A Study on The Nursing Needs of Users and their Service Status at Short-Stay Centers for The Elderly (단기노인보호소 이용자의 간호요구도 및 서비스 실태연구)

  • Aging population is a global trend and Korea is no exception. Due to the progress of the medical and scientific sectors, increase in the average lifetime incurred increase in the number of elderly people. Meanwhile, the family function of supporting the elderly has become a major issue in a society due to the changes in the society's structure and increased entry of the women into the workforce. As a means to substitute the protection of the elderly who are not protected at their respective homes and to replace the tending needs, weekly protection centers and short-stay centers for the elderly were established and operated in Seoul since 1992. However, structured and satisfactory services are not yet to be provided, thus this research intends to understand the current situation at the short-stay centers for the elderly, to identify users' characteristics and to conduct study on nursing need for the health reasons so that the results of the research can be utilized as basic data for the development of nursing care program. Towards this end, this research targeted 142 elderly people who were undergoing treatment at the 11 short-stay centers for the elderly out of total of 13 located in Seoul that were willing to cooperate. The research was conducted from April to May 2002, with the measurement tool developed by Rhee, Seon-ja (2001) to measure the level of nursing need. Then data was collected and analyzed using SPSS program. The results are as follows. The general characteristics of users of the short-stay centers for the elderly were: 123 females $(86.6\%)$, 19 males $(13.4\%)$ and 72 elderly were over the age of 80 $(50.7\%)$. Among these, 24$(16.9\%)$ were living by themselves. Among the illnesses that the subjects were inflicted with were: dementia $(54.2\%)$, stroke $(54.1\%)$ and high blood pressure $(50.7\%)$, in the respective order. Despite the fact that they were diagnosed with illnesses, $47.2\%$ responded that they do not undergo special treatment due to financial reasons. When the level of needs for nursing care among the elderly who use the short-stay centers is divided into informational. physical. and emotional need levels and scored between zero to two, the level of emotional need is the highest with 1.80, followed by physical (1.45) and informational (1.44) need levels. As for the realities of services at the centers and improvement areas, restriction on term was cited as the greatest inconvenience with $38.7\%$, followed by lack of budget (24 respondents, $16.9\%$). Wishful services were financial assistance $(46.5\%)$, medical and nursing services $(40.1\%)$, meal provision service $(8.5\%)$ and counseling service $(4.9\%)$, in the respective order. Based on the above mentioned results, the research identified the need to develop nursing program for the patients of dementia that can be applied onto the short-stay centers and the need to develop volunteer service, backed by societal support. Also, placement of specialized cadre of professionals is needed so that medical and nursing service functions can be provided in a complementary manner.

Movable Anchorage System for Mitigation of Cable Vibration in Cable-Stayed Bridges with Sag (Sag가 고려된 사장교 케이블의 진동저감을 위한 Movable Anchorage 시스템)

  • Rain-wind induced cable vibration can cause the damages in the cable-stayed bridge due to very little inherent damping characteristics and low fundamental frequency. External Dampers attached to stay cables near anchorages have been shown to be effective means at short stay-cables. However, installation locations of external dampers are limited to a particular range due to aesthetic and practical reasons for very long stay-cables. A recent study by the authors showed that the stay-cable vibration system can perform better than the optimal passive viscous damper, thereby demonstrating its applicability in large cable-stayed bridges. This paper extends the previous study on the taut string representation of the cable by adding cable sag and inclination. The response of the proposed system compared to those of the cable with and without an external damper, and the movable anchorage system provides very effective mitigation of cable vibration. Cable damping ratio is seen to be remarkably reduced by movable anchorage system for a wide range of cable sag. This result shows that the sag effects of the proposed system should be considered.

Appropriateness Evaluation of Preoperative Hospitalization: A Case Study of A Tertiary Care Hospital (수술전 재원기간의 적절성 평가)

