Review of Clinical Studies for Korean Medicine Treatment on Nocturnal Enuresis in Children (소아 야뇨의 한의학적 치료에 대한 국내외 임상연구 동향 - 2000년 이후 발표된 연구를 중심으로 -)

  • Objectives The purpose of this study is to identify the research trends and to evaluate the effectiveness and safety of Korean medicine treatment by analyzing the recently published clinical studies of Korean medicine treatment on nocturnal enuresis in children. Methods The literatures were searched from OASIS, KISS, NDSL, RISS, Cochrane, CINAHL, Embase, Pubmed, and only clinical studies published from 2000 to October 2019 were included. The publication year, study type, type of enuresis, demographic information of participants, duration of illness, intervention type and details, follow-up period, outcome measurements and adverse events of selected literatures were analyzed. Results A total of 38 studies were selected. In the selected studies, acupuncture was the most frequently used treatment for nocturnal enuresis, followed by herbal medicine, electrical stimulation treatment, moxibustion, acupressure and plaster therapy. The most commonly used acupoints for acupuncture were SP6 and CV4, and the most used herbal medicine was Chukcheonwhan, while Alpiniae Fructus (益智仁) and Dioscoreae Rhizoma (山藥) were the most used herbal materials. Most of the studies showed that the total effective rate of Korean medicine treatment was high. All the adverse events occurred in the Korean medicine treatment group were caused by acupuncture or puncture. Conclusions This study analyzed clinical studies of Korean medicine treatment on nocturnal enuresis in children, identified the research trends and evaluated the effectiveness and safety of the Korean medicine treatment.

A Review of Clinical Researches on Socheongryong-Tang (소청룡탕의 임상연구 분석)

  • Objective : This study's purpose was to review the clinical studies of Socheongryong-Tang. Method : We searched papers about Socheongryong-Tang using KISS, KTKP, PUBMED, Embase, Science Direct, and the key words "Socheongryong-Tang", "Shoseiryuto", "Xiao Qing Long Tang", and "Minor Green-Blue Dragon Decoction" were used. Papers not matched with inclusion criteria were excluded. Until today, there have been 143 studies on the effects of asthma, allergic rhinitis, common cold, eczema and other various effects of Socheongryong-Tang. Of these, 15 were classified as clinical research papers. Results : There were 4 cases of asthma, 2 cases of allergic rhinitis, 2 cases of common cold, 1 case of pneumonia in patients with dementia, 1 case of eczema, 1 case of drug interaction study, 1 case of ephedrine drug removal, 3 cases of side effects causing pneumonia. Conclusion : It can be seen that Socheongryong-Tang has established the basis for application for the purpose of asthma, allergic rhinitis, wind-cold type cold, preventing pneumonia in patients with dementia, dyshidrotic eczema. On the other hand, considering three cases that cause side effects of pneumonia, it is necessary to confirm whether or not the symptoms of pneumonia are certainly progressing when observing the progress of the patient.

A Systematic Review on Nurse-Led Transitional Care Programs for Discharged Patients from Hospital to Home (퇴원환자를 위한 간호사 주도 전환 프로그램의 내용과 효과에 관한 체계적 문헌고찰)

  • Purpose: This study was to systematically review the contents and effects of nurse-led transitional care programs for discharged patients from hospital to home. Methods: Randomized controlled trials published between 2005 and 2015 were searched in Pubmed, Embase, Cochrane(Central Register of Controlled Trials) and CINAHL. Data were analyzed using Cochrane Review Manager(Revman) software 5.3. Results: Nine studies were selected and analyzed. Patient assessment, education and discharge planning were included in pre-discharge phase. Referring, communication and care planning were performed by nurses in transition phase. Home and phone visits, monitoring and multidisciplinary advices were included in post-discharge phase. Various outcome measures such as hospital utilization(30 days readmission and emergency department visit), quality of life, and cost were used to identify effectiveness of nurse-led transitional care programs. 30 days readmission(OR=.73, 95% CI 0.54, 0.98; p=.03) and emergency department visit(OR=.67, 95% CI 0.50, 0.88; p=.005) were statistically significant in meta-analysis. However, participant blinding was not done in seven studies which put at the risk of performance bias. Conclusion: The results indicated that nurse-led transitional care program is effective in reducing unnecessary hospital utilization. Nevertheless, small sample size and risk at performance bias are the limitation of this study. Thus, we suggest that well-designed randomized controlled trials need to be conducted.

