Renal scar formation after urinary tract infection in children

  • Urinary tract infection (UTI) is a common bacterial illness in children. Acute pyelonephritis in children may lead to renal scarring with the risk of later hypertension, preeclampsia during pregnancy, proteinuria, and renal insufficiency. Until now, vesicoureteral reflux (VUR) has been considered the most important risk factor for post-UTI renal scar formation in children. VUR predisposes children with UTI to pyelonephritis, and both are associated with renal scarring. However, reflux nephropathy is not always acquired; rather, it reflects reflux-associated congenital dysplastic kidneys. The viewpoint that chronic kidney disease results from renal maldevelopment-associated VUR has led to questioning the utility of any regimen directed at identifying or treating VUR. Despite the recognition that underlying renal anomalies may be the cause of renal scarring that was previously attributed to infection, the prevention of renal scarring remains the goal of all therapies for childhood UTI. Therefore, children at high risk of renal scar formation after UTI should be treated and investigated until a large clinical study and basic research give us more information.

Interaction of Antitubercular Drug (II) - Drug Interaction of Rifampicin and Isoniazid (항결핵약물의 상호작용 (II) - 리팜피신과 이소니아짓의 약물상호작용)

  • Rifampicin is an indispensable drug along with isoniazid for the control of tuberculosis and is usually prescribed as the combination of rifampicin and isoniazid. This paper is attemtped to investigate the interaction of rifampicin and isoniazid. Isoniazid was administered orally at a dose of 30mg/kg of rabbits pretreated with rifampicin 7.5mg/kg, 15mg/kg, and 30mg/kg, respectively twice daily for 9 days. The results are as follows: The blood level and relative bioavailability of isoniazid were decreased significantly (p<0.05) by rifampicin at a dose of 15mg/kg and 30mg/kg. The renal clearance of total isoniazid and ratio of its metabolites to isoniazid were increased significantly (p<0.05) by rifampicin at a dose of 15mg/kg and 30mg/kg. It seemed to be due to enzyme induction by rifampicin. Elimination rate constant ($\beta$) of isoniazid was increased and half life ($t_{1/2$\beta}$) was decreased by rifampicin pretreatment. Dosage regimen of isoniazid after long term administration of rifampicin should be adjusted carefully.

Interaction of Furosemide and Angiotensin Inhibitor (푸로세미드와 안지오텐신 차단제와 상호작용)

  • This paper was attempted to investigate effect of angiotensin inhibitor (loading dose 25, 50, $100{\mu}g/kg$ and maintenance dose 12.5, 25, $50{\mu}g/kg/hr$) on the pharmacokinetics of furosemide (5 mg/kg i.v) in rabbit. The plasma concentrations of furosemide increased by angiotensin inhibitor and the relative bioavailability of furosemide increased from 118.1% to 193.2% by the inhibitor. The protein binding of furosemide decreased by angiotensin inhibitor in bovine serum albumin ($2.17\;{\times}\;10^{-4}M$) by equilibrium dialysis method. Consequently, dosage regimen of furosemide might be adjusted carefully when furosemide is administered with angiotensin inhibitor.

  • Objectives : The aim of this study was the collection of dietary treatments of atopic dermatitis Methods : We surveyed the western and oriental medical book concernig the atopic dermatitis. Results : The 20~30% of patients with atopic dermatitis have hypersensitivity reaction on the major food antigen. The food restriction to prevent allergy reaction need to take effect early during infancy and childhood. In oriental medicine, a heat-poison(熱毒) caused by food affects a fetus of pregnancy. So the food, the regimen and the harmony of the five tastes is stressed the need. When certain foods are restricted to prevent allergy reaction, infant and children with food related Atopic Dermatitis need extra dietary efforts to maintain optimal nutrition as they are in the growth period.

Drug interaction of Cimetidine and Rifampicin in normal Human subjects (시메티딘과 리팜피신의 상호작용)

  • The intraction between cimetidine and rifampicin was studied pharmacokinetically in normal human subjects. The serum level and the area under the serum concentration curve(AUC) of rifampicin administrated orally were elevated significantly by cimetidine. Volume of distribution, total clearance and elimination rate constant of rifampicin were reduced significanyly by cimetidine. Biological half-life of rifampicin was prolonged significantly by cimetidine. The mechanism of this results is probably related to the inhibition of rifampicin metabolism(deacetyl form) or reduction of blood flow in the liver. It is desirable that dosage regimen of rifampicin shoud be adjusted when combined with cimetidine in clinical pharmacy practice.

