Comparison of the Use of Midazolam only with Midazolam Combined with Fentanyl or Propofol in IV sedation (정맥하 진정요법에서 Midazolam 단독 사용과 Midazolam, Fentanyl 또는 Propofol 병용 요법의 비교)

  • Background: The purpose of this study was to compare the use of midazolam only with midazolam with fentanyl or propofol in IV sedation. Methods: 24 cases were divided to midazolam group (M group), midazolam + fentanyl group (MF group), midazolam + propofol group (MP group) and midazolam + fentanyl + propofol group (MFP group). In M group, 2 ml midazolam was injected at first, than at 2 minutes interval 1-2 ml injected continuously depending on the level of sedation. In MP, MFP groups, propofol was injected at the speed of 15-20 ml/hr by infusion pump. In this study, the sedation level was evaluated by using OAA/S scale. In each groups, the recovery time was measured until OAA/S scale score level was 5, and pre and postoperative blood pressure change was measured. Each group's data was statistically analyzed using one-way ANOVA. If significant statistical difference were observed, Dunnet test was performed, and control group was M group. Results: Pre and postoperative blood pressure change were not represent significant statistical difference in 4 groups (P value = 0.679 [systolic], P value = 0.206 [diastolic]). But recovery time were represent significant statistical difference (M group: 35.6, MF group: 32.5, MP group: 17.9, MFP group: 19.6 [P value = 0.002]). The result of Dunnet test on recovery time showed significant statistical difference on MF, MFP group when M group was control group. In MFP group, sedation was increased by using supplemental fentanyl, and postoperative pain control was dominant. Conclusion: To achieve the effect of anxiolysis, analgesia, amnesia effectively, and short recovery time, MFP group is mostly recommended.

Analysis of Impact on the Circulating Water System due to an Installation of Helical Current Turbine at the Discharge Channel of the Power Plant (헬리컬 조류수차 설치로 인한 발전소 배수로 계통 영향 분석)

  • In this study, the impact on the circulating water system has been analyzed due to an installation of helical turbine to develop hydro-kinetic energy at the discharge channel of the power plant. Numerical simulations of velocity and pressure variations have been performed when one set of $3.6\;m\;{\times}\;1.5\;m$ sized helical turbine is installed at the outlet of discharge culvert. In case of mean sea level, change of downstream water surface elevation does not affect upstream elevation of the weir because its propagation is blocked by the seal well weir. However in case of highest high water level, change of downstream elevation affects upstream elevation because flow pattern in discharge culvert becomes the full pipe flow with submerged weir. Although an unstable pressure change occurs in upstream of the weir during the intial 10 minutes after beginning of the discharge, it becomes stable after that time. In addition, a rise of water surface elevation by 0.2 m is observed but it is concluded that it hardly affects the safety of circulating water pump (CWP) although its required power is increased more or less. Therefore, the increase of required power of CWP needs to be considered for evaluation of the helical turbine applicability.

Analysis of High-Resolution Manometry Results in LPRD Patients Who Do Not Response to PPI Medication (양자펌프 억제제에 반응을 보이지 않은 인후두 역류질환 환자에서 고해상도 식도 내압검사의 분석)

