Purpose: The purpose of this study was to assess the effects of virtual reality-based continuous slow exercise on muscle strength and dynamic balance capacity, in older adults over 65 years of age. Methods: Twenty-six volunteers were randomly divided into two groups; a Virtual Reality (VR) exercise-group ($67.8{\pm}4.1$ yrs) and a Control group ($65.5{\pm}5.2$ yrs). The VR group participated in eight weeks of virtual reality exercise, utilizing modified Tai-Chi provided by a motion capture system, and the Control group had no intervention. The hip muscle strength and dynamic balance of the members of both the VR group and the Control group were measured at pre- and post-intervention, using a multimodal dynamometer, and backward stepping test, respectively. Results: 1. After the 8-week VR-based exercise, the VR group showed significant improvement of hip strength, compared to the control group: hip extension (p=0.00), flexion (p=0.00), abduction (p=0.00), and adduction (p=0.00). 2. After the 8-week VR-based exercise, the VR group showed significant improvement of dynamic balance capacity as ground reaction force, compared to the control group. Eyes opened backward stepping test: Fx (+) (p=0.00), Fy (-) (p=0.02), Ver (+) (p=0.02) direction. Eyes closed backward stepping test: Fx (+) (p=0.04), Fy (-) (p=0.04), Ver (+) (p=0.03) direction. Conclusion: The VR group showed improvement of their hip muscle strength, and dynamic balance capacity. Therefore VR-based continuous slow exercise would contribute to reducing the risk of falls in the elderly.

Purpose: The objective of this study was a virtual reality-based balance training program effective for improvement in physical function, examined the balance ability and fall efficacy of elderly women with experienced falls, intending to examine the program's usefulness as an exercise program to prevent the recurrence of a fall. Methods: The participants were 30 elderly women aged 65 or older who met the conditions. The participants were randomly assigned to either a balance training group (BT) or a virtual reality-based balance training group (VT) and received the training three times per week, 30 minutes per day, for six weeks. To measure static balance, the Tetrax Portable Multiple System (Tetrax Ltd, Israel) were used. To measure dynamic balance, the Berg Balance Scale (BBS) and functional reach test (FRT) was used, and regarding fall efficacy, the Korean Fall Efficacy Scale (K-FES) was used. Results: Tetrax significantly improved after the intervention in both the BT group and the VT group (p<0.05). The comparison between the two groups was not significantly. BBS and FRT result significantly improved after the intervention in both the BT and VT groups (p<0.001), while K-FES was significantly ameliorated in the VT group only (p<.05). Comparing the groups, there were more significant changes in the BBS (p<0.05) and FRT (p<0.01) result of the VT group than the BT group. Conclusion: A virtual reality-based balance training program is considered to be usable as an exercise program to prevent recurrence of falls in elderly women.
본 연구는 완전 몰입형 가상현실 게임을 통하여 완전 몰입형 가상현실 동작과 실제 동작에서의 근수축 개시시간을 비교 하여 보고자 한다. 정상성인 30명을 대상으로 가상현실 동작(펀치, 활쏘기)과 실제 동작(펀치, 활쏘기)에서의 근수축 개시시간을 비교하여 보았다. 본 연구 결과 가상현실 동작과 실제 동작에서의 근수축 개시시간에는 유의한 차이가 없었다(p>0.05). 본 연구를 통하여 물리치료와 완전 몰입형 가상현실을 융합하여 새로운 완전 몰입형 가상현실 콘텐츠를 개발 하여 물리치료에 적용 할 수 있을 것으로 생각되며, 다양한 치료 방법이 개발 될 것으로 보인다. 또한 추후 연구에서 뇌졸중 환자를 대상으로 완전 몰입형 가상현실을 적용하여 치료에 적용 할 수 있을지 알아볼 필요가 있다.
