Nationwide Study on the Usage and Characteristics of Patients Visiting Korean Medical Facilities - Based on the Treatment of Major Disorders, Effectiveness, Satisfaction and Occurrence Rate of Side Effects From the Ministry of Welfares Report on Usage and Consumption of Korean Medicine in 2011 - (전국민을 대상으로 한 한의원과 한방병원 외래이용환자의 이용실태 및 특성비교연구 - 2011년 한방의료이용 및 한약소비실태조사 보고서(보건복지부)중 이용환자의 질병치료방법 및 치료효과를 중심으로 -)

  • A survey conducted on 1,103 patients or caretakers visited Korean medical clinics and hospitals between August 25, 2011 to September 30, 2011 by the Ministry of Health and Welfares and Korea Institute for Health and Social Affairs was analyzed and obtained following results: 1. For sociodemographic distribution, female(813) outnumbered male(290) patients with majority of patients ranging from 30s to 60s. Married patients(793) outnumbered unwed(150) patients and 65.0% with higher than high school education. Statistical significance was seen in gender, age, marital status, and education level but no significant difference for status of employment, income level, and types of insurance between the clinics and hospitals. (P<0.05). 45.1% had less than \2,000,000 in monthly salary and most of coverage was provided by either regional or work insurance. 2. 67.9% of the patients rated health conditions to be better than average and 32.1% listed as poor. People in good health showed tendency to visit Korean medical facilities. Musculo-skeletal conditions such as arthritis, ankle sprain, lumbago, muscular injury, and frozen shoulder were common conditions, followed by gastric disorders, common cold and herbal tonics. No significant difference was observed between the clinics and hospitals for above conditions, but significance was seen in atopic dermatitis, stroke, and sequela from traffic accidents (P<0.05). 3. Ten most common conditions addressed at Korean medical facilities were lumbago, arthritis, muscular injury, back sprain, gastric disorders, ankle sprain, common cold, herbal tonics, frozen shoulder and stroke. Major treatment modalities rendered were herbal medicine, herbal supplements, acupuncture and moxibustion, cupping, tuina, and Korean midical physical therapy. No significant difference existed between the clinics and hospitals. 4. All modalities showed at least 85% effectiveness. No statistical significant difference between the clinics and hospitals except for herbal decoction. (P=0.0452) 5. 88.3% of responses showed treatment satisfaction with significant difference between the clinics and hospitals (P=0.002). The occurrence of side effects was at 2%, mostly corning from treating digestive, skin, kidney disorders and neurological issues. No significant difference was observed between the clinics and hospitals. From the above results, the typical population visiting Korean medical facilities can be summarized as being middle aged female with relatively higher education and moderately low income. The health condition is generally good and the purpose of visit is to receive traditional treatments of acupuncture, herbal medicine, and physical therapy. Treatment efficacy and satisfaction were high with no significant differences between the clinics and hospitals.

Value of Freight Travel-Time Savings for Road Investment Evaluation (도로사업의 투자분석을 위한 화물운송시간가치 산정)

  • Most investment evaluations and economic assessments of road transport proposals in Korea omit a valuation of the time spent in transit for loads of freight. These days there were few attempts to estimate value of freight travel-time savings in Korea, but most of them included rail or marine with statewide area so that couldn't obtain unique travel-time savings for road freight transport. This study applied revealed Preference method and associated binominal logit models to estimate the value of travel-time savings in transit from an statewide survey of road freight transport in 1997. Data sets were segmented according to transport areas and business types. The results of this study showed that the value of freight travel-time savings varied wide ranges from 53,449 won per hour in urban transport to 29,397 won per hour in regional transport, that the use of statewide value of freight travel-time savings can drives wrong results into economic assessment, and that the use of adequate value of freight travel-time savings according to assessment area is very important.

  • The purpose of this study is to examine the impact of social enterprise's shared value creation on corporate performance and to develop policy measures to enhance competitiveness of social enterprises. The main results and implications are as follows. First, according to the general characteristics of social enterprises, the difference between shared value creation and organizational performance was not statistically significant. In other words, although the social enterprise has various organizational characteristics, it does not differ from the shared value creation or the organizational performance that it creates. The rapid increase of social enterprises led to a rapid increase in various types of social enterprises, but there is no difference between shared value creation and organizational performance. Second, the results of analyzing the impact of shared value creation on the organizational performance of social enterprises revealed that social performance was statistically significant influenced by re-recognition of product market and regional cluster among sub-factors of shared value creation. Among the sub-factors of social enterprise's economic performance, the re-recognition of the product market had a statistically significant effect on the overall organizational performance of social enterprises. Among the sub-factors of shared value creation. In the future, it will be necessary to make effort to improve corporate performance by introducing the concept of shared value creation for social enterprise and making policy efforts to activate it.

