Polymerization of light-activated restorations results in temperature increase caused by both the exothermic reaction process and the energy absorbed during irradiation. Within composite resin, temperature increases up to 2$0^{\circ}C$ or more during polymerization. But, insulation of hard tissue of tooth lowers this temperature increase in pulp. However, many clinicians are concerned about intrapulpal temperature injury. The purpose of this study was to evaluate temperature changes in the pulp according to various restorative materials and bases during curing procedure. Caries and restoration-free mandibular molars extracted within three months were prepared Class I cavity of 3$\times$6mm with high speed handpiece fissure bur. 1mm depth of dentin was evaluated with micrometer in mesial and distal pulp horns. Pulp chambers were filled with 37.0$\pm$0.1$^{\circ}C$ water to CEJ. Chromium-alumina thermocouple was placed in pulp horn below restorative materials for evaluating of temperature changes. This thermocouple was connected to temperature-recording device(Multiplication analyzer MX, 6.000, JAPAN). Temperature changes was evaluated from initial 37.$0^{\circ}C$ after temperature changes to 37.$0^{\circ}C$. Tip of curing unit was placed in the center of prepared cavity separated 1mm from restorative materials. Curing time was 40s. The restorative materials were used with Z 100, Fuji II LC, Compoglass flow and bases were used with Vitrebond, Dycal. Resrorative materials were placed in 2mm. The depth of bases were formed in 1mm and in this upper portion, resin of 2mm depth was placed. This procedure was performed 10 times. The results were as follows. 1. All the groups showed that the temperature in pulp increased as curing time increased 2. The temperature increase of glass ionomer was significantly higher than that of Resin and Compomer during curing procedure (P<0.05). 3. The temperature increase in glass ionomer base was significantly higher than that of Calcium hydroxide base during Resin curing procedure (P<0.05).

The purpose of this study was to investigate early histological changes and calcitonin gene-related peptide (CGRP) expression in the dental pulp of the rat after Er:YAG laser preparation. Occlusal cavities were prepared in the upper first molars using either Er:YAG laser and conventional bur. At 48 hours after cavity preparation, the teeth were processed for hematoxylin-eosin stain and CGRP immunohistochemistry. The results were as follows : 1. The cavity floor by Er:YAG laser preparation was more irregular shape compared with those by bur preparation and there are some cracks in the directions of dentinal tubules. 2. There were more inflammatory cell infiltration and disruption of odontoblast in the dental pulp by Er:YAG laser preparation in comparison with the dental pulp by bur preparation. 3. CGRP expression in the pulp tissue by both Er:YAG laser and bur preparations were increased and higher than in the normal pulp. The expression pattern of CGRP was more strong in the pulp by Er:YAG laser preparation. These results indicate that Er:YAG laser is useful in the operative dentistry such as caries removal and cavity preparation if properly applied.
Objectives: The purpose of this study was to investigate the involvement of TRPA1 in the cinnamaldehyde-induced pulpal blood flow (PBF) change in the feline dental pulp. Materials and Methods: Mandibles of eight cats were immobilized and PBF was monitored with a laser Doppler flowmetry at the mandibular canine tooth. To evaluate the effect of cinnamaldehyde on PBF, cinnamaldehyde was injected into the pulp through the lingual artery at a constant rate for 60 seconds. As a control, a mixture of 70% ethanol and 30% dimethyl sulfoxide (DMSO, vehicle) was used. To evaluate the involvement of transient receptor potential ankyrin 1 (TRPA1) in PBF change, AP18, a specific TRPA1 antagonist, was applied into the pulp through the Class V dentinal cavity followed by cinnamaldehyde-administration 3 minutes later. The paired variables of experimental data were statistically analyzed using paired t-test. A p value of less than 0.05 was considered as statistically significant. Results: Administration of cinnamaldehyde (0.5 mg/kg, intra-arterial [i.a.]) induced significant increases in PBF (p < 0.05). While administration of a TRPA1 antagonist, AP18 (2.5 - 3.0 mM, into the dentinal cavity [i.c.]) caused insignificant change of PBF (p > 0.05), administration of cinnamaldehyde (0.5 mg/kg, i.a.) following the application of AP18 (2.5 - 3.0 mM, i.c.) resulted in an attenuation of PBF increase from the control level (p < 0.05). As a result, a TRPA1 antagonist, AP18 effectively inhibited the vasodilative effect of cinnamaldehyde (p < 0.05). Conclusions: The result of the present study provided a functional evidence that TRPA1 is involved in the mechanism of cinnamaldehyde-induced vasodilation in the feline dental pulp.
