Implementation and Evaluation of a Nutrition Education Program to Improve the Nutritional an Physiological Status of Female Gymnasts (여자체조선수의 영양생리학적 요인 개선을 위한 교육프로그램의 효과분석)

  • This study was conducted with 20 female gymnasts to examine the relationship between eating patterns, diet menstrual function and hematological status. According to the baseline data a nutrition counseling and education program was developed and evaluated improved the nutritional status and health of female gymnasts. Mean body weight at the onset of the study was 42.1$\pm$7.0kg and was reduced to 41.8$\pm$6.1kg after the nutrition counseling and education program. The percent of body fat was significantly reduced from 13.9$\pm$3.7% to 13.1$\pm$3.1%(p<0.01) skinfold thickness of subscapular and thighs was reduced significantly(p<0.01, p<0.05) Mean daily intake levels of energy, protein calcium iron thiamin riboflavin and niacin were significantly elevated after the nutrition counseling and education program but were lower than the Recommenced Dietary Allowances. For the nutrition knowledge and food habits, the posttest mean scores showed a significant increase. The hematological status(hematocrit, serum ferritin) and the early follicle level of estradiol were elevated to a mild degree although it was not significant,. The follicular stimulating hormone level was elevated significantly(p<0.01) Gymnastica has been one of the sports implicated by the medical profession as having probable detrimental effects. The implications of such training to childs growth and maturation have yet to be determined . Most female athletes, however, experience poor nutritional status and delayed puberty The priorities were to prepared a more effective nutrition program and education material status and delayed puberty. The priorities were prepared a more effective nutrition program and educational material for athletes coaches and adminstrators to prepare guidelines for the team physicians and coaches to follow for the physical and physiological examinations of female athletes.

The Utility of Basal Serum Luteinizing Hormone Levels for Screening Central Precocious Puberty in Girls

  • Background: This study was conducted to examine if basal luteinizing hormone (LH) levels could be useful for screening central precocious puberty (CPP) in girls. Methods: A total of 90 girls under the age of 8 years were included in this study. They underwent the gonadotropin-releasing hormone (GnRH) stimulation test at Good Gang-An Hospital from March 2008 to December 2012 for evaluation of premature sexual development. Patients were classified into two groups: the pubertal response group of patients who had 5 IU/L peak LH levels in the GnRH stimulation test, and the prepubertal response group of patients who had LH levels <5 IU/L. Chronological and bone ages, height, weight, body mass index, gonadotropin response to GnRH stimulation, and basal levels of LH, follicle-stimulating hormone, and estradiol were studied in both groups. The relationship between basal LH and peak-stimulated LH was evaluated using Spearman's correlation. To determine the optimal cut-off values of basal LH levels for differentiating between two groups, the receiver operating characteristic (ROC) curves were analyzed. Results: When the correlation between basal LH levels and peak LH after GnRH stimulation was analyzed in all subjects (N=90), basal LH levels had a statistically significant positive correlation with peak stimulated LH levels (rs=0.493, p<0.001). The cut-off level of optimal basal LH was 0.1 IU/L, according to the ROC curves. Its sensitivity was 73.3%, and its specificity was 77.8%. Conclusion: The study results showed that serum basal LH levels are useful for screening CPP in girls.

Usefulness of pelvic ultrasonography for the diagnosis of central precocious puberty in girls

  • Purpose: It is difficult to differentiate between central precocious puberty (CPP) and premature thelarche (PT) in girls. The aim of this study was to investigate the diagnostic usefulness of pelvic ultrasonography to distinguish between CPP and PT in girls with early breast development. Methods: This study included girls with early breast development who visited the clinic between January 2012 and December 2013. Clinical, laboratory, and pelvic ultrasonographic data were evaluated. CPP and PT were confirmed using the gonadotropin-releasing hormone stimulation test. Results: A total of 248 girls aged 7-8 years were included, among whom 186 (75.0%) had CPP and 62 (25.0%) had PT. The uterine length, transverse diameter, fundus, volume, and cross-sectional area were significantly larger in the CPP group (uterine length, $2.45{\pm}0.50cm$ vs. $2.63{\pm}0.49cm$, P=0.015; uterine volume, $0.95{\pm}0.62cm^3$ vs. $1.35{\pm}0.76cm^3$, P<0.001). However, there were no differences in the fundus/cervix ratio and ovarian measurements. In receiver operating characteristic analysis, a uterine volume of at least $1.07cm^3$ was the most predictive parameter for CPP with an area under the curve of 0.670 (95% confidence interval, 0.593-0.747). Conclusion: Uterine measurements by pelvic ultrasonography in girls with early pubertal development were significantly larger in the CPP group. However, the diagnostic value of ultrasonographic parameters was not high because of a considerable overlap of values between the two groups. Therefore, pelvic ultrasonography in combination with clinical and laboratory tests may be useful to distinguish between CPP and PT in girls.

