마우스 종양에서 저선량 방사선이 Apoptosis의 유도에 미치는 영향 (Effect of Small Dose of Radiation on Induction of Apoptosis in Murine Tumors)

  • 목적 : 방사선에 의한 apoptosis 유도에 저선량 방사선이 미치는 영향을 관찰하여 adaptive response 현상이 관계되는지를 마우스 종양에서 분석하고 관련되는 기전에 관하여 연구하고자 하였다. 대상 및 방법 : 마우스 동종암인 HCa-I, OCa-I에서 저선량 (0.05 Gy) 방사선 조사 후 고선량 (25 Gy)을 조사하여 이로부터 일정시간 후 종양에서 유도된 apoptosis 수준을 비교분석하였다. 또한, apoptosis의 조절 물질인 p53, Bcl-2, Bax, Bcl-X 등의 발현을 Western blotting으로 분석하여 관련된 기전을 연구하였다. 결과 : OCa-l에서 0.05 Gy를 전처치 후에 25 Gy를 조사한 군에서 apoptosis의 유도 수준은 세포 1000개당 229로서 예상되는 값인 324 에 비하면 약 $30\%$정도 감소된 결과로 나타나서(p<0.05) 저선량의 방사선에 의하여 apoptosis의 유도 수준이 감소한 것으로 나타났다. 반면 HCa-l기에서는 예상된 apoptosis수준과 실제 관찰치간에 변화가 없었다. 유전물질의 발현에서 p53은 두 종양 공히 0.05 Gy 조사군, 25 Gy 조사군 및 0.05+25 Gy 조사군에서 발현이 증가되었다. Bcl-2와 Bax는 두 종양 모두 발현 수준의 등락이 현저하지는 않았으나 OCa-l의 0.05+25 Gy 조사군에서 Bcl-2의 발현이 Bax를 상회하는 결과를 보였다. Bcl-X는 HCa-l에서 0.05 Gy 정도의 저선량에서부터 높은 상승을 보인 반면, OCa-l기에서는 천혀 발현되지 않았다. 결론 : 마우스 종양의 일부에서 0.05 Gy의 저선량이 고선량 방사선에 의한 apoptosis 유도에 대하여 adaptive response를 보이는 것으로 나타났다. 이는 Bcl-2, Bax의 발현 수준과 BcI-X 등이 관련되는 것으로 보였다. 본 연구는 방사선에 의한 apoptosis에서 adaptive response의 관련성이 일정치 않다는 것을 마우스 종양에서 보여주었다.

국소 진행된 자궁경부암의 방사선치료와 저용량 cisplatin 항암요법 동시치료시 급성독성 밀 초기반응 평가 (Low Dose Cisplatin as a Radiation Sensitizer in Management of Locally Advanced Scluamous Cell Carcinoma of the Uterine Cervix : Evaluation of Acute Toxicity and Early Response)

