Pre-operative Concurrent Chemoradiotherapy for Stage IlIA (N2) Non-Small Cell Lung Cancer (N2 병기 비소세포 폐암의 수술 전 동시화학방사선요법)

  • Purpose: This is to evaluate the acute complication, resection rate, and tumor down-staging after pre-operative concurrent chemoradiotherapy for stage IIIA (N2) non-small cell lung cancer. Materials and Methods Fifteen patients with non-small cell lung cancer were enrolled in this study from May 1997 to June 1998 in Samsung Medical Center. The median age of the patients was 61 (range, 45~67) years and male to female ratio was 12:3. Pathologic types were squamous cell carcinoma (11) and adenocarcinoma (4). Pre-operative clinical tumor stages were cT1 in 2 patients, cT2 in T2, and cT3 in 1 and all were N2. Ten patients were proved to be N2 with mediastinoscopic biopsy and five had clinically evident mediastinal Iymph node metastases on the chest CT scans. Pre-operative radiation therapy field included the primary tumor, the ipsilateral hilum, and the mediastinum. Total radiation dose was 45 Gy over 5 weeks with daily dose of 1.8 Gy. Pre-operative concurrent chemotherapy consisted of two cycles of intravenous cis-Platin (100 mg/m$^{2}$) on day 1 and oral Etoposide (50 mg/m$^{2}$/day) on days 1 through 14 with 4 weeks' interval. Surgery was followed after the pre-operative re-evaluation including chest CT scan in 3 weeks of the completion of the concurrent chemoradiotherapy if there was no evidence of disease progression. Results : Full dose radiation therapy was administered to all the 15 patients. Planned two cycles of chemotherapy was completed in 11 patients and one cycle was given to four. One treatment related death of acute respiratory distress syndrome occurred In 15 days of surgery. Hospital admission was required in three patients including one with radiation pneumonitis and two with neutropenic fever. Hematologic complications and other acute complications including esophagitis were tolerable. Resection rate was 92.3% (12/l3) in 13 patients excluding two patients who refused surgery. Pleural seeding was found in one patient after thoracotomy and tumor resection was not feasible. Post-operative tumor stagings were pT0 in 3 patients, pTl in 6, and pT2 in 3. Lymph node status findings were pN0 in 8 patients, pN1 in 1, and pN2 in 3. Pathologic tumor down-staging was 61.5% (8/13) including complete response in three patients ($23.7%). Tumor stage was unchanged in four patients (30.8%) and progression was in one (7.7%). Conclusions : Pre-operative concurrent chemoradiotherapy for Stage IIIA (N2) non-small cell lung cancer demonstrated satisfactory results with no increased severe acute complications. This treatment shceme deserves more patinet accrual with long-term follow-up.

  • This study intends to clarify the key elements of designing low energy residential building construction by planning out residential construction in nature oriented designing method utilizing nearby environment and nature oriented energy from designing stage instead of construction of low energy residential building. Development of building technology is proportional to the development of technology that lasts already. However, what is no less important than the advancement of technology, it is the study of fundamental phenomena energy use in response to climate, reduction, such as recycling. It is possible in such a purpose, it is assumed that there is a need to study elements implementation plan in accordance with the climatic characteristics of the study. For through the analysis of this case, corresponding to the phenomenon of the features of the macro climate and micro climate due to climate change, a combination building blocks of classification placement of each, shape, structure, elevation, space, of the material appeared in various it was possible to know the construction characteristics were. As shown in each case, construction method to address climate change has been found to apply to a comprehensive analysis climatic characteristics of each region, in response to this, the construction of element each corresponding.

Growth of Daphniphyllum macropodum and Climatic Factors at Mt. Naejang, Korea (내장산 지역 굴거리나무의 연륜생장과 기후요소와의 관계)

  • The growth of Daphniphyllum macropodum at Mt. Naejang National Park(Naejang temple area) was investigated in connection with the climatic factors by the use of tree-ring analysis. A pair of cores was extracted from each of 18 trees in the D. macropodum community area near Younjabong for the analysis. The period of mater chronology based upon D. macropodum covers from 1915 to 1998. The growth rates of D. macropodum were very poor in the years 1920, 1932, 1934, 1937, 1942, 1946, 1964, 1969 and 1985, respectively. Response function was employed to understand the relationship between the growth of D. macropodum and climatic factors. The response function of the growth rates of D. macropodum indicated significant negative correlation with the precipitation of August and September. Poor growth of D. macropodum during the August and September nay be due to the frequent rain periods during the summer The heavy rain during the summer seems to decrease the solar radiation, which eventually caused the decrease of photosynthesis capacity. In conclusion, we hypothesize that the decrease of the photosynthesis rates during the rainy summer seasons may cause the slower growth of D. macropodum.

  • The power strength of electric wave of reader looked radiation forms from the gate type RFID system. The more distant from antennas, the weaker of power strength of electric wave. The power strength of electric wave with 3 antennas showed stronger than its with 2 antennas. With 2 antennas, the coefficient of correlation between the power strength of electric wave and response ability of tag was 0.665 (p=0.009), and it showed results at significance level. With 3 antennas, the coefficient of correlation was 0.711 (p=0.004), correlationship between the power strength of electric wave and the response ability of tag showed at significance high level. From the analysis result about tag reading performance on the gate type RFID system, the reading rate was good when tags were attached on a acrylic plate which were located in front of 3 or 4 antennas. As tags were getting away from antennas, the reading rate was decreased. The nearer from antennas, the higher the reading rate at the center. When tags were turned at right angles with antennas, the longer distant from antennas, the lower reading rates at the center, right and left locations. When the number of antennas and distance are constant, respectively, the significant difference of the tag reading rates according to the locations of left, right and center were not found.

