Background This study evaluated the outcomes of a new modified Wies technique for patients with involutional lower eyelid entropion without horizontal eyelid laxity. Methods This case series retrospectively analyzed consecutive patients with entropion who underwent surgery between January 2014 and March 2019 by the same experienced surgeon. Horizontal eyelid laxity, lower eyelid retractor function, and orbicularis muscle overriding were recorded before and after surgery. The recurrence rate and complications were also evaluated. This technique consisted of modified everting sutures combined with reattachment of the lower eyelid retractors to the inferior tarsal plate. Results This new technique was performed on 28 eyes in 25 patients (mean age, 71.0±8.0 years; range, 56-87 years). Nine patients (36%) were women and 16 (64%) were men. Lower lid entropion was present in the right eye in 14 patients (56%), the left eye in eight patients (32%), and both eyes in three patients (12%). The mean follow-up period was 27.3±12.4 months (range, 6-60 months). No intraoperative complications were observed. All patients' symptoms were alleviated. One patient (3.6%) had recurrence after 2 years (success rate, 96.4%). The remaining 27 eyes maintained a satisfactory and comfortable eyelid position. No patients had problems with scarring. Conclusions The approach described herein proved to be safe and feasible in eyes with involutional lower eyelid entropion without horizontal eyelid laxity. These advantages of this procedure include the lack of a conjunctival scar, punctal eversion, and lateral canthal angle deformation. A low recurrence rate and a long interval to recurrence were also observed.
Objectives : The present study was carried out to provide basic information necessary for the prevention and efficient treatment of stroke through a comparison between thr patients of the first attack and those of recurrence. Methods : The observation f3r the current study was made on 210 cases of stroke that were confirmed through brain CT-scan. The patients were hospitalized at one of two oriental medical hospitals in Seoul during 2006. Result : The main results were as follows. First, the male-to-female ratio of stroke patients were 1:1.26, with more primary stroke far females and more recurrent stroke for males. Second, in the age distribution, seventies was the top, and sixties, fifties, and forties were next in the order of frequency. A large city was the most frequent residential site and unemployment was the most frequent occupation to have stroke. The incidence of stroke became higher as patients had a taste for spicy and salty food. Third, the most common preceding disease was hypertension. In the relationship of diastolic blood pressure with recurrence, there was significance in the test of independence. Fourth, the most important precipitating conditions at the onset of stroke were rest and steeping. The most common precedent symptoms were verbal disturbance, numbness, and dizziness. In the stroke patients with hemiparesis, male and female patients usually showed It. hemiparesis. Fifth, as a result of brain CT-scan, cerebral hemorrhage was inclined to reduce but cerebral infarction was inclined to increase due to senility and change of lifestyle. Finally, total cholesterol findings disclosed that 22.4% were hypercholesteremia, 18.6 % were hyperlipemia in triglyceride findings, and 19.0% were glycosemia in glucose findings. Conclusions : The above results suggested avoidance of meat and salty fDod and positive control of hypertension and diabetes mellitus in order to prevent stroke.
Objective: Chronic subdural hematoma (CSDH) is one of the most common types of traumatic intracranial hemorrhage, usually occurring in the older patients, with a good surgical prognosis. Burr hole craniostomy is the most frequently used neurosurgical treatment of CSDH. However, there have been only few studies to assess the role of the number of burr holes in respect to recurrence rates. The aim of this study is to compare the postoperative recurrence rates between one and two burr craniostomy with closed-system drainage for CSDH. Methods: From January 2002 to December 2006, 180 consecutive patients who were treated with burr hole craniostomy with closed-system drainage for the symptomatic CSDH were enrolled. Pre- and post-operative computed tomography (CT) scans and/or magnetic resonance imaging (MRI) were used for radiological evaluation. The number of burr hole was decided by neurosurgeon's preference and was usually made on the maximum width of hematoma. The patients were followed with clinical symptoms or signs and CT scans. All the drainage catheters were maintained below the head level and removed after CT scans showing satisfactory evacuation. All patients were followed-up for at least 1 month after discharge. Results: Out of 180 patients, 51 patients were treated with one burr hole, whereas 129 were treated with two burr holes. The overall postoperative recurrence rate was 5.6% (n = 10/180) in our study. One of 51 patients (2.0%) operated on with one burr hole recurred, whereas 9 of 129 patients (7.0%) evacuated by two burr holes recurred. Although the number of burr hole in this study is not statistically associated with postoperative recurrence rate (p> 0.05), CSDH treated with two burr holes showed somewhat higher recurrence rates. Conclusion: In agreement with previous studies, burr hole craniostomy with closed drainage achieved a good surgical prognosis as a treatment of CSDH in this study. Results of our study indicate that burr hole craniostomy with one burr hole would be sufficient to evacuate CSDH with lower recurrence rate.
