The Influence of Nursing Practice Environment, Compassion Fatigue and Compassion Satisfaction on Burnout among Clinical Nurses (간호근무환경, 공감피로 및 공감만족이 간호사의 소진에 미치는 영향)

  • Purpose: The purpose of this study was to identify the influence of nursing practice environment, compassion fatigue and compassion satisfaction on burnout among clinical nurses in Korea. Methods: A descriptive crosssectional study was conducted. The sample consisted of 210 clinical nurses from three tertiary hospitals located in Seoul. Measurement instruments included the Korean version of the Practice Environment Scale of Nursing Work Index (K-PES-NWI) and Stamm's professional quality of life (ProQOL). Descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression were used to analyze the data. Results: Nursing practice environment had moderate negative influence on burnout (r=-.38, p<.001). Compassion fatigue had strong positive influence on burnout (r=.50, p<.001), while compassion satisfaction had strong negative influence on burnout (r=-.61, p<.001). The regression model explained 63% of variance of burnout and the compassion satisfaction was the most influencing factor for nurses' burnout. Conclusion: This study identified nursing practice environment, compassion fatigue and compassion satisfaction as influencing factors for nurses' burnout. Strategies to decrease compassion fatigue, enhance compassion satisfaction and create better nursing practice environment are recommended to decrease nurses' burnout.

Combination Treatment of Korean Medicine with Ohrim-san for Lower Urinary Tract Symptoms (LUTS) after Laparoscopic Myomectomy Patient : A Case Report (복강경하 근종적출술 후 발생한 하부요로증상에 대한 오림산을 포함하는 복합한의진료의 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on a post-laparoscopic myomectomy patient's lower urinary symptoms (LUTS). Methods: The patient who diagnosed with uterine myoma had Laparoscopic myomectomy. After the operation, the patient complained of dysuria, nocturia, frequent urination, urgent urination and urinary retention. The patient received combination treatment of Korean medicine during 14 days of hospitalization. The treatment included Ohrim-san, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through the Numeric Rating Scale (NRS), Overactive Bladder Symptom Score (OABSS), Frequency-Volume chart, and 5-Level version of EuroQol-Five Dimension (EQ-5D-5L). Results: After the treatment, the clinical symptoms such as dysuria, nocturia, frequent urination, urgent urination, urinary retention were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the combination treatment of Korean Medicine with Ohrim-san may be effective for treating lower urinary tract symptoms (LUTS) after laparoscopic myomectomy.

Lower Urinary Tract Symptoms of Elderly Prostate Cancer Patients (전립선암 노인 환자의 하부요로증상)

  • Purpose: The purpose of this study was to investigate the degree of LUTS (Lower Urinary Tract Symptoms) and factors affecting LUTS in patients with prostate cancer. Methods: Fifty-four patients with prostate cancer participated and they completed a structured questionnaire at a hospital in Seoul. The LUTS level was measured by using the International prostate symptom score (1992) and the data were analyzed by Wilcoxon rank sum test and Kurskal-Wallis test. Results: Most subjects obtained a higher than moderate LUTS score. The participants who had a lower perceived health status and did not drink alcohol were more likely to had a lower LUTS score than other patients who had higher perceived health status and drink alcohol. In addition, eye diseases were significantly associated with the LUTS score. In LUTS, all the patients received higher IPSS-QOL (International prostate symptom score quality of life) and frequency, nocturia. Conclusion: For proper nursing care related to LUTS, nurses should focus on the factors identified when performing nursing assessments and should develop nursing intervention programs for symptoms management.

A Web-GIS Based Monitoring Module for Illegal Dumping in Smart Cities

  • This study was conducted to develop a Web-GIS based monitoring module of smart city that can effectively respond, manage and improve situation in all stages of illegal dumping management on a city scale. First, five technologies were set for the core technical elements of the module configuration. Five core technical elements are as follows; video screening technology based on motion vector analysis, human behavior detection based on intelligent video analytics technology, mobile app for receiving civil complaints about illegal dumping, illegal dumping risk model and street cleanliness map, Web-GIS based situation monitoring technology. The development contents and results for each set of core technical elements were evaluated. Finally, a Web-GIS based 'illegal dumping monitoring module' was proposed. It is possible to collect and analyze city data at the local government level through operating the proposed module. Based on this, it is able to effectively detect illegal dumpers at relatively low cost and identify the tendency of illegal dumping by systematically managing habitual occurrence areas. In the future, it is expected to be developed in the form of an add-on module of the smart city integration platform operated by local governments to ensure interoperability and scalability.

