우리나라 직장인 스트레스의 역학적 특성 (Epidemiology of Psychosocial Distress in Korean Employees)

  • Objective : To estimate the magnitude of psychosocial distress and examine eligible factors associated with the development of psychosocial distress in Korean employees, using a nationwide sample. Methods : A total of 6,977 workers were recruited from 245 companies. A structured questionnaire was used to assess sociodemographics, health-related behaviors, job characteristics, social support at work, personality traits (locus of control, type A behavior pattern), self-esteem, and psychosocial distress. Results : The results showed that 23 % of workers were categorized as high stress, 73% as moderate, and 5% as normal. Hierarchical multiple regression analysis showed that psychosocial distress was more common in younger workers, both male and female. Regular exercise was negatively associated with increase of psychosocial distress. In job characteristics, as expected, low decision latitude, high job insecurity, and low social support at work were related to high psychosocial distress. Personality traits such as locus of control and type A behavior pattern, and self-esteem were more powerful predictors of psychosocial distress than general characteristics, health-related behavior, and job characteristics. There were some gender differences. While men who are less educated and single (unmarried, divorced, and separated) experienced higher levels of psychosocial distress than those who are educated and married, women who feel high job demand experienced higher levels of psychosocial distress than those who feel low job demand. Conclusions : The proportion of the high stress group was higher than expected, and psychosocial factors like social support and personality characteristics (e. g. locus of control, type A behavior pattern and self-esteem) were more significant factors for psychosocial distress than other variables. This finding suggests that some psychosocial factors, especially inadequate social support, low self-esteem and lack of internal locus of control for the development of psychosocial distress, will also operate as an intervention strategy in the worksite stress reduction program. It is strongly required that worksite stress reduction programs should be established in at both occupational and level as well as in individual levels.

특수병과의 과거 군 소음 노출이 소음 노출 작업자의 청력에 미치는 영향 (The Effect of working Noise Exposure and Military Background on the Hearing Threshold)

  • 과거 군 복무 기간 동안 청력에 영향을 미치는 사격 및 포격 등 소음에 노출된 군 경력이 현재 소음 노출 근로자에게 어떤 청각학적 영향을 미치며, 소음성 난청의 발생과 관련이 있는지를 평가하고자 하였다. 중소규모 선박수리 및 건조업체 15개 사업체 440명을 대상으로 근로자의 일반 사항, 임상증상, 현재의 청력과 관련한 과거 이과적 병력, 군 복무시 충격소음의 상시적 노출 등의 군 경력, 이명 등의 자각증상에 관한 설문조사와 순음청력검사 및 중이검사를 시행하였다. 현 직종에서의 소음 노출군 중 과거 군에서 소음 노출군이 각 주파수별 청력역치와 평균 청력역치가 가장 컸으며, 특히 2-8kHz의 주파수 역치에서 차이가 크게 나타났다. 청력손실자의 비율 및 중증도의 정도가 현 직종에서의 소음 노출군 중 과거 군에서 소음 노출군이 가장 높은 비율을 보였으며, 특히 감각신경성 난청으로서 소음성 난청자가 통계적으로 유의하게 더 많았다. 이명은 현재 소음 노출 직종에 있는 군에서 높았으며, 그중 군 소음 노출군이 46.2%로 유의하게 더 높았다. 소음성 난청 여부를 종속변수로 한 로지스틱 회귀분석 결과, 연령, 현 직종 근무기간, 군 소음 노출력, 현 직종에서의 소음 노출 여부가 유의한 독립변수로 작용하였다. 우측 귀의 경우, 소음성 난청에 대한 군 소음 노출력의 비차비는 4.5이었고, 현 직종의 소음 노출의 비차비는 7.9이었다. 군 소음 노출 및 현 작업 소음 노출 여부에 따른 각 군별 좌우측 귀의 4 kHz 청력역치와 평균 청력역치는 I군에서 좌우측 귀 모두 나이 및 근무기간이 증가할수록 가장 크게 영향을 미치며 유의하게 증가하였으며 II군, III군, IV군 순으로 청력 역치가 증가하였다. 군에서 노출되는 충격소음은 청력손실과 이명 등 청각학적 영향을 미치며 사업장에서 소음 노출에 따른 소음성 난청의 발생에도 부가적인 영향을 미친다. 따라서 군에서의 소음 노출에 따른 청각학적 영향을 예방하기 위해서뿐만 아니라 소음 노출 근로자에 대한 청력 관리 측면에서도 이의 조기 진단, 치료, 보상 및 재활을 위해서는 군에서의 소음 노출 문제까지 함께 다루어야 함을 시사한다.

