맥진 데이터의 온톨로지 표현과 진단 서비스 추론 시스템 (Ontology Representation of Pulse-Diagnosis Data and an Inference System for the Diagnosis Service)

  • 본 논문에서는 온톨로지로 구축된 맥 정보를 이용하여 유비쿼터스 컴퓨팅 환경에서 의료 정보 시스템의 상황 인식 서비스를 할 수 있도록 하는 기반 구조를 제안한다. 피검사자의 웨어러블 신호, 온도, 습도 그리고 시간 등을 요소로 사용하는 진맥 인식을 통하여 얻어진 맥진 데이터를 온톨로지로 표현하고 이를 바탕으로 진료 서비스 시나리오를 작성하는 진단 서비스 추론 시스템을 설계 및 구현하였다.

편작학파(扁鵲學派)의 진단(診斷)에 관한(關) 연구(硏究) (A Study on Diagnosis of the Bianque's School)

  • Objectives : Literatures related to Bianque are studied to discover the path of development and the impact of Bianque school's pulse diagnosis system. Methods : Texts regarding Bianque were searched in history books such as Shiji and Zhanguoce, and medical texts such as the medical books of Mawangdui Han Tomb, Huangdineijng, Maijing, and Qianjinyifang to understand how the Bianque school's pulse diagnosis system was developed. Results : 1. Bianque school's pulse diagnosis system was used to inspect the distribution pattern of blood vessels and discover the location of the disease including the palpatation realm such as only hard or only fall. 2. The system of inspection was created when the diagnosis method that uses the color of the pulse by using the color of blood vessels was added to the diagnostic method of pulse condition. 3. Adding the concept of pulse to the visual information that derives from pulse condition becomes pulsation. This is a diagnostic method that falls under the realm of palpation, and it was used to discover the location of disease. 4. The qi of pulse is motor that induces pulse, and this concept is used in order to understand how normal and abnormal pulsations appear, and to treat the circulation disorder of qi and blood. 5. Cubit skin examination is a method that comprehensively take into account the upper arm skin's cold and heat, slippery and roughness, and relax and tension state. This method was used together with other diagnostic methods. As described above, it seems that the diagnostic method with blood vessels used by Bianque school seems to have developed from Bianque's special inspection ability to the stage where it uses palpation, and then to the stage of cubit skin examination which uses both palpation and inspection.

  • Pulse diagnosis is one of the typical examination methods in traditional oriental medicine. However, it is necessary to obtain the objectivity for the standardization of this diagnosis. For this purpose, the pulse analyzer has been devised and studied, and the tonometric pulse analyzer displaying the pulse wave and its variables was developed recently. In this study, we investigated the effect of the sexuality of the individuals on the differences of the pulse wave as well as the correlation between the weight and the pulse wave to interrogate whether the pulse difference is not caused by the difference of sex but the size of individual.

실을 통한 맥진, 소위 현사진맥(懸絲診脈)에 관하여 (On the Pulse Diagnosis via a Thread, Namely "Xuanxizhenmai")

  • Objectives Although the faith that pulse diagnosis via a thread, namely "Xuanxizhenmai", had been applied to some women in royal families, is widely spread in East Asian countries, but it is still controversial that whether this faith is based on historical facts or just originated from some folk tales. So we provided some reasonable clues to interpret that faith. Methods The digitalized Annals of Joseon Dynasty and Twenty Five Books of Chinese History were used for historical example search. Conventional internet search engines are widely used for investigation of other examples and related interpretations. Additionally, a pilot observation with nylon threads and optical vibration detection devices was performed to confirm it's feasibility. Results Although there are a few evidences supporting Xuanxizhenmai's existence in Qing dynasty, no evidence was found to show it's existence in authoritative annals of Korea and China. The pilot observation showed that in optimal environment, some intense arterial pulse could be propagated dozens of centimeter, but it was not applicable to clinical needs. Conclusions Pulse propagation via a thread was proved to be reproducible within limited extents, but pulse diagnosis via a thread, namely Xuanxizhenmai, seem to have never been used for proper clinical purpose.

식사가 정상인의 맥에 미치는 영향 분석 (Effect of Taking Meal on Pulse Diagnosis in Healthy Subjects)

  • The pulse diagnosis studies reported to date has mainly been performed to clinically reveal the pulse wave characteristics according to the specific diseases, whereas no attempts have been made to study the effects on the pulse wave characteristics of the daily activities such as taking meals, exercise, and sleep, etc. This work reports the effect of feeding stimulus on the healthy subjects on the pulse wave pattern which has quantitatively been analyzed using the objective model for the pulse diagnosis in oriental medicine. The pulse waves right before/after the meal and 30 minutes after the meal were measured using the pulse analyzing equipment (3D-Mac, Daeyo Medi, Korea) and at the same time oriental medicine doctors' diagnoses were given. The pulse parameters obtained from the equipment and clinical records on the subjects were statistically processed and the variables showing statistically significant differences were analyzed. The results indicate that the pulse pressure, the pulse rate, and the respiratory rate increase while the blood pressure decreases after the meal. For the floating/sinking and the deficient/excess coefficients characterizing the pulse states described in the oriental medicine, the floating/sinking coefficients were observed to decrease whereas the deficiency/excess coefficients increase after the meal. The results indicates that besides the standard bio-indicators like blood pressure and respiratory rate, etc., the pulse wave characterization in terms of the pulse classifications in the oriental medicine using the floating/sinking, deficient/excess pulse states provide an important piece of biomedical information.

