The Third Nationwide Korean Heart Failure III Registry (KorHF III): The Study Design Paper

  • With advancements in both pharmacologic and non-pharmacologic treatments, significant changes have occurred in heart failure (HF) management. The previous Korean HF registries, namely the Korea Heart Failure Registry (KorHF-registry) and Korean Acute Heart Failure Registry (KorAHF-registry), no longer accurately reflect contemporary acute heart failure (AHF) patients. Our objective is to assess contemporary AHF patients through a nationwide registry encompassing various aspects, such as clinical characteristics, management approaches, hospital course, and long-term outcomes of individuals hospitalized for AHF in Korea. This prospective observational multicenter cohort study (KorHF III) is organized by the Korean Society of Heart Failure. We aim to prospectively enroll 7,000 or more patients hospitalized for AHF at 47 tertiary hospitals in Korea starting from March 2018. Eligible patients exhibit signs and symptoms of HF and demonstrate either lung congestion or objective evidence of structural or functional cardiac abnormalities in echocardiography, or isolated right-sided HF. Patients will be followed up for up to 5 years after enrollment in the registry to evaluate long-term clinical outcomes. KorHF III represents the nationwide AHF registry that will elucidate the clinical characteristics, management strategies, and outcomes of contemporary AHF patients in Korea.

Age Distribution of Breast Cancer from a Thailand Population-Based Cancer Registry

  • Background: Breast cancer is a common cancer worldwide. With the establishment of Thailand's population-based cancer registry and availability of complete data from 2002-2011, it is of interest to investigate the epidemiologic and clinic-pathological profiles of breast cancer based on the population-based registry data. Methods: The data of all breast cancer patients in the registry for the period of 2002-2011 were included. All medical records of the patients diagnosed from documents of National Cancer Registry of Thailand were retrieved and the following information abstracted: age, clinical characteristics, and histological variables. Thailand census data for the period of 2002-2011 were used to provide the general population's statistics on age, gender, and other related demographic factors. Results: Over the 10 year-period, 7,711 breast cancer cases were included. The disease incidence under age 40 years was relatively low (4.13/$10^5$) while the incidence in the age groups 40 and older was very high (39.2/$10^5$). The vast majority of breast cancer cases (88.8%) were diagnosed by histology as primary lesions in the breast. The most common of patients with breast cancer (36.4%) had regional lymph node involvement and the most common of histopathology diagnosed in patients (84.2%) was an infiltrating duct carcinoma. Conclusions: This study showed a high incidence of breast cancer in older subjects, and high rate of breast cancer in Thailand. Future studies should explore clinical and molecular disease patterns.

Epidemiology of Primary Brain and Central Nervous System Tumors in Korea

  • Objective : The aim of this report is to provide accurate nationwide epidemiologic data on primary central nervous system (CNS) tumors in Korea. Despite its importance, there are no accurate statistics on primary CNS tumors in Korea. We analyzed primary CNS tumors diagnosed in 2005 from the nationwide registry. Methods : Data on primary CNS tumors diagnosed in 2005 were collected from the Korean Central Cancer Registry and the Korean Brain Tumor Society. Crude and age-standardized rates were calculated in terms of gender, age, and histological type. Tumors of uncertain histology were investigated individually at the corresponding hospitals and had their diagnoses confirmed. Results : A total of 5,692 patients diagnosed with primary CNS tumors in 2005 were included in this study. CNS tumors occurred in females more often than in males (female to male, 1.43 : 1). The most common tumor was meningioma (31.2%). Glioblastoma accounted for 30.7% of all gliomas, and 19.3% of all malignant primary CNS tumors. In children under 19 years of age, both germ cell tumor and embryonal/primitive/medulloblastoma were the most common tumors. Conclusion : This article is the first nationwide primary CNS tumor epidemiology report in Korea. Data from this study should provide valuable information regarding the understanding of primary CNS tumors epidemiology in Korea.