  • Background and Objectives : Laryngopharyngeal reflux disease (LPRD) originates from regurgitation of gastric contents to the laryngopharynx, and it is relatively common disease in otolaryngology. Proton-pump inhibitor (PPI) medication is frequently using treatment method in present time, but controversies exist regarding treatment for it. High-resolution manometry (HRM) system is a newly developed device that using 36 channels and pressure topography plotting program, checking the function and status of esophagus. The aim of this study was to evaluate clinical usefulness of HRM in LPRD patients who do not response to PPI medication. Materials and Method : From July 2009 to July 2010, a total of 99 patients who were suspected LPRD according to symptoms (Reflux Symptom Index >13) and laryngoscopic findings (Reflux Finding Score >7) were retrospectively enrolled in this study. Patients were consisted of 31 men and 68 women, the mean age was 51.8 years, the mean BMI was 22.46. Patients have taken PPI medication for 2 months, after medication, they performed HRM. Comparative analysis was performed with results of the difference in symptoms. Results : In 99 patients, 37 patients (37.4%) show not improvement of LPRD symptoms after PPI medication. Among them, 18 patients (48.6%) showed abnormal findings that were classified as Peristaltic dysfunction (38.9%), Diffuse esophageal spasm (11.1%), Relaxation impairment of LES (11.1%), Achalasia (5.6%), Hypotensive LES (11.1%), Relaxation impairment of UES (11.1%), Nutcracker esophagus (5.6%), Decreased resting pressure (5.6%). Other 62 patients (62.6%) show improvement of LPRD symptoms, 23 patients (37.1%) showed abnormal findings. Most common finding was Peristaltic dysfunction (43.4%) More prevalent abnormal findings of HRM were found in patients who revealed no response to PPI medication (48.6% vs. 37.1%), but it does not reach the statistical significance. Conclusion : Although statistical significances is not showed, the difference observed in the frequency of HRM abnormal finding between response and no response for PPI medication. It has been estimated that HRM may be used to determine differential diagnosis in patients with LPRD. Further studies in lager population containing normal controls will be needed to prove clinical usefulness.

  • The propose of this study was to evaluate the effect of cellular activity on PDL cells dependent on intermittent and continuous compressive force by determining the alkaline phosphatase activity. An intermittent and continuous compressive forces were applied on PDL cells at the confluent stage. The alkaline phosphatase activity was measured on control and experimental groups every 24, 48, 72hours. The experimental group were consist of continuous and intermittent compressive group which were compressed by $300g/cm^2$ of diaphram pump. The intermittent compressive group was connected by timer which was worked on 10 minutes and off 10minutes. The results were as follows ; 1. The alkaline phosphatase activity of intermittent compressive group was lower than control group at 24 hours(P<0.05). 2. The alkaline phosphatase activity between each groups showed no significant differences at 48hours. 3. The alkaline phosphatase activity of continuous compresssive group was significantly higher than control group at 72 hours(P<0.01).

Aspirin Reduces Acute Lung Injury in Rats Subjected to Severe Hemorrhage (Aspirin이 출혈성 쇼크로 인한 급성 폐손상에 미치는 효과)

  • Background : Hemorrhagic shock and trauma are two of the most common causes of acute lung injury. The activation of cyclooxygenase is one of the important causes of acute lung injury. This study investigated the effect of aspirin, a well-known cyclooxygenase inhibitor, on severe hemorrhage-induced acute lung injury in rats. Methods : The hemorrhagic shock was induced by withdrawing blood; 20ml/kg of B.W., through the femoral artery in 5 min. The mean arterial pressure was recorded through the femoral artery on a polygraph. Results : In the present investigation, the lung tissue myeloperoxidase activity, protein contents and leukocyte counts, in bronchoalveolar lavage fluid, increased significantly 2 and 24 h after the hemorrhage induction. Although the decreased mean arterial pressure spontaneously recovered, acute lung injury occurred after severe hemorrhage. These changes were effectively prevented by a single intravenous injection of aspirin (10 mg/kg of B.W.) 30 min before the hemorrhage. Conclusion : These results suggest that severe hemorrhage-induced acute lung injury is mediated, in part, by the activation of cyclooxygenase. Furthermore, pretreatment of aspirin in acute lung injury-prone patients, or prophylactic treatment of aspirin to the patients with precipitating conditions, could be helpful in the prevention of acute lung injury.

Design and Construction of a Bottoming Organic Rankine Cycle System for an Natural Gas Engine (가스엔진용 유기랭킨사이클의 설계 및 제작)

  • ORC system was designed and constructed for utilizing the heat of the exhaust gas and coolant released from the gas engine which was modified to use natural gas as a fuel. In this paper the components of the ORC system were designed and manufactured based on measured data of the gas engine. The components are composed of two plate heat exchanger, the 5kW-class expander and multi stage centrifugal pump. The thermodynamic performance of the ORC system was analyzed by using the electric heater. Also, the developed ORC system was implemented to modified natural gas engine. Two gas engines were used to supply heat to the ORC system. As a result of test bench, when the heat source temperature is $110^{\circ}C$ expander shaft power, the pressure ratio and cycle efficiency is 5.22kW, 7.41, 9.09%. As a result of field test, when the heat source temperature is $86^{\circ}C$ expander shaft power, the pressure ratio and cycle efficiency is 2kW, 3.75, 6.45%.

