The Development of Evaluation Criteria Model for Discriminating Specialized General Hospital (종합전문요양기관 인정기준 모형 개발)

  • This study was conducted to verify the current criteria and classification system used to determine specialized general hospitals status. In this study, we proposed a new classification system which Is simpler and more convenient than the current one. In the new classification system clinical procedure was chosen as the unit of analysis in order to reflect all the resource consumption and the complexities and degree of medical technologies in determining specialized general hospitals. We developed a statistical model and applied this model to 117 general hospitals which claim their national insurance through electronic data interchange(EDI). Analysis based on 984 clinical procedures and medical facilities' characteristic variable discriminated specialized general hospital in present without misclassification. It means that we can determine specialized general hospital's permission In new way without using the current complicated criteria. This study discriminated specialized general hospital by the new proposed model based on clinical procedures provided by each hospital. For clustering the same types of medical facilities using 984 clinical procedures, we executed multidimensional scale analysis and divided 117 hospitals into 4 groups by two axises : a variety of procedure and the Proportion of high technology Procedure. Therefore, we divided 117 hospitals into 4 groups and one of them was considered as specialized general hospital. In discriminating analysis, we abstracted proportion of 16 clinical procedures which effect on discriminating the specialized general hospital in statistical system also we identify discriminating function which include these variables. As a result, we identify 2 discriminating functions, one is for current discriminating system and the other two is for new discriminating system of specialized general hospital.

  • Recently many construction claims related to the ommission and discrepancy of design document are alleged since 1998 and various dispute resolution methods are appeared. But many contractors alleged claims are not satisfied with the result because of not sufficiently reflecting construction practice. The purpose of this study is to analyze the liability of contract privity from the view point of the contract related law and to analyze the claims and claim factor, and to suggest construction contract practice in the public project. The construction claims related to the design document may be minimized, through (1) the accumulation and retrieval of standard forms by owners' type, (2) the usage of specified specification considering project characteristics, (3) the materialization of detail subsurface design and various contract method in heavy engineering construction.

  • With increase of Korean economy scale and globalization, permanent alien residents in Korea are increasing day by day. Living in Korea, they maintain their original nationalities, but their life in Korea is greatly affected by political decision made by Korea. It has been taken for granted that foreigners's suffrage were not recognized due to national sovereignty. The claim that foreigners should not be allowed to participate in government has constantly been met with counterargument on the basis of instability of sovereignty principle, trends of advanced countries, intrinsic differences between federal and regional governments, or actual state of foreigners. It is unreasonable to deal with foreigners' suffrage and eligibility for election differently in that the current public offices election law does not allow foreigners to be eligible for election without any special reason while allowing foreigners to vote in the local election. It is discrimination against foreigners not to allow foreigners to be eligible for election when there is no rationale to differently deal with foreigners' suffrage and eligibility for election. This paper deals with constitutional argument regarding foreigners' electoral eligibility, takes a look at legislative cases of Japanese and European countries, and examines possibilities of including foreigners' eligibility for election in our public offices election law.

Comparative Study on the Empathic Ability Between Nursing and Non-Nursing Students (간호계 학생과 비간호계 학생의 감정이입 능력에 관한 연구)

  • The term 'nursing' is often defined as a therapeutic and interpersonal process that ultimately functions to help clients. In the process of helping, empathic ability(ability to entering into the another's feeling or motives) is an essential factor that has been proved in the fields of psychology and social psychology. In the sense, nursing personnel with high level of empathy is required to accomplish the comprehensive nursing. The purpose of this study was (1) to investigate empathy between nursing and non-nursing students, (2) to examine changes of empathic ability in accordance with their academic years, and (3) to show relationships, if any, between the two concepts, empathy and need-affiliation. For the survey, 325 respondents for nursing department and 378 from non-nursing departments were randomly selected from the two universities in Seoul. The instruments used in this questionaire survey method were Hogan's Empathy Scale and Mehrabian's Need-Affiliation Scale, each of them has established its validity and reliability in many different people and settings. For the analysis, descriptive stative statistics were mostly used by calculating frequencies, percentages and mean scores. T-test, analysis of variance and correlation coefficient were employed to test the differences and the statistical significance. The results of this study are summarized as follows: (1) There was no significant difference in empathic ability detween the nursing and non-nursing student (2) The levels of empathy among the nursing students, 'surprisingly enough, were negatively correlated with their academic years, even though statistical significations was not found. The scores of the empathy are somewhat dropped as academic year going up. (3) No significant relationship was revealed between empathy and need-affiliation. In conclusion, the results of the study repeatedly support the claim that the nursing education must empathize and, also, include the programs that enable nursing students to develop the ability of empathy for their clients.

  • Nanotechnology is now widely believed to be one of the most influential techno-sciences of the future. Or at least it is believed that way in most of industrial countries including South Korea. Naturally, the governments of these industrial countries are currently investing a sizable portion of their human and material resources in order to go ahead in this prominent area of research. Consequently, nanotechnology in these countries has become one of the social sectors where reasonable accountability is normally called for. I claim that reasonable accountability of nanotechnology should take into account the risk-perception of citizens as regards nanotechnology as well as its risk itself. Risk-perception could be variable, changeable, and prone to be polarized; still we should pay more attention to public risk-perception because it is clearly effective in mobilizing public movements, objective in the intersubjective way, and legitimate in a democratic society.

