Static and Dynamic Analysis of Efficiency of Korean Regional Public Hospitals (지방의료원의 효율성에 대한 정태적 및 동태적 분석)

  • The purpose of this paper is to analyze the efficiency change and its determinants of the regional public hospitals. We utilize 34 regional public hospital's panel data for 6 years from 2003 to 2008. We use DEA(Data Envelopment Analysis)-CCR, BCC model, DEA/Window model, and DEA Profiling. The empirical results show the following findings. First, technical efficiency shows that approximately 3.6% of inefficiency exists on the regional public hospitals and it reveals that the cause for technical inefficiency is due to scale inefficiency. Second, DEA/Window results show that the stable dissimilarity by standard deviation, LDP of CCR. Third, the results of partial efficiency by DEA Profiling show that increase efficiency depends on the number of beds, doctors, and nurses.

Two Cases of Surgical and Medical Treatment of Infectious Intracranial Aneurysms - Case Report - (감염성 두개강내 동맥류의 수술 및 내과적 치험 2례 - 증 례 보 고 -)

  • Object : To determine whether to use surgical or medical therapy in treatment of infectious intracranial aneurysms, we reviewed two recent cases of infectious intracranial aneurysms and others known previous reports of aforementioned cases. Hence, we attempted to compare the validity and effectiveness of surgical and medical treatment. Method : Recently, we treated two cases of ruptured infectious intracranial aneurysms. In former case, the aneurysm was located distal to the middle cerebral artery in a patient with mild mitral regurgitation of the heart. In latter case, the aneurysm was multiple with varying hemorrhage. The hemorrhage was located bilaterally and a moderate mitral regurgitation and infective endocarditis were accompanied in this patient. Result : Due to the large size of the intracranial hematoma, stable medical condition, and easy resectability, we treated the former patient surgically. And, because of successive hemorrhage by multiple aneurysmal rupture, and the risk of heart failure, we treated the latter patient medically with serial follow-up angiography. Both patients are at present in good health. Conclusion : Because of the variability in associated factors, such as the patient's health, the number of lesions, location, anatomy of the aneurysms and the causative organism, each patient's care must be individualized and tailored to the patient's particular clinical situation.

Neurenteric Cyst in Upper Thoracic Spinal Canal - Case Report - (상흉추강내에 발생한 신경장 낭종 - 증 례 보 고 -)

  • Spinal neurenteric cyst results from the persistence of an abnormal communication between endodermal and ne-uroectodermal layer. Embryologically, neurenteric cyst is derived from endoderm that is fused with the developing notochord during the third week of gestation. It is a rare malformation that lead to spinal cord compression. The patient is 19-year-old male presented with chest pain, paresthesia and progressive weakness in his low extremities(grade II/II). Preoperative MR imaging revealed intradural extramedullary cyst with intracystic hemorrhage in T1 and T2 level that is ventrally located and compressed the spinal cord. Involved vertebral bodies were scalloped and fused. The cystic tumor were totally removed through costotransversectomy approach. Postoperatively, motor weakness of the low extremities were improved to the level of grade IV/V. And chest pain and paresthesia were gradually disappeared. Postoperative MR imaging showed the decompression of the thoracic spinal cord. Histologic examination revealed a ciliated columnar epithelial neurenteric cyst. The pre- and postoperative clinical, radiological features of a case of upper thoracic neurenteric cyst is described with review of literature.

The Role of Radiologic Study in Diagnostic Work-up of Headache Patients (두통환자에 대한 방사선학적 검사의 진단적 가치)

  • Objective : The goal of this study is to identify the significant radiologic abnormalities in patients complaining headache and to determine predictive factors for clinically significant radiological abnormalities. Method : The study population was 410 patients having underwent CT or MRI study among 1000 patients complaining headache in outpatient basis between 1996-1999. All of these patients answered self-administered questionaire about their headaches. We reviewed the patient's charts and the questionaires and examined the radiologic study results. Result : Of the 410 patients referred for CT or MRI study, male : female ratio was 1 : 1.97. Twenty-five patients(6.1%) revealed clinically significant organic lesions. Mean age was 46.1 in radiologic abnormal group and 48.4 in normal group. Short symptom duration(p<0.01), motor weakness(p<0.05), vomiting(p<0.05), cranial nerve palsy(p<0.05), and trauma history(p<0.05) were factors indicated higher incidence of radiologic abnormality. But, patients age, and severity of headache were not associated with clinically significant radiologic lesion. The ratio of radiologic abnormality was 0.8% in patients not having any risk factor. Conclusion : The results indicate that radiologic study should be done in headache patients having the risk factors such as short symptom duration, motor weakness, vomiting, cranial nerve palsy, trauma history. For patients without any such a risk factor, the radiologic study doesn't seem mandatory.