Ossifying fibroma is a rare benign tumor of the maxilla and the mandible. And its nosology and diagnosis have had many problems. It occurs more common in young adult, especially in female, and there is a predilection for the mandible. The lesion is generally asymptomatic and recurrence is rare after conservative excision. Among ossifying fibroma, juvenile active ossifying fibroma grows rapidly, produces many symptoms, and frequently recurs after conservative excision. Authers experienced a case of juvenile active ossifying fibroma which recurred after two times of conservative surgery, and report is with review of the literatures.
Objectives: This research was aimed 1) to find out various subjective symptoms cancer patients had, 2) to find out the quantitative difference of symptoms occurring according to the area of cancer, current treatment methods, and relapse of the cancer and 3) to analyze the relationship between the occurrence of symptoms. Methods: This study was a survey about the various subjective symptoms cancer patients had. The subjects were 48 cancer patients in a double-blinded randomized controlled trial about the effectiveness of Korean Red Ginseng (KRG) treatments. Surveys were examined before Korean Red Ginseng (KRG) treatments. The questionnaire consisted of demographic research, patient history and accompanying symptoms. The symptoms part consisted of 4 questions on general symptoms (vertigo, hot flush, rash, insomnia), 2 questions on digestion symptoms (anorexia, dyspepsia), 2 questions on excretions (diarrhea, constipation), and 5 questions on symptoms in different body parts (headache, dryness of lips, chest pain, aphthous ulcer, nose bleeding). The questionnaire was used to research presence of symptoms and relationships between the occurrence of symptoms. Results: A total of 48 patients took part in this survey. Dizziness was the most frequent subjective symptom patients had (64.58%), followed by dry mouth (43.75%), headache (43.75%), insomnia (41.67%), anorexia (37.5%) and chest discomfort (37.5%). The presence of these subjective symptoms was partially related to the patient's history details - type of cancer, method of cancer treatment (chemotherapy, radiotherapy), termination of treatment, presence of pain, fatigue and recurrence, etc. Factor analysis was conducted to analyze the relationship between the occurrence of symptoms. It suggested 5 factors as a result, but there was a limitation that only a low level of correlation was shown among them. Conclusions: We were able to analyze the occurrences and the relationships among them for accompanying subjective symptoms in cancer patients. However the results of the study are limited in that only 48 patients participated. The symptoms in cancer patients showed a tendency for clustering, occurring simultaneously or continuously rather than occurring alone. The study of these symptom clusters is worth further study, for it is similar to the Symptom Differentiation System in Traditional Korean Medicine. For effective approach to the treatment of cancer in Traditional Korean Medicine, further research on the Korean traditional view of cancer patients should be done based on this research, regarding various symptoms and those relationships on a larger scale.
Objectives The purpose of this study is to report the effectiveness of Korean medicine on treating paralytic strabismus in childhood. Methods We treated a 4-years-old patient who was diagnosed with paralytic strabismus with herbal medicine, laser acupuncture, T-acupuncture and moxibustion. Results After 5 months of the Korean medicine treatment, the child's strabismus, diplopia, and limitation of abduction were improved. There was no recurrence of the symptoms for 1 year and 4 months. Conclusions Korean medicine treatment can be effective in improving symptoms of paralytic strabismus. However, further studies are needed to demonstrate the treatment benefits to abducens nerve palsy.
