Regional Difference of Health Care Utilitzation in Korea (의료이용의 지역간 격차 -3차성 내과계 진단군을 중심으로-)

  • This study is conducted to investigate the current status on the utilization of health care and plan for solving this problem. The claims data of the fiscal tear 1995 obtained from the regional health insurance society are used for the study. The main findings of the study are summarized as follows. Indexes(The Extremal Quotient(EQ), coefficients of variance(CV's))which represent the regional difference in the admission rate of the tertiary medical diagnosis group report that there is difference in quantity and quality of utilization of health care. The admission rate is lower in the big city areas, Kyoungkido, Kangwondo and Chunlapukdo. Even after age-sex adjustment, the admission rate is still low in Kangwondo, Chunlapukdo and Kyoungsangpukdo. The big city areas tend to have higher rates in the expenses per claim, hospital days per claim, and daily expenses but the rates are still low in some area in Kangwondo, Chunlanamdo and Kyoungsangpukdo. This result remains as same after age-sex adjustment. There is a large regional difference in average utilization rate for the tertiary hospital of the tertiary medical diagnosis group: 57.2%(SD 11.53). The utilization rates for the tertiary hospital in their large catchment area are 96.34%, 83.19% and 73.22% in each Kyoungin, Kyoungnam and Kyoungpuk areas whereas it is lower in a Chungpuk and Chungnam areas. The regional differences of health care utilization of the tertiary medical diagnosis group gave some relationships with their geographical characteristics such as socio-economic characteristics and supply factors of medical services. It is important that many medical policies should be developed in order to minimize and balance out the regional differences of health care utilization. The service allocation policy should include the reconstruction of manpower policy, developing the resource allocating formula, finding the self-sufficient catchment area and reforcing of public health services. Moreover, in order to achieve the balanced development by region, they should investigate and consider each county's microscopic properties under the consistent macrocopic policy. The further studies to find causes of regional difference are needed.

Evaluation of Impact Energy Absorption Characteristics of Flexible Sand Asphalt Pavement for Pedestrian Way (보도용 연성 샌드 아스팔트 포장의 충격흡수 특성 평가)

  • More than 90% of roadway in the world are paved as asphalt concrete pavement due to its excellent properties compared with other paving materials; excellent riding quality, flexibility, anti-icing property and easy maintenance-ability. In this study, to make best use of the softer property of the asphalt mixture, the flexible sand asphalt mixture (FSAM) was developed for pedestrian ways. The mix design was conducted to prepare FSAM using PG64-22 asphalt, screenings (sand) less than 5mm, crumb rubber, hydrated lime and limestone powder without coarse aggregate. The deformation strength ($S_D$), indirect tensile strength (ITS) and tensile strength ratio (TSR) tests were conducted to make sure durability of FSAM performance. The impact energy absorption and flexibility were measured by drop-boll test and the resilient modulus ($M_R$) test. The impact energy absorption of FSAM was compared with normal asphalt pavement, concrete pavement, stone and concrete block for pedestrian way. As a result of drop-boll test, FSAM showed higher impact energy absorption compared with other paving materials with the range of 18% to 43%. Impact energy absorption of FSAM increased with increasing test temperature from 5 to $40^{\circ}C$. The results of $M_R$ test at $5^{\circ}C$ showed that the flexibility of FSPA was increased further, because the $M_R$ value of the sand asphalt was measured to be 38% lower than normal dense-graded asphalt mixture (WC-1). Therefore, it was concluded that the FSAM could provide a high impact absorbing characteristics, which would improve walking quality of the pedestrian ways.

  • Using extensive job posting big data, this study investigates the industrial and locational characteristics of decent work from the supply side. The analysis revealed that manufacturing is pivotal in supplying decent work, accompanied by a stark regional disparity, most notable in the Seoul Metropolitan Statistical Area (MSA), which constitutes nearly half of all decent work opportunities. The study further uncovered that the distribution of decent work varies significantly across MSAs, with a pronounced inclination towards a higher supply in peripheral rather than central areas. These findings bring to light the critical need for policies that bolster manufacturing, aiming to enhance the availability of high-quality jobs and to bridge the job quality gap between the Seoul MSA and other regions. Moreover, the results emphasize the necessity for customized job supply strategies in each MSA, prioritizing strategies that account for the proximity between workplaces and living areas in the job supply process.