  • Background : The purpose of this research was to evaluate the appropriateness of preoperative hospital days in a tertiary care hospital and to examine the reasons of the inappropriateness, so as to provide basic information and policy for enhancing appropriateness of preoperative hospitalization and benefit of patients and hospital. Methods : The subjects of the research were the 344 patients who received operation among discharged patients during January, 1996 in surgical departments including general surgery, neurosurgery, orthopedic surgery, plastic surgery and ophthalmology. Their medical records were reviewed and appropriateness of hospital days was evaluated by the Appropriateness Evaluation Protocol. Result : The results of evaluating the appropriateness of preoperative hospitalization showed that inappropriate hospital days were 80.8%. The reasons of inappropriate hospital stays were the tests or preparation which could be done in outpatient basis' followed by 'possible tests or preparation on the operation day' and 'cancelation of operation'. Conclusion : In order to shorten the inappropriate preoperative length of stay, it is recommended that lengthening of laboratory running time and doing most of tests necessary for operations on the outpatient basis prior to admission should be considered. In addition, the operation at the same day of hospitalization and usage of day surgery should be encouraged. Finally there should be changes in the inpatient management system and attitudes and behaviors of surgeons to shorten unnecessary preoperative and maximize the benefit for patients and hospital.

The Relationship between the Quality of Life, Care Services, and Family Support of Elderly Residents in Long-Term Care Facilities (노인요양 시설 노인의 삶의 질과 시설 서비스, 가족 지지의 관계)

  • The aim of this study is to identify the influence of care services on the quality of life of elderly residents in long-term care facilities, and to determine the mediating effect of family support. The subjects of the study were 192 older adults living in long-term care facilities in the Daegu and Gyeongsangbuk-do region. The collected data were analyzed using the SPSS 22.0 program, and the methods used include descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and regression analysis. The results of the study found that the subjects' quality of life differed depending on their age, education, income level of their children, reasons for admission, and length of stay. The factors that influenced their quality life included age, education, the income level of their children, reasons for admission, length of stay, and care services. The study also revealed that family support was the mediating effect in the relationship between care services and quality of life. The results of the study will provide insight in establishing a nursing intervention plan to improve the quality of life of elderly residents in care facilities.

Appropriateness Evaluation of Hospitalization for the Cerebral Ischemia Patients (허혈성 뇌졸중 환자의 재원적절성 평가)

  • The purpose of this study was to survey appropriateness of admission and days of care for the cerebral ischemia patients as a basis to provide an effectiveness of hospitalization. The authors retrospectively reviewed the medical records of cerebral ischemia patients in two hospitals from November 1997 to February 1998. Of 194 medical records reviewed, there were 2108 medical days. I t is used a 'Appropriateness Evaluation Protocol' previously developed by Gertman and Restuccia (1981) and translated by Department of Health Management, Seoul National University and Korea Institute for Health Services Management (1993), It was found that the 'Appropriateness Evaluation Protocol' had a high inter-rater reliability(k=.92), Statistical significant was tested by using the percentage, mean, and logistic regression by SAS 6.12. The results were as follows; 1. The appropriate admissions were 87.6%, days of care 63.4%, and the average length of stay $10.9{\pm}6.7$ days. 2. The reasons of inappropriate admissions were for work-up(75.0%) and conservative care (25.0%). Major reasons of inappropriate days of care were 'cases in which the medical purpose of hospitalization has been accomplish or can be addressed in a less setting(45.0%)', and 'cases in which there is a delay in performing the work-up or treatment which required patients is hospitalized (44.4%)'. 3. Appropriate days of care were higher as ageing. Appropriate days of care were higher in patients with lower accademic back ground than those of upper college graduates, and in the patients who enter a hospital via emergency room than out-patients department. Appropriate days of care were higher in the patient with MCA infarction, and lower in the patient with cerebellar infarction than the patient with lacunar infarction. Appropriate days of care were higher in attack first than attack above second, in nomortension patients than hypertensive, and lower in groups who engaged in semi-private room and public room than private room in hospital. Appropriate days of care were higher in shorter length of stay than longer length of stay. 4. Diagnosis, admission path, and appropriate days of care explained appropriate admissions. Diagnosis, appropriate admissions, hypertension explained appropriate days of care. According to the above results, author confirms the substantial amount of inappropriate hospital bed utilization. To reduce inappropriateness, it is necessary to develop some alternative services such as home care services or nursing home with which can be replaced inpatient services and to introduce policy such as case management which includes Critical Pathway for consistent management. And, it should be followed the further study for the effectiveness.

  • In this study, we developed the severity-adjusted length of stay (LOS) model for acute myocardial infarction patients using data from the hospital discharge survey and proposed management of medical quality and development of policy. The dataset was taken from 2,309 database of the hospital discharge survey from 2004 to 2006. The severity-adjusted LOS model for the acute myocardial infarction (AMI) patients was developed by data mining analysis. From decision making tree model, the main reasons for LOS of AMI patients were CABG and comorbidity. The difference between severity-adjusted LOS from the ensemble model and real LOS was compared and it was confirmed that insurance type and location of hospital were statistically associated with LOS. And to conclude, hospitals should develop the severity-adjusted LOS model for frequent diseases to manage LOS variations efficiently and apply it into the medical information system.