A Comparison of the Cancer Incidence Rates between the National Cancer Registry and Insurance Claims Data in Korea

  • Although much health services research has been conducted using national health insurance claims data in Korea, the validity of this method has not been ascertained. The objective of this study was to validate the use of claims data for health services research by comparing incidence rate of cancers found using insurance claims data against rates of the national cancer registry of Korea. An algorithm to estimate incidence rates using claims data was developed and applied. The claims data from 2005-2008 were acquired and the patients admitted to hospitals due to cancer in 2008 without admission to hospital from 2005-2007 by the same diagnosis code were regarded as incident cases. The acquired results were compared with the values from the National Cancer Registry of Korea. The incidence rate of all cancers found using claims data was 363.1 per 100,000 people, which is very similar to the 361.9 per 100,000 rate of the national cancer registry. Also the age-, gender- and disease-specific rates between the two data sources were similar. Therefore, national health insurance claims data may be a worthwhile resource for health services research if appropriate algorithms are applied, especially considering the cost effectiveness of this method.

  • Interest in conformance and the issues related to conformance and interoperability testing in ebXML products are rapidly gaining momentum among standards developers, implementers, and users. The goal is to obtain implementations of the standard that correctly perform the functionality specified in the standard. Conformance testing helps to achieve correct implementation. In this paper, we design and implement the CPP(Collaboration Protocol Profile)/CPA(Collaboration Protocol Agreement) and registry for ebXML conformance and interoperability testing tools that observe the standard specification in conformity with ebXML Message Service v2.0, ebXML Registry Service v2.0 and ebXML CPP/A v2.0.

  • In the Hive file format, the sk(Security Key) cell provides access control to registry key. An attacker can figure out secret information on registry or change the security set-up if she could apply modified hive files on system. This paper presents various methods to change access control of registry key by modifying or replacing cell on hive file. We also discuss threats by access control modification and signs of attacks analysis by modified hive files.

  • Trauma care is evolving throughout the world to meet the demand resulting from rapidly increasing rates of mortality and morbidity related to external injuries. The State Major Trauma Service was designated to Royal Perth Hospital in 2004 to provide comprehensive care for trauma patients in Western Australia (WA), which is the largest state by area in the country. The State Major Trauma Unit, which was established in 2008, functions as a level I center and admits over 1,000 major trauma patients per year, making it the second busiest trauma center in Australia. The importance of recording data related to trauma was identified by the trauma service in WA to inspire higher standards of patient care and injury prevention. In 1994, the service established a trauma registry, which has undergone significant changes over the last two decades. The current State Trauma Registry is linked to a statewide database called the Data Linkage System. The linked data are available for policy development, quality assurance, and research. This article discusses the evolution of the trauma service and the registry database in the WA health system. The State Trauma Registry has enormous potential to contribute to research and quality improvement studies along with its ability to link with other databases.

  • Nowadays, ebXML and Web Services are two major e-business frameworks, and ebXML Registry and UDDI play important roles in publishing and inquiring business partners' meta information. These two types of registries should be interoperated in order to interoperate two frameworks. In this paper we propose interoperation procedures for each case of interoperation, i.e. ebXML-based, WS-based, or Integrated Model-based Interoperation.

  • 본 논문은 현재 18M, 마이크로소프트, SAP등이 운용하고 있는 대표적인 공용 UBR(UDDI Business Registry) 노드들과 국내에서 개발된 K-UDDI, 일본의 NTT에서 운용하는 UBR노드에 대해 각각의 특징을 서로 비교해 본다. 또한 간단한 모니터링 프로그램을 구현하여, 각각의 UDDI 비즈니스 레지스트리에 등록되고 있는 전체 비즈니스 개체와 비즈니스 서비스 수의 증감 추이를 관찰하고, 등록된 비즈니스 개체별 링크된 WSDL파일의 유효함의 여부를 실행한 결과물을 통해 현재 운용중인 공용 UDDI 레지스트리의 활성화 정도를 분석해 본다.
