Psychosocial Factor Influencing Suicidal thoughts in Community Dwelling Elderly in Jeonnam Province (지역사회 노인의 자살사고에 영향을 미치는 심리사회적 요인 : 전남지역을 대상으로)

  • Objectives : The purpose of this study was to examine psychosocial factors influencing suicidal ideation of community dwelling elderly, using the senior community center in Jeonnam Province. Methods : A total of 2,202 subjects (369 males, 1,833 females) were recruited. We evaluated sociodemographic factors, using a self-reporting questionnaire. Subjects completed the Geriatric Depression Scale-Short Form Korean Version (SGDS), Multi-dimensional Scale of Perceived Social Support (MSPSS), Korean version of the General Health Questionale-12 (GHQ-12) and Satisfaction with Life scale (SWLS), to assess psychosocial factors affecting suicidal ideation. Results : Among 2,202 subjects, 179 (8.1%) reported recent suicidal ideation. Self-perceptive health status (p<0.001) and physical disease (p=0.002) revealed differences between two groups. The scores of four scales in the suicidal group were significantly different from the control group: SGDS (p<0.001) and GHQ-12 (p<0.001) were higher, while MSPSS (p<0.001) and SWLS (p<0.001) were lower, in the suicidal ideation group than the control group. Multivariate logistic regression analysis revealed that physical disease (OR 2.575, 95%CI 1.022-6.492), SGDS (OR 1.181, 95%CI 1.120-1.246) and GHQ-12 (OR 1.192, 95%CI 1.108-1.283), were significantly associated with suicidal ideation. Conclusion : Findings support that physical disease, depression, and general mental health may correlate to suicidal ideation in the elderly.

Factors Associated with Self-rated Health Status for the Elderlies in Urban Areas (일부 도시지역 노인들의 주관적 건강상태와 관련 요인)

  • The purpose of this study was to determine factors affecting self-rated health status among the elderly in urban areas. The study subjects were 390 persons aged 65 years and older who had lived more than 5 years in D city. Interviews from the questionnaire were conducted from June through August 2016. The distribution of self-rated health status was rated in terms of dependent variables, with odds ratios and their 95% confidence intervals calculated using unconditional logistic models. As a result, 67.2% of the study subjects answered that they were healthy, whereas 32.8% were unhealthy. In the logistic regression analysis, disability and lowered IADL greatly lowered self-rated health status. The group without an occupation, living expenses from governmental subsidies, frequency of relative contents is seldom, anxiety is high, subjective sleep quality is low, satisfaction of daily life is low, had low levels of self-rated health status. Above results suggest that the self-rated health status of the elderly in urban areas is closely related to sociodemographic characteristics, physical health status, social activity participation, and psychosocial factors.

Predictors of Health Status in Patients with Hemodialysis: A Two-year Longitudinal Study (혈액투석 환자의 건강상태 예측요인: 2년 종단연구)

  • Purpose: The purpose of this study was to examine the effects of physical, psychological, and social factors on the health status of patients undergoing hemodialysis using longitudinal data. Methods: Two surveys were conducted within a period of two-year. A total of 75 patients participated in both surveys. Eight variables including symptoms, treatment belief, hope, meaning-focused coping, family support, friend/peer support, healthcare provider support, and health status were measured. Data were analyzed by paired t-test, Pearson's correlation coefficients, and hierarchical multiple regressions. Results: The mean age of the participants was 47.31 and the average duration of dialysis was 8.27 years. There was no difference in scores on perceived health status between the two year-interval surveys. The two-year follow-up on perceived health status was correlated with baseline perceived health status, friend/peer support, meaning-focused coping, hope, and symptoms. As the result of the regression analysis, friends/peer support (${\beta}=.31$, p=.013) and baseline perceived health status (${\beta}=.30$, p=.020) were found to be significant predictors of subsequent health status which explained 39.0% of the variance. Conclusion: This study suggests the importance of psychosocial resources for positive health outcomes in patients with a chronic illness. Nursing interventions aimed to provide renal peer support need to be developed and tested.