  • The purpose of this study was to analyze the claim data of Health Insurance Review & Assessment Service to determine what factors affected the length of stay in patients aged 65 and older and undergoing total knee arthroplasty due to the principal diagnosis of gonarthrosis by the type of medical institutions. As a result of making an analysis, gender, age, medical security type, severity, residential area and the number of sickbeds were identified as the factors that influenced the length of stay in each type of medical institutions. At this point in time when an increase in the elderly population triggered by population aging and another subsequent increase in medical expenses put a heavy strain on household and national economy, it's necessary to consider how to shorten the length of stay and how to ensure the efficient management of sickbeds based on the findings of this study. In addition, this study is of significance in that it could be used as basic data on quality life-care for elderly patients by the introduction of a systematic management system geared toward lessening patient burden for medical expenses.

Changes in the Hospital Standardized Mortality Ratio Before and During the COVID-19 Pandemic: A Disaggregated Analysis by Region and Hospital Type in Korea

  • Objectives: The coronavirus disease 2019 (COVID-19) pandemic has led to a global shortage of medical resources; therefore, we investigated whether COVID-19 impacted the quality of non-COVID-19 hospital care in Korea by comparing hospital standardized mortality rates (HSMRs) before and during the pandemic. Methods: This retrospective cohort study analyzed Korean National Health Insurance discharge claim data obtained from January to June in 2017, 2018, 2019, and 2020. Patients' in-hospital deaths were classified according to the most responsible diagnosis categories. The HSMR is calculated as the ratio of expected deaths to actual deaths. The time trend in the overall HSMR was analyzed by region and hospital type. Results: The final analysis included 2 252 824 patients. In 2020, the HSMR increased nationwide (HSMR, 99.3; 95% confidence interval [CI], 97.7 to 101.0) in comparison to 2019 (HSMR, 97.3; 95% CI, 95.8 to 98.8). In the COVID-19 pandemic zone, the HSMR increased significantly in 2020 (HSMR, 112.7; 95% CI, 107.0 to 118.7) compared to 2019 (HSMR, 101.7; 95% CI, 96.9 to 106.6). The HSMR in all general hospitals increased significantly in 2020 (HSMR, 106.4; 95% CI, 104.3 to 108.5) compared to 2019 (HSMR, 100.3; 95% CI, 98.4 to 102.2). Hospitals participating in the COVID-19 response had a lower HSMR (HSMR, 95.6; 95% CI, 93.9 to 97.4) than hospitals not participating in the COVID-19 response (HSMR, 124.3; 95% CI, 119.3 to 129.4). Conclusions: This study suggests that the COVID-19 pandemic may have negatively impacted the quality of care in hospitals, especially general hospitals with relatively few beds. In light of the COVID-19 pandemic, it is necessary to prevent excessive workloads in hospitals and to properly employ and coordinate the workforce.

A Study on the Improvement of Freedom of Information Act (「정보공개법」 개선방안 연구)

  • This study aims to discuss ways to improve the information disclosure system centered on the Freedom of Information Act by analyzing the revision history of the Act and interviewing information disclosure experts. First, the study analyzed the revision history and contents of the Act from its enactment to today. It found that although positive amendments have been made in terms of expanding the right to know, they have been limited to operational aspects, and no amendments have been made to the criteria for determining information subject to disclosure. The interview was organized into three areas: achievements, problems, and improvements of the Act. The analysis was conducted by breaking down the content of each area and organizing it into themes. In conclusion, the study proposed eight recommendations for improvement, including expanding the scope of information disclosure, categorizing the information to be disclosed, securing a procedure for appealing the non-disclosure of information, increasing the effectiveness of the committee, introducing penalties and disciplinary regulations, establishing an Information Disclosure Appeals Tribunal, establishing systematic controls to prevent misuse of the right to request information, and merging the Public Data Act with the Freedom of Information Act.

The Consequence Argument and the Supervenience Argument (결과논변과 수반논변)

  • In this paper, I explore Turner's argument against libertarianism. He claims that libertarians can't be compatible with naturalism, without abandoning the Consequence Argument. In order to support this claim, he proposes the Supervenience Argument. According to him, the Supervenience Argument can't be rejected without abandoning either the Consequence Argument or naturalism. But I show that libertarians don't have to accept his Supervenience Argument, even though they stick to the Consequence Argument, and that even though they accept his Supervenience Argument, they don't have to abandon the freedom of agent. For these reasons, I conclude that Turner's argument against libertarianism turns out a failure.

  • With the opening in construction market, more diversified and internationalized construction culture is imported, but the level of our construction culture and policy is still not met the international standards. For example, there is a difference in the liquidated damages at delay due to the reason of builder imputation during the public construction project comparing the international standards like damages. It may lead to the potential claim. In this study, via comparing and analysis on the relevant international standards and extension expense for terms of work, actual compensation is selected, and improvement of liquidated damages like damages is presented.