Radiography is one of the important tool adopted in daily dental practice and medical diagnosis. To visualize soft tissuechange various contrast media has Been introduced. Any cavity or space can be easily determined by increasing the X-ray absorption of the cavity using the radiopaque contrast media which widely employed in medical radiography to show much of the digestive, cardiovascular, pulmonary, and renal system. The essential part of any radiopaque medium is a heavy element that can absoarb most of the X-ray beam. The element must be noninjurious and easily eliminated. Both aqueous and oil suspensions of iodine containing compounds' are available to the dental profession, for example Lipiodol and Dionosil. The study was designed to determine toxic effect of Lipiodol to the vital pulp and to confirm visualization ioprovement in pulp canal. 1. Thin mixture of Calcium hydroxide and Lipiodol was applied to 19 deep vital cavities for 24 hours. Only one case complained slight pain for short time. 2. Cotton pellet over-saturated in Lipiodol was inserted in coronal chamber of which 6 were non vital and 5 were vital. The transference of Lipiodol was not noticed in every case after 24 hours. 5 cases with vital pulp tissue in the canals showed no clinical symptome. 3. Extracted 20 teeth were routinely prepared for endodontic treatment and applied Lipiodol in conjunction with cotton fiber as deep as midportion of the roots. After 24, hours the medicament reached to nearly the end of apex, but there were no evidence of penetration in dentine layer and migrate into ramified canal.
The author experienced a failed ase of root resection which qas performed on the abnormal upper right lateral incisor. The patient was 16year old female. Clinically the effected tooth had neither a carious cavity nor periondontal diseases, ut, a fistula was present on the gum tissue near the root apex. The roentogenogram revealed a pathological area around the apex of the tooth. The results of failed apicoectomy lead to following conclusions. 1. The operated tooth was a case of dense invaginatus which had two pulp cavities and two canals seperated along their entire length. 2. It was the main cause of failure that the second pulp cavity and root canal were not obturated.
This study was performed to reveal the effect of self-curing acrylic resin to dental pulp when the resin was used directly on the freshly cut dentin. Class V cavity preparations were made on the teeth of dogs with No. 701 carbide bur at a speed of 200,000 rpm and with an air water spray. Petrolatum jelly was coated on the inner surfaces of the cavities. After that, doughs of self-curing acrylic resin were filled in these cavities and before completely set these were removed and cemented with zinc oxide and eugenol cement. The dogs were sacrified between 1 day and 3 weeks at the intervals of one week and routinely processed with hematoxylin and eosin stain for histopathologic interpretation. The results were as followings; 1. The inflammatory changes of the pulp can be seen after the use of self-curing acrylic resin on freshly cut dentin. 2. The inflammatory changes of the pulp were not completely subsided until the end of third experimental week.
Purpose: The primary objective of this study was to evaluate the change in the temperature of the adhesive resin in polycrystalline ceramic brackets irradiated using a diode laser at different irradiation energy levels and times. Materials and Methods: For the measurement of the temperature of the adhesive resin, it was applied at the base of the ceramic bracket, a thermocouple was placed at the center of the base surface, the bracket was placed on prepared resin specimens for light curing, and a laser was irradiated to the center of the bracket slot at 5, 7, and 10 W. For the measurement of the temperatures of the enamel under the bracket and pulp cavity, extracted premolar was fixed to a prepared mold and the ceramic bracket was bonded to the buccal surface of the premolar. The Kruskal-Wallis H test and Friedman test were used for statistical analysis. Result: At 5 W, the temperature of the adhesive resin did not reach the resin softening temperature of 200℃ within 30 seconds. At 7 W, it reached 200℃ when the ceramic bracket was irradiated continuously for 28 seconds. At 10 W, it reached 200℃ when the ceramic bracket was irradiated continuously for 15 seconds. During laser irradiation, the temperature of the enamel under the bracket increased by over 5℃ within 15 seconds. Conclusion: The use of diode laser irradiation for bracket debonding should be carefully considered because the pulp cavity temperature increases by over 5℃ within the irradiation time for resin thermal softening.