A Study on the Recent Trend of Chief Complaint of Korean Pediatric and Adolescent Outpatients (한방소아·청소년과 외래 환자 주소증의 최근 경향에 대한 연구)

  • Objectives The purpose of this study is to investigate about chief complaints of Korean pediatrics and adolescent outpatients in the ${\bigcirc}{\bigcirc}$ university hospital for their recent trend. Methods The study was composed of 4,677 new patients aged between 1 month and 20 years who had been visited pediatrics in ${\bigcirc}{\bigcirc}$ university hospital from 2012 January to 2015 December. Results 1. Among those patients, majority was early childhood as 51.4%, followed by early childhood, late childhood, infancy, and adolescence. 2. Chief complaints in this population have been recorded in the fall the most as 29.6%, but the distribution was inconsistent. 3. To categorize, digestive-related chief complaints were the highest as 30.6%, followed by respiratory, supplemental use, growth, psychiatrics. Respiratory-related chief complaints increased dramatically in the fall. 4. In infancy and early childhood, digestive/respiratory complaints were common, whereas, growth complaints were common in late childhood and adolescence. 5. Digestive/respiratory complaints have been over 50% out of all chief complaints annually. Psychiatric and growth-related complaints have been trending up, whereas, supplemental/skin complaints have decreased every year. 6. Specifically, anorexia were the most common in digestive complaints, and rhinitis/sinusitis were top among respiratory complaints. Precocious puberty and developmental disability increased every year. Conclusions The most common chief complaints were digestive and respiratory-related, and precocious puberty and growth issues have been trending up lately. Based on this result, more studies that targets precocious puberty and growth problems are needed.

The Effects of Caffeine on the Long Bones and Testes in Immature and Young Adult Rats

  • This study was to evaluate the age-dependent effects of caffeine exposure on the long bones and reproductive organs using male rats. A total of 15 immature male rats and 15 young adult male rats were allocated randomly to three groups: a control group and two groups fed caffeine with 120 and 180 mg/kg/day for 4 weeks. Exposure to caffeine at either dose significantly reduced body weight gain; a proportional reduction in muscle and fat mass in immature animals, whereas a selective reduction in fat mass with relatively preserved muscle mass in young adult animals. The long bones of immature rats exposed to caffeine were significantly shorter and lighter than those of control animals along with decreased bone minerals. However, there was no difference in the length or weight of the long bones in young adult rats exposed to caffeine. Exposure to caffeine reduced the size and absolute weight of the testes significantly in immature animals in comparison to control animals, but not in young adult animals exposed to caffeine. In contrast, the adrenal glands were significantly heavier in caffeine-fed young adult rats in comparison to control animals, but not in caffeine-fed immature rats. Our results clearly show that the negative effects of caffeine on the long bones and testes in rats are different according to the age of the rat at the time of exposure, and might therefore be caused by changes to organ sensitivity and metabolic rate at different developmental stages. Although the long bones and testes are more susceptible to caffeine during puberty, caffeine has negative effects on body fat, bone minerals and the adrenal glands when exposure occurs during young adulthood. There is a need, therefore, to educate the public the potential dangers of caffeine consumption during puberty and young adulthood.

