승모판막성형술 실패 직후에 시행한 재수술 (Immediate Reoperation for Failed Mitral Valve Repair)

  • 승모판성형술 직후에 시행한 재수술에 대한 연구보고는 많지 않다. 저자들은 승모판성형술 직후 성형술 실패로 판단되는 환자에서 즉시 시행한 재수술 결과에 대해 알아보았다. 대상 및 방법: 1995년 4월부터 2001년 7월까지 세종병원에서 승모판성형술을 받은 환자 중 체외순환 이탈 직후 시행한 경식도초음파 검사에서 승모판막폐쇄부전 혹은 협착이 의미있게 잔존하거나 다른 이유로 재수술이 즉시 필요하였던 18명을 대상으로 후향적으로 조사하였다. 남녀비는 5 : 13이었고 평균 연령은 44세였다. 승모판막 질환은 폐쇄부전 12명, 협착 3명, 그리고 혼합형이 3명이었다. 원인은 류머치스성 9명, 퇴행성 8명, 그리고 심내막염이 1명이었다. 재수술의 원인은 잔존 승모판폐쇄부전 13명, 협착 4명, 그리고 좌심실천공이 1명이었다. 14명(77.8%)에서 재성형술을, 4명에서는 인공기계판막치환술이 시행되었다. 결과: 조기사망은 없었다. 조기결과는 승모판막치환을 한 4명을 제외한 14명 중 13명(92.9%)에서 0-I도의 폐쇄부전을 보였고 협착은 14명 모두 경도 이하 상태였다. 평균 33개월을 추적조사 한 결과 1명이 술 후 4개월 후 심기능부전으로 사망하였다. 승모판폐쇄부전은 9명(64.3%)에서 0-I도를, 승모판협착은 11명(78.6%)에서 경도 이하였고 재수술은 1명에서 시행되었다. 6년 생존율과 재수술로부터의 자유도는 각각 94%와 90%였다. 4년 후 승모판폐쇄부전 및 협착 재발로부터의 자유도는 각각 56%와44%였다. 결론: 승모판막성형술 직후 재수술은 양호한 조기 및 중기 생존율을 보이며 일차성형술 실패 후에도 높은 빈도에서 재성형술이 가능하다. 하지만 재성형술 시 특히 류머치스성 판막질환에서는 판막 기능부전 발생률이 높기 때문에 성형술 후 판막부전의 재발을 줄이기 위해서는 성형술의 적절한 적용 및 적응증 선별이 중요할 것으로 생각된다. 7명의 환자에서는 완전 방실전도차단이 발생하였고, 이 중 6명의 환자에서 영구 인공심박조율기의 삽입이 필요하였다. 걸론: 부분방실중격결손증은 낮은 사망률로 수술을 시행할 수 있었다. 재수술의 원인은 좌측 방실판막페쇄부전이 가장 많았다. 방실판막구조물의 기형이 동반된 경우 재수술의 가능성이 높아지는 경향을 보였으나 통계적 의미는 없었다. 또한, 술후 좌심실유출로협착여부가 재수술의 위험인자였으므로 좌측 방실판막 및 좌심실 유출로의 해부학적인 구조에 대한 정확한 이해가 요구된다. 좌심실유출로협착은 발생시 대동맥하부의 조직에 대한 절제술을 시행할 수 있으나 재발 가능성이 크고, 필요시 수정 Konno수술로 만족할만한 결과를 얻을 수 있었다. 또한, 방실전도차단은 초기시행단계에서 많이 발생하였으며, 심장 전도계에 대한 정확한 해부학적 이해와 경험축적으로 극복할 수 있었다.로 사용될 수 있음을 입증하였다.첨가군들의 카드뮴 함량은 6주째까지 모든 시험군에서 증가하는 경향을 보였으며, 6주 이후에는 감소되는 경향을 보였다. 7. 분변 중 카드뮴 함량은 C4군이 가장 높았고, 결명자 첨가군들의 카드뮴 함량은 C3군과 C4군을 제외한 나머지 군들에서 완만하게 증가하는 경향을 보였으며, 6주째에 카드뮴 함량이 감소되는 경향을 보였다가 6주 이후에 다시 카드뮴 함량이 증가하는 경향이었다.erefore the guidelines for iron supplementation and nutritional education to improve their iron status should be provided.한 질소제거를 N-balance로부터 구해보면, R3 반응조의 경우가 가장 높은 제거율(40.9%)을 보였다. 이상의 결과들을 볼 때, Bncillus 균주는 호기적 탈질을 일으킬 수 있는 가능성이 있고, Bncillus 균주를 이용한 B3 공정은 탈질에 이용되는 탄소량이