  • Changed the business environment, private sector focuses on the performance management, so to manage the performance he uses BSC that overcomes the limitation of financial indicators. Also, it is getting emphasized to evaluate the performance about services that the public sector provides and obliged for the public sector to be responsible for managing and improving its performance as the private sector has been doing. But lacks of knowledge and experience on performance management and evaluation are blocking the public sector from introducing and implementing new performance management systems. To solve those problems, this study has derived general guidelines and KPIs that public sector should keep in mind when BSC is introduced new management system. This study focused on the public hospital which is the performance management and evaluation system of Government administration.

Comparative Analysis of Value Added to Personnel Expenses between General Hospitals and Regional Public Hospitals (민간종합병원과 지방의료원의 인건비 투자효율성 비교분석)

  • Objectives : This study compared Value added to personnel expenses between 12 private general hospitals and 12 regional public hospitals, based on location and size, to examine industry competitiveness in terms of management. Methods : From 2011 to 2015, the value added and value added to personnel expenses were calculated by year. and a SPSS statistical program was used to determine and influential factors between private general hospitals and regional public hospitals. Results : The total value added to personnel expenses was 26.85 percent lower than general hospitals compared to regional public hospitals. The product category most influenced by the value added to personnel expenses was Stationeries & expendables at general hospitals and Outsourcing at regional public hospitals. Conclusions : Regional public hospitals have relatively low value added to personnel expenses compared to the general hospitals. Therefore, it is necessary for hospital management to reexamine gross revenue relative to total manpower.

  • The purpose of this study is to understand the context of mental hospital admission and discharge related with the stakeholder and to find out issues about public management on mental health disabilities. To this process, the more effecive alternative policies for mental health will be offered. As a research tool, the qualitative study was used, and the 6 case were analyzed. Through this study, we find 5 theme such as admission type occurred outside public management, uneffective public management in mental hospital treatment, long-term treatment mechanism occured in mental hospital, helplessness and role distortion of public follow-up system. According to the mental health law, mental hospital admission and discharge on mental health disabilities is to managed public system. But public management with mental health disabilities did not work on. In this results, we are find infringement on mental health disabilities. According to these findings, several suggestions are offered for better policies about admission and discharge procedures managed by public system.

Process Evaluation and Influencing Factors of Building up a Network System between a National University Hospital and a Public Community Hospital in a Korean Provincial Area (한 광역자치도에서 국립대병원과 지방의료원간 협력체계 구축 사례의 성립 요인 및 과정평가)

  • This study aimed to explore influencing factors on the establishment of the network system between public hospitals and to make a process evaluation of it. we analyzed the case of a strategic alliance contracted by a National University Hospital(NUH) and a Community Hospital(CH). Main points of the project were regular dispatch of clinical specialists in the NUH such as gastroenterologist and running teleradiology program. The NUH considered the improvement of it's image as a public hospital as a successful element of the network program. The provincial office which have to manage the CH satisfied with these program in terms of helping CH in need of clinical specialists. Staffs in the CH pointed out the problem of discontinuity for patients who visited the CH. Three institutes argued that continuous support of central government in the relevant institution and budget could play the most important role in the advance of the network system between public hospitals.

The patient satisfaction of medical services and the revisiting intention in a public hospital (일개 공공병원의 의료서비스 만족도와 재이용의사에 관한 연구)

  • The aim of this study was to provide the basic data for the betterment of public medical services. A questionnaire survey on the quality of the services and the intention of revisiting the hospital was administered to 330 patients in a public hospital located in Seoul. The mean satisfaction of hospital medical services was 4.11, whereas that of the revisiting intention was 3.94. The relationship between the satisfaction of medical services and the intention of revisiting was significant (t=11.335, p<.001). The intention of revisiting increased with increasing satisfaction of medical services of a hospital. Therefore, to enhance satisfaction, a hospital should identify the needs of people looking for a public hospital, increase rapport with the neighboring communities, and provide public amenities.