A Comparative Study of Self-Esteem, Health Status and Self-Care in the Rural and Urban Elderly (농촌노인과 도시노인의 자아존중감, 건강상태와 자가간호에 관한 비교 연구)

  • Purpose: This study was conducted to identify the relationships of self-esteem, health status and self-care, and compare them between the rural and urban elderly. Method: The subjects were 126 persons with age over 65: rural(69 persons) and urban(57 persons). The data was collected from 1st Sep to 10th Oct, 2004 by questionnaires, and was analyzed with t-test, ANOVA, Least Significant Difference and Pearson's correlation coefficient in the SPSS-Win 10.0. Results: The level of self-esteem, health status and self-care of the elderly in rural were lower than those of the elderly in urban. The higher group self-esteem in the rural elderly shows more health status and self-care than the lower group. Significant differences between two groups in the urban elderly were not found. Self-esteem, health status and self-care were positively correlated each other in the rural elderly. Significant correlations were found between self-esteem and health status, and between self-esteem and self-care in the rural elderly. Conclusion: Self-esteem forms the foundation of psychosocial health and provides a measure for the quality of life of the elderly in long term care. As nursing is in a unique position to promote self-esteem, the nurse can plan and provide nursing intervention with the consideration of characteristics of the rural and urban elderly to promote the self-esteem, health status and self-care of the elderly.

Relationship Between Job Stress and Psychosocial Stress Among Clerical Public Officers (일부 행정 공무원의 직무스트레스와 사회심리적 스트레스와의 관련성)

  • This study investigated the relationship between job stress and psychosocial stress among clerical public officers in Daejeon City. The self-administered questionnaire survey was administered to 386, during the period between June 1, 2010 and July 31, 2010. As a results, In terms of various levels of psychosocial stresses, 8.8% of the subjects were healthy group, 64.5% were potential stress group, and 26.7% were high risk stress group. On job specifications, the high risk stress group were significantly higher those with higher level of job demand, lower job autonomy and lower colleagues support than their respective counterparts. The level of psychosocial stress was positively correlated with job demand and negatively correlated with job autonomy and social support. Multiple regression analysis revealed that the factors of influence on psychosocial stresses included subjective status of health, satisfaction in work, smoking, leisure activities, past history of visits to medical clinics, social support from colleagues.

Analysis of the Influence of Hopelessness, Psychosocial Stress and Depression on Suicide Ideation Among Middle School Students Using the Structural Equation Model (구조방정식모형을 이용한 중학생의 절망감, 사회심리적스트레스 및 우울이 자살생각에 미치는 영향 분석)

  • This study was performed to determine the suicide ideation and its association with hopelessness, psychosocial stress and depression among some middle school students. The survey was administered to 975 middle school students in Gyeongin area during the period, May $1^{st}$ to June $30^{th}$, 2013. Structured self-administered questionnaires were delivered and collected without the respondents' personal information. As a result, concerning the correlation of depression with various factors, the level of suicide ideation was positively correlated with hopelessness, psychosocial stress and depression. Multiple linear regression analysis showed that the factors influencing the suicide ideation included sex distinction, experience of alcohol drinking, hobbies and leisure life, subjective health status, grade, school violence experience, hopelessness, psychosocial stress and depression. An analysis of the covariance structure, hopelessness (BHOP) was more influential on the suicide ideation than psychosocial stress (PWI) and depression (CES-D). The study found that higher hopelessness, severe distress and higher depression tends to increase the suicide ideation.

The Effects of Stress on Salivary Cortisol Level of Some of the Dental Hygienists (일부 치과위생사의 스트레스가 타액 코티졸 농도에 미치는 영향)

  • The purpose of this study was to research sociodemographic characteristic, work-concerned characteristic, health behavior, life stress, psychosocial stress, job stress and self-esteem of dental hygienists and to find their effects on cortisol level in saliva. The target of this study is 219 dental hygienists registered in Daegu and Gyeongbuk Dental Hygienists Association as of February 2010. For life stress, there was significant difference depending on working years of current job(p<0.05), self health status and regular meal(p<0.01). For psychosocial stress, there was significant difference depending on age(p<0.05), daily working hours(p<0.01), self health status, sleeping hours, regular meal and hobby and leisure activity(p<0.01). For job stress, there was significant difference depending on academic background (p<0.05), job experience, position, man power appropriateness in department and working years of current job(p<0.05). For cortisol level in saliva, extra working days(p<0.05) and self health status(p<0.05) showed significant difference and job stress and cortisol level in saliva showed the negative correlation(p<0.01). Health behavior gave a significant effect to life stress by path coefficient -36.807(p<0.05) and that to psychosocial stress by path coefficient -5.209(p<0.001). And health behavior also gave a significant effect to self-esteem by path coefficient 0.561(p<0.05). Life stress gave a significant effect to psychosocial stress by 0.012(p<0.05) with self-esteem to psychosocial stress by -2.437(p<0.001) and psychosocial stress to job stress by 0.523(p<0.001). Job stress gave a significant effect to cortisol level in saliva by -0.060 (p<0.05). It was found that the most significant factor that affects cortisol level in saliva was job stress, but showed negative correlation. It can be assumed that chromic job stress cause shortage in cortisol excretion, and caused lower cortisol level in saliva.