수술용 내시경 로봇(AESOP)을 이용한 최소 침습적 개심술과 동 기간에 시행된 전통적인 개심술의 결과에 대한 비교 (Comparison of the Operative Results of Performing Endoscopic Robot Assisted Minimally Invasive Surgery Versus Conventional Cardiac Surgery)

  • 배경: 내시경 장비와 수술 로봇의 발달로 작은 절개를 이용한 최소 침습적 심장수술이 점점 증가하고 있으나 이에 대한 수술 결과를 고식적인 수술 방법과 비교, 분석한 국내 보고는 드문 편이다. 대상 및 방법: 2005년 12월부터 2007년 6월까지 수술용 내시경 로봇(AESOP2000)을 이용하여 심장수술을 받은 48명(A군)과 동 기간에 정중흉골절개로 수술을 받은 50명(B군)을 후향적으로 조사하여 수술 시간, 수술 결과, 술 후 통증, 회복 정도 등을 객관적으로 비교, 분석하였다. 결과: 수술 사망은 없었으며 술 후 합병증 발생률도 두 군간에 큰 차이는 없었다. 평균 수술시간($292.7{\pm}61.7$분, $264.0{\pm}47.9$분; p=0.01), 체외순환시간($(128.4{\pm}37.6$분, $101.7{\pm}32.5$분; p<0.01)은 A군에서 더 길었으나 대동맥차단시간 ($82.1{\pm}35.0$분, $87.8{\pm}113.5$분; p=0.74), 평균 인공호흡기 사용시간($18.0{\pm}18.4$시, $19.7{\pm}9.7$시; p=0.57)은 차이가 없었으며 중환자실 평균 입원시간($53.2{\pm}40.2$시, $72.8{\pm}42.1$시; p=0.02) 및 평균 재원기간($9.7{\pm}7.2$일, $14.8{\pm}11.9$일; p=0.01)은 A군에서 더 짧았다. 입원기간 중 수혈을 받은 환자는 B군에서 더 많았으며 평균 수혈양도 B군에서 더 많았으나 통계적 유의성은 없었다(p=0.06). 술 후 1주에서 4주까지 통증 점수는 A군에서 의미 있게 적었으며 일상생활의 수행 능력을 측정하는 듀크 활동상태지수 (Duke activity status Index)에 따른 기능 점수(functional status score)는 A군에서 현저하게 높았다. 술 후 심초음파 결과 승모판 성형술을 시행한 환자의 역류 정도($0.7{\pm}1.0,\;0.9{\pm}0.9$; p=0.60)와 삼첨판 성형술을 시행한 환자의 역류 정도($1.0{\pm}0.9,\;1.1{\pm}1.0$; p=0.89)는 두 군간에 통계적으로 유의한 차이가 없었다. 판막주위유출은 각각 1명의 환자에게 있었으며 그 외 판막 관련 합병증은 없었다. 결론: 본 연구 결과 로봇을 이용한 치소 침습적 심장수술은 수술 후 통증 감소, 환자의 빠른 회복, 미용적인 효과, 짧은 입원기간 등의 장점뿐만 아니라 수술 성적도 고식적 수술 방법에 비해 떨어지지 않음이 확인되었다.

엡스타인 기형의 20년 수술 치험 (20 Years Surgical Experiences for Ebstein's Anomaly)