Comparison of NPS Pollution Characteristics between Snowmelt and Rainfall Runoff from a Highland Agricultural Watershed (고랭지 밭 유역에서 융설과 강우유출로 발생하는 비점오염원의 특성 비교)

  • Runoff, NPS pollution load and flow-weighted mean concentration (FWMC) occurred by snowmelt and rainfall runoff were compared by a variance analysis. Snowmelt runoff ranged between 1,449 and $19,921m^3$. The peak snowmelt runoff was similar to the runoff that occurred by about 40mm/day rainfall. And average snowmelt runoff was not significantly different from the runoff that occurred by 25.5 mm/day rainfall. Average values of SS loads and FWMCs were 5,438 kg/day and 954.9 mg/L, respectively. SS loads and FWMCs were in the similar range with those that occurred by 39.0 mm/day and 53.0 mm/day rainfall, respectively. Daily SS and COD loads and FWMCs occurred by snowmelt and rainfall were analyzed not to be significantly different. Overall assessment led that the NPS pollution loads by snowmelt runoff had a similar characteristics with the loads by about 40 mm/day rainfall runoff. It was recommended that the agricultural fields in snowy region needs to managed not only for rainfall runoff but also snowmelt runoff for an effective water quality management.

Categorization of Regional Delivery System for the Elderly Chronic Health Care and Long-Term Care (지역별 노인 만성기 의료 및 요양·돌봄 공급체계 유형화)

  • Background: By applying the suggested criteria for needs-based chronic medical care and long-term care delivery system for the elderly, the current status of delivery system was identified and regional delivery systems were categorized according to quantity and quality of delivery system. Methods: National claims data were used for this study. All claims data of medical and long-term care uses by the elderly and all claims data from long-term care hospitals and nursing homes in 2016 were analyzed to categorize the regional medical and long-term care delivery system. The current status of the delivery system with a high possibility of transition to a needs-based appropriate delivery system was identified. The necessary and actual amount of regional supply was calculated based on their needs, and the structure of delivery systems was evaluated in terms of the needs-based quality of the system. Finally, all regions were categorized into 15 types of medical and care delivery systems for the elderly. Results: Of the total 55 regions, 89.1% of regions had an oversupply of elderly medical and care services compared to the necessary supply based on their needs. However, 69.1% of regions met the criteria for less than two types of needs groups, and 21.8% of regions were identified as regions where the numbers of institutions or regions with a high possibility of transition to an appropriate delivery system were below the average levels for all four needs groups. Conclusion: In order to establish an appropriate community-based integrated elderly care system, it is necessary to analyze the characteristics of the regional delivery system categories and to plan a needs-based delivery system regionally.

  • For us, any regional art exist? Almost all art events today take place mostly in Seoul. Concentrating on the reinforcement of its power, the regional art world remains isolated without any connection with the Seoul art community. It is completely closed off from the central art scene. The regional art world seems to consider that more helpful in consolidating its status. It is in real sense suffering the absence of art criticism and art media, lack of an understanding of art and backwardness of art education. Many regional artists are dreaming of moving out from their domain, aspiring to be a member of the central art world. They make an effort to assimilate into a refined, modern style of the central art circle, rather than striving to create works imbued with regionality and locality. As the artists living in the provinces, the characteristics of their district should be above all else reflected in their art. Although they are less informed about the latest trend of culture and art, their art has to be something passionately and obstinately embodying the intrinsic quality of their regions. They have to find a practical solution more positively, instead of merely complaining about the poor condition of the regional art community. There is the need to bring about a turnabout in our awareness that regional art is confined to any limit and cannot be in the center of the entire art world. What' s most significant is to dissolve factionalism and not to depend on school ties or regional networks. As a reaction against authoritarianism, regional art has to put emphasis on attaining its originality. For this, regional art should respect regional tradition, history and background and break away from an old convention of wielding power.