Purpose: Exact knowledge of the location and dimension of the pulp chamber help to maintain the pulp healthy during operative procedure and also reduces the risk of perforation of pulp chamber during root canal treatment. This in-vivo study was carried out to measure critical morphology of pulp chamber of mandibular molar using intra-oral periapical radiograph. Materials and Methods: Mandibular molar teeth of 56 patients were evaluated. The mandibular molar teeth whose pulp chamber was not violated by caries, restoration, fracture crown and those having closed apex were included in the study. The intraoral periapical radiographs were taken with paralleling angle technique using radio-opaque grid with 1 mm space. This grid was placed directly on the film. Results: In 94% of the mandibular first molars specimens the pulp chamber ceiling was at the level of the cemento-enamel junction. The measurements showing the lowest percentage variance were buccal cusp to furcation (approximately 11%) and buccal cusp to pulp chamber ceiling (approximately 15%). The distance from the cusp tip to pulp chamber ceiling height was approximately 6.0 mm, the distance from the pulpal floor to the furcation was approximately 3.0 mm, and the average height of a pulp chamber was 1.5 to 2.0 mm. Conclusion: The exact knowledge of distances of pulp chamber from various anatomical landmarks helps in proper assessment of root canals and ultimately avoids the failure of root canal treatment.
The purpose of this study was to evaluate the effects of bleaching agent through the dentinal tubules of cervical area in the intracoronal bleaching of pulpless teeth on cutured fibroblast cells. Extracted human incisors were enlarged to # 40 K-file and obturated with gutta-perella and AH 26 sealer. The gutta-percha was removed to 2mm below the cementoenamel junction of the root The teeth were divided into 3 experimental and control groups. Experimental groups; Experimental group 1: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity. Experimental group 2: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after placement of ZOE cement to cementoenamel junction. Experimental group 3: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after application of Copalite to cementoenamel junction. Control group: Temporary inlay wax filled without 30% $H_2O_2$ in pulp cavity under the same condition at each experimental group. Each tooth was immersed in well of multidish cultured fibroblast cell for 48 hours. The cellular multiplication and cell viability were calculated at the interval of 1, 3, 5. 7 hours and the morphological changes in well were observed and their photographs were taken with inverted microscope. The obtained results were as follows : CD The cellurar multiplicaton and cell viability decreased in all experimental groups at 1 hour after experiment and the morphology of fibroblast cell was changed from star shape to round (2) The cell viability was lowered to 34 % in experemental group 1, 44 % in experimental group 2, and 38 % in experemental group 3 at 3 hours after experiment (3) The cell multiplication was decreased to 54% in experemental group 1. 47% in experimental group 2, and 40% in experemental group 3 at 7 hours after experiment. (4) The decrease of cell number and morphological changes of fibroblast cell were remarkable in experimental group 1, group 3 and 2 in order. These results suggest that the fibroblast cells receive severe damage by 30% $H_2O_2$ solution leaked through the dentinal tubules and the dentinal tubules are able to be obturated better by ZOE cement than by Copalite.
Purpose: The purpose of present study was to develop a method for assessing the chronological age of Korean adults based on the relationship between age and size of pulp cavity using dental radiographs of mandibular first molars. Methods: A total of 325 dental radiographs of Korean adults with known age and gender were selected for the study (199 males and 126 females) which were taken in the period between January 2009 and June 2014 at the Pusan National University Dental Hospital. The measurements were carried out on both orthopantomographs (OPGs) and intraoral periapical radiographs of mandibular first molar and the following ratios were calculated: pulp chamber floor height ratio (F/L), pulp chamber ceiling height ratio (R/L), and pulp chamber depth ratio (D/L). Results: The ratios of measurements on intraoral periapical images of mandible first molar generally produce more reliable data than the measurements on OPGs. The pulp chamber floor height ratio and pulp chamber thickness ratio showed significant correlation with age, whereas the pulp chamber ceiling height ratios showed weak correlation with age. It was found that the best correlations between the ratios and age were found for pulp chamber thickness ratios (r=-0.731 to -0.751). The multiple regression models were derived using 3 ratios that were significantly correlated with age. The determination coefficients ($R^2$) of the models ranged from 0.556 to 0.596. Conclusions: Our results indicate that the pulp chamber thickness and pulp chamber floor height in mandibular first molar are an age-dependent variable in adults which can be used to estimate age with reasonable accuracy. The higher image quality of dental radiographs will probably narrow the age estimation error and improve dental age estimation.