Research Trends of the Ear-Acupuincture Treatment for Menstrual Pain (월경통의 이침 치료에 대한 최신 연구 동향)

  • Objectives: The purpose of this study was to examine clinical studies on the treatment of dysmenorrhea treatments and analyze the results to provide sufficient evidence to provide adequate evidence of the efficacy of the ear-acupuncture treatment on dysmenorrhea patients. Methods: The search for foreign papers used 'Pubmed', a research engine in the America National Library of Medicine. Used searching terms were 'dysmenorrhea' and 'menstrual pain' in all cases. And among these studies, we searched by using key word 'auricular acupuncture' or 'ear acupuncture' or 'auricular acupressure' or 'ear acupressure'. Results: Overall 53 studies, 7 studies were finally selected to this study. There was 1 controlled study, 3 clinical trials, and 3 randomized-control studies. About acupoints in study, the endocrine was used in all 7 papers. Internal genitals, shenmen, sympathesis, kidney, liver were used in 4 papers, central rim, hypo-cortex, uterus were used in 2 papers, central of cymba auriculae was used in 2 paper. In 4 papers, the effect of ear acupuncture treatment was effective in relieving menstrual pain. Moreover, we can see that ear acupuncture therapy was effective in reducing menstrual pain than in oriental herb medicine. Conclusions: As a result of the 7 papers, we found that patients with menstrual pain decreased after ear acupuncture therapy. However, there was little research and it was limited to achieving the results of the analysis. A vigorous clinical study should be conducted to demonstrate the efficacy of ear acupuncture therapy and standardize treatment methods.

The first review study on association of DNA methylation with gastric cancer in Iranian population

  • Background: Gastric cancer (GC) is the second leading cause of cancer-related death worldwide. Several environmental, genetic and epigenetic factors have been suggested to have a role in GC development. Epigenetic mechanisms like histone changes and promoter hyper-methylation are now being increasingly studied. Associations between methylation of many gene promoters with the risk of gastric cancer have been investigated worldwide. Such aberrant methylation may result in silencing of specific genes related to cell cycling, cell adhesion, apoptosis and DNA repair. Thus this molecular mechanism might have a key role in proliferation and migration of cancerous cells. Materials and Methods: In this review article we included studies conducted on DNA methylation and gastric cancer in Iranian populations. Using Science direct, Pubmed/PMC, Springer, Wiley online library and SciELO databases, all published data until 31 January 2016 were gathered. We also searched Science direct data base for similar investigations around the world to make a comparison between Iran and other countries. Results: By searching these databases, we found that the association between methylation of seven gene promoters and gastric cancer had been studied in Iran until 31 January 2016. These genes were p16, hLMH1, E-cadherin, CTLA4, $THR{\beta}$, mir9 and APC. Searching in science direct database also showed that 92 articles had been published around the world till January 2016. Our investigation revealed that despite the importance of GC and its high prevalence in Iran, the methylation status of only a few gene promoters has been studied so far. More studies with higher sample numbers are needed to reveal the relation of methylation status of gene promoters to gastric cancer in Iran. Conclusions: Further studies will be helpful in identifying associations of DNA methylation in candidate genes with gastric cancer risk in Iranian populations.

Current concept for anti-inflammatory mechanism of acupuncture;based on the latest studies (침의 염증 제어 기전에 관한 최근 견해;최신 연구를 바탕으로)

  • Objectives : The acupuncture has been in the classic arsenal of Oriental medicine over inflammatory diseases. Its physiological mechanism is not fully understood but being known better everyday. We reviewed several papers to describe current concepts over anti-inflammatory mechanism of acupuncture. Methods : Some computerized literature searches were done using the key words of 'acupuncture' AND 'Anti-inflammatory' in Medline via Pubmed between March 2007 and December 2007. Only rationally-designed studies were picked among practically associated materials. A well-known hypothesis on acupunctural physiology was adapted for integration. Result : 18 studies were selected. 17 studies were laboratory experiment and 1 was a clinical study. Data was classified into some comprehensive categories. Author's opinion was added at the end of each category. Study results supported the hypotheses on acupunctural physiology; Acupuncture has some influences on autonomic nerve system(ANS). And it stimulates several receptors from target cells like macrophage, and finally inhbits cytokines like TNF-${\alpha}$, IL-1${\beta}$ andIL-10 which are inflammation-mediated. Acupuncture increases the opioid releasing therefore relieves inflammation. And acupuncture inhibits cyclooxgenase(COX) but its mechanism is controversial until now. Conclusion : Current concepts over anti-inflammatory mechanism of acupuncture are as follows. Acupucture suppresses inflammation by stimulation of ANS, increasing of opioid releasing and inhibition of COX. But more studies are needed to fully describe the anti-inflammatory effect of acupuncture.

