간호사 국가시험 방향설정을 위한 임상수행 능력- 기본간호학, 성인간호학, 정신간호학, 여성건강간호학, 지역사회간호학, 아동간호학, 간호행정을 중심으로 - (Clinical Competency for Directing of Registerd Nurses정 National Examination.)

  • For producing large numbers of professional nurses who could manage 21th century's human health, it is necessary to review the direction of registered nurses' national examination which evaluates the nursing education and is granted a licence. For adapting to social expectation of the nurse, we have to nurture the nurses' problem solving capability in clinical setting. Seven divisions of Korean Academy of Nursing suggested clinical competency according to their categories. This paper was presented in the work-shop for setting up direction of registered nurses' national examination. We expect that this paper would be more refine and confirm through reviewing subdivisions' learning objectives and discussing clinical minimum level of competence contents with clinical leaders.

재미한인간호사의 사회적 지지, 문화적응 스트레스가 우울에 미치는 영향 (Impacts of Social Support and Acculturative Stress on Depression among Korean Registered Nurses in the United States)

  • Purpose: The purpose of this study was to identify the impacts of social support and acculturative stress on depression among Korean registered nurses in the United States. Method: In total, 137 nurses were recruited through direct interviews and online surveys. The data were collected through a structured questionnaire from June 1, to September 16, 2012, and analyzed using descriptive statistics, t-test, ANOVA, Scheff$\acute{e}$'s test, Pearson correlation coefficient, and stepwise multiple regression with SPSS WIN 14.0. Results: Social support significantly differed by the length of one's nursing career in US, monthly income and perceived health status. Acculturative stress significantly differed by the length of one's nursing career in US and types of employment setting. Depression significantly differed by the total length of one's nursing career, type of employment setting, and perceived health status. Moreover, Depression was negatively correlated with social support; positively correlated with acculturative stress; social support was negatively correlated with acculturative stress. Factors influencing depression were acculturative stress, perceived health status, and social support, which explained about 23% of the total variance. Conclusion: These results suggest that an adaptation program that decreases acculturative stress and improves social support should be developed and implemented for Korean nurses to help them successfully integrate into the healthcare system of new country.

신생아중환자실 간호사 업무수행 평가도구 개발 (Developing a Performance Appraisal Tool for Neonatal Intensive Care Unit Registered Nurses)

  • Purpose: This study was done to develop a performance appraisal tool (PAT) for neonatal intensive care unit (NICU) registered nurses (RNs). Methods: The PAT was developed in three steps: 1) a standard of NICU nursing was established; 2) a draft was made; and 3) the PAT was ratified and its reliability and validity were tested. Results: The standard of practice of NICU nursing was predominantly based on role description and responsibilities for nurses. We identified 4 domains of nursing: professional practice, education, research, and leadership. The validity score for each item ranged from 3.93 to 3.27. The PAT consisted of 76 indicators, 49 reflecting components of professional practice, 5 related to responsibility for education, 3 representing research, and 19 relating to leadership. Cronbach's ${\alpha}$ averaged 0.99 for the 76 items. Conclusions: The PAT for NICU RNs that we created was found to be reliable and valid. This PAT would be very useful in evaluating nursing performance and facilitate the professional growth of nurses.

간호대학생의 국가고시 관련 시험불안, 자아존중감과 정신건강에 대한 연구 (Impact of Self-Esteem and Test Anxiety on Mental Health among Nursing Students Taking the National Examination for Registered Nurses)

  • Purpose: The purpose of this study was to examine the relationship between test anxiety, self-esteem and mental health. Method: Data were collected by structured questionnaires from 306 nursing students in a dormitory for the national examination for registered nurses. Collected data were analyzed by t-test, ANOVA, scheffe's test and Pearson's correlation coefficient and hierarchial multiple regression through the SPSS WIN program. Result: The results of the analysis showed that the mean for test anxiety was $79.70{\pm}18.74$ points, the degree of self-esteem was $28.26{\pm}5.02$, and mental health was $24.31{\pm}13.07$. In the correlation analysis, mental health positively correlated with test anxiety (r=.559, p=.000) and negatively correlated with self-esteem (r=-.141, p=.013). Test anxiety and subjective health status explained 36.4% of mental health. Conclusion: This finding of this study recommends that test anxiety control and self-esteem enhancement programs should be development for mental health promotion in nursing students taking the national examination for registered nurses.

