Non Pharmacological Approaches in the Irritable Bowel Syndrome (과민성 장 증후군의 증상 경감을 위한 비약물적 접근 고찰)

  • There have been many trials of clinical efficacy of multi component and single component treatments for irritable bowel syndrome(IBS). We reviewed effects of non phamacological treatments in the IBS. Though the efficacy of multi-component approaches was unclear, several results suggest that cognitive behavioral therapy was effective in improving gastrointestinal symptoms of IBS. As a single component, cognitive therapy and relaxation with or without biofeedback could improve the symptoms and psychological health of IBS patients. Yoga, meditation, self-help information and hypnotherapy could be applicable to IBS.

An Analysis of Experimental Research on Foot Reflexology from Year 2000 to 2020 (발반사요법의 실험연구 문헌분석: 2000-2020년)

  • The purpose of this study was performed to analyze the trend of foot reflexology and effectiveness. This study analyzed 118 articles published over 20 years on the effect of intervention therapy. Year of presentation, type of research, characteristics, results of intervention variables, and so on of preceding papers on foot reflexology were analyzed. Collected data were analyzed using SPSS/PC Ver. 26.0 statistical program. Results of this study confirmed that foot reflexology therapy was effective in psychological state, physical state, body systems, vital signs, lipids, and blood. Results of this study could be used as basic data for the development of interventional methods of foot reflexology for human health.

Spleen Daoyin Method (脾腸導引法) for Functional Dyspepsia Patients: A Case Report (비장도인법(脾腸導引法)을 적용한 복부 팽만감 주소의 기능성소화불량 증례보고)

  • A 42-year-old patient with functional dyspepsia had suffered from early satiation and postprandial fullness for 1 year despite Western medicine treatment. We treated her with the Spleen Daoyin method, along with acupuncture, moxibustion, and herbal medicine and scored her symptoms using a 4-point Functional Dyspepsia-related Quality of Life (4p-FD-QoL) questionnaire, the Gastrointestinal Symptom Rating Scale (GSRS), and a visual analogue scale (VAS). A 7-day Spleen Daoyin treatment decreased her 4p-FD-QoL score from 61 to 51, with a marked decrease in the emotional part (18 to 10). Her GSRS score did not change, but her bloating score decreased from 3 to 1. Her VAS score decreased from 6.6 to 4.3 for upper abdomen discomfort, but lower abdomen discomfort was unchanged. The patient showed high compliance and satisfaction with exercise therapy and reported no adverse events. Spleen Dayoin may be a useful therapy for functional dyspepsia, especially when accompanied by psychological problems.

Treatment of Peripheral Facial Palsy with Skin Damage Caused by Folk Remedies Using Korean Medicine: A Case Report (민간요법으로 피부손상을 동반한 말초성 안면마비에 대한 한의치료 증례 보고)

  • Peripheral facial palsy generally results from damage to the facial nerve. As facial asymmetry is observed, it accompanies not only functional problems but also psychological and social difficulties. Medical institutions treat most patients, however, there are still patients who rely on invasive methods by unqualified practitioners for fear of sequelae. This case describes a 61-year-old woman who experienced facial palsy twice. She visited our hospital with skin damage after folk patch therapy for her facial palsy. Combined Korean medicine treatment was administered during treatment period. After treatments, the symptoms of facial palsy and skin lesions improved. This case showed that Korean Medicine was efficacious in improving symptoms of facial palsy with damaged skin caused by folk patch therapy. Education and awareness of uncontrolled invasive treatments for facial palsy are needed.

A Case Report on Korean Medicine Psychotherapy in an Adolescent Patient with Major Depressive Disorder Accompanied by Suicidal Ideation and Behavior (자살생각과 자살행동을 동반한 청소년 주요우울장애 환자의 한의정신요법 활용 1례 보고)

  • Objectives: Major depressive disorder is a severe disease that is associated with suicidal ideation and behavior. In this case, Korean medicine psychotherapy and Korean medicine treatment were used to treat an adolescent patient with major depressive disorder. Methods: The Beck Depression Inventory, Reynolds Suicidal Ideation Questionnaire, and Crisis Triage Rating Scale were used as psychological evaluation scales. Korean medicine psychotherapy focuses on Giungoroen-therapy and IiGyeungByunQi-therapy and Korean medicine treatment combined with acupuncture, moxa, and herbal medicine. Results: Comparison before and after treatment showed improvement in clinical symptoms, Beck Depression Inventory, Reynolds Suicidal Ideation Questionnaire, and Crisis Triage Rating Scale. Conclusions: As a result of applying Korean medicine, psychotherapy and Korean medicine treatment reduces to an adolescent patient with major depressive disorder, depression, suicidal thoughts, and suicidal behavior.