  • 배경: 엡스타인 기형 환자들에서 수술적 치료의 임상적 결과를 살펴보기 위해 후향적 조사를 실시하였다. 대상 및 방법: 1984년 2월부터 2006년 6월까지 엡스타인 기형으로 수술적 치료를 시행 받은 50명의 환자들을 대상으로 후향적 연구를 시행하였다. 환자들의 평균 연령은 26.9세였고, 남자가 19명이었다. 동반 심질환으로는 심방중격결손(33), 동맥관개존(2), 심실중격결손(1), 폐동맥협착(4) 등이 있었고 90% (45/50)의 환자에서 중증도 이상의 삼첨판막 폐쇄부전을 동반하였다. Carpentier type은 A형 6명, B형 26명, C형 14명, D형 4명이 속하였고 10명의 환자에서 WPW증후군을 동반하였다. 수술방법으로는 31명의 환자에서 보존적 수술(삼첨판 성형술, 심방화 우심실 주름잡기술(plication of atrialized RV) 등)이 가능하였으며, 폰탄씨 수술은 4명, 삼첨판막 치환술 12명, 고식적 수술은 2명의 환자에서 시행되었다. 13명의 환자에서 양방향성 상대정맥-폐동맥 단락술을 시행하였고(one and a half ventricular repair), WPW 증후군을 동반한 10명의 환자와 심방세동의 4명의 환자에서 부정맥에 대한 수술적 치료가 함께 시행되었다. 걸과: 술 후 뉴욕심장학회 기능분류(New York Heart Association Functional Class) (중간값 3 (범위: $1{\sim}4$)${\rightarrow}\;(1{\sim}3)$)와 평균 심흉비$(0.65{\rightarrow}0.59)$는 술 전에 비해 통계학적으로 의미 있게 감소하였다(P<0.001, p=0.014). 평균 산소포화도$(86.6{\rightarrow}94.1%)$와 삼첨판막역류 정도(중간값 4 (범위: $1{\sim}4$)${\rightarrow}1\;(0{\sim}4)$) 역시 술 전에 비해 술 후 통계학적으로 의미 있게 향상되었다 (p=0.004, p<0.001). 양방향성 상대정맥-폐동맥 단락술과 보존적 수술간의 비교에서는 수술 전 우심실 압력(33.0 vs 41.3 mmHg), 중환자실 기간(2.86 vs 1.89일), 입원기간(10.6 vs 16.8일), 그리고 수술 후 좌심실 구축률(64.3 vs 72.8%)이 통계적으로 의미 있는 차이를 보였다. 술 후 조기사망이 3명(6%)의 환자에서 있었는데 2명은 양심실 부전, 나머지 1명은 패혈증으로 사망하였다. 평균 추적관찰 기간은 101.5개월이었으며 추적 조사 결과, 심장관련사망이 1명(폰탄 실패)이 있었고 6명의 환자에서 재수술을 요하였다(조직판막 퇴행성 변화(2), 폰탄 전환술(4)). 수술 후 10년 생존율은 90.2%였고, 10년 재수술 자유도는 78.9%, 심장관련 합병증의 자유도는 49.2%이었다. 걸론: 엡스타인 기형의 수술적 치료는 비교적 안전하게 시행될 수 있으며 양방향성 상대정맥-폐동맥 단락술의 추가가 고위험군에서는 도움이 되는 것으로 생각한다. 적절한 술기의 적응이 결국 장기 생존율의 호전과 재수술의 감소를 기대할 수 있을 것으로 생각한다.

감염성 승모판 심내막염의 중단기 수술 성적 (Early and Mid-term Results of Operation for Infective Endocarditis on Mitral Valve)

  • 배경: 감염성 심내막염은 다른 심장질환에 비해 유병률과 사망률이 높고, 대부분의 임상연구가 대동맥판에 발생한 심내막염에 편중되어 있어, 승모판 심내막염에 대한 보고는 상대적으로 드문데 이에 대한 임상양상 및 수술 결과를 분석하여 보고하고자 한다. 대상 및 방법: 1995년 6월부터 2003년 5월까지 전남대학교병원에서 감염성 승모판 심내막염으로 단일 술자에 의해서 외과적 치료를 받았던 23 예을 대상으로 하였다. 2예는 인공판막 심내막염이었고 나머지 21예는 자가판막 심내막염이었다. 환자의 평균 연령은 44.8$\pm$15.7 (11∼66)세였고 연령분포에는 큰 차이가 없었다. 17예에서는 혈역학적 불안정이나 거대 우종으로 응급수술을 시행하였다. 수술 전 12예에서 울혈성 심부전, 4예에서 신부전, 2예에서 비장 및 신장경색, 2예에서 일시적 뇌손상, 1예에서 뇌농양 소견을 보였다. NYHA 기능 분류상 Grade II가 7예, Grade III가 9예, Grade IV가 6예이었다. 술 전 심초음파상 승모판 역류우세가 19예, 협착우세가 4예이었으며, 우종은 20예(86.9%)에서 관찰되었다. 혈액배양 검사상 10예(43.4%)에서 원인균이 동종되었는데 Streptococcus viridans 5예, methicillin sensitive Staphylococcus aureus 2예, 그리고 Corynebacterium, Haemophillis, Gernella 각각 1예씩 이었다. 수술 적응증은 ACC/AHA 지침에 근거하였으며, 평균 외래 추적관찰기간은 27.6$\pm$23.3 (1∼97)개월이었다. 결과: 13예에서 승모판치환술을 시행하였는데 9예에서 기계판막을 사용하였고 4예에서 조직판막을 사용하였다. 10예에서 다양한 수기의 승모판성형술을 시행하였다. 동반수기로는 6예에서 대동맥판 치환술, 2예에서 삼첨판 성형술, 1예에서 변형된 미로수술, 1예에서 심실중격결손 낙합술을 시행하였다. 술 후 합병증으로는 출혈에 의한 재수술 2예, 종격동염 1예, 저심박출증 1예, 페렴 1예가 있었다. 술 후 30일 이내의 조기사망이나 병원 내 사망 및 판막 연관성 합병증은 없었다. 1예의 환자가 수술 3개월 후 다시 판막성형술을 받았고, 술 전 우종 전색에 의한 뇌경색을 보인 1예의 환자가 술 후 31개월에 뇌출혈로 사망하여 만기 전 체판막 연관성 사망률은 4.3%, 판막연관성 합병증은 8.6%였다. 1, 3, 5년 valve-related event free rate는 90.8%, 79.5%, 79.5%이었고 1, 3, 5년 생존율은 100%, 88.8%, 88.8% 이었다. 결론: 승모판 심내막염의 외과적 치료 시에는 감염된 조직의 완전 절제가 중요하며, 감염된 조직을 완전히 제거하면 대체물이나 수술 기법상의 차이가 결과에 미치는 영향은 크지 않을 것으로 판단되고, 술 전에 감염이나 우종 전색에 의한 뇌손상이 의심되는 환자에서는 술 후 추적관찰 중에도 뇌혈관 손상 가능성에 대해 주의 깊게 관찰하여야 할 것으로 생각된다.