Sleep disturbances are frequently associated with neurological disorders. Sleep disorders interfere with rehabilitation of patients with neurological disorders such as stroke and may increase the severity of their symptoms and recurrence rate of stroke. The treatment of sleep apnea syndrome is particularly important in managing patients with cerebral infarction of whom 50-80% have moderate to severe sleep apnea. Sleep apnea produces not only poor quality sleep but also excessive daytime sleepiness, fatigue and lack of energy. Sleep problems frequently found in patients with dementia are sleep-wake cycle abnormality, fragmentation of sleep, nocturnal insomnia, decreased slow wave sleep and REM sleep, and sleep disordered breathing. The management of sleep disturbances is very important for controlling symptoms such as nocturnal wandering and sundowning syndrome in patients with dementia. Parkinson's disease and epilepsy are other neurological disorders that may have sleep disturbances.
A Reports were done on 3 cases of proriasis, 1 case of chronic urticaria which were admition and treated by fasting therapy in Dept. surgery, opthalmology & otolaryngology, college of Oriental Medicine, Kyungsan University, Pohang, Korea, from March 1999 to October 2000. The results were as follows: 1. In the case chronic urticaria, symptom and sign(jtching, wheal, erythema), except of pigment, were showed conciderable improvement. 2. In the case proriasis, it is different of person, bellow 5cm sized erythematous popules were disappeared during fasting therapy, above 5cm sized erythematous papules and plaque were improved by using medication after fasting therapy. Silver-white colored scales was improved about $50-60\%$, itching was disappeared during fasting therapy. 3. After fasting therapy there was a recurrence, but about $40-50\%$ decreased symptoms were showed. 4. Besides upper diagnosis and treatment based on overall of symptoms and signs considered by documentary investigation, medications by Sasang prescription also can be expected good treatment effect.
Objectives: The purpose of this study is to review the research in the cyclic treatment of endometriosis in traditional Korean medicine. Methods: By October 2019, the relevant studies on the periodic therapy of endometriosis were searched using the Chinese National Knowledge Infrastructure Database (CNKI) and Oriental Medicine Advanced Searching Integrated System (OASIS). Results: Finally, three Randomized controlled trials, one case study, two clinical observational studies were selected. In all studies, herbal medicine periodic therapy has been effective in improving symptoms of endometriosis. Blood-activating and stasis-dispelling medicinal were the most commonly used before menstruation and during menstruation, and tonifying and replenishing medicinal were the most commonly used after menstruation. According to the one study, which reported safety, herbal medicine was safe. Conclusions: Although limited due to a lack of literature, periodic therapy is effective in improving endometriosis related symptoms and preventing recurrence. A well-designed study and experimental studies for the identification of the mechanism are needed.
Drug-induced autoimmune hepatitis (DIAIH) is an increasingly recognized form of drug-induced liver injury that leads to a condition similar to idiopathic autoimmune hepatitis. A number of drugs have been associated with DIAIH, minocycline is one of the most well characterized. Minocycline is a semisynthetic tetracycline antibiotic used in the treatment of acne vulgaris. Minocycline-induced autoimmune hepatitis presents with serologic and histologic features similar to idiopathic autoimmune hepatitis. However, the natural history and outcomes of these two conditions differ significantly. The majority of patients with minocycline-induced autoimmune hepatitis experience complete resolution of symptoms after withdrawal of the medication. Some patients may require a short course of steroids and rarely use of an immunomodulator to achieve resolution of disease. Recurrence of symptoms is rare and typically only occurs with reintroduction of minocycline. It is important for primary care providers to consider minocycline-induced autoimmune hepatitis when liver injury develops during minocycline therapy.
