as a negative prognostic factor for triple-negative breast cancer after surgery

  • Purpose: This study aimed to evaluate the prognostic effects of lymphovascular invasion (LVI) in triple-negative breast cancer (TNBC) patients who underwent surgical resection. Materials and Methods: A total of 63 non-metastatic TNBC patients who underwent surgical resection were retrospectively investigated from 2007 to 2016 in Inje University Busan Paik Hospital. Pathological tests revealed that 12 patients (19.0%) had LVI. Approximately 61.9% (n = 39) of the patients' samples stained positive for p53. Additional chemotherapy and radiotherapy (RT) were performed in 53 (84.1%) and 47 (74.6%) patients, respectively. Results: The median follow-up period was 39.5 months (range, 5.9 to 123.0 months). The pathological T stage (p = 0.008), N stage (p = 0.014), and p53 positivity (p = 0.044) were associated with LVI. Overall, the 3-year disease-free survival (DFS) rate and overall survival (OS) rate were 85.4% and 90.2%, respectively. Ten patients (15.9%) experienced relapse. LVI (n = 12) was associated with relapses (p = 0.016). p53 positivity was correlated with poor DFS (p = 0.048). Furthermore, LVI was related to poor DFS (p = 0.011) and OS (p = 0.001) and considered as an independent prognostic factor for DFS (p = 0.039). The 3-year DFS of patients with LVI (n = 12) was only 58.3%. Adjuvant RT minimized the negative prognostic effect of LVI on DFS (p = 0.068 [with RT] vs. p = 0.011 [without RT]). Conclusion: LVI was related to the detrimental effects of disease progression and survival of TNBC patients. Thus, a more effective treatment strategy is needed for TNBC patients with LVI.

자궁내막암의 수술 후 방사선치료 결과에 영향을 미치는 예후인자 (Prognostic Factors Influencing the Result of Postoperative Radiotherapy in Endometrial Carcinoma)

  • 목적: 수술 후 보조적으로 방사선치료를 받은 자궁내막암 환자의 전체생존율, 무병생존율, 재발 부위 등을 분석하여 이와 관련된 예후인자를 알아보고자 하였다. 대상 및 방법: 1992년 4월부터 2003년 5월까지 부산대학교병원에서 수술 후 방사선치료를 받은 자궁내막암 환자 중 조직유형이 선암인 환자 54명을 대상으로 후향적 분석하였다. 전체 환자의 중앙 나이는 55세(35-76세)였고, 병기 분포는 FIGO 병기 I군이 34명(63.0%), 병기 II군이 8명(14.8%), 병기 III군이 12명(22.2%)이었다. 모든 환자는 수술 및 외부 방사선조사($41.4{\sim}54.0Gy$, 중앙값 50.4 Gy)를 받았고, 20명(전체 환자의 37.0%)의 환자에서 추가로 질내 근접치료($15.0{\sim}24.0Gy$, 중앙값 15.0 Gy)를 받았다. 전체추적기간은 $5{\sim}115$개월로 중앙추적기간은 35개월이었다. 분석 결과 유의 인자로 나타난 조직 분화도(histologic grade), 림프-혈관 침범(Iymphovascular space invasion), 그리고 자궁근 침범 정도(myometrial invasion depth)를 점수화(GLM 점수)하여 생존분석을 시행하였다. 생존분석은 Ka-plan-Meter 법을, 단변량 및 다변량 통계분석은 각각 log-rank 검정과 Cox 회귀분석을 사용했다. 결과: 전체 자궁내막암 환자의 5년 생존율은 87.7%였고, 5년 무병생존율은 87.1%였다. 단변량 통계분석에서는 조직분화도, 림프-혈관 침범, 그리고 자궁근 침범 정도가 전체생존율 및 무병생존율과 관련 있는 인자였고, 다변량 통계분석에서는 림프-혈관 침범이 무병생존율과 관련 있는 인자였다(p=0.0158). GLM 점수는 전체생존율 및 무병생존율과 의미 있는 관계를 나타냈고(각각 p=0.0090, p=0.0073), 원격재발에도 유의한 예후인자로 나타났다 (p=0.0132). 전체 환자 중 6명(11%)의 환자에서 재발을 보였고, 재발 부위는 대동맥 림프절 2명, 폐 2명, 쇄골상부 림프절 1명, 질 1명이었다. 결론: 수술 및 수술 후 방사선치료를 받은 자궁내막암 환자의 예후는 수술 후의 병리 소견과 밀접한 관계를 가지고 있다. 더 많은 연구를 통해 자궁내막암 환자의 예후인자를 체계화한다면, 병의 진행양상을 예견하고 대처하는데 도움이 될 것이다.