  • In Korean context, the innovative cities imply new towns or urban clusters in the Non Capital Regions, all of which would accommodate the relocated Capital based public organizations. The central government-initiated innovative cities have provoked pros and cons towards their effectiveness and efficacy for the balanced territorial development. From a broader prospective, this paper firstly examines the current status quo of the innovative cities. Based on their master plans, it analyzes physical and non physical factors which would exert significant impact on the innovative cities. Secondly, it pays attention to how key factors strengthen or weaken their behavioral patterns in terms of dynamic location and implementation policies of the innovative cities. Using System dynamics approaches, it sets up couples of scenarios, categorizing between supporting and opposing arguments towards the innovative cities. Lastly, after divulging systematic structure of the innovative cities, it proposes a series of practical alternatives which would contribute to minimizing unexpected side effects or unwanted social cost in the long run. In order to guarantee reinforcing structure of the innovative cities, the paper suggests that QOL (quality of life) variables, which would require continuous investment in the social infrastructure, are pivotal in achieving original goals of the innovative cities. Otherwise, the innovative cities would not be innovative per se. In the worst case, they might be degraded into the unpopular ghost towns.

Effects of a Forest Therapy Program on Reducing PTSD and Depression and Improving Mood States in Fire Officers

  • Background and objective: This study was conducted to investigate the effects of forest therapy programs on reducing post-traumatic stress disorder (PTSD), depression and improving mood states in fire officers. Methods: To determine PTSD, depression, and mood states before and after the forest therapy program, the survey results of 185 fire officers were analyzed. Frequency analysis was conducted to identify the demographic characteristics of the participants, and Wilcoxon signed-rank test to analyze the differences among group. Results: As a result of a comparative analysis of PTSD before and after the program, there was a significant decrease in PTSD from 10.65 ± 12.00 to 5.64 ± 8.29 after the program. Depression also showed a statistically significant difference from 3.21 ± 4.00 to 2.21 ± 3.47 after the program. As a result of comparatively analyzing mood states before and after the program, there was an increase in positive factors and a decrease in negative factors. Total mood disturbance (TMD) also showed a statistically significant decrease from 5.78 ± 17.37 to -7.38 ± 10.35 after the program. Conclusion: This study has significance in verifying that forest therapy programs can bring psychological changes to fire officers, and these results can be used as a basis for stress management and relief of fire officers.

  • The world has become interested in aging problems. We will have many problems also in the PT field. Japanese aging rate was as high as 16.7 percent in the year of 1999. Its PT has dealt with the aging problem in various ways. This research helps you understand the background and history of Japanese PT from the both clinical and literal sides, which attaches importance to exercise treatment and local rehabilitation, through the PT charge and its related law. Japan and Korea are the neighbors and history, society and culture of both countries have much in common. Korea will also face the aging society soon. I hope it is a good way for Korea to deal with such problems in the future to know of the Japanese PT and refer to it.

Moxibustion for Benign Prostatic Hyperplasia: A Systematic Review and Meta-analysis (전립선 비대증에 대한 뜸치료의 효과 : 체계적 문헌고찰과 메타분석)

  • Objective: This study evaluates the effectiveness and safety of moxibustion for benign prostatic hyperplasia (BPH). Methods: Using the keywords "benign prostatic hyperplasia", "benign prostatic hypertrophy", "benign prostatic enlargement", "prostatic hyperplasia", and "moxibustion", we searched papers in numerous databases, including National Discovery for Science Leaders (NDSL), Korean Traditional Knowledge Portal (KTKP), Oriental Medicine Advanced Searching Integrated System (OASIS), Research Information Sharing Service (RISS), PubMed, Embase, and CENTRAL. The search range included randomized controlled trials (RCTs). Papers not matched with inclusion criteria were excluded. The methodological quality of each RCT was assessed using the Cochrane risk-of-bias tool. Where appropriate, meta-analyses were performed. Results: Initially, 77 studies were found. Of these, 11 duplicate studies were removed and 27 were excluded following title and abstract screening. After the remaining 39 papers were scanned, 13 RCTs were selected and analyzed. Among these 13 RCTs, five compared moxibustion therapy and oral medication, seven compared moxibustion plus acupuncture therapy and oral medication, and one compared moxibustion plus acupuncture therapy and sham-moxibustion. The meta-analysis showed positive results for the use of moxibustion therapy in terms of International Prostate Symptom Score (IPSS), Quality Of Life (QOL), Maximum Flow Rate (Qmax), Prostate Volume (PV), and the efficacy rate. The meta-analysis showed positive results for the use of moxibustion plus acupuncture therapy in terms of IPSS, QOL, and the efficacy rate. Conclusions: This meta-analysis of clinical trials suggests that moxibustion is effective intreating BPH patients. The results of this study could be applied to clinical treatment of BPH. However, additional large-scale clinical researches should be conducted.