5단계 가압에 대한 맥파 변화 분석에 의한 맥 패턴 분류와 부침맥(浮沈脈) 연구 (A study on floating and sinking pulse by classification of pulse pattern through analysis of P-H volume-curve at 5 applied pressure levels)

  • Objectives: The information on the depth where pulse wave appears is as important as pulse waveform. The aim of this study was to classify pulse pattern using pressure-height(P-H) volume-curve by 5 applied pressure levels to find out the information on the depth of pulse and interpret the floating & sinking pulse in oriental medical pulse diagnosis. Methods: We used 3 dimensional pulse imaging analyser (DMP-3000, DAEYOMEDI Co., Korea), which measures radial pulse waveforms noninvasively by way of tonometric method at 5 applied pressure levels, and shows P-H volume-curves by applied pressure. 448 subjects were enrolled, pulse waveforms were measured and the P-H volume-curves were gained on the three locations of Chon, Kwan, and Cheok. Results: Gained P-H volume curves were classified into 3 types ; increase type, decrease type, and increase-decrease type. Increase-decrease type appeared more often on Chon and Kwan, while increase type appeared more often on Cheok. In a few cases, decrease-type appeared on Chon and Kawn, however it never appeared on Cheok. Conclusions: Through the classification of pulse by P-H volume-curve, we gained the information on the depth of pulse. We speculate the decrease type as floating pulse, the increase-decrease type as middle pulse, and the increase type as sinking pulse in oriental medical pulse diagnosis. After more researches on P-H volume-curve by applied pressure, the P-H volume-curve may be used as an important factor for pulse diagnosis.

중풍환자의 변증에 따른 맥상의 분포 (Study on the Pulse Diagnosis for Pattern Identifications in Stroke Patients)

  • This study is aim to evaluate pulse diagnosis as indicators for classification of the pattern identifications in stroke patients. To get the clinical information, we recruited the onset 1 month stroke patients through the multicenter network which consists of 13 oriental hospitals. The clinical informations about three pairs of pulse wave form and levels of their significancy based on the case report form (CRF) were collected and their distribution in each pattern identification were analyzed. The results are as follows Fire-Heat group shows high portions of floating pulse, rapid pulse and solid pulse. Qi Defficiency group has a greater portion of deep pulse, slow pulse, deficient pulse. The well-defined character of Phlegm-Retained Fluid, Yin Defficiency, Static Blood groups cannot be explained by pulse wave form. These results show a rough relationship between the pulse diagnosis and pattern identifications of stroke therefore, further studies are required to determine the pulse diagnosis as significant indicators of stroke pattern identification.

『찬도방론맥결집성』의 장원소 「진맥입식해」 연구 (A Study on the 「Zhenmairushijie」 by Zhang Yuansu in 『Chandobangronmaekkyulgipseong』)

  • Objectives : By studying the contents of Zhang Yuansu's "Zhenmairushijie", which are introductory remarks of "Chandobangronmaekkyulgipseong", this study attempts to clarify the academic meaning expressed in it and consider his real significance. Methods : First, based on previous studies on Zhang Yuansu and "Wangshuhemaijue", this study divides the contents of "Zhenmairushijie" into 4 chapters and read out the original text. Next, based on Zhang Yuansu's notes and other writings on the original text, this study examines contents in detail. Finally, based on the discussion, this study examines the current significance of academic thoughts expressed in Zhang Yuansu's "Zhenmairushijie". Results & Conclusions : "Zhenmairushijie" emphasizes the combination of nervation and Byeonggi in the process of feeling the pulse for diagnosis, the combination of Byeonggi and Yongyak to declare that the feeling of the pulse for diagnosis is the principle of differential diagnosis. The combination principle of nervation-Byeonggi can be found in comprehensive pulse methods of "Nanjing", and the combination of Byeonggi-Yongyak should follow Ohaeng's Bubuheoshilsajeongbosa principle. Pulse methods of "Wangshuhemaijue" integrated Byeonggi expressed in Uigyeong and Byeonggi in Gyeongbang in the process of the feeling of the pulse for diagnosis to present the principle of diagnosis to perform Byeongjeungronchi. Therefore, "feeling the pulse for diagnosis ipsik" is not only an introductory remark of feeling the pulse for diagnosis, but an comprehensive remark of whole diagnosis as well. It is an introductory remark of the entire medical field due to the nature of oriental medicine which emphasizes diagnosis.

3 Stage 2 Switch Application for Transcranial Magnetic Stimulation

  • Transcranial magnetic stimulation utilizes the method of controlling applied time and changing pulse by output pulse through power density control for diagnosis purposes. Transcranial magnetic stimulation can also be used in cases where diagnosis and treatment are difficult since output pulse shape can be changed. As intensity, pulse range, and pulse shape of the stimulation pulse must be changed according to lesion, the existing sine wave-shaped stimulation treatment pulse poses limitations in achieving various treatments and diagnosis. This study actualized a new method of transcranial magnetic stimulation that applies a 3 Stage 2 Switch( power semiconductor 2EA) for controlling pulse repetition rate by achieving numerous switching control of stimulation coil. Intensity, pulse range, and pulse shape of output can be freely changed to transform various treatment pulses in order to overcome limitations in stimulation treatment presented by the previous sine wave pulse shape. The method of freely changing pulse range by using 3 Stage 2 Switch discharge method is proposed. Pulse shape, composed of various pulse ranges, was created by grafting PFN (Pulsed Forming Network) through AVR AT80S8535 one-chip microprocessor technology, and application in transcranial magnetic stimulation was achieved to study the output characteristics of stimulation treatment pulse according to delaying time of the trigger signal applied in section switch.
