Breast Reconstruction with Microvascular MS-TRAM and DIEP Flaps

  • The free muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) and deep inferior epigastric perforator (DIEP) flaps involve transferring skin and subcutaneous tissue from the lower abdominal area and have many features that make them well suited for breast reconstruction. The robust blood supply of the free flap reduces the risk of fat necrosis and also enables aggressive shaping of the flap for breast reconstruction to optimize the aesthetic outcome. In addition, the free MS-TRAM flap and DIEP flap require minimal donor-site sacrifice in most cases. With proper patient selection and safe surgical technique, the free MS-TRAM flap and DIEP flap can transfer the lower abdominal skin and subcutaneous tissue to provide an aesthetically pleasing breast reconstruction with minimal donor-site morbidity.

Trunk Stabilization Muscle Activity in Chronic Low Back Pain Patients during One Leg Stance (만성요통환자의 한발서기 시 체간 안정화 근육의 활성화 변화)

  • Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).

The Internal Mammary Vessel as a Recipient Site for Delayed Breast Reconstruction (지연 유방 재건시 수혜부 혈관으로써 내유동정맥)

  • In breast reconstruction with a free flap following mastectomy, the recipient vessels most widely used are in the axillary system, which limits flap movement and flexibility in breast shaping. In addition, scarring and fibrosis can make dissection of the vessels difficult. We have performed 43 breast reconstructions using a free transverse rectus abdominis myocutaneous(TRAM) flap. In 10 cases out of 20 delayed reconstruction, we anastomosed to the internal mammary vessels rather than subscapular system. There has been no flap failure. The surgical techniques, advantages and limitations of the internal mammary system are presented. The internal mammary vessel are compared with the axillary vessels as a recipient vascular system.

A Study for Maximal Force Exertion on Upward Slopes (상승면상의 최대 발휘근력에 대한 연구)

  • Many studies have been done to determine the magnitudes of force a man can exert on the objects. However, very little attention has been paid to those In postures on slopes. This paper was aimed to evalute how human work postures would affect the push/pull force and to grasp the relationships between the rectified EMG(REMG) measured at the trunk muscles and force exerted on upward slopes. Two subjects participated in the experiment. The results showed no lineal relationship between the REMG and exerted force at handle. But as the slope and handle height increased, exerted force and muscular stress on erector spinae or rectus abdominis were generally increased. It was notified that since ANOVA did not detect any statistical significance in REMG variation due to dominant muscles, careful application and interpretation of the REMG should be required in analyzing maximal force exertions.

Comparison of hamstring muscles activity between subjects with normal and shortened hamstring muscle during plank exercise

  • Background: Plank exercise (PE) is an effective exercise to enhance lower back stability by strengthening the core and lower limb muscles. However, in patients with a shortened hamstring muscle (HAM), PE may cause abnormal movement of the pelvis and lower back due to HAM hyperactivity. Therefore, the objective of this study was to investigate the effects of PE on the core muscles and HAM in subjects with a shortened HAM. Design: Cross-sectional study. Methods: Subjects were divided into a normal length of HAM group (NHG; 9 subjects) and a shortened length of HAM group (SHG; 14 subjects). The activities of the erector spinae (ES), rectus abdominis (RA), external oblique (EO), and HAM muscles were measured using surface electromyography. Results: The results showed that RA, EO, and ES muscle activities were higher in the NHG than in the SHG; however, no significant differences were detected. Conclusion: HAM activity was significantly higher in the SHG than in the NHG. In subjects with a shortened HAM, PE may hyperactivate the HAM, adversely affecting the pelvis and lower back.

Exercise-induced traumatic muscle injuries with active bleeding successfully treated by embolization: three case reports

  • Muscle injuries caused by indirect trauma during exercise are common. Most of these injuries can be managed conservatively; however, further treatment is required in extreme cases. Although transcatheter arterial embolization is a possible treatment modality, its role in traumatic muscle injuries remains unclear. In this case series, we present three cases of exercise-induced muscle hemorrhage treated by transcatheter arterial embolization with successful outcomes. The damaged muscles were the rectus abdominis, adductor longus, and iliopsoas, and the vascular injuries were accessed via the femoral artery during the procedures.

Phylogenetic Characteristics of Fasciola hepatica Isolated from a Korean Patient

  • Fascioliasis is a parasitic infection caused by liver flukes. Although several cases have been reported in Korea, phylogenetic analysis of isolates is lacking. In this study, a 66-year-old woman with right upper quadrant (RUQ) abdominal pain was diagnosed as fascioliasis involving abdominal muscle by imaging study. She received praziquantel treatment, but symptoms were not improved. Lateral movement of the abscess lesion was followed. Trematode parasite was surgically removed from the patient's rectus abdominis muscle. The fluke was identified as Fasciola hepatica based on sequence analysis of 18S rDNA. To determine the phylogenetic position of this Fasciola strain (named Korean Fasciola 1; KF1), the cox1 gene (273 bp) was analyzed and compared with the genes of 17 F. hepatica strains isolated from cows, sheep, goats, and humans from various countries. Phylogenetic analysis showed that KF1 was closely related with the isolates from China goat.