  • 목적 :국소적으로 진행된 자궁경부암 환자에서 저용량의 Cisplatin을 방사선민감제로 사용하여 방사선치료와 동시 치료하였을 때의 급성독성 평가와 초기 반응을 평가하기 위하여 연구를 시행하였다. 대상 및 방법 : 본 연구는 1996년 12월부터 1999년 1월까지 FIGO Stage IIB-IIIB의 진행된 자궁경부암 환자 38명을 대상으로 하였다. 16명은 자궁경부암의 크기가 4cm이상인 환자들로 방사선 치료와 저용량의 cisplatin을 매일 동시에 치료 하였으며, 나머지 환자에선 방사선 치료 단독으로 치료하였다. 방사선치료는 골반강에 외부 방사선치료로 4500 cGy(3060cGy시행이후 midline block 추가), 자궁옅조직으로 침범이 있는 경우에는 자궁옅조직으로 방사선치료 부위를 줄여 900-1000cGy 추가치료를 시행하였고, 내부치료는 Ir$^{192}$고선량 근접 치료기(micro-Selectron HDR)로 6극회의 강 내조사(point "A"에 3000 to 3500cGy, 500cGy/fx, 2회/week)를 시행하였다. 방사선치료와 저용량의 cisplatin을 동시에 사용한 군에서는 방사선치료 첫날부터 방사선치료 20일째 되는 날까지 저용량의 cisplatin 10mg을 방사선치료 30분 전에 투여하였다. 급성독성의 평가는 expanded common toxicity criteria of the NCI Clinical Trial을 이용하였다. 초기반응의 평가는 방사선치료 종료이후 최소 4주이상의 추적조사가 가능한 사람들을 대상으로 시행되었다. 결과 : 급성독성 평가는 전체 38명에서 평가 가능하였으며, 방사선치료와 저용량의 cisplatin을 병용한 군에서 16명 중 6명(37.5$\%$)에서, 방사선 단독으로 치료한 군에서는 22명중 1명(6.2$\%$)에서 3등급 이상의 백혈구감소를 보였으며, 통계학적으로 의미있는 차이를 보였다 (P=0.030). 3등급 이상의 급성 위장간 독성은 저용량의 cisplatin을 병용한 군에서만 4명이 있었으나, 2-3일의 치료 중단이후 증상이 완화되어서 치료를 계속할 수 있었으며, 치료중 5kg이상의 체중감소는 방사선치료와 저용량의 cisplatin을 병용한 군에서는 16명중 3명(18.7$\%$), 방사선 단독으로 치료한 군에서는 22명중 2명(9.1$\%$)으로 통계학적으로 의미 있는 차이를 보이지 않았다(P=0.63). 초기반응은 추적조사가 4주 이상 가능하였던 34명을 대상으로 하였으며, 저용량의 cisplatin을 병용한 군에서 14명중 11명 (78$\%$), 방사선 단독으로 치료 군에서 20명중 16명(80$\%$)으로 통계학적으로는 의미있는 차이를 보이지 않았다(P=0.126). 결론 :국소 진행된 자궁경부암에 대한 방사선치료와 저용량의 cisplatin 병용요법 치료시 3등급 이상의 백혈구감소가 방사선치료 단독으로 치료시보다 많았으나, 1주 이하의 치료중단 이후 치료를 계속 할 수 있었으며, 4등급 이상의 백혈구감소와 치료에 의한 사망은 없었다. 초기반응에 대한 평가는 저용량의 cisplatin병용 군에서 4cm 이상의 자궁경부암을 가진 환자가 많음에도 불구하고 양 군에서 비슷한 정도의 반응을 보였으므로 진행된 자궁경부암의 치료에서 저용량의 cisplatin을 방사선민감제로 사용하여 치료결과의 향상을 기대할 수 있게 되었다. 따라서 저용량의 clsplatin과 방사선치료를 병용요법하였을 때의 치료효과를 판정하기 위해서는 phase III study가 필요하다.

Gold nanoparticles enhance anti-tumor effect of radiotherapy to hypoxic tumor

  • Purpose: Hypoxia can impair the therapeutic efficacy of radiotherapy (RT). Therefore, a new strategy is necessary for enhancing the response to RT. In this study, we investigated whether the combination of nanoparticles and RT is effective in eliminating the radioresistance of hypoxic tumors. Materials and Methods: Gold nanoparticles (GNPs) consisting of a silica core with a gold shell were used. CT26 colon cancer mouse model was developed to study whether the combination of RT and GNPs reduced hypoxia-induced radioresistance. Hypoxia inducible $factor-1{\alpha}$ ($HIF-1{\alpha}$) was used as a hypoxia marker. The 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay and terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) staining were conducted to evaluate cell death. Results: Hypoxic tumor cells had an impaired response to RT. GNPs combined with RT enhanced anti-tumor effect in hypoxic tumor compared with RT alone. The combination of GNPs and RT decreased tumor cell viability compare to RT alone in vitro. Under hypoxia, tumors treated with GNPs + RT showed a higher response than that shown by tumors treated with RT alone. When a reactive oxygen species (ROS) scavenger was added, the enhanced antitumor effect of GNPs + RT was diminished. Conclusion: In the present study, hypoxic tumors treated with GNPs + RT showed favorable responses, which might be attributable to the ROS production induced by GNPs + RT. Taken together, GNPs combined with RT seems to be potential modality for enhancing the response to RT in hypoxic tumors.

