Objectives: Restless legs syndrome (RLS) is a common sleep disorder in adults with diabetes. This study investigated the frequency of RLS and clinical correlations in children and adolescents with type 1 diabetes. Methods: This study included 55 consecutive patients (21 males, age $12.6{\pm}3.4years$) with type I diabetes that were regularly treated at the Department of Pediatric Endocrinology. RLS was diagnosed by intensive interviews which also included the Epworth Sleepiness Scale (ESS) and International RLS Rating Scale (IRLSRS). Patients also received neurological examinations and laboratory tests for diabetes, iron metabolism and renal function. Results: Thirteen patients (23.6%, 6 males) were compatible for the diagnostic criteria of RLS. None of the RLS patients showed abnormal findings in neurological evaluations and 7 patients had familial history of RLS. Demographic and laboratory findings were not different between the patients with or without RLS. The RLS group showed significantly increased ESS and IRLSRS scores. Conclusion: RLS was prevalent in children and adolescents with type 1 diabetes. The association between RLS and diabetes-related laboratory findings requires further investigation.

This study is aimed at establishing the categories and items for ecological assessment and evaluation of the environmental friendliness of golf courses in the capital region of Korea. The categories and items for the assessment have been derived based on the existing literature and interviews with golf experts. This study covers 32 golf courses in the capital region of Korea that are available in terms of data and on-site surveys. In order to conduct a comprehensive assessment of the environmental friendliness of the golf courses, the assessment area was divided into 4 categories that include a total of 14 sub-categories. The 4 categories encompass 1) location, 2) topography, 3) vegetation, and 4) construction. As its sub-categories, the location category includes current land use and zoning in the National Land Use Management Law. Topography has 2 sub-categories in the damage ratio of existing topography, gradient, cut area, and slope height. The assessment of vegetation is largely based on site surveys in the categories of preservation of the existing vegetation, the use of natural resources and existing trees, the component ratio of native tree species, the multi-layered structure of vegetation, and the utilization of water purification plants. In the aspect of construction, afforestation on tile slopes and the utilization of existing surface soil were evaluated. The examination of comparative analysis among the 10 items as a ratio measure showed that the scores were low in the sub-categories of current land we, the use of existing trees, and the multi-layered structure of vegetation. However, the rating results were satisfactory in the 2 sub-categories including cut area, and the utilization of native tree species. Those proved to be contributing factors in the ecological health of the golf courses. According to correlation analysis of the 10 items to the overall ecological rating of each golf course, the sizes of the 32 golf courses were mainly affected by the damage ratio of existing topography, gradient, preservation of vegetation and slope height. This study has the initiative to conduct an ecological assessment of golf courses in the country based on site surveys. The study results revealed that location factors such as current land use, damage ratio of topography and gradient and topographical factors were the main factors affecting the environmental friendliness of golf courses. This indicates indicating the significance of these factors in the future construction practices of golf courses. Furthermore, this study raises the need for follow-up studies to establish more detailed assessment criteria and to develop assessment techniques for areas such as slope afforestation and water purification plants that need a qualitative approach.
Objective : Panic disorder is a chroni, debilitating illness, and has been known to be affected by high anxiety sensitivity and a poor quality of life. We examined if 12 weeks of pharmacotherapy with escitalopram could affect anxiety sensitivity, quality of life, and general disability in patients with panic disorder. Method : We enrolled 27 patients who met the DSM-IV-TR criteria for panic disorder, with or without agoraphobia, and completed 12 weeks of escitalopram treatment. We enrolled 29 normal control subjects who were matched for age and sex compared with the panic patients. All subjects were assessed using the Hamilton Rating Scale for depression (HAM-D), Hamilton Rating Scale for Anxiety (HAM-A), Panic Disorder Severity Scale (PDSS), Anxiety Sensitivity Index-Revised (ASI-R), WHO quality of life assessment (WHO-QOL) and Sheehan Disability Scale (SDS) before and after 12 weeks of treatment. Result : Panic disorder patients had a higher Anxiety sensitivity, a poorer Quality of life, and significant disability than normal control subjects at baseline. After the treatment, Panic disorder patients showed significant improvement in the severity of panic symptoms in terms of the HAM-A p<0.001), HAM-D (p<0.001) in addition, the reached a remission state in terms of the PDSS score. They also showed significant improvement in the ASI-R (p<0.001), WHO-QOL (p<0.001) and SDS (p<0.001), but mean scores of the ASI-R, WHO-QOL and SDS in panic disorder patients were did not reach normal levels. Conclusion : This study suggests that 12 weeks of pharmacotherapy with escitalopram could improve anxiety sensitivity, quality of life, and disability in panic disorder patients, although their anxiety sensitivity, quality of life and disability were not yet normalized after short-term pharmacotherapy. Thus, long-term pharmacotherapy may be necessary for panic disorder patients in order to get an optimal clinical response in terms of anxiety sensitivity, quality of life, and general disability.