  • To resolve global environmental problems, many automobile companies are making a great deal of effort to develop so-called green-cars. One of the solutions is an electric vehicle equipped with the PEMFC (Proton Exchange Membrane Fuel Cell) stack. However, the cost of the stack, at the moment, is still too high. This obstacle must be resolved for commercialized fuel cell cars. One of the reasons for high cost is a relatively large amount of Pt (platinum) in the membrane. In this paper, a method for reducing the Pt-loading is introduced. Furthermore, the durability of the stack will be important for a company to stay competitive in world markets (in the future). For this, the diagnosis of the stack must be conducted on-line. Some diagnosis methods as key technology are also introduced in this paper.

  • The scale of wind turbines has continuously increased over the last decade. Especially, the rapid growth of the rotor diameter has brought about the increase of the tower height and the load on the rotor blade, as can be seen in the case of a 5MW class wind turbine with 126m rotor diameter. This trend means the increasing possibility of system failure. In addition to that, it is impossible for human operators to stay and manage all the turbines in the case of a large-scale wind farm. For these reasons, the operation and maintenance technology is getting more importance. In this paper, we present an unmanned remote monitoring system for MW class wind turbines and its application to YeungHeung wind test bed.

  • Dispute plays a key role in maintaining the desirable performance of trade transaction. In an effort to stay competitive in a global marketplace, the Korean companies need to become more aware of alternatives to costly and time-consuming litigation. Korean companies, therefore, should be more concerned with ADR(Alternative Dispute Resolution) system and should utilize ADR to settle their disputes effectively and efficiently. ADR encompasses all process of dispute resolution as a substitute for the traditional litigation. Generally, three kinds of ADR are available in Korea: Negotiation, mediation, and arbitration. This article investigates reasons why ADR isn't used well in Korea and suggests ways how ADR can work best in international trade disputes. To expand ADR system in international trade disputes, it is very important for both the company and the scholar to recognize the concept and usefulness of ADR system. The Korean Commercial Arbitration Board also must help both Korean companies and scholars recognize the mechanism of dispute resolution and utilize ADR system in international trade disputes.

Practices of Nutritional Ergogenic Aids Usage by Elite Bodybuilders and Weight Lifters (엘리트 보디빌더와 역도선수의 영양 보조물(nutritional ergogenic aids) 섭취 실제)

  • The objective of the present study was to determine the prevalence and type of nutritional ergogenic aids use, and to determine the frequency, reasons for use of nutritional ergogenic aids. Thirty-four male bodybuilders (mean age = 27.0 years), twenty-four male weight lifters (mean age = 20.9 years) participated in the study. Participants completed a comprehensive survey detailing their usage patterns. In this study, 78.1% of bodybuilders and 79.2% of weight lifters reported using nutritional ergogenic aids. The most frequently taken nutritional ergogenic aids, in ranking order, were protein/amino acid powders (79.4%), multivitamin/minerals (67.7%) and creatine (67.6%) for bodybuilders, in contrast to sports drinks (100.0%), protein/amino acid powders (50.5%) and creatine (50.5%) for weight lifters. Over the half of the respondents, 79.4% of bodybuilders and 50.6% of weight lifters, used protein/amino acid powders to gain muscle mass and to stay healthy. Bodybuilders, 67.6% and weight lifters, 41.7%, used multivitamin/minerals to stay healthy and for energy. The intakes of most vitamin and minerals through diet and nutritional ergogenic aids were much greater than RDA. Vitamin $B_1$, vitamin $B_2$, niacin, vitamin $B_6$ and folate intakes were ranged at 400-900%. Vitamin C intake was 1285.4% (for bodybuilders) and 1322.6% (for weight lifters). The correct answer rate of nutritional ergogenic aids was 46.0% for bodybuilders and 52.0% for weight lifters. Both bodybuilders and weight lifters took highly nutritional ergogenic aids and it tended to be taken irrespective of scientific background. Specific sport nutrition education applicable to athletes, especially strength athletes, is recommended. The findings of this investigation could be used to enable the professionals (sports dietician and physician) to identity common misconceptions regarding nutritional ergogenic aids and to implement educational programs.