동병하치 확산을 위한 전략적 방향과 이행방안 (Strategic Direction and Road Map of Expanding Prevention of Winter Disease in the Summer)

  • Objectives : The purpose of this study was to propose appropriate strategic directions and road maps for successful achievement of programs preventing winter disease in the summer. Methods : Details on programs preventing winter disease in the summer such as clear concept, theoretical basis, current status, intervention or available prescriptions and indication/contraindication/caution were prepared through the related journal review, upon which an observational study was devised and done for simulation to find out even a trivial problem and to guarantee the safety beforehand. The experimental group was divided into 5 groups by the size of pill and the way ginger is treated; 1cm pill with ginger group, 3cm pill group without ginger, 3cm pill group dipped into ginger, 3cm pill group applying ginger to acupoints and 3cm pill group with ginger Results 1. program preventing winter disease in the summer was defined as representative winter diseases such as common cold, influenza, chronic asthma, chronic bronchitis, allergic rhinitis, emphysema, chronic gastritis and rheumatoid arthritis, and preventive care in the summer, reinforcing deficient yang qi of five viscera by using exuberant yang qi from summer heat. 2. It was based upon historically established theories which is 'nourishing yang qi in the spring and summer', 'long summer, namely rainy spell in the summer overwhelms the winter, because of earth winning water according to the five phases theory' and 'To replenish yang qi is major principle to treat winter diseases, which can be most appropriately and timely applied to the patient with deficient yang qi of five viscera inherently, especially in the three dog days of the summer, because of exuberant exterior yang qi and deficient interior yang qi in the five viscera'. 3. In the adjacent China and Taiwan, acupoint applying method in the three dog days named 'San Fu Tie' have been stirring a boom throughout the nation, in which Xiaochuan Gao was used as a basic prescription and it mainly was applied at bilateral $BL_{13}$, $_{15}$ and $_{17}$ for about 4 hours. As far as domestic current status, the necessity of adopting the above method prior to Herbal formula was also recognised, because not a few koreans have apprehension for the safety of it including medicinal herbs and are reluctant to take it any more due to negative advertisement of narrow minded doctors' association. 4. Indication of acupoint applying method in the three dog days included most of winter diseases such as common cold, influenza, chronic asthma, chronic bronchitis, allergic rhinitis, emphysema, chronic gastritis. contraindication was pregnant woman and the weak such as infants and the old. More attention was paid to grasp firmly the normal reaction following the treatment for preventing side effect and teasing blister. recommendation was also given to abstain from food inducing phlegm and dampness such as meat, shrimp and crab as well as cold drinks and foods 5. In the simulation observational study based upon the above findings following review the related articles, no blister was shown on the acupoints icluding bilateral $BL_{13}$, $_{15}$ and $_{17}$ in every experimental group during 24hr observation following the acupoint applying treatment with pills made by modified and devised prescription. At 4 hr, the effectiveness of it reached a peak showing redness and mild tenderness and there is little difference between groups 3cm pills groups regardless of the way ginger was treated. abdominal distention and growling was found in all the volunteers during the treatment at CV 8. Strategic directions and road maps : Through successful fulfillment of the program preventing winter disease in the summer, Korean traditional medicine should be integrated into mainstream national health care services. Cultural access was thought to be as important as Scientific EBM approach. First of all, To evoke potential cultural homogeneity from campaigns and press advertisement was needed for promoting public awareness about preventing winter disease in the summer by enhancing immunity via acupoint applying treatment in the three dog days, and then indigenous name as Sambokcheop, protocol, Clinical Research Form for data collection of it should be developed and prepared. Once the first step was taken this summer, through a thorough data collection and scrutinized scientific evaluation, drawbacks should be compensted for and the efficacy and safety should be substantiated.