Air-side Heat Transfer and Pressure Drop of a Fin-and-Tube Heat Exchanger Under Low Temperature Condition (저온 조건에서 핀-관 열교환기의 공기측 열전달 및 압력손실)

  • Currently, residential air conditioners operate as a heat pump during winter. In this case, the outdoor heat exchanger acts as an evaporator obtaining heat from cold air. On the other hand, it acts as a condenser during summer transferring heat to hot air. The outdoor temperature changes significantly from high to low. Generally, the air-side j and f factors are obtained at a standard outdoor temperature. Therefore, the applicability of the j and f factors under different outdoor conditions needs to be checked. In this study, tests were conducted for a two-row louver finned heat exchanger changing the outdoor temperature to subzero. The effects of the tube-side brine flow rate were also checked. The results showed that air-side j and f factors were essentially constant and independent of the outdoor temperature, suggesting that an extension of j and f factors obtained under standard conditions to a low outdoor temperature is acceptable. All j and f factors agreed within 9% and 3%, respectively. Tests were also conducted by changing the coolant flow rate. Both the j and f factors did not change according to the flow rate, suggesting that the tube-side heat transfer correlation is acceptable.

  • A remote monitoring panel and control system was developed to control various valves and access control chambers, including gas shutoff valves used in CBR(Chemical, Biological and Radiological) facilities. The remote monitoring panel consisted of a main panel installed in the NBC (Nuclear, Biological and Chemical) control room and auxiliary panel installed in the clean room, and the size was divided into pure control and control including CCTV. This system can be monitored and controlled remotely according to the situation where an explosion door and gas barrier door can occur during war and during normal times. This system is divided into normal mode and war mode. In particular, it periodically senses the operation status of various valves, sensors, and filters in the CBR facilities to determine if each apparatus and equipment is in normal operation, and remotely alerts situation workers when repair or replacement is necessary. Damage due to the abnormal operation of each device in the situation can be prevented. This enables control of the blower, supply and exhaust damper, emergency generator, and coolant pump according to the state of shutoff valve and positive pressure valve in the occurrence of NBC, and prevents damage caused by abrupt inflow of conventional weapons and nuclear explosions.

Evaluation of Dust Removal Efficiency on Roadway Structures Using Ultrafine Bubble Water Jet (초미세기포 water jet을 이용한 도로 시설물 분진 제거 효율 평가)

  • A road structure washing vehicle equipped with a 4 HP, 80 LPM ultrafine bubble generator was used to clean a tunnel wall and the surface of the surrounding structure, consisting of concrete and tiles, in a heavy traffic area around an apartment complex in the city. Ultrafine bubbles were generated by supplying air at 2 to 3 LPM and using a specially designed nozzle, whereas fine bubbles made by an impeller in a gas-liquid mixing self-priming pump were produced with an average diameter of 165.4 nm and 6.81 × 107 particles mL-1. Using a high pressure washer gun that can perform high-pressure cleaning at 150 bar and 30 LPM, ultrafine bubbles were used to wash dust adsorbed on the surface of the road structures. The experimental analysis was divided into before and after washing. The samples were analyzed by applying ISO 8502-3 to measure surface contamination of dust adsorbed on the surface. Using the transparent tape attached to the surface, the removal rate was calculated by measuring the weight of the dust, and the number of particles was calculated using the gravimetric method and the software, ImageJ. The results of the experiment showed that the number of dust particles adsorbed on the tile wall surface before and after washing were 3,063 ± 218 particles mL-1 and 20 ± 5 particles mL-1, respectively, with weights of 580 ± 82 mg and 13 ± 4 mg. Particles on the surface of the concrete structure before and after washing were 8,105 ± 1,738 particles mL-1 and 39 ± 6 particles mL-1, respectively, with weights of 1,448 ± 190 mg and 118 ± 32 mg.