Study of the Thermal Properties of Warm Needle and the Development of Warm Needle Apparatus (온침의 열특성과 온침기기 개발에 관한 연구)

  • Objectives : To understand the strength and weakness of traditional warm needle acupuncture based on existing research outcomes in Korea mostly and to suggest how to build the desirable warm needle acupuncture apparatus by overcoming demerits of traditional and currently existing ones. Methods : We searched warm needle relating papers in Korean with the key words of '온침, 화침, 열자극' by using DBpia, Journal of Korean Oriental Medical Society, Journal of Korean Acupuncture & Moxibustion Society and Journal of Society for Meridian and Acupoint. Degree dissertations in Korean were also searched with the same words through the search engine of Library of Kyung Hee University. We also searched with words of 'warm, thermal, heat, needle, acupuncture, temperature' via Pubmed and found a small number of English written papers and large number of Chinese written ones. To find english translated version of those papers, we googled with the same words with no success. Results : About 20 papers on warm needle acupuncture written in Korean were found and analyzed with respect to experimental factors that affected the thermal properties or the amount of heat stimulus of the acupuncture. More rigorous descriptions seemed to be required on the insertion depth, duration and manipulation of warm needle treatment. A basic heat transfer model was presented for the clarification of heat loss through the needle of warm needle acupuncture. Environmental factors such as air flow on the properties seems to be considered for the warm needle acupuncture. Conclusions : Papers on warm needle acupuncture were reviewed and analyzed based on their thermal properties and tools such as needles and moxa cones. Several suggestions were made on the descriptions relating the properties. Necessary specifications were introduced to aim modernized warm needle systems.

  • Sinus floor elevation is a predictable and standard procedure for the treatment of the posterior maxilla before insertion of dental implants. Although overall complication rates are low, complications can occur including sinus membrane perforation, infection of sinus, swelling and hemorrhage. The most common complication is membrane perforation. Recently, various techniques and materials for repair of perforation to the sinus membrane have been proposed. The purpose of this article is to report on various techniques and materials that can be used for repair of perforations to the sinus membrane. The search protocol used was the following electronic database: Pubmed, with a time limit from 1998 to 2009. The key words such as 'sinus lift', 'sinus augmentation', 'sinus floor elevation', 'sinus graft', 'sinus perforation', 'repair of sinus perforation' and 'repair of sinus membrane' were used, alone and in combination, when searching the database. Various techniques have been proposed to manage of perforation of sinus membranes. These include that the use of collagen membranes, demineralized freeze-dried human lamellar bone sheets, processed human allografts, lamella bone, buccal fat pads and suturing. Implant success rate ranges from 69.9% to 98.9%.

A Systematic Review of Bee Venom Acupuncture for Ankle Sprain (족관절 염좌의 봉독 약침 치료에 대한 체계적 문헌 고찰)

  • Objectives To evaluate the evidence supporting the effectiveness of bee venom acupuncture for ankle sprain. Methods We conducted search across 11 electronic databases (Pubmed, EMBASE, Cochrane CENTRAL [CENTRAL], KoreaMed, Kmbase, Koreanstudies Information Service System [KISS], National Digital Science Library [NDSL], Korea Institute of Science and Technology Information [KISTI], China National Knowledge Infrastructure [CNKI], Wanfang and Chinese Scientific Journals Database [VIP] database) to find clinical trials that used bee venom acupuncture as treatment for ankle sprain. The methodological quality of randomized controlled clinical trials (RCTs) were assessed using the Cochrane Risk of Bias (RoB) tool, while non-randomized controlled clinical trials (NRCTs) were assessed using the Cochrane Risk of Bias Assessment tool for Non-randomized Study (RoBANS) tool. Results Four RCTs and one nRCT met our inclusion criteria. Almost studies showed that bee venom acupuncture has positive effect on ankle sprain. 4 studies of same intervention and control included in the meta-analysis. When comparing bee venom acupuncture and acupuncture with acupuncture, the effect size of standardized mean difference (SMD) was -0.19 (95% confidence interval [CI]: -1.95~1.56, Z=0.55, p=0.83). And when comparing bee venom acupuncture with acupuncture, the effect size of SMD was -0.17 (95% CI: -0.65~0.31, Z=0.71, p=0.48). Conclusions Although our systematic review found encouraging but limited evidence of bee venom acupuncture for ankle sprain, most of the studies included in the analysis were evaluated as methodologically high risk of bias. From now on further well-designed RCTs should be encouraged.

Chuna Manual Therapy for Headache : A Systematic Review and Meta-analysis (두통에 대한 추나요법의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives : To determine the effectiveness of Chuna manual therapy for headache Methods : We searched 6 electronic databases(Pubmed, CAJ, Oasis, RISS, DBPIA, KoreanTK) and 2 journals up to Oct 2015. We included randomized controlled trials(RCTs) using Chuna manual therapy for headache. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results : 9 RCT studies were eligible in our review. The meta-analysis of 6 studies showed favorable results for the use of Chuna manual therapy. High risk of bias were observed in all studies. Conclusions : Although there are favorable results with meta-analysis, our systematic review arehighly dependent on the single source of Chinese electrical database, CAJ. Now limited evidence is available tosupport Chuna manual therapy for headache and further well-designed RCTs should be encouraged.