고식적 Damus-Kaye-Stansel 술식의 임상적 적용 및 결과 (The Clinical Application and Results of Palliative Damus-Kaye-Stansel Procedure)

  • 배경: Damus-Kaye-Stansel (DKS) 술식은 체심실유출로 협착과 폐동맥고혈압을 완화하기 위하여 근위주폐동맥을 대동맥에 문합시키는 고식적수술이다. 본 연구는 DKS 술식의 적응증, 결과 및 결과에 영향을 주는 요인, 그리고 반월판막 기능과 DKS 경로의 장기 결과들을 알아보고자 하였다. 대상 및 방법: 1994년 5월에서 2006년 4월 사이에 본원에서 DKS 술식을 시행받은 28명의 임상기록을 토대로 후향적으로 분석하였다. 수술시 연령은 중앙값 5.3개월($13일{\sim}38.1$개월), 체중은 중앙값 5.0 kg ($2.9{\sim}13.5\;kg$)이었고, 술전 체심실 유출로 압력차는 $25.3{\pm}15.7\;mmHg$ ($10{\sim}60\;mmHg$)였다. 18명은 초기고식적 수술로 폐동맥교약술을 시행하였으며, 술전 진단은 양대혈관 우심실 기시증이 9명, 심실-대혈관연결 불일치가 있는 양방실판막 좌심실유입이 6명, 그 외 기능성 단심실이 5명, Criss-cross 심장이 4명, 완전 방실중격 결손증이 3명, 그리고 좌심 저형성증후군의 이형이 1명이었다. 수술방법은 팻취를 이용한 측단문합이 14명, 고전적 측단문합이 6명, Lamberti 방법이 3명, 그 외가 5명이었으며, 양방향성 상대정맥 폐동맥 단락술을 6명에서, 폰탄수술을 2명에서 같이 시행하였다. 결과: 병원내 사망은 4명(14.3%)이었고, 평균 $62.7{\pm}38.9$개월 ($3.3{\sim}128.1$개월)의 추적관찰기간 동안 만기사망은 3명(12.5%)이었으며, 10년 누적 생존율은 $71.9{\pm}9.3%$였다. 다변량 분석에서 우심실형 단심실(위험도=13.960, p=0.006)과 일차 DKS 술식의 시행(위험도=6.767, p=0.042)이 사망의 유의한 인자였다. DKS 술식 후 4명은 단계적 양심실성 교정을, 그리고 13명은 폰탄수술까지 시행하였으며, 체심실 유출로 협착은 1명 이외에는 발생하지 않았다. 중등도 이상의 반월판막역류나, 반월판막에 대한 재수술은 없었으나, DKS경로에 의한 폐동맥협착으로 인한 재수술이 고전적 측단문합을 시행했던 1례(3.6%)에서 필요하였다. DKS경로나 반월판막에 대한 재수술의 자유도는 10년에 87.5%였다. 결론: DKS 술식은 체심실 유출로 협착을 동반한 환자군에서 유용한 방법이며, 출생직후 양심실교정이 어려웠던 일부 환자군에서 단계적 양심실 교정을 안전하게 시행할 수 있었다. 체심실유출로 협착이 심하지 않은 경우에 DKS 술식전에 폐동맥 교약술을 시행하여 폐혈관 저항을 감소시킨 후 DKS 술식의 성공률을 향상시킬 수 있었다. DKS 술식후 반월판막기능, DKS 경로와 체심실유출로협착의 완화에 대한 장기 결과는 만족할만하다.