Congenital Tuberculosis as a Result of Disseminated Maternal Disease: Case Report

  • Although tuberculosis is highly prevalent worldwide, congenital tuberculosis is one of the least common manifestations of the disease. The diagnosis is usually difficult because of the non-specific clinical presentation and the lack of awareness of maternal disease prior to pregnancy and delivery. We present the case of a preterm neonate with congenital tuberculosis, born to a previously healthy mother who had developed severe disseminated tuberculosis during her pregnancy. Once the diagnosis was confirmed in the mother, the congenital infection was confirmed by isolation of Mycobacterium tuberculosis in gastric aspirates, and positive polymerase chain reaction in a cerebrospinal fluid examination. Treatment for tuberculosis with a four-drug regimen resulted in an adequate clinical response in both the mother and infant.

Stress, Yangsaeng and Subjective Happiness Among Female Undergraduate Nursing Students in the Republic of Korea (여자 간호대학생의 스트레스, 양생(養生)과 주관적 행복감)

  • Purpose: This study investigates the relationship among stress, Yangsaeng and subjective happiness in female undergraduate nursing students in the Republic of Korea. Method: The subjects of this study were 283 female undergraduate nursing students in Korea. Data were collected using a self-reporting questionnaire. Data analysis included descriptive statistics, Pearson's correlation coefficients, independent t-tests, one-way ANOVA, Scheffe test, Stepwise multiple regression and Cronbach's ${\alpha}$. Results: Yangsaeng negatively correlated with stress (r=-.299, p<.001) but positively correlated with subjective happiness (r=.440, p<.001). Stress negatively correlated with subjective happiness (r=-.238, p<.001). Stepwise multiple regression revealed that Yangsaeng and satisfaction with the field of nursing explained 25.4% of subjective happiness. Conclusion: The Yangsaeng oriental health care regimen could lower stress and heighten subjective happiness. Therefore, Yangsaeng could be recommended as a feasible means of promoting health and subjective happiness.

The Relationship between Yangsaeng and Happiness among Nurses in the Small and Medium-sized Hospitals (중소병원 간호사의 양생과 행복에 관한 연구)

  • Purpose: This study was designed to assess the degree of Yangsaeng (oriental health care regimen) and happiness of nurses who work in small and medium-sized of hospitals and to identify correlations of between variables in Korea. Methods: Participants were 131 nurses in hospitals in Seoul and Gyeonggido. Data were analyzed using descriptive statistics, Pearson's correlation coefficients, t-test, ANOVA, Scheffe's test, multiple regression and Cronbach's ${\alpha}$. Results: Mean scores were 2.99 (5 point scale) for Yangsaeng and 4.64 (7 point scale) for happiness. Yangseang positively correlated with happiness (r=.463, p<.001). Job satisfaction was a significantly correlated with Yangsaeng and happiness. Yangsaeng, job satisfaction and age explained 28.0% of happiness. Conclusion: We found Yangsaeng could improve happiness. Therefore, Yangsaeng could be recommended as a way of promoting health and happiness.

A Retrospective Study of the Clinical Outcome of Guided Tissue Regeneration in infrabony defects (비흡수성 차폐막을 이용한 치조골재생술의 임상적 효과)

  • The purpose of this study was to evaluate the extent and predictability of periodontal regeneration with barrier membranes in deep infrabony defects. 25 patients(40% smokers) were included in this study. Fourty-one deep infrabony defects treated with membranes(PPD>6mm) were evaluated 1 year postoperatively following a plaque control regimen. Probing pocket depth(PPD), gingival recession(REC), and probing attachment level(PAL) were evaluated at baseline and postoperative 1 year. Plaque score at baseline was 16.2 and plaque score at 1 year was 9.9 A PAL gain of $4.1{\pm}2.5mm$ along with a PPD reduction of $5.0{\pm}2.3mm$ were observed. A PAL gain of $4.1{\pm}2.5mm$ was observed at the smoking group and a PAL gain of $4.0{\pm}2.5mm$ was observed at the non-smoking sroup. It was concluded that periodontal regeneration with membrane represented the predictable and effective treatment modality in the deep infrabony defects.

Diagnosis and Treatment of Extrapulmonary Tuberculosis

  • Extrapulmonary tuberculosis (EPTB) constitutes about 20% of all cases of tuberculosis (TB) in Korea. Diagnosing EPTB remains challenging because clinical samples obtained from relatively inaccessible sites may be paucibacillary, thus decreasing the sensitivity of diagnostic tests. Whenever practical, every effort should be made to obtain appropriate specimens for both mycobacteriologic and histopathologic examinations. The measurement of biochemical markers in TB-affected serosal fluids (adenosine deaminase or gamma interferon) and molecular biology techniques such as polymerase chain reaction may be useful adjuncts in the diagnosis of EPTB. Although the disease usually responds to standard anti-TB drug therapy, the ideal regimen and duration of treatment have not yet been established. A paradoxical response frequently occurs during anti-TB therapy. It should be distinguished from other causes of clinical deterioration. Surgery is required mainly to obtain valid diagnostic specimens and to manage complications. Because smear microscopy or culture is not available to monitor patients with EPTB, clinical monitoring is the usual way to assess the response to treatment.