요양병원 간호인력 배치수준이 환자결과에 미치는 영향에 관한 경로분석 (Path Analysis of the Effects of Nurse Staffing on Patient Outcomes in Long-Term Care Hospitals)

  • Purpose: The study aimed to examine the effects of nurse staffing levels on patient outcomes in long-term care hospitals in South Korea using path analysis. Methods: We conducted a secondary analysis of national data, which included the 2021 hospital assessment results of long-term care hospitals from the Health Insurance Review and Assessment Service. Data collection was performed between June 19 and June 27, 2023. The study sample consisted of 1,215 hospitals, and we analyzed the data using SAS 9.4 and Mplus 8 software. Results: The average numbers of patients per registered nurse, certified nursing assistant, and nursing staff, including both registered nurses and certified nursing assistant, was 10.00, 7.43, and 4.00, respectively. Path analysis revealed that the number of patients per registered nurse had direct effects on improvements in activities of daily living and indirect effects on indwelling catheterization, pressure ulcer improvement, and weight loss. The number of patients per certified nursing assistant had direct effects on new pressure ulcer development and pressure ulcer improvement, with no indirect effects. The number of patients per nursing staff had direct effects on pressure ulcer improvement and no indirect effects on other patient outcomes. Conclusion: The findings suggest that establishing policies to evaluate staffing levels of registered nurse and certified nursing assistants separately is necessary in order to improve registered nurse staffing levels and patient outcomes in long-term care hospitals.

상급종합병원 간호사의 근거기반실무에 대한 지식과 태도 및 수행 (Registered Nurses' Knowledge, Attitudes, and Practice about Evidence-Based Practice at General Hospitals in Korea)

  • Purpose: Evidence-based practice (EBP) has emerged as a marker for healthcare quality and a global issue in nursing care. This study was conducted to identify Korean registered nurses' knowledge, attitudes, and practice about EBP in Korea. Methods: With a cross-sectional design, the EBP questionnaire (EBPQ) was administered to a convenience sample of nurses (N=453) recruited from 44 general hospitals. To increase the response rate, the directors of nursing were individually contacted by the researchers. The final response rate was 95.7%. Results: The majority of participants were females (99.3%), staff nurses (57.8%), and with bachelors degree (40.6%). Most of them (314, 73.4%) had ever heard about EBP. The overall mean score for the EBPQ was $4.72{\pm}0.69$ out of maximum score of 7. The mean score for the knowledge of EBP, attitudes towards EBP, and practice/use of EBP were $4.61{\pm}0.75$, $4.85{\pm}0.94$, and $4.90{\pm}0.88$, respectively. Educational level, age, and exposure to EBP were statistically significant predictors of Korean nurses' EBP perception. Conclusion: The findings showed that nurses with higher education are more likely to use and value the best available evidence for practice. This study suggests needs not only for further exploration but also for developing educational plans provided by each institute to assist nurses with the process of EBP and the associated skills.

야간전담간호사와 3교대간호사의 표준 교대근무 지표(Standard Shiftwork Index) 차이 (A Comparison of Standard Shiftwork Index between Night Shift Fixed Nurses and Rotating Shift Nurses)

  • Purpose: The purpose of this study was to compare the working status and Standard Shiftwork Index (SSI) between night shift-fixed nurses and rotating shift nurses. Methods: Participants were 367 nurses working as registered nurses at 7 different medical institutions. The data were collected from August 8, 2016 to September 30, 2016. Results: Night shift fixed nurses were more satisfied with work schedule than rotating shift nurses. Rotating shift nurses reported higher SSI scores, such as chronic fatigue, musculoskeletal problems, than night shift fixed nurses. In SSI's sub-items, rotating shift nurses reported higher scores, such as the amount of sleep, feeling well after sleep, heartburn or stomach-ache and heartbeat irregularities, than night shift fixed nurses. Conclusion: The results indicated that night shift fixed system might be an effective method to improve nurses' work satisfaction. It is necessary to figure out any suitable night shift fixed system based on the characteristics of each nursing organization. The SSI might be an useful tool to measure the nurses' satisfaction in reference to their work shift.

병원간호사의 부정적 감정조절, 정서억제, 직무만족 및 조직몰입 간의 관계 (The Relationship of Negative Emotion, Emotion Suppression, and Job Satisfaction to Organizational Commitment in Hospital Nurses)

  • Purpose: The purpose of this study was to examine the relationships among negative emotion, emotion suppression, job satisfaction and organizational commitment in Hospital Nurses. Method: The participants were 155 registered nurses working in 1 hospital in Seoul. Data were obtained by self-report questionnaires. Data were collected from October 1 through October 9, 2010. Data were analyzed using frequency and percentage, t-test, ANOVA and Scheffe's test and stepwise multiple regression. Result: The influencing factors for organizational commitment of hospital nurses were job satisfaction, position, total clinical career, and negative emotion. These variables explained 56.6% of the variance of the organizational commitment. Conclusion: These results indicate that improving job satisfaction is the best way to enhance nurses' organizational commitment and thus, a need to improve the job satisfaction of hospital nurses.