The concept of extraction in orthodontic treatment has been changed many times. Even today, criteria of extraction or nonextraction is still changing. In this study, changes depending on the evaluator's perception of treatment outcomes were compared in both extraction and nonextraction cases. In this study, premolar extracted 59 patients and nonextracted 60 patients, totally 119 patients who finished orthodontic treatment in Dankook University Dental Hospital orthodontic clinic were enrolled. Evaluation sections made up of specialists and laypersons assessed soft tissue traced from lateral cephalometric radiographs with visual analogue scale before and after the treatment. And the results were statistically analyzed. Thus, the conclusions drawn are as follows: 1. Average score is 5.76 in extraction, which is larger than 5.28 of nonextraction case. Improvement of facial profile was more favorably accepted in extraction case. 2. 5.875 in the group of specialists were higher evaluation than 5.165 in the group of layperson. 3. Specialists gave significantly higher ratings in the extraction than nonextraction. 4. A higher rating in extraction case of the layperson group has no significant difference with nonextraction case. 5. Nonextraction patients were given higher ratings from specialist group. 6. A higher rating of specialist group in extraction case has no significant difference with layperson group.

Objectives: Patients with premenstrual dysphoric disorder(or PMDD) have impairments of the social, occupational or academic function due to psychological or somatic symptoms, which have the characteristic pattern of symptom exacerbation in the week before menses begin and remission shortly after the onset of menses. In the chronobiological view, many researchers have assumed that the etiology of PMDD is the advanced circadian rhythm. It has been suggested that light has a therapeutic effect on PMDD, because evening light results in phase delay of circadian rhythm through the biochemical changes including melatonin. Methods: The authors investigated the therapeutic effect of light therapy on four patients with prospectively diagnosed PMDD by DSM-IV criteria using clinical psychiatric interview, Premenstrual Assessment Form(PAF) and Daily Rating Form(or DRF). In the evening(6:30pm-8:00pm), the 2,500 lux light administered for seven consecutive days during the symptomatic late luteal phase of menstrual cycle. Beck Depression Inventory(or BDI), Hamilton Rating Scale for Depression(or HAM-D), Spielberg State Anxiety Inventory(or SA), and DRF were evaluated before and after seven days of light therapy. Results: Premenstrual symptoms of PMDD could be effectively treated with the evening bright light therapy, especially in PMDD patients with atypical symptoms. In addition, the light therapy seemed to more effective on the psychologic symptoms than the somatic symptoms of PMDD. There was no significant side-effect of light therapy, except the transient and mild eye-strain in one case. Conclusions: In spite of the results of limited data from our clinical trial, the authors suggest that the potential use of light therapy as an alternative to the pharmacological management of patients with PMDD.

Three kinds of typical practices to reflect the values of Agile Development Methodology were selected from a previous study. The purpose of this study is to validate the Reactive-Proactive Aggression Questionnaire (RPQ), a tool for measuring reactive-proactive aggression, in the context of South Korea. A thorough translation was conducted in collaboration with the original author. An exploratory factor analysis (EFA), exploratory structural equation modeling (ESEM), rating scale model (Rasch), differential item functioning (DIF), and convergent validity were performed on a sample of 510 South Korean individuals. The results revealed a two-factor structure of reactive and proactive aggression after removing one item showing dual loading. Rating scale analysis based on the Rasch model indicated the appropriateness of the 3-point Likert scale, with all items meeting fit criteria. Although the separation index and separation reliability of proactive aggression was marginally lower, the overall discrimination between participants and items was satisfactory. Examination of participant-item distribution indicated a suitable alignment between reactive aggression and participant ability levels, whereas proactive aggression exhibited slightly elevated item difficulty. Furthermore, three items were found to function differently based on gender. A moderate but statistically significant positive correlation was found between the Barratt Impulsiveness Scale-11-R (Korean version) and RPQ from the results of the convergent validity evaluation. Overall, this study employed rigorous statistical methods to validate the suitability of the RPQ for use in Korea, taking cultural nuances into account, and introduced the concepts of reactive and proactive aggression to the Korean general population.