Evaluation of cephalometric characteristics and skeletal maturation of the cervical vertebrae and hand-wrist in girls with central precocious puberty

  • Objective: This study aimed to evaluate the differences in cephalometric characteristics and skeletal maturation in girls with central precocious puberty (CPP) via lateral and hand-wrist radiographs. We also aimed to identify the indicators that are most effective for determining skeletal maturity in these patients. Methods: The study included 70 Korean girls (mean age, 8.5 ± 0.5 years) diagnosed with CPP at the Department of Pediatrics, and 48 normal healthy age-matched girls who visited the Department of Orthodontics and had no history of hormone treatment or growth problems. Skeletal maturation was evaluated using lateral cephalometric and hand-wrist radiographs using cervical vertebrae maturation indicators (CVMI) and skeletal maturity indicators (SMI). Results: The mean mandibular plane angle was smaller in the CPP group than in the control group (35.8° ± 4.9° vs. 39.0° ± 6.5°), resulting in greater posterior facial height (p = 0.003). SMI was significantly greater in the CPP group (3.5 ± 1.4 vs. 2.0 ± 1.0) than in the control group (p = 0.001) and was significantly associated with CPP (r = 0.492; p = 0.001), whereas CVMI was not. Conclusions: In comparison with the control group, the CPP group exhibited a smaller mandibular plane angle, greater posterior facial height, and greater skeletal maturation. SMI may be more suitable than CVMI for determining skeletal maturation in CPP. Hand-wrist radiography is recommended in addition to lateral cephalogram for predicting growth in girls with CPP.

Association of the ubiquitin specific peptidase 9X -linked and Afadin expression patterns with sexual maturation in boar testis

  • Closely correlated expression patterns between ubiquitin specific peptidase 9X-linked (USP9X) and adherens junction formation factor (Afadin) in mouse testis development suggests that Usp9x regulates the deubiquitination of Af-6 (also known as Afadin, AFDN), and subsequently, the cell adhesion dynamics during gametogenesis. However, this relationship has not yet been tested in other domestic animals. The study was examined the temporal and spatial expression patterns of porcine USP9X and AFDN from the pre-pubertal to adult stages using real time-PCR and immunohistochemistry. Furthermore, we detected the transcripts of USP9X and AFDN in the testis of 1-, 6- and 12-months old boar, respectively. USP9X and AFDN were found to have similar expressions patterns, with basal expression after 1 month followed by a significant up-regulation from 6 months (puberty) onwards. In addition, neither the AFDN or USP9X proteins were detected in spermatogenic cells but they were expressed in the leydig cells and sertoli cells. USP9X was detected around the basal lamina during pre-puberty, and predominantly expressed in the leydig cells at puberty. Finally, in adult testis, USP9X was increased at the sertoli cell-cell interface and the sertoli cell-spermatid interface. In summary, closely correlated expression patterns between USP9X and AFDN in boar testis supports the previous findings in mice. Furthermore, the junction connections between the sertoli cells may be regulated by the ubiquitination process mediated via USP9X.

  • Pubertal onset is known to result from reactivation of the hypothalamic-pituitary-gonadal (HPG) axis, which is controlled by complex interactions of genetic and nongenetic factors. Most cases of precocious puberty (PP) are diagnosed as central PP (CPP), defined as premature activation of the HPG axis. The cause of CPP in most girls is not identifiable and, thus, referred to as idiopathic CPP (ICPP), whereas boys are more likely to have an organic lesion in the brain. ICPP has a genetic background, as supported by studies showing that maternal age at menarche is associated with pubertal timing in their offspring. A gain of expression in the kisspeptin gene (KISS1), gain-of-function mutation in the kisspeptin receptor gene (KISS1R), loss-of-function mutation in makorin ring finger protein 3 (MKRN3), and loss-of-function mutations in the delta-like homolog 1 gene (DLK1) have been associated with ICPP. Other genes, such as gamma-aminobutyric acid receptor subunit alpha-1 (GABRA1), lin-28 homolog B (LIN28B), neuropeptide Y (NPYR), tachykinin 3 (TAC3), and tachykinin receptor 3 (TACR3), have been implicated in the progression of ICPP, although their relationships require elucidation. Environmental and socioeconomic factors may also be correlated with ICPP. In the progression of CPP, epigenetic factors such as DNA methylation, histone posttranslational modifications, and non-coding ribonucleic acids may mediate the relationship between genetic and environmental factors. CPP is correlated with short- and long-term adverse health outcomes, which forms the rationale for research focusing on understanding its genetic and nongenetic factors.