State of education regarding ultrasound-guided interventions during pain fellowships in Korea: a survey of recent fellows

  • Background: Recently, the use of ultrasound (US) techniques in regional anesthesia and pain medicine has increased significantly. However, the current extent of training in the use of US-guided pain management procedures in Korea remains unknown. The purpose of the present study was to assess the current state of US training provided during Korean Pain Society (KPS) pain fellowship programs through the comparative analysis between training hospitals. Methods: We conducted an anonymous survey of 51 pain physicians who had completed KPS fellowships in 2017. Items pertained to current US practices and education, as well as the types of techniques and amount of experience with US-guided pain management procedures. Responses were compared based on the tier of the training hospital. Results: Among the 51 respondents, 14 received training at first- and second-tier hospitals (Group A), while 37 received training at third-tier hospitals (Group B). The mean total duration of pain training during the 1-year fellowship was 7.4 months in Group A and 8.4 months in Group B. Our analysis revealed that 36% and 40% of respondents in Groups A and B received dedicated US training, respectively. Most respondents underwent US training in patient-care settings under the supervision of attending physicians. Cervical root, stellate ganglion, piriformis, and lumbar plexus blocks were more commonly performed by Group B than by Group A (P < 0.05). Conclusions: Instruction regarding US-guided pain management interventions varied among fellowship training hospitals, highlighting the need for the development of educational standards that mandate a minimum number of US-guided nerve blocks or injections during fellowships in interventional pain management.

Ways to Improve the Efficiency of the Self Quality Inspection Analysis for RTE Kimbab (김밥의 자가품질검사 항목설정의 합리화 방안)

  • Purpose - The self quality inspection which is mandatory in Republic of Korea can play an important role in promoting food safety, but the cost for self quality inspection is also a big burden especially for small businesses. In addition, excessive or duplicated inspection costs resulting from the standard gaps of various ministries and agencies ultimately effect negative social utility. Thus, this paper aims to suggest ways to improve the analysis of self quality inspection for the Ready-to-Eat Foods in terms of dried laver, in order to reduce costs caused by overinclusion. Research design, data, and methodology - The focus of interest is seaweed in Kimbab, which is to be exempted from the Vibrio parahaemolyticus test on the three grounds. First, existing literature provides little support for the necessity of performing a microbiological test. Second, laver products do not require the V.parahaemolyticus test by international standards such as the CODEX. Third, no case was found for seaweed products on the list of information on failed food items over the 2015~16 period. Results - Other types of seaweed such as exportable and seasoning seaweed are not subject to the test. Hence, exclusion of the V.pahaemolyticus test on seaweed is a valid point, bringing about large expected cost savings to many small businesses. Conclusions - Based on a complete survey of 75 food-testing agencies from March 20th to 30th, 2018, this paper finds that the proposed revision of the Kimbab test is likely to save an average of 130,000 won per business per year. Especially, in the case of the testing fee of Salmonella spp.(n,c,m,M), the cost difference by agencies was found to be up to five times. The regional gap in testing fees can be considered an unfair barrier. Accordingly, it is necessary to examine whether the testing fee is set at an appropriate level through the cost-extraction program proposed by the Ministry Food and Drug Safety. Hence, the survey results also point out that harmonization of testing fees charged by different food-testing agencies is appropriate with respect to transparency and efficiency.

Breast Cancer Characteristics and Survival Differences between Maori, Pacific and other New Zealand Women Included in the Quality Audit Program of Breast Surgeons of Australia and New Zealand

  • Background: The Quality Audit (BQA) program of the Breast Surgeons of Australia and New Zealand (NZ) collects data on early female breast cancer and its treatment. BQA data covered approximately half all early breast cancers diagnosed in NZ during roll-out of the BQA program in 1998-2010. Coverage increased progressively to about 80% by 2008. This is the biggest NZ breast cancer database outside the NZ Cancer Registry and it includes cancer and clinical management data not collected by the Registry. We used these BQA data to compare socio-demographic and cancer characteristics and survivals by ethnicity. Materials and Methods: BQA data for 1998-2010 diagnoses were linked to NZ death records using the National Health Index (NHI) for linking. Live cases were followed up to December $31^{st}$ 2010. Socio-demographic and invasive cancer characteristics and disease-specific survivals were compared by ethnicity. Results: Five-year survivals were 87% for Maori, 84% for Pacific, 91% for other NZ cases and 90% overall. This compared with the 86% survival reported for all female breast cases covered by the NZ Cancer Registry which also included more advanced stages. Patterns of survival by clinical risk factors accorded with patterns expected from the scientific literature. Compared with Other cases, Maori and Pacific women were younger, came from more deprived areas, and had larger cancers with more ductal and fewer lobular histology types. Their cancers were also less likely to have a triple negative phenotype. More of the Pacific women had vascular invasion. Maori women were more likely to reside in areas more remote from regional cancer centres, whereas Pacific women generally lived closer to these centres than Other NZ cases. Conclusions: NZ BQA data indicate previously unreported differences in breast cancer biology by ethnicity. Maori and Pacific women had reduced breast cancer survival compared with Other NZ women, after adjusting for socio-demographic and cancer characteristics. The potential contributions to survival differences of variations in service access, timeliness and quality of care, need to be examined, along with effects of comorbidity and biological factors.