승모판협착증 환자에서 경피적 풍선확장판막성형술의 폐기능 및 운동부하 검사에 대한 효과 (Effects of Percutaneous Balloon Mitral Valvuloplasty on Static Lung Function and Exercise Performance)

  • 연구배경 : 승모판 협착증 환자들은 폐기능검사상 제한성 및 폐쇄성 환기장애를 동시에 나타낼 수 있으며, 또한 운동능력의 장애가 있다는 것은 잘 알려져 있다. 이러한 폐기능의 장애는 수술적 판막교정술에 의해 일부 호전이 있었다고 보고되어 왔다. 이에 저자들은 흉부폐쇄하에서의 경피적풍선확장 승모판막성형술에 의해 승모판 협착증을 교정한후, 폐기능검사 및 운동능력의 호전 정도 및 시술후 사간적 경과에 따른 변화를 보기 위해 본 연구를 시행하였다. 방법 : 승모판 협착증을 가진 46명을 대상으로 하였으며 심초음파상 좌심방혈괴가 보이거나, 승모판 폐쇄부전증이 grade 2이상인 경우는 제외하엿다. 승모판 협착증의 교정전후 폐활량측정법, 폐확산능 및 운동부하검사를 시행하였으며 폐기능의 시간적 경과에 따른 변화를 보기 위하여 폐기능 및 운동부하검사의 추적검사를 11명의 환자에서 평균 5개월 후에 실시하였다. 그리고 동율동을 보인 29명의 환자를 폐동맥압에 의해 A, B 양군으로 나누고, 양군간의 판막성형술 전후 폐기능 및 운동부하 검사결과를 비교하였다. 결과 : 경피적풍선 확장 판막성형술후 평균 10일째 시행한 폐기능 및 운동부하검사결과를 수술전 검사 결과와 비교시, 노력성폐활량(FVC), 1초간노력성호기량($FEV_1$), 노력성호기중간유량($FEF_{25-75%}$), 최고호기유속(PEF), 최대산소섭취량($\dot{V}O_2$ max) 및 무산소역치(AT)등의 유의한 증가가 있었으며, 폐확산능(DLco)의 유의한 감소가 있었다. 한편 5개월뒤 추적 검사한 11명의 환자에서 시술직후와 비교시 폐기능 검사 및 운동부하 검사결과의 유의한 증가는 없었다. 동율동을 보인 29예를 폐동맥압이 35mmHg보다 높은 경우 A군(16예), 35mmHg 이하인 경우 B군(13예)으로 분류하였다. A군에서는 판막성형술전 FVC, $FEV_1,\;\dot{V}O2$ max가 B군보다 유의하게 낮았으나 판막성형술후 A군에서 B군보다 FVC, $FEV_1,\;\dot{V}O_2$ max의 유의한 증가를 보였다. 그리고 A군에서 판막성형술후 AT 및 $\dot{V}O2$ max에서의 분당 산소섭취량에 대한 분당환기량($\dot{V}E/\dot{V}O_2$)과 분당 이산화탄소 생산량에 대한 분당 환기량($\dot{V}E/\dot{V}O_2$)이 유의하게 감소하였으나, B군에서는 AT에서의 $\dot{V}E/\dot{V}O_2$$\dot{V}E/\dot{V}CO_2$는 유의한 감소를 보였으나, $\dot{V}O_2$ max에서의 $\dot{V}E/\dot{V}O_2$$\dot{V}E/\dot{V}CO_2$는 유의한 변화가 없었다. 결론: 1) 승모판협착증 환자에서 경피적풍선확장 판막성형술(PMV)후 1개월 이내 시행한 폐기능검사 및 운동부하 검사상 유의한 호전이 있었다. 2) 중등도 이상의 폐고혈압을 보인 승모판협착증 환자에서 폐동맥압 상승이 경미한 예보다 운동수행 능력의 유의한 증가가 있었다. 3) 중등도 이상의 폐고혈압을 보인 승모판협착증 환자에서 판막성형술후 운동능력의 유의한 호전은 분당 산소소모량에 대한 분당 환기량의 감소에 기인한 것으로 사료되었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times.