1차 관해된 급성 골수성 백혈병에서 자가 조혈모세포 이식을 위한 Cytarabine, Melphalan, 전신 방사선치료의 효과 (Effect of Cytarabine, Melphalan, and Total Body Irradiation as Conditioning for Autologous Stem Cell Transplantation for Patients with AML in First Remission)

  • 목적: 급성 골수성 백혈병에서 자가 조혈모세포 이식은 무병생존율에 도움을 주며 1차 관해된 급성 골수성 백혈병에서 자가 조혈모세포 이식은 점차 늘어나고 있는 추세이다. 본 연구는 1차 관해된 급성 골수성 백혈병에서 자가 조혈모세포 이식을 위한 전처치 요법으로 cytarabine, melphalan과 전신 방사선치료를 시행하여 그 효과를 알아보고자 하였다. 대상 및 방법: 1995년 1월부터 1999년 12월까지 급성 골수성 백혈병으로 1차 관해 후 자가 조혈모세포 이식을 받은 29명을 대상으로 하였다. 환자의 중앙연령은 33세(16~47세)이었다. 자가 조혈모세포 이식을 위한 전처치 요법은 cytarabine ($3.0\;gm/m^2$, 3일), melphalan ($100\;gm/m^2$, 1일)과 전신 방사선치료를 시행하였다. 전신 방사선치료는 6MV 선형가속기를 이용하여 200 cGy를 1일 2회씩 5회 분할 조사하여 총 조사선량은 1000 cGy이었다. 결과: 추적 관찰기간은 3~58개월이었으며 중앙값은 40개월이었다. 전체 환자의 4년 무병생존율은 69.0%이었으며 중앙생존기간은 41.5개월이었다. 4년 재발률은 27.6%이었다. 무병생존율과 재발률에 영향을 미치는 인자 분석에서는 FAB 분류만이 유의한 예후인자로 분석되었다($M_3$군 vs. $M_3$를 제외한 군; p=0.048, p=0.043). 대 상 환자 중 9명에서 사망하였으며 치료와 관련된 사망은 1명이었고 8명은 재발로 사망하였다. 결론: 1차 관해된 급성 골수성 백혈병에서 자가 조혈모세포 이식을 위한 melphalan, cytarabine과 전신 방사선치료는 비교적 효과적인 전처치 요법이었다.

자궁내막암의 수술 후 보조적 방사선치료 (Postoperative Adiuvant Radiation Therapy in Endometrial Carcinoma)