충청권 대학생의 감각처리 중재 후 삶의 질 (Quality of Life in Chungcheong area University Students according to their Sensory Processing Intervention)

  • 본 연구는 대학생의 감각처리능력을 조사하고 그 중 감각처리능력에 문제가 있는 대상을 선별하여 일상생활에서 스스로 실천 할 수 있는 감각처리중재 방법을 교육한 후 삶의 질에 영향을 미치는지 알아보고자 하였다. 연구대상은 2013년 5월 초 충청지역에 소재한 3개 대학의 동일 계열 전공 대학생 230명으로 하였다. 230명 모든 대상자에게 감각처리능력, 개인적 특성, 삶의 질에 대해 설문조사를 실시하고 감각처리능력에 문제가 있는 32명의 대상자를 각각 16명씩 실험군과 대조군으로 구분하여, 실험군에게는 감각처리 중재를 실시하고 대조군에게는 중재를 실시하지 않았다. 감각처리 중재 방법을 6주 동안 스스로 시행 한 후 삶의 질을 재평가 하였다. 연구 결과 중재 후 삶의 질 총 점수는 실험군 98.69점, 대조군 84.81점(p=0.001), 신체적 건강 점수는 실험군 16.43점, 대조군 14.64점(p=0.008), 심리적 건강 점수는 실험군 14.71점, 대조군 11.75점(p<0.001), 사회적 점수는 실험군 14.67점, 대조군 13.17점(p=0.032), 생활환경 점수는 실험군 14.66점, 대조군 12.34점(p=0.006)으로 나타났다. 실험군은 삶의 질 모든 영역에서 유의하게 점수가 향상되었고 대조군은 삶의 질에 변화가 없었다. 감각처리 문제를 가진 대상자에게 일상생활에서 실천 가능한 감각처리중재 방법을 교육하는 것이 삶의 질을 향상시키는 효과가 있는 것으로 보인다. 앞으로 성인을 대상으로 하는 감각처리중재 프로그램의 다양한 적용과 삶의 질 향상을 도모해야 할 것이다.

대사증후군 환자의 치주질환과 치아상실에 관한 연구 (A Study on Periodontal Disease and Tooth Loss in Metabolic Syndrome Patient)

  • 본 연구는 40세 이상 중 노년 남녀의 대사증후군과 치주질환, 상실치의 관련성을 조사하여 대사증후군과 구강질환의 효과적인 관리법을 모색하고자 국민건강영양조사 제5기(2010, 2012년) 자료를 이용하여 8,225명을 최종 분석하였고, 다음과 같은 결론을 얻었다. 남자의 치주질환에 영향을 미치는 대사증후군 구성요소는 공복혈당으로 정상군에 비해서 비정상군에서 교차비가 1.27배(95% CI, 1.04~1.54) 높았다. 여자의 치주질환에 영향을 미치는 대사증후군 구성요소는 HDL 콜레스테롤, 복부비만으로 HDL 콜레스테롤이 정상군에 비해 비정상군에서 1.45배(95% CI, 1.22~1.72) 높았고, 복부비만이 정상군에 비해 비정상군에서 치주질환 교차비가 1.44배(95% CI, 1.17~1.77) 높았다. 대사증후군에 따른 치주질환 연관성은 정상군을 기준으로 위험군에서 여자의 치주질환 교차비가 1.55배(95% CI, 1.19~2.01) 높았고, 대사증후군에서 교차비가 2.25배(95% CI, 1.68~3.02) 유의하게 높았다. 여자의 상실치에 영향을 미치는 대사증후군 구성요소는 고혈압으로 혈압이 정상인 군에 비해 비정상군에서 상실치 교차비가 1.41배(95% CI, 1.10~1.82) 높았다. 여자의 대사증후군에 따른 상실치 연관성은 정상군을 기준으로 대사증후군에서 상실치 교차비가 1.48배(95% CI, 1.07~2.04)로 유의하게 높았다. 치주질환과 상실치는 대사증후군의 구성요소가 많아질수록, 위험군보다 대사증후군에서 더 영향을 받는 것으로 나타났다.