Objectives : Autism is a well-known psychiatric disorder that is presumed to have a neural basis. To investigate the underlying neurofunctional abnormalities of autism, the authors performed single photon emission computed tomography (SPECT) on children with autism. Methods : Fifty-five children with untreated autism (47 boys and 8 girls, mean age=$50.6{\pm}20.28$ months) were selected from among the patients visiting the child and adolescent psychiatric clinic of Seoul National University Hospital. Psychiatrists had diagnosed the participants according to the DSM-IV criteria for autistic disorder and the Childhood Autism Rating Scale (CARS) criteria for a diagnosis of autism. All participants were examined using 99mTC-HMPAO Brain SPECT. Using statistical parametric mapping (SPM) analysis, we compared the participants' SPECT images to standardized SPECT images of normal children, which had been retrospectively selected by the authors, on a voxel by voxel basis. Voxels with a p-value less than .001 were considered to be significantly different. Results : The autistic group showed significant hypoperfusion in the right medial frontal gyrus, right precentral gyrus, and left precuneus gyrus. In addition, they showed no significant hyperperfusion areas when compared to the control group. Conclusion : The findings of hypoperfusion in the medial-frontal lobe and precuneus are accord with hemodynamic abnormalities that have been already reported. Therefore, these findings are compatible with the recently suggested "theory of mind" hypothesis and the disturbances in attention shifting that have been observed in autistic children.
In recent years, Ministry of Environment (MOE) has been implementing a phased strengthening of the effluent standards for sewage treatment plants. In this regard, a comprehensive system should be developed to help check the appropriateness of such standards by specifying the grounds for standard-setting and investigating the current operation of sewage treatment plants clearly. It is necessary to establish a new standard-setting system for the effluent that is in a closer connection with the environmental criteria and rating systems. In the United States, the federal government provides guidelines on the least provisions and requirements for the Publicly Owned Treatment Works (POTWs). Local governments set the same or stricter guidelines that reflect the characteristics of each state. In Japan, the sewage treatment plants are subject to both the effluent standards and the discharge acceptable limits to pubic waters under the sewerage law. Specific requirements and limits are set in accordance with local government regulations. The European Union imposes sewage treatment plants with different provisions for effluent standards, depending on the sensitivity of public waters to eutrophication. The effluent standards for sewage treatment plants are classified by pollutant loads discharged to receiving waters. MOE also needs to introduce systems for setting new parameter standards on a POTW effluent by applying statistical means and treatment efficiencies or optimal treatment techniques, as seen in the cases of the US National Pollutant Discharge Elimination System (NPDES) or the EU Integrated Pollution Prevention and Control (IPPC).

Objective : Surgical treatment of lumbosacral foraminal stenosis requires an understanding of the anatomy of the lumbosacral area in individual patients. Unilateral facetectomy has been used to completely decompress entrapment of the L5 nerve root, followed in some patients by posterior lumbar interbody fusion (PLIF) with stand-alone cages Methods : We assessed 34 patients with lumbosacral foraminal stenosis who were treated with unilateral facetectomy and PLIF using stand-alone cages in our center from January 2004 to September 2007. All the patients underwent follow-up X-rays, including a dynamic view, at 3, 6, 12, 24 months, and computed tomography (CT) at 24 months postoperatively. Clinical outcomes were analyzed with the mean numeric rating scale (NRS), Oswestry Disability Index (ODI) and Odom's criteria. Radiological outcomes were assessed with change of disc height, defined as the average of anterior, middle, and posterior height in plain X-rays. In addition, lumbosacral fusion was also assessed with dynamic X-ray and CT. Results : Mean NRS score, which was 9.29 prior to surgery, was 1.5 at 18 months after surgery. The decrease in NRS was statistically significant. Excellent and good groups with regard to Odom's criteria were 31 cases (91%) and three cases (9%) were fair. Pre-operative mean ODI of 28.4 decreased to 14.2 at post-operative 24 months. In 30 patients, a bone bridge on CT scan was identified. The change in disc height was 8.11 mm, 10.02 mm and 9.63 mm preoperatively, immediate postoperatively and at 24 months after surgery, respectively. Conclusion : In the treatment of lumbosacral foraminal stenosis, unilateral facetectomy and interbody fusion using expandable stand-alone cages may be considered as one treatment option to maintain post-operative alignment and to obtain satisfactory clinical outcomes.