  • 목적 : 수술 및 수술 후 보조적 방사선치료를 시행한 자궁내막암 환자의 예후인자, 재발 장소 및 생존율 등을 분석해 보고자 하였다. 대상 및 방법: 1991년 9월부터 1997년 12월까지 서을중앙병원에서 수술 및 방사선치료를 받은 27예의 자궁내막암환자를 대상으로 하였다. 수술은 단순자궁적출술이 6예, 단순자궁적출술 및 림프절 절제술이 8예, 근치적 자궁적출 술이 13예에서 시행되었다. 모든 환자에서 외부 방사선치료가 시행되었으며 이 중 16예에서 ovoid를 이용한 질내조사가 추가되었다. 조사선량은 외부 방사선치료는 총 50.4 Gy를 조사하는 것을 원칙으로 하였고, 질내조사는 1회 분할선량 4$\~$5 Gy씩 4내지 5회 시행하였다. 추적기간은 6개월에서 95개월로서 중앙추적기간은 30개월이었다. 결과:FIGO 병기는 I-18예(67$\%$), II-1(4$\%$), III-8예(29$\%$)이었고, 조직분화도는 1$\~$14예(52$\%$), 2$\~$6예(9$\%$), 3$\~$7예(26$\%$)이었다. 수술 후 조직 소견상 조직분화도가 나쁠수록, 자궁근층의 침범 정도가 심할수록, 자궁부속기의 침범이있는 경우, 림프-혈관 침범이 있는 경우 골반내 림프절이 양성을 보이는 경우가 많았으나 통계적인 의미는 없었다. 추적이 종료된 시점에서 사망한 환자가 한 예도 없어 전체 생존율은 100$\%$이었으나 3예에서 병이 재발되어 3년 및 5년 무병생존율이 각각 89.6$\%$,, 76.8$\%$이었다. 재발 장소는 골반내, 대동맥방림프절, 폐를 포함한 다발성 등이 각각 I예 이었고 질 재발은 관찰되지 않았다. FIGO 병기(p=0.01), 림프-혈관 침범(p=0.03), 골반내 림프절 침범(p=0.0001)등이 무병생존을에 영향을 미치는 것으로 나타났고 조직분화도가 나쁠수록 무병생존율이 저조한 경향을 보였다(p=0.1). 치료부작용으로는 1예에서만 Grade 1의 직장출혈이 나타났으나 중등도 이상의 합병증을 보인 경우는 없었다. 결론 : 수술후 조직학적 소견에 따른 외부방사선치료 및 추가 질내조사는 국소재발을 억제하는데 효과적인 것으로 판단되며 결과적으로 생존율을 향상시키는데 기여하는 것으로 생각된다. 다만, 병기 및 조직분화도에 따라 질내조사를 추가하지 않는 경우 질 재발이 호발하는 환자군에 대한 연구가 더 진행되어야 할 것으로 판단된다.

Institutional Experience of Interstitial Brachytherapy for Head and Neck Cancer with a Comparison of High- and Low Dose Rate Practice

  • Aims: To describe our institutional experience with high dose rate (HDR) interstitial brachytherapy (IBT) compared with previously reported results on the low dose rate (LDR) practice for head and neck cancer. Materials and Methods: Eighty-four patients with oral cavity (n=70) or oropharyngeal cancer (n=14) were treated with 192Ir HDR-IBT. Seventy-eight patients had stage I or II tumour. The patients treated with IBT alone (n=42) received 39-42 Gy/10-14 fractions (median=40 Gy/10 fractions). With respect to the combination therapy group (n=42), prescription dose comprised of 12-18 Gy/3-6 fractions (median=15 Gy/5 fractions) for IBT and 40-50 Gy/20-25 fractions (median=50 Gy/25 fractions) for external radiotherapy. Brachytherapy was given as 2 fractions per day 6 hours apart with 4 Gy per fraction for monotherapy and 3 Gy per fraction for combination therapy. Results: Four patients were not evaluable in the analysis of outcome. The primary site relapse rates were 23.8% (10/42) and 68.4% (26/38) in patients treated with IBT alone and combination therapy, respectively (p<0.001). Salvage surgery was performed in 19 patients. The 5-year local control rate was estimated at 62% and the disease-free survival (DFS) rate at 52% for all patients. Local control with respect to T1 and T2 tumours was 84% and 42%, respectively. Conclusions: Our present series on HDR-IBT and the previous report on LDR-IBT for head and neck cancer demonstrated similar DFS rates at 5 years (52%). The rate of regional failure in node-negative patients was <20% in both of our series. HDR-IBT offers similar results to LDR-IBT for head and neck cancer.