24시간 소변분석과 음식섭취빈도지를 사용한 우리나라 성인들의 나트륨 섭취량과 지역별, 음식군별 나트륨 섭취량의 비교 (Sodium Intakes of Korean Adults with 24-hour Urine Analysis and Dish Frequency Questionnaire and Comparison of Sodium Intakes According to the Regional Area and Dish Group)

  • This study was performed to assess the sodium intakes of Korean adults using a 24-hr urine analysis and dish frequency questionnaire (DFQ) according to each dish group and the regional area. The subjects of this study were comprised of 522 adults (male : 267, female : 285), aged 20-59yr residing in the metropolitan area (N=200), Chungcheng-Do (N=117), Jeolla-Do(N=117), and Gueongsang-Do provinces (N=118). The subjects were recruited from the residents who once participated or are participating in the various health programs offered by the public health center. The number of subjects who completed the 24-hr urine collection was 205 (male : 110, female : 95). The mean age and BMI of the subjects were $39.0{\pm}$11.7y and $23.1{\pm}2.9 kg/m^2$, respectively. The mean systolic and diastolic blood pressure was $119.5{\pm}15.4 mmHg$, and $77.1{\pm}11.1 mmHg$, respectively. Eighteen percent of the subjects responded that they are currently smoking, 36% drinking and 50.4% exercising. Twenty point six percent of the subjects were assessed as having hypertension according to their systolic or diastolic blood pressure($SBP{\ge}140mmHg$ or $DBP{\ge}90mmHg$) measurements in the present study. Salt intake of the subjects estimated with 24-hr sodium excretion was 12.7g/d (male : 13.4g/d, female : 12.1g/d) based on the sodium excretion rate as 82%. Salt intake estimated with DFQ was 14.7g/d (male : 16.2g/d, female : 13.4g/d), 2 g more than the salt intake estimated with 24-hr urine analysis. The four dish groups that contributed most to the sodium intake in order were kimchi (11571.4mg), soup and stew (1260.5mg), fish and shellfish(706.3mg) and noodle and ramyeon(644.3mg). Salt intake estimated with DFQ was the highest in the subjects of Gueongsang-Do(17.0g/d), second highest Chungcheong-Do (16.4g/d) and the lowest in the metropolitan area (13.0g/d). Subjects of Gueongsang-Do showed the highest sodium intakes in most of the dish group, whereas subjects of the metropolitan area showed the lowest. Residents of Chungcheong-Do revealed the highest sodium intake with kimchi and of Jeolla-Do the higher sodium intake with main dish (meat, fish and beans). The highest salt percentage of kimchi ($3.0{\pm}0.8%$) and soybean paste ($14.5{\pm}5.1%$) were observed in Gueongsang-Do, whereas individuals of the metropolitan area were observed as having kimchi ($1.6{\pm}0.5%$) and soybean paste ($7.4{\pm}1.6%$) with the lowest salt percenage. Men were observed as having more salty kimchi ($2.4{\pm}0.1%$) than women ($2.1{\pm}0.1%$).