가상현실(Virtual Reality) 기술은 우리 시대에 인간의 희로애락(喜怒哀樂)을 담아내는 진보적 수단으로 자리 잡고 있다. 최근에 인지심리학 분야에서 치료도구로서 사용하기 위한 다양한 시도들이 일어나고 있다. 오감을 통한 가상세계속의 실재감은 현실세계의 인지(Cognition) 의 의미를 재해석해야 할 만큼 우리에게 폭넓게 다가오고 있다. 이러한 패러다임의 변화를 바탕으로 가상현실 기술들을 활용한 새로운 치료방법들이 가능해지고 있는데 그 대표적인 경우가 공황장애를 극복하기위한 행동치료 분야라고 할 수 있다. 특히 가상공간 내에서의 유연한 상호작용(Interaction) 기술의 발전은 환자들이 접할 수 있는 물리적 체험환경을 심리체험의 장으로 유도해 낼 수 있다는 장점을 가지고 있다. 상호작용 기술은 사용자의 물리적 오감을 보다 적극적으로 자극할 수 있는 환경을 제공하며, 가상세계로 부터의 체험정보는 인간으로 하여금 잠재된 에너지들을 새롭게 환기(喚起) 시켜 현실적 경험들로 체득시키기에 매우 유용한 것이다. 가상현실치료의 장점은 장애 환자들의 증상에 따른 맞춤형 치료가 가능하고 치료단계별로 차별화시켜 적용할 수도 있다는 것이다. 각각의 증상과 단계별로 이루어지는 치료과정을 거치면서 환자들로 하여금 현실세계 환경과 상황에 점차 익숙해지게 하여 장애를 치료하는 것이다. 감성공학을 기반으로 한 몰입형 가상현실장비와 반작용 되먹임(force-relative feedback)기술과 입체음향, 그리고 후각을 자극하는 기술들은 인간의 오감을 자극하여 경험을 환기(喚起)시키는데 유용하다. 가상세계에 대한 몰입 감이 주는 장점으로는 도전, 상호작용, 사실성, 환상, 협동성과 같은 현상들이 확대 된다는 점일 것이다. 이러한 현실보강(augmented reality)기능을 확장시켜 치료분야에 적용하고 있는 것으로 최근에는 인터넷을 통한 가상공간과 데이터의 공유기술, 그리고 저렴한 가용성이 그 저변을 확대시키고 있다. 인지적 경험을 위한 능동적인 상호작용 환경을 제공하는 가상현실 치료기술은 두려움 때문에 실제 상황에 직면할 수 없는 환자들에게 적절하게 조절된 환경을 제시해 줄 수 있으며, 비밀이 보장되고, 안전하며, 경제적이라는 이점이 있다. 또한, 실시간(Realtime) 내비게이션을 하는 것을 원칙으로 하기 때문에 빠른 수정보완이 가능하며 안경 없이 입체영상을 볼 수 있는 렌티큘라(lenticular)기술의 보급으로 보다 사실적인 실재감(Presence) 구현이 가능해져서 사이버 멀미현상을 줄일 수 있게 되었다. 그리고 대화식 기술의 발전으로 가상공간 속에서 직접 자신의 손을 이용해서 내비게이션을 하거나 물체를 조종함으로서 실세계와 유사한 실재감에 근접해 가고 있다. 본 논문은 제한적이기는 하지만 장애치료에 활용하고 있는 감성공학기반 가상현실 기술의 특성과 적용범위와 문제점들을 살펴보고 미래의 가능성을 조망해 보고자 한다.

Background: The purpose of this study is to investigate the effects of virtual reality on gait, balance, and upper extremity functions compared to other independent variables or no variables. Additionally, the possibility of virtual reality for stroke patients was discussed. Design: Meta-analysis. Methods: The search for this study was a search term that combined stroke, virtual reality, and training, and the electronic search was conducted through EMBASE, MEDLINE, and Cochrane Library. As a result of the search, 21 studies satisfying the selection criteria of the target study were confirmed as the final analysis target. This study consisted of 21 randomized experimental studies and 21 randomized controlled trials, and the total number of participants was 642. [Experimental group (n=314), control group (n=328); total 642]. As a result of the study, upper extremity function was assessed using a box and block test, a modified Ashworth scale, and a scale including range of motion. The balance was evaluated by the berg balance scale. Gait was a Timed Up and Go test (TUG), stride length, and gait function. Scales including a walking rate scale were evaluated. The effect size for the intervention of the analytical study was meta-analyzed with the RevMan 5.3.3 program of the Cochrane library. Results: The results of the study showed that the function of walking was statistically significant. Balance showed statistically significant results. The upper extremity function showed no statistically significant results. Conclusion: Through this rehabilitation treatment by applying virtual reality environment to the rehabilitation of stroke patients in the future can be proposed as an effective intervention method for the balance and gait function of stroke patients.