The Comparison of the Physical Characteristics between Boys and Girls at Puberty (사춘기 남녀 청소년의 인체 특징에 관한 비교 - 만 $10{\sim}14$세를 중심으로 -)

  • The purpose of this study was to analyze the body measurements of boys and girls at puberty and to provide the fundamental data for pubescent apparel manufacturers to produce clothing that reflect their physical characteristics. A total of 549 boys and 529 girls aged between 10 and 14 were measured in the capital area from March 4 to April 3, 2004. Data were collected from 35 anthropometric items and 12 photographic items per a person. SPSS Ver. 12 program was used in data analysis including means, standard deviation, t-test and Duncan test. The main results of this study were as follows. They showed the significant difference of their growth in accordance with the increase of their ages. There were also the difference between boys and girls. As for height and length items, boys showed a slow growth at the age of $10{\sim}11\;and\;12{\sim}13$. Those at the age of $11{\sim}12\;and\;13{\sim}14$ showed rapid growth. That is, an active growth was followed by a slow growth and that phenomenon repeatedly occurred. On the other hand, girls showed remarkable growth at the age $10{\sim}11$ and the growth rate gradually slowed down afterward. Regarding circumference items, boys at the age of $11{\sim}12\;and\;13{\sim}14$ showed remarkable growth. This results showed that boys at the age of $11{\sim}12$ had vertical growth and horizontal growth at the same time and for those at the age of $13{\sim}14$, growth was more conspicuous in horizontal direction. Meanwhile, for girls, the growth rate was high at the age of $11{\sim}12$, somewhat later than the age of the growth of height and length. As for breadth-related items and depth-related items, for both sexes two items grew steadily throughout the ages, breadth-related items showed a higher growth rate than that of depth-related items. This study analyzed the body measurements of pubescent boys and girls and the results showed that, for boys, an active physical growth took place at the age of 13 according to previous studies, but the findings of this study suggested that the phenomenon now occurred at the age of $11{\sim}12$, which proved that physical growth took place earlier than before. Also, an active growth was followed by a slow growth. Girls at puberty showed remarkable growth of height at the age of $10{\sim}11$ that is consistent with previous studies and then showed horizontal growth at the age of around 12, having a voluminous body shape.

A Study on the Importance of Contents in Middle-School Home-Economics (중학교 남.녀학생이 인식한 가정과 교육내용의 중요성에 관한 연구 - 전라북도 남.녀학생을 중심으로-)

  • The purpose of this study was to identify the contents that students want to learn the most on Home-Economics. Participants were 710 middle school students(359 boys and 351 girls) from 9 schools located in 4 cities and 5 counties in CholaBukDo. The results are as follows: 1. Of 32 contents on Family life, boys were interested in 'the puberty', 'pregnancy and the ethics on sex', 'social needs', 'the importance of family relationship', and 'adolescents' physical development'. On the other hand, girls were more intered in 'pregnancy' and 'the ethics on sex', 'the puberty', 'development of career consciousness' and 'career preparation', 'the adolescents' physiolosical and physical development', and 'the importance of family relationship'. 2. Of 37 contents of Managing home resource and Consumer life subarea, boys showed their interests on 'environmental problems', 'environmental conservation', 'environmental pollution', 'management of time and work', 'knowledge', 'management of their abilities'. And girls were more interested in 'environmental conservation', 'environmental pollution', 'environmental problems', 'adolescents life', and 'management of time and work'. 3. Of 49 contents on Dietary life, boys were more interested in 'puberty and the development', 'importance of nutriment', 'nutrition and health' and 'smoking and the health'. However, girls were more interested in 'smoking and health', 'puberty and the development', 'the importance of nutriment', 'nutrition and health', 'a realistic plan to reduce the food-based environmental pollution'. 4. Of 40 contents on Clothing life subarea, while boys were interested in 'the suitable cloths on oneself', 'clothing design', 'regulation of temparature', physical protection' 'merchandising for ready-made clothing'. On the other hand, girls were mostly interested in 'the suitable cloths on oneself', and next 'the clothing design', 'merchandising for ready-made clothes','physical protection', and 'the appropriate posture and physical adornment'. 5. Of 18 contents on Residence life subarea, teachers emphasized the importance of ventilation for adjusting the room environment and students were more interested in the effective use of the residing space. In general, those middle school students in Cheolabukdo showed the biggest interest on 'Clothing life', 'Residence life' the second, 'Eating life' the third, 'Human development and Family relationship' the fourth, and Management of family resource and Consuming life' was their fifth interest. Such result was consistent with Ⅱ-Rok Park's previous research(1997). Since the participants for this study were sampled form 9 schools located in CheolaBukDo, it is not suitable to apply the results nationwide. The restricted sampling must be considered and further studies are needed to make comparisons among areas.