  • The population concentration in the Capital region of Korea has become an important issue for the pursuit of the balanced regional human capital development. Considering migration both as a geographic and a social movement, migration to the capital region could be examined in the push factors and the selective migrant characteristics from the out-migration region. Their relative importance reveals that age and education level are important in almost all years, but the importance of the percentage of manufacturing sector and rural/urban region moves to the years of education, the percentage of unskilled occupation and manufacturing sector and unemployment ratio recently. Since the brain drain has been occurring under the highly unbalanced regional development in Korea, the results suggest that regional human capital investment should be accompanied with enlarging quality employment opportunities to reap the benefits.

Effect of Mixed Soybeans Materials on Qulity Characteristics of Traditional Soybean Paste(Doenjang) during Aging (혼합콩이 전통된장의 숙성 중 품질에 미치는 영향)

  • The effects of five colored soybean (blue, red, yellow, white, and black) varieties on the quality characteristics of soybean paste (Doenjang) were investigated for proximate composition, salt content, pH, color, organic acid content, amino acid content, and mineral content after 80 days of aging. Soybean paste (Doenjang) samples were divided into TDS (traditional Doenjang prepared with soybean) and TDM (traditional Doenjang prepared with mixed soybeans, kidney bean, black soybean, kind of small bean, sword bean, green bean, red bean, at a radio of 1:1:1:1:1(w/w)). Moisture content was 49.66% for TDM and 49.52% for TDM. Protein and salt contents of the TDS sample were higher than those of TDM. The "L" values of the TDB sample increased the aging period was decreased. In contrast, the "L" and "$\alpha$" values of TDM did not show any significant differences. The main organic acids detected were malic acid > lactic acid > tartaric acid in the TDS and TDM samples. Glutamic acid contents were 2.83 g/100 g for the TDS sample and 2.15 g/100 g for the TDM. The levels of K, Mg, and Ca of the TDS sample were higher than those of TDM. No significant difference occurred between TDS and TDM regarding their quality characteristics after 80 days of aging. Further studies needed include sensory evaluation and functionality of soybean paste made from mixed soybean materials.

  • This research was establishment of landscape formation plan to Yusang village region Yeongcheon city. Literature research, field research, surveys carried out for this purpose. In addition, by utilizing design code that appears in the common characteristics of the rural scenic views of the region in improving the quality of life of the local population and preserve the indigenous landscape as well as improve the quality of rural landscapes, management, capital town of the region to form Yeongcheon the center was established in the landscape plan form. The natural environment of the capital region for the landscape formation plan, Humanities and Social Environment, Landscape Resources Status, Social Consciousness, landscape map, landscape design, code identification and basic design and planning through the basic steps to derive design guidelines were applied to the landscape formation plan. Formation landscape plan for the regional launch of the regional landscape improvements, region entrance space and yusang reservoirs place, rest zone in the sacred tree and nurse trees around the plan, and repair the town hall. residents living village road maintenance to improve the environment, and develop a detailed plan, such as the lungs maintenance. The plan is likely to be able to contribute to improving the quality of life of the people and landscapes to improve regional, creating a guide for future business landscape is formed to have a significant advantage as a basis.

Development of Models for Regional Cardiac Surgery Centers

  • Background: This study aimed to develop the models for regional cardiac surgery centers, which take regional characteristics into consideration, as a policy measure that could alleviate the concentration of cardiac surgery in the metropolitan area and enhance the accessibility for patients who reside in the regions. Methods: To develop the models and set standards for the necessary personnel and facilities for the initial management plan, we held workshops, debates, and conference meetings with various experts. Results: After partitioning the plan into two parts (the operational autonomy and the functional comprehensiveness), three models were developed: the 'independent regional cardiac surgery center' model, the 'satellite cardiac surgery center within hospitals' model, and the 'extended cardiac surgery department within hospitals' model. Proposals on personnel and facility management for each of the models were also presented. A regional cardiac surgery center model that could be applied to each treatment area was proposed, which was developed based on the anticipated demand for cardiac surgery. The independent model or the satellite model was proposed for Chungcheong, Jeolla, North Gyeongsang, and South Gyeongsang area, where more than 500 cardiac surgeries are performed annually. The extended model was proposed as most effective for the Gangwon and Jeju area, where more than 200 cardiac surgeries are performed annually. Conclusion: The operation of regional cardiac surgery centers with high caliber professionals and quality resources such as optimal equipment and facility size, should enhance regional healthcare accessibility and the quality of cardiac surgery in South Korea.