치과의사의 태도와 치과위생사의 서비스가 치과불안에 미치는 영향 (Impact of Dentists' Attitudes and Dental Hygienists' Services on Dental Anxiety)

  • 본 연구는 치과의사의 태도와 치과위생사의 서비스가 치과불안에 미치는 영향을 파악하고 환자들의 치과불안을 줄 일 수 있는 방안을 모색하기 위한 기초자료를 제공하고자 조사되었다. 무작위 표본추출을 통해 선정된 225명을 대상으로 하였고, 2017년 4월 25일부터 5월 10일까지 실시되었다. 본 연구는 IBM SPSS Statistics 20.0 for Windows 통계프로그램을 사용하여 분석하였으며, 얻어진 결과는 다음과 같다. 성별은 여자(54.7%)가 다소 많았고 마지막 치과 방문으로는 1년 미만(59.6%)이 과반수로 나타났으며, 학력은 (전문)대학교 졸업이상(79.1%)이 약 80%를 차지하였다. 직업군으로는 학생(38.1%)이 가장 많았으며, 수입은 200만원 미만(53.8%)이 가장 많았다. 또한 일반적 특성(성별, 학력, 직업군, 마지막 치과 방문, 수입)은 치과불안에 영향을 미치지 않았다. 치과불안은 치과의사의 환자경시, 치과의사에 대한 불신, 치과의사 태도 합과 통계적으로 유의하였고(p<0.01), 불안자극은 치위생 교육수준, 치위생 지식수준, 치위생 진료처치, 치과위생사의 서비스 총합, 치과의사의 환자경시, 치과의사 불신, 치과의사 태도 총합, 치과불안과 통계적으로 유의하였다(p<0.01). 치과불안 및 불안자극에 영향을 미치는 요인으로는 치과의사의 태도 하위요인 중 의사불신(p<0.05), 치과위생사의 서비스 하위요인 중 치위생 지식수준으로 나타났다. 이상의 결과를 토대로 환자들의 치과의사와 치과위생사에 대한 신뢰가 치과불안에 영향을 미치는 것을 알 수 있다. 치과스텝은 환자들의 치과불안을 감소시키기 위해 구강보건 전문가가 할 수 있는 역할에 대한 중요성이 있음을 시사한다. 따라서 일반인들의 구강보건전문가에 대한 인식을 개선하기 위하여 노력해야 한다. 치과의사의 환자에 대한 존중이 필요하고, 치과위생사들은 전문성을 갖추기 위하여 더욱 증진할 필요가 있다고 생각된다. 또한 치과불안을 줄일 수 있는 다양한 프로그램이 개발되어서 환자들이 편하게 치과치료를 받을 수 있도록 개선이 필요하다.

병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.

Astonish TF 재구성 기법의 적용을 통한 체적 크기의 변화에 따른 표준섭취계수(SUV)에 관한 고찰 (Consideration of Standardized Uptake Value (SUV) According to the Change of Volume Size through the Application of Astonish TF Reconstruction Method)

  • PET 영상의 질 향상과 더불어 많은 연구를 통해 다양한 프로그램의 개발이 이루어지고 있다. 그 중 Philips 사의 Astonish TF 재구성 기법은 기존보다 빠른 재구성 속도와 함께 2 mm의 영상 재구성이 가능하여 병변의 향상된 대조도를 확인할 수 있다. 본 연구에서는 전신 $^{18}F-FDG$ PET 영상에서 기존의 4 mm와 2 mm 재구성 기법에 따른 표준섭취계수(SUV)를 비교 평가하였다. GEMINI TF 64 PET/CT (Philips, Cleveland, USA)를 사용하여 팬텀실험은 NEMA IEC Body Phantom (sphere: 10, 13, 17, 22, 28, 37 mm)으로 영상을 획득하였고, 임상영상은 유방암 진단을 받은 여자 30명(연령: $55.1{\pm}11.3$세, BMI: $24.1{\pm}2.9$)을 대상으로 $^{18}F-FDG$ PET/CT 검사를 시행한 후, 각각 4 mm와 2 mm로 영상을 재구성하였다. 획득된 영상은 EBW (Extended Brilliance Workstation) NM ver.1.0을 통해 팬텀과 임상영상에 관심영역을 설정하고, 표준섭취계수를 측정하였으며, SPSS ver.17.로 통계 분석하였다. 팬텀실험에서 90 sec로 획득한 영상의 4 mm와 2 mm 재구성 영상의 $SUV_{Max}$를 비교한 결과, 열소의 크기가 작을수록 $SUV_{Max}$의 편차가 크게 나타났고, 150 sec로 획득한 영상의 4 mm와 2 mm 재구성 영상의 $SUV_{Max}$를 비교한 결과에서도 같은 성향을 나타냈다. 90 sec와 150 sec로 획득한 영상의 $SUV_{Max}$의 편차 정도는 90 sec로 영상을 획득하였을 경우 보다 150 sec로 획득한 영상에서 열소의 크기가 작을수록 큰 차이를 나타냈고, 열소의 크기가 클수록 작은 차이를 나타냈다. 임상영상에서는 4 mm와 2 mm 재구성 기법을 분석한 결과, 표준섭취계수는 4 mm보다 2 mm 재구성 기법에서 높게 나타났고, 또한 체적이 작을수록 변화율이 증가하였다. Astonish TF 재구성 기법을 적용한 팬텀실험과 임상영상의 분석 결과, 체적의 크기가 작을수록 표준섭취계수의 변화율이 증가하였다. 그러므로 임상에서 대조도 및 병변 감별력이 우수한 2 mm 재구성 기법의 정확하고 적극적인 활용을 위하여 표준섭취계수 보정에 대한 추가적인 연구가 필요할 것으로 사료된다.