Stratigraphical Research of the Quaternary Deposits in the Korean Peninsula (韓半島 第四紀 地層의 層序的 考察)

  • With regard to the Quaternary formations in the Korean Peninsula, very few studies have been done specially from a stratigraphic viewpoint. The alluvial sediments filling in the valleys have often been considered as the only formation of Quaternary age (more precisely of the Holocone) and so the Pleistocene was regarded as an erosional or nondepositional episode. This is apparently evident from a quick look at the general geological maps of the Peninsula, which show a lithological sequence of Mesozoic or Paleozoic substrata immediately overlain by Holocene alluvium. Likewise, the Pleistocene period was described in terms of unconformity in most local or regional stratigraphical successions of the Peninsula. Recently several different types of Quaternary formation, besides the so-called Holocene alluvium in the valley plain have been found around the Peninsula. They consist of coastal deposits, marine or fluviatile terrace deposits, ancient valley fill deposits or slope deposits. Some parts of the volcanic sediments in Jeju Is. are also known as the Quaternary sequence. Thus the Quaternary deposits in the Peninsula are far more developed than previously known to gelolgists. Moreover the importance of Quaternary research became recently apparent in Korea due to the shortage of raw materials and to the policy of an optimum land-use. Advanced constructions and land reclamation have required more precise engineering parameters of loose materials and an estimation of land stability. This does not imply only the engineering, or the structural properties of the loose material, but at the same time the basic study of the sediments from the stratigraphical and environmental viewpoints has been necessary. In this connection, Quaternary outcrops specially along the coastal fringe of the Peninsula have been mapped, profiled and sampled for sedimentological, clay-mineralogical and palaeomagnetic studies. All these results are compiled for the core of the Quaternary stratigraphy of the Peninsula.

Separate type heat pipe performance comparison by the heat exchanger shapes (열교환기 형상에 따른 분리형 히트파이프 성능 비교)

  • This study compared fin-tube and parallel-flow heat pipes for their sensible heat exchange rate, heat recovery amount, and air-side pressure drop. Tests were done with different refrigerant charging rates of 40-60% vol. and air flow rates of 300-1,400. The sensible heat exchange rate was highest for both types of heat pipes at a working fluid charge of 40% vol. and low flow rate. For the parallel-flow heat pipe, the 60% vol. charge is too high and results in a low sensible heat exchange rate. The reason is that the thicker liquid film of the tube wall deteriorates the heat transfer effect. Hence, the optimal charging rate is 40 to 50% vol. The evaporator heat pipe has a larger air-side pressure drop than the condenser section heat pipe. The reason is considered to be condensation water arising from the evaporator surface. Compared to the fin-tube heat pipe, the parallel-flow heat pipe showed better performance with a working fluid charging rate of 48%, volume of 41%, and an air-side pressure drop about 37%.

Summer Precipitation Variability in the Han River Basin within the Context of Global Temperature Gradients (전지구 온도지표를 이용한 한강유역의 여름철 강우특성 변화 분석)

  • In this study, two global simple indices are used to investigate climate variability and change in observations. Land-Ocean Contrast (LOC) is an index of area-averaged surface temperature contrast between land and ocean. Meridional Temperature Gradient (MTG) is defined as the mean meridional temperature gradient in the Northern Hemisphere from mid to high latitude and sub-tropical zonal bands. These indices have direct or indirect effects on changing in atmospheric circulations and atmospheric moisture transport from north-south or east-west into East Asia (EA). In addition, warm season hydrometeorology in EA is highly associated with water supplies for coupled human and natural systems including drinking water, irrigation, hydropower generation as well as fisheries. Therefore, in this study, we developed an empirical separation approach for summer rainfall from typhoon and monsoon. An exploratory analysis was also conducted to identify the regional patterns of summer monsoon precipitation over the Korean peninsula within the context of changes in different types of temperature gradients. The results show significant and consistent changes in summer monsoon rainfall during the summer season (June-September) in South Korea.

A Study on Improvement of Korean Port Incentives (우리나라 항만 인센티브제도 개선방안 연구)

  • This study explores the incentive system in Korean ports to become a global logistics hub in Asia or to be competitive among Korean ports as a regional hub. First, we identified the types of port incentives in Korea and in overseas ports. We selected potential incentives applicable to Korean ports and compared whether they were meaningful, efficient, and effective in the short and long term. We selected IPA (Importance-performance Analysis) as an analytical method and surveyed users and experts. The results show that it is necessary and effective to provide incentives for new shipping companies, both export and import shippers, while setting criteria for reasonable incentives. Factors needed for improvement included simplifying and calculating the incentive request process, incentives for terminal operators, and new incentives, in addition to existing incentives. Policy-making organizations will also need to collect feedback on institutional improvements and raise awareness among users. Key factors for improvement include providing incentives for existing shipping companies. In particular, in order to actively manage systematic and efficient policies, incentive criteria for existing shipping companies need to be reconsidered.