Purpose: The purpose of this study was to investigate the effects of virtual reality based treadmill training on muscle architecture of gastrocnemius in chronic stroke patients. Methods: Thirty chronic stroke patients were randomly assigned to either the virtual reality based treadmill training (VRTT) group (n=15) or treadmill training (TT) group (n=15). Both groups participated in a standard rehabilitation program; in addition, the VRTT group participated in virtual reality based treadmill training for 30 minutes per day, three times per week, for 6 weeks, and TT group participated in treadmill walking training for 30 minutes per day, three times per week, for 6 weeks. Ultrasound image was used for measurement of pennation angle and muscle thickness of the medial gastrocnemius muscle at rest and during maximum voluntary contraction. Results: In the paretic side medial gastrocnemius muscle, greater improvement on the pennation angle and muscle thickness while resting and maximal voluntary contraction were observed in the VRTT group compared with the TT group. Conclusion: Findings of this study demonstrated that the virtual reality based treadmill training has an effect on muscle architecture of medial gastrocnemius in chronic stroke patients.
본 연구는 가상현실 프로그램을 사용한 재활치료의 효과성에 대해 체계적으로 분석하여 국내 임상현장에서 재활치료에 종사하는 의료인, 의료기사 및 연구자들에게 다양한 가상현실 기반의 재활치료와 효과성에 대한 정보를 제공하여 가상현실 기반의 재활치료가 임상에서 적절하게 적용될 수 있도록 하고자 한다. 연구 방법으로 학술연구정보서비스(Research Information Sharing Service; RISS), Google Scholar, Pubmed 데이터베이스를 사용하여 2007년 1월부터 2017년 12월까지의 논문을 검색하였다. 주요 검색어는 'Virtual Reality AND Rehabilitation', 'Virtual reality AND Effect'를 사용하였다. 최종적으로 10편의 해외 학술지에 게재된 논문을 본 연구의 분석 대상으로 선정하였다. 선정된 논문은 PICO (Patient, Intervention, Comparison, Outcome) 형식을 사용하여 정리하였다. 연구 결과 10개의 논문에서 참여한 대상자를 유형별로 분석한 결과, 대상자는 총 4가지로 분류되었으며 그 종류는 뇌졸중, 뇌종양, 파킨슨병, 신장질환이었다. 중재에 사용한 가상현실 기반 도구들은 IREX 3건(30%), X-box Kinect 2건(20%), Nintendo Wii 2건(20%), [Existing tool + VR]이 1건(10%), [Mobile application + VR]이 1건(10%)이었다. 중재 결과 10개의 논문 모두 기능향상이 나타났으며 신체기능이 24회(66%), 정신기능 6회(17%), 인지기능 5회(14%), 일상생활활동이 1회(3%)였다. 따라서 본 연구는 가상현실 기반 재활치료와 그 효과를 정리하여 재활치료서비스의 전문성과 질을 향상시키고, 영역을 확대하는 기초자료로 활용될 수 있을 것이다.