Meta-analysis of the Factors Related to Self-rated Health among Elderly -Focused on Psychological Dispositions, Social-Economic Status- (노인의 주관적 건강상태 관련 요인에 대한 메타분석 -사회경제적 지위, 심리사회적 요인 중심으로-)

  • The purpose of this study was to meta-analyze and compare the effect sizes of socioeconomic status and psychosocial factors on self-rated health among older adults. Also, to examine whether those effect sizes had been affected by certain moderator like gender through meta-regression analysis was attempted. A total of 487 effect sizes computed from 80 studies published in Korea before 2013 were analysed. The results of this research were as follows. Firstly, spousal support had the largest effects, followed by income, educational level, occupation, support of children, assets, non-family support, family support and social activities. Secondly, while the positive effect of non-family support on self-rated health increased for women than men, the positive effect of income decreased. The findings suggest the following; The importance of spousal support should be recognized for later life's health and the health support for those who had been bereaved must come first. Income security policies and non-family support for elderly women must be strengthened.

Association of Psychosocial Factors in Developing Childhood Depression and ADHD in a Community Low Income Family Children (지역 저소득층 아동의 우울증상과 주의력결핍-과잉행동증상에서 사회경제적 요소의 관련성)

  • Ovjectives: As the attention devoted to children's mental health increases, medical costs and burdens mount as well. In the present study, we evaluated the association between socioeconomic status(SES) and major child psychiatric symptoms. Methods: The subjects of this study were children of recruited from a mental health screening program in the Seoul Sungbuk mental health center over the course of 3 months. To establish the SES of each child, we collected data about each child's medical insurance, years of parental education, household income, family structure and housing. 149 children & parents completed questionnaires including the Childhood Depression Inventory(CDI) and the Korean Attention Deficit Hyperactivity Disorder Rating Scale(K-ARS). Results: The mean K-ARS-P score was $12.1{\pm}11.1$ and the suspected prevalence of ADHD was 20.8%(n=31). The mean cm score was $12.9{\pm}7.9$, and the prevalence of suspected depression was 16.8%(25). Depressive symptoms and ADHD symptoms were both more severe than those observed in a previous epidemiologic study in Korea. Depressive symptoms were more closely associated with family SES status. Conclusion: SES status is one of the most important risk factors in the development of major child psychiatric symptoms. In our study we found that depressive symptoms in particular were most tightly associated with psychosocial factors. Evaluation of the risk factors, early screening and intervention for low SES children would be valuable mental health management tactics to implement in a community mental health system.

The Effect of social Support on Chronic Stress and Immune System in Male Manufacturing Workers (사회적 지지가 만성적 스트레스와 면역체계에 미치는 영향)

  • Objectives : To examine whether cumulative chronic stress influences the immune status, and to verify the effect of social support on the relationship between these two dimensions in male manufacturing workers. Methods : A total of 39 workers were recruited for this study. A structured-questionnaire was used to assess general characteristics, job characteristics (work demand and decision latitude), psychosocial distress, and social support. The serum levels of CD4 and CD8 were measured as immune markers, and were collected between 8:00 and 10:00am in order to standardize the markers. Nonparametric statistics were used to estimate the differences between job characteristics and the immune markers. Results : General characteristics, and health-related behaviors, were not associated with CD4, CD8 or CD4/CD8. No relationships were found between job characteristics and the mean levels of immune reactivity. These results were consistent, even after controlling for social support. Social support failed to modify the relationship toward work demand, decision latitude or psychosocial distress to CD4, CD8, and CD4/CD8. Conclusion : Cumulative chronic life stress might not influence the immune status, and the effects of social support on the immune function under chronic stress, may not play a crucial role in modifying the relationships. This implication supports that the effect of stress on the immune function may be determined by the characteristics of that stress. further research should effectively considers the type, magnitude and timing of a stress event, and modifiable factors, such as personality traits, coping style, and hormone excretion levels, on the alteration of immune status.