은퇴준비필요도와 은퇴준비가 삶의 질에 미치는 영향 : 은퇴교육참여도의 조절된 매개효과 (The Degree of Requirements for Retirement Preparation and the Effect of Retirement Preparation on Quality of Life: The Moderated Mediating Effect of the Degree of Participation in Retirement Education)

  • 본 연구는 은퇴교육필요도와 은퇴교육준비도가 삶의 질에 미치는 영향에서 은퇴준비의 매개효과와 조절변수인 은퇴교육참여도가 어떻게 조절되는지를 파악하고자 수행하였다. 연구대상은 조사의 편리성을 고려하여 유의표집하였으며, 2017년 5월부터 7월까지 충청남도 9시 군 212명의 데이터를 활용하였으며, 남자 58.5% 여자가 41.5%로 남자가 많았으며, 거주지역은 시지역이 76.9%, 군지역이 23.1%로 시 거주자가 많았다 세부연구문제는 첫째로 일반적 특성에 따라 삶의 질의 차이, 둘째로 은퇴준비필요도, 은퇴준비, 은퇴교육, 삶의 질의 상관관계, 셋째로 은퇴교육은 은퇴준비와 삶의 질 간의 간접효과를 조절을 보았다. 연구결과 대상자의 첫째, 일반적 특성에 따른 삶의 질 차이 검증결과 거주지역에 따라서는 시지역에 거주하는 사람이 군 지역에 사는 사람보다, 대학원 졸업이상이 대학이하 집단보다 높았다. 직업에 따라서는 공무원, 회사원이 자영업보다 높았으며, 경제수준과 건강수준에 만족하는 집단이 삶의 질에 만족하는 것으로 나타났다. 둘째, 상관분석결과 은퇴준비와 삶의 질, 은퇴준비 필요도와 양의 상관관계를 나타났다. 또한 삶의 질과 은퇴교육, 은퇴교육 필요도와의 양의 상관관계를 보였으며, 은퇴교육과 은퇴준비필요도에 따라서는 양의 상관관계를 보였다. 셋째, 은퇴교육참여는 은퇴교육 필요도, 은퇴준비도를 거쳐서 삶의 질에 미치는 간접효과를 조절하였다. 즉, 은퇴교육 필요도는 은퇴준비에 영향을 주고 은퇴교육 간접효과에 의해 삶의 질에 영향을 준다. 따라서 은퇴준비에 은퇴교육의 조절된 매개효과가 커지는 것으로 확인되었다. 이는 은퇴교육의 필요성에도 삶의 질은 달라질 수 있다는 것을 알 수 있었다.