목적 : 본 연구는 가상현실 프로그램을 통해 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향을 알아보고자 하였다. 연구방법 : 뇌졸중 진단을 받은 환자를 대상으로 총 4주 동안 가상현실 프로그램 중재를 받는 실험군 10명과 작업치료 중재를 받는 대조군 10명으로 나누어 실시하였다. 치료 중재 전 후 낙상 효능감을 비교하기 위하여 낙상 효능감 척도(Fall Efficacy Scale: FES)를 시행하였고, 일상생활활동 수행능력을 비교하기 위하여 기능적 독립척도(Functional Independence Measure: FIM)를 시행하였다. 결과 : 실험군과 대조군의 치료 중재 전 후 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력은 통계학적으로 유의한 차이를 보였고(p<.05), 실험군이 대조군에 비해 통계학적으로 유의하게 낙상 효능감과 일상생활활동 수행능력이 향상되었다(p<.05). 결론 : 작업치료와 더불어 가상현실 프로그램을 적용할 때 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 향상시킨다는 것을 알 수 있었다. 따라서 가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 위한 치료방법으로 작업치료와 더불어 임상에서 유용하게 적용될 수 있을 것이다.
20세기 초부터 대안적 심리치료의 관심과 발전과 더불어 예술치료의 영역도 발전, 확장되어 가고 있다. 특히 근래 들어 기술의 발전과 코로나 사태(Covid 19 Pandemic)가 불러온 언택트(Untact) 시대는 디지털 예술치료 콘텐츠에 대한 개발을 가속화 하고 있다. 그 중 가상현실(Virtual Reality)에 대한 높은 관심은 전통적 예술치료를 넘어서는 새로운 매체의 치료적 효과성에 대한 기대와 의문을 동시에 불러오고 있다. 이 연구에서는 통합 미술치료이론인 ETC(Expressive Therapies Continumm)의 개념 틀과 치료적 구성요소에 기반한 체계적 문헌분석을 통해 가상현실 미술치료(VRAT:Virtual Reality Art Therapy) 콘텐츠의 치료적 요인의 특성을 알아보고자 하였다. 연구결과로는 VRAT 콘텐츠는 사용자의 동적, 지각적, 인지적 요인을 촉발하는 좌반구 대상의 치료적 요인이 주로 나타났고 감각적, 정서적, 상징적 요소에 치중된 우반구 대상의 치료적 요인은 상대적으로 드물게 나타났다. 그 이유로는 VRAT 매체의 특성 및 기술적 한계 외에도 실험적 단계의 특성상 치료사의 적극적 개입과 장기적 세션 구성의 부재, 사용자들의 VRAT 에 대한 두려움과 서툼, 낯섦 등에 기인한 것으로 보인다. 연구의 한계로는 적은 수의 분석 대상 문헌과 미술치료라고 하기엔 미비한 현 VRAT 의 형식이며 본 연구의 결과로서 도출된 VRAT 콘텐츠 매체와 환경에 기인한 치료 요인의 특징과 가능성이 향후 VRAT 콘텐츠의 적절한 개발에 유용하게 쓰이길 기대한다.
Objective: The objective of this study is to investigate the effect of treadmill training with real optic flow scene on functional recovery of balance and balance self-efficacy in stroke patients. Design: Single blind, Randomized controlled trial. Methods: Nine patients following stroke were divided randomly into the treadmill with optic flow group (n=3), treadmill with virtual reality group (n=3), and control group (n=3). Subjects in the treadmill with optic flow group wore a head-mounted display in order to receive a speed modulated real optic flow scene during treadmill training for 30 minutes, while those in the treadmill with virtual reality group and control group received treadmill training with virtual reality and regular therapy for the same amount of time, five times per week for a period of three weeks. Timed up and go test (TUG) and activities-specific balance confidence scale (ABC scale) were evaluated before and after the intervention. Results: TUG in the treadmill training with optic flow group showed significantly greater improvement, compared with the treadmill training with virtual reality group and control group (p<0.05). Significantly greater improvement in the ABC scale was observed in the treadmill training with optic flow group and the tread mill training with virtual reality group, compared with the control group (p<0.05). Conclusions: Findings of this study demonstrate that treadmill training with real optic flow scene can be helpful in improving balance and balance self-efficacy of patients with chronic stroke and may be used as a practical adjunct to routine rehabilitation therapy.