IMPACT OF UV RADIATION SENSITIVITY ON DERMATOPHYTES (Microsporum boullardii)

  • An experiment has been conducted to measure the impact of UV radiation sensitivity on dermatophytes (Microsporum boullardii) by different UV radiation exposure time interval (1 min, 2 min 5 min, 10 min and 20 min) in degradation of keratin (Feather) in growth promoting substances of protein, cysteine, cystine and methionine from 7 to 28 days of incubation period. Mutant strain caused maximum weight loss with 1 minutes of UV radiation exposure at 21 day and mutant strain became immune in sensitivity at 14 days for decomposition of feathers. Maximum protein caused at 21st days with 20 minutes U.V radiation exposure and immune sensitivity had deducted with other UV radiation exposure time. On 28 days, mutant strains became immune with all exposure times, Whereas maximum methionine caused at 21st days with 20 minutes UV radiation exposure. Maximum cysteine caused at $14^{th}$ day with 5 minutes UV radiation exposure and mutant strain showed immune response at all time periods. Cystine production was also followed by cysteine at 21 day and also showed complete immune response with 1 and 2 minutes UV radiation exposure at7 and 14 days. Thus mutant strain of Microspornm boullardii can be used as a biotechnological tool for production of growth promoting substances.

국소적으로 진행된 자궁경부암에서 방사선과 Cisplatin의 동시병합요법의 치료결과 (Therapeutic Results of Concurrent Chemoradiation in Locally Advanced Uterine Cervical Cancer)

  • Purpose : Despite a development of therapeutic machines and advance in modern radiation therapy techniques, locally advanced cervical carcinoma has shown high rate of local failure and poor survival rate, Combination of chemotherapy and radiotherapy demonstrated benefit in improving local control and possibly the overall survival. Our study was performed to evaluate effect of concurrent chemoradiation on locally advanced uterine cervical cancer. Methods and Materials : Twenty six patients with locally advanced stage(FIGO stage IIB with ${\geq}5cm$ in diameter, III, IVA) were treated with combination of radiation therapy and concurrent cisplatinum between May of 1988 and September of 1993 at our hospital. Radiation therapy consisted of external irradiaton and 1-2 sessions of intracavitary irradiation. Cisplatinum was administered in bolus injection of 25mg/$m^2$ at weekly intervals during the course of external radiation therapy. Results : Of the 26 Patients, twenty-five patients were evaluable for estimation of response. Median follow-up period was 25 months with ranges from 3 to 73 months. Stage IIB, III, and IVA were 16, 5, 4 patients, respectively, Twenty patients were squamous cell carcinoma. Response was noted in all 25 patients: complete response(CR) in 17/25($68\%$), Partial response(PR) in 8/25($32\%$). Of the 24 patients except one who died of sepsis at 3 months follow-up, seventeen patients($70.8\%$) maintained local control in the pelvis: 16/17($94.1\%$) in CR, 1/17($14.3\%$) in PR. Fourteen of the 17 patients with CR are alive disease free on the completion of follow-up. Median survival is 28 months for CR and 15 months for PR. Analysis of 5-year survival by stage shows 11/16($59.8\%$) in IIB, 3/5($60.0\%$) in III, and 1/4($25.0\%$) in IVA. Overall 5-year survival rate was $55.2\%$. Ten patients recurred: 4 at locoregional, 3 in distant metastasis and 3 with locoregional and distant site. Toxicity by addition of cisplatinum was not excessive. Conclusion : Although the result of this study was obtained from small number of patients, it is rather encouraging in view of markedly improved response rate compared with the results of historical group.

Initial clinical outcomes of proton beam radiotherapy for hepatocellular carcinoma

  • Purpose: This study aimed to evaluate the initial outcomes of proton beam therapy (PBT) for hepatocellular carcinoma (HCC) in terms of tumor response and safety. Materials and Methods: HCC patients who were not indicated for standard curative local modalities and who were treated with PBT at Samsung Medical Center from January 2016 to February 2017 were enrolled. Toxicity was scored using the Common Terminology Criteria for Adverse Events (CTCAE) version 4.0. Tumor response was evaluated using modified Response Evaluation Criteria in Solid Tumors (mRECIST). Results: A total of 101 HCC patients treated with PBT were included. Patients were treated with an equivalent dose of $62-92GyE_{10}$. Liver function status was not significantly affected after PBT. Greater than 80% of patients had Child-Pugh class A and albumin-bilirubin (ALBI) grade 1 up to 3-months after PBT. Of 78 patients followed for three months after PBT, infield complete and partial responses were achieved in 54 (69.2%) and 14 (17.9%) patients, respectively. Conclusion: PBT treatment of HCC patients showed a favorable infield complete response rate of 69.2% with acceptable acute toxicity. An additional follow-up study of these patients will be conducted.