Effects of shoulder rotation according to stance posture and plane of motion on EMG response of shoulder rotator cuff and Trunk muscles. (스탠스 자세와 운동면의 차이에 따른 위팔어깨관절의 돌림운동이 어깨돌림근군과 몸통근군의 근전도 반응에 미치는 영향)

  • The purpose of this study is to provide the basic data for the shoulder strengthening exercise by analyzing the% MVIC of the muscle activity in the shoulder rotator cuff by the difference of the stance posture and the anatomical plane. 8male subjects were randomly assigned to perform the shoulder rotation exercise 10 times on the frontal plane, the horizontal plane, the sagittal plane and the two legs stance posture, the one leg stance posture, the lunge posture. Measured muscle activity of supraspinatus, infraspinatus, teres minor, anterior deltoid, rectus abdominis, erector supinea, pectoralis major, lattisimus dorsi during exercise. A repetitive one-way ANOVA was performed using the SPSS 22.0 statistical program. First, during the external rotation on the frontal plane, the erector spinea was higher in the lunge posture than in the two legs stance posture and the one leg stance posture, And during the internal rotation on the frontal plane, the muscle activity of suprapinatus was higher in one leg stance posture than in the two legs stance posture and more so in the lunge posture. Second, during the external rotation on the horizontal plane, the muscle activity of deltoid anterior was higher in the one legs stance posture and in the lunge posture than in the two legs stance posture, and during the internal rotation on the horizontal plane, the muscle activity of infraspinatus was higher in the lunge posture than in the two legs stance posture and one leg posture, and the muscle activity of pectoralis major was higher in two leg stance posture than in the one legs stance posture and more so in the lunge posture. Third, during the external rotation on the sagittal plane, muscle activity of rectus abdominis was higher one leg stance posture in the lunge posture than in two leg stance posture. During the internal rotation on the sagittal plane, muscle activity of supraspinatus was higher one leg stance posture in the lunge posture than in two leg stance posture. And muscle activity of infraspinatus was higher in the lunge posture than in two leg stance posture, one leg stance. And muscle activity of Rectus abdominis was higher in the lunge posture and one leg stance posture than in the two legs stance posture. And muscle activity of Erector spinea was higher in the two legs stance postur and lunge posture than in the one leg stance posture. In conclusion, the differences in stance and shoulder anatomy have different effects on the muscle activity of the shoulder rotator exercises, and this is expected to be a more positive exercise program when applied to the shoulder strengthening exercise program.

The Effect of Microcurrent Application on Muscle Fatigue of Pes Planus during Gait (미세전류 적용이 편평족을 가진 사람들의 보행근육 피로도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of microcurrent on fatigue of muscles in people who were flat-footed during gait. Methods: 10 flat-footed university students volunteered to participate in this study. 10 flat-footed subjects were divided into 2 groups, one group was experimental group of 5subjects(This group put on microcurrent induction shoes but the subjects were not able to feel the current.) and the other group was the control group of 5subjects(This group put on the general shoes which were similar in shape but microcurrent was not induced.) to perform double blind test and random sampling. Their gait muscle fatigue of 6 regions (vastus medialis, gastrocnemius, tibialis anterior, biceps femoris, erector spinae, and rectus abdominis muscle.) was measured by EMG MP150, Delsys Inc Boston, USA during walking and then they carried out the Harvard step with a platform (It was a arbitrarily made wooden platform of 100cm long, 50cm wide, 60cm high. They carried out climbing it for one second and descending it for one second by using the Metronome program, total 5minutes) for 5minutes. Right after that, the subjects walked on a treadmill at a speed of 4km/h for 10minutes and then their gait muscle fatigue of 6regions was assessed while they were walking on the ground as equally as before exercise. Results: The experimental group has resulted in lower average differences in gait muscle fatigue before and after exercise than those of the control group average 12.24Hz(P=0.009) at vastus medialis, average 8.52Hz(P=0.016) at gastrocnemius, average 9.16Hz(P=0.009) at tibialis anterior, average 8.66Hz(P=0.047) at biceps femoris, average 7.53Hz(P=0.016) at erector spinae, and average 7.80Hz(P=0.047) at rectus abdominis. All of the assessments of muscles have shown significant difference statistically. Conclusions: This result has shown that the use of micro current could decrease gait muscle fatigue of flat-footed people. It is recommended to use a microcurrent to reduce their gait muscle fatigue.

Effect of Different Supporting Surfaces on Trunk Muscle Activities during Core Stabilization Exercises (지지면 차이에 따른 안정화 운동 시 몸통의 근활성도에 미치는 효과)

  • Purpose : The purpose of this study was to compare the effects of different supporting surfaces on trunk muscle activities during core stabilization exercises. Methods : A total of 20 healthy college students participated in this study for 6 weeks. In this study, we divided participants into a stable surface exercise group and an unstable surface exercise group. Each group performed core stabilization exercises of Curl-ups, Bridges, and Quadrupedal position. Core stabilization exercises were performed 3 times a week for 30 minutes during 6 weeks. The stable surface exercise group used a yoga mat while the unstable surface exercise group used AERO step. This study was designed using pre-test and post-test measurements. We used Surface Electromyograpy (sEMG) to measure for the rectus abdominis, external abdominal oblique, and multifidus muscle of trunk muscle activities. Data was processed using a paired sample t-test on SPSS 18.0. Results : For the stable surface exercise group there was a meaningful improvement in left rectus abdominis, left external abdominal oblique, and right multifidus (p<0.05). For the unstable surface exercise group, a meaningful improvement was seen in the left external abdominal oblique and right multifidus. Conclusion : From the experiment, we concluded that differences in surface can make various degrees of improvement in muscles activities, which suggests patients can choose a better option under their own conditions when planning to have a core stability exercise.