Effects of Korean Medical Combination Treatment for Lumbar Spinal Herniated Intervertebral Disc : accompanying or not High-intensive zone(HIZ) (요추 추간판 탈출증에 대한 한방 치료의 효과 연구 : High-intensive zone(HIZ) 유무에 따라)

  • Objectives : The purpose of this study was to analyze the effects of lumbar spinal herniated intervertebral Disc on MRI accompanying or not High-intensive zone(HIZ) by Korean Medical Combination Treatment Methods : 324 patients who received inpatient treatment from January 2017 to September 2017 in the Daejeon-Jaseng of Korean Medicine Hospital were divided into 2 groups by High-intensive zone(HIZ) confirmed with a Lumbar spine magnetic resonance imaging. All patients received a combination of treatment including acupunture, chuna manual therapy, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index), physical examination as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 18.0 for windows. Results : After treatment, Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index), physical examination was improved respectively. But there was no statistically significance between 2 group's improvement after treatment. Conclusions : After inpatient treatment by Korean Medical Combination Treatment, Most patient's pain, disability and Health Related Quality of Life was improved significantly. But there was no statistically difference between accompanying or not High-intensive zone(HIZ). So We think that future research of higher quality and correct statistics shall be necessary.

Quality of Life after Esophageal Surgery for Esophageal Surgery (식도암 환자의 수술 후 삶의 질에 관한 연구)

  • Background: We study the QOL (quality of life) & functional improvement in patients underwent esophageal surgery for cancer by the viewpoint of the patient and would like make the guideline of recovery course. Material and Method: Between Dec. 1996$\sim$Aug. 2002, 250 patients were operated and 57 patients was enable interview, didn't have evidence of recurrence & survived more than 1 year postoperatively at Asan Medical Center. Questionnaire made by direct or telephone interview & include diet habitus, change of body weight, G-E (gastroesophageal) reflux, dumping symptom, change of daily life. Result: There were 53 men (93%), 4 women (7%) with mean age of $62.05{\pm}8.7$ (range: $37{\sim}94$). Operation method was Ivor Lewis operation in 43 case (75.4%), Esophagocolojejunostomy 4 case (7.1%), Mckeown operation 10 case (17.5%). In Diet habitus, 55 patients (96.5%) ate more than three times in a day with mean diet frequency was 3.5 times/day, 51 patients (89.5%) have been ate solid, regular diet, 5 patients (8.8%) enable to eat liquid diet. To compare with preoperative state, 32 patients (56.9%) had a diet speed more than 80%, 28 patients (39.1%) had a diet amount more than 80%. 32 patients (56.9%) had a little change of body weight within a 10%, 25 patients (43%) had a improvement rather than preoperative state. In G-E reflux, 4 patients (7%) had a reflux after every diet, 27 patients (47.4%) had a little reflux after diet. In dumping symptom. 7 patients (12.3%) had a diarrhea after meal. 38 patients (66.6%) had a normal activity 19 patients (33.3%) had a decreased activity. At present state, majority (53 patient-93%) of patient were satisfactory to the operation result, in spite of discomfort from time to time. In the emotional status, 50 patients (88%) had a well sleeping without disturbance. 65% of patients were comfortable in the psychology state. 39% of patients had a libido. Compare with their median age (63 y), it's a normal varient. Conclusion: QOL in patients underwent esophageal surgery for esophageal cancer nearly recovery to preoperative state.