Three-dimensional conformal radiotherapy for portal vein tumor thrombosis alone in advanced hepatocellular carcinoma

  • Purpose: We sought to evaluate the clinical outcomes of 3-dimensional conformal radiation therapy (3D-CRT) for portal vein tumor thrombosis (PVTT) alone in patients with advanced hepatocellular carcinoma. Materials and Methods: We retrospectively analyzed data on 46 patients who received 3D-CRT for PVTT alone between June 2002 and December 2011. Response was evaluated following the Response Evaluation Criteria in Solid Tumors. Prognostic factors and 1-year survival rates were compared between responders and non-responders. Results: Thirty-seven patients (80.4%) had category B Child-Pugh scores. The Eastern Cooperative Oncology Group performance status score was 2 in 20 patients. Thirty patients (65.2%) had main or bilateral PVTT. The median irradiation dose was 50 Gy (range, 35 to 60 Gy) and the daily median dose was 2 Gy (range, 2.0 to 2.5 Gy). PVTT response was classified as complete response in 3 patients (6.5%), partial response in 12 (26.1%), stable disease in 19 (41.3%), and progressive disease in 12 (26.1%). There were 2 cases of grade 3 toxicities during or 3 months after radiotherapy. Twelve patients in the responder group (15 patients) received at least 50 Gy irradiation, but about 84% of patients in the non-responder group received less than 50 Gy. The 1-year survival rate was 66.8% in responders and 27.4% in non-responders constituting a statistically significant difference (p = 0.008). Conclusion: Conformal radiotherapy for PVTT alone could be chosen as a palliative treatment modality in patients with unfavorable conditions (liver, patient, or tumor factors). However, more than 50 Gy of radiation may be required.

절제 불가능한 식도암에서 고선량 외부조사 방사선 치료의 결과 (Treatment Results of Increased Dose External Beam Radiation Therapy for Unresectable Esophageal Cancer)

  • Purpose : To evaluate the treatment outcome for patients with locally advanced, unresectable esophageal cancer treated with relatively high dose radiation therapy(RT). Materials and Methods : From January 2000 to December 2008, 32 patients with locally advanced unresectable or medically inoperable esophageal cancer were treated with radiation therapy(RT) with or without concurrent chemotherapy. Ten patients were excluded from analysis because of distant metastasis and drop off. Patient distributions according to AJCC stages II, III IVa were 7(31.8%), 12(54.6%), 3(13.6%) respectively. The locations of tumor were cervical/upper thorax 3 (13.6%), mid thorax 13(59.1%), and lower thorax/abdominal 6(27.3%), respectively. Eleven patients received RT only, and 11 patients received cisplatin based concurrent chemoradiotherapy(CCRT). Median radiation dose was 65 Gy(range 57.6~72 Gy). Results : The median follow-up was 9.1 months(range 1.9~43.8 months). The response rates for complete response, Partial response, stable disease and Persistent disease were 6(27.3%), 11(50.0%), 4(18.2%) and 1(4.5%), respectively. Two patients(9.1%) suffered from esophageal stenosis and stents were inserted. Two patients(9.1%) had Grade 3 radiation pneumonitis and one of them expired due to acute respiratory distress syndrome(ARDS) at 36 days after completion of radiation therapy. The recurrence rate was 11(50.0%). The patterns of recurrence were persistent disease and local progression in 5(22.7%), local recurrence 3(13.7%) and concomitant local and distant recurrence in 3(13.7%). The overall survival(OS) rate was 32.1% at 2 years and 21.4% at 3 years(median 12.0 months). Disease free survival(DFS) rate was 17.3% at 2 and 3 years. All patients who had no dysphagia at diagnosis showed complete response after treatment and 100% OS at 3 years(p=0.0041). The OS for above 64.8 Gy group and 64.8 Gy or below group at 3 years were 60.6% and 9.1%(p=0.1341). The response to treatment was the only significant factor affecting OS(p=0.004). Conclusion : Relatively high dose radiation therapy in unresectable esophageal cancer tended to have a better outcome without increased complication rate. Further study with more patients is warranted to justify improved result.