  • Parametric study on submerged body shape of an oscillating hemisphere point absorber was conducted to predict the optimal relation between radius and draft of the body. As an additional damping due to power takeoff system, the optimal damping same as wave radiation damping was applied to the PTO system to produce the maximum wave power. Body response spectrum and power spectrum were obtained for various peak frequencies on wave spectra. It was found that the maximum power can be generated when the peak frequency of available wave power was 20% greater than that of wave spectrum.

Radiotherapy and Concurrent Chemo-Radiotherapy in Locally Advanced Hypopharyngeal Cancers - A Hospital Registry Based Analysis

  • Background: The survival of patients with hypopharyngeal cancer is low amongst head and neck cancer cases. The incidence rates of hypopharyngeal cancers in our population are amongst the highest in the world and there are limited data available on the literature on varied responses to first course of treatment with radiotherapy (RT) and concurrent chemo-radiotherapy (CRT) in our population. Materials and Methods: Clinical characteristics and initial responses to treatment in patients who had received radiotherapy and chemo-radiotherapy in a regional cancer center from January 2010 to December 2013 were evaluated. The data were obtained from the hospital cancer registry, and analysis was carried using descriptive statistics. Pearson's chi-square was used to test for differences in the variables and p<0.05 was considered statistically significant. Results: A total of 554 patients were included in the analysis, 411 (74.2%) receiving RT and 143 (25.8%) being given CRT. There was significantly lower number of patients above 70 years with a higher proportion of patients below 50 years who had received CRT (p<0.05). Some 79.3% and 84.6% of patients in the RT and CRT groups respectively presented with a favorable performance status, and in the RT group 240 (58.4%) showed complete response (CR), and in the CRT group 103 (72.0%) showed CR at the first follow-up (p<0.05). Conclusions: Concurrent chemo-radiotherapy gives better short term response to treatment in locally advanced hypopharyngeal cancers.

  • In this paper, we proposed the image-based 3D ray tracing indoor propagation model using a patch scattering model for fading analysis of indoor propagation environments. An image-based 3D ray tracing technique is mainly used, which allows the rapid generation of the complex channel impulse response for any given location and polarization of transmitter and receiver. Due to the site specific nature of indoor environment, we took into account the location and the electrical properties of individual walls and objects such as windows, doors and plaster board in our propagation model. Besides, the 3D radiation beam-patterns and polarizations of arbitrary antennas were considered, and using patch scattering model we can consider the change of the polarization due to the scattering from the various indoor objects, like desks, chairs and etc. the are almost impossible to be modeled with the basic image-based ray tracing method. The model will predict the impulse response, the rms delay spread, the fading characteristics of the channel and performances of the polarization diversity schemes.

  • Dose response curves using absorbed dose to the biological effect are usually available in case of conventional X beam. However, absorbed dose is not consider in treatment planning for carbon beam such as heavy ions. Because the biological effects also depend on other quantities such as the local variation, which is often characterized by the linear energy transfer (LET). So LQ model cannot explain the entire response of fractionated carbon beam irradiation. The variation in LET with penetration depth leads to substantial differences in biological effect of carbon beam. And it is therefore essential in treatment planning to calculate not only the absorbed dose but also the LET to estimate the biological outcome of the radiation of interest. LET variation plays an important role in the fractionated irradiations. It is suggested that consideration of LET is necessary in biophysical model.

Clinical Study on Lobaplatin Combined with 5-Fu and Concurrent Radiotherapy in Treating Patients with Inoperable Esophageal Cancer

  • Objective: To investigate short- and long-term treatment effects and side reactions of lobaplatin plus 5-Fu combined and concurrent radiotherapy in treating patients with inoperable middle-advanced stage esophageal cancer. Methods: Sixty patients with middle-advanced stage esophageal squamous cell cancer were retrospectively analyzed. All patients were administered lobaplatin (50 mg intravenously) for 2 h on day 1, and 5-Fu ($500mg/m^2$) injected intravenously from day 1 to 5 for 1 cycle, in an interval of 21 days for totally 4 cycles. At the same time, late-course accelerated hyperfractionated three-dimensional conformal radiotherapy was performed. Patients were firstly treated with conventional fractionated irradiation (1.8 Gy/d, 5 times/week, a total of 23 treatments, and DT41.4 Gy), and then treated with accelerated hyperfractionated irradiation (1.5 Gy, 2 times/d, a total of 27 Gy in 9 days, an entire course of 6-7 weeks, and DT 68.4Gy). Results: All patients completed treatment, including 10 complete response (CR), 41 partial response (PR), 7 stable disease (SD), and 2 progressive disease (PD). The total effective rate was 85.0% (51/60). Thirty-nine patients had an increased KPS score. One-, 2-, and 3-year survival rates were 85.3%, 57.5%, and 41.7%, respectively. The median survival time was 27 months. The adverse reactions included myelosuppression, which was mainly degree I and II. The occurrence rate of radiation esophagitis was 17.5%. No significant hepatic or renal toxicity was observed. Conclusion: Lobaplatin plus 5-Fu combined with concurrent radiotherapy is safe and effective in treating patients with middle-advanced stage esophageal cancer. However, this result warrants further evaluation by randomized clinical studies.

  • In this paper, the method calculating hydroelastic responses of very large floating structure close to a breakwater in waves is presented. The source-dipole distribution method is used to calculate the generalized radiation problem considering breakwater effects and the diffraction problem is analyzed by using the source-dipole distribution andvelocity potential continuation method. The response of a VLFS is approximated by anexpansion in terms of a free-free beam. Calculated model is a VLFS with 1000m in length in a sea with a straight breakwater. The vertical displacements and bonding moments around a VLFS are calculated by variations for distance between a VLFS and a breakwater and incident wave angle to know the effect of a breakwater.