A Study on the Correlation between Social Support and Quality of Life of Older Adults (노인의 사회적지지와 삶의 질과의 연관성에 관한 연구)

  • Purpose : This study investigated the correlation between social support and older people's quality of life. Methods : We conducted a questionnaire survey with 350 older participants and analyzed 320 completed surveys. The collected data were analyzed using SPSS 25.0 for Windows. The general characteristics of the participants and quality of life subdomains were analyzed using descriptive statistics and frequency analyses. Furthermore, the differences between the subdomains of social support and quality of life were analyzed using independent t-tests and a one-way ANOVA. Post-hoc tests were performed using the Scheffé test. A significance level of α=.05 was used to verify statistical significance. Results : Upon examining the sub-domain level of social support and considering general characteristics, it was found that older age correlates with increased dependency among patients with associated diseases (p<.05). Similarly, an investigation into participants' quality of life at the subdomain level, considering general characteristics, revealed higher correspondence to lower quality of life (p<.05). Differences emerged between social support and quality of life among older individuals, with a negative correlation observed between the two (p<.05). Conclusion : This study suggests the importance of actively supporting older people with chronic diseases who experience emotional and psychological instability. It advocates for promoting the use of various services, including visiting medical services, convalescent care, and assistance, to meet their needs effectively.

Effect of a Low-Intensity Combined Exercise Program using Props on the Daily Living Fitness and Quality of Life of Elderly Women (저강도 복합운동프로그램이 여성노인의 일상생활체력과 낙상효능감에 미치는 영향 )

  • PURPOSE: The purpose of this study was to examine the effectiveness of a low-intensity combined exercise program on daily physical fitness parameters such as balance, flexibility, muscle strength, and fear of falling of elderly women in the community. METHODS: This study assigned 30 elderly women randomly into two groups: The control and experimental groups. The control group (n = 15) underwent routine gait. The experimental group (n = 15) underwent the low-intensity combined exercise. The exercise program in this study comprising combined exercise, including balance, flexibility, muscle strength training, and the exercise program using props was conducted twice a week for 8 weeks. RESULTS: The average age of the control and experimental groups was 77.27 years and 78.33 years, respectively. There were significant differences in static balance (t = -4.167, p < .001), dynamic balance (t = 2.463, p < .001), (t = -3.870, p < .001), (t = -2.262, p < .001), (t = -5.732, p < .001), (t = -6.573, p < .001), and fear of falling (t = -5.129, p < .001). CONCLUSION: The results show that low-intensity combined exercise is an effective intervention that improves physical health fear of falling in older women. The combined exercise program was found to be more effective in terms of physical function and fall-related psychological function compared to the control group that only walked.

The Association of Activity Limitation on Health-Related Quality of Life and Depression in Elderly Korean Stroke Patients

  • Purpose: The purpose of this study was to help the numerous health care workers who participate in the rehabilitation of stroke patients by understanding how limitations on the activities of stroke patients affect the health-related quality of life and depression. Methods: We investigated 527 stroke patients from the Korean's National Health and Nutrition Examination Survey (KNHANES, 2013-2018). The subjects were divided into two categories: with and without activity limitation. A medical doctor's diagnosis of depression and the EQ-5D, which measures life satisfaction connected to health, were the study's factors. Based on their level of activity limitation, the individuals' depression and health-related quality of life were compared. The odds ratios relating to activity limitation's relationships with depression and stroke patients' quality of life were computed using logistic regression analysis. Results: The diagnosis of depression in subjects with activity limitation was 16.0%, while in those with no limitation on activity it was 5.6%, and the EQ-5D index was 0.67±0.02, 0.85±0.01. For every item on the EQ-5D, there existed a significant difference in the odds ratio. Furthermore, when comparing depression with activity limitation to non-activity limitation, the odds ratio was 4.09 (2.12-7.788). Conclusion: Limitation of activities of stroke patients significantly reduces the health-related quality of life and increase the probability of depression. Therefore, treatment of stroke patients should be approached taking into consideration their psychological condition. It is also deemed necessary to have a systematic and continuous rehabilitation program.

Awareness of disaster and post traumatic stress disorder in occupational therapy students (재난 및 외상 후 스트레스장애에 대한 작업치료 전공자의 인식조사)

  • This study examined the perception of disasters and post-traumatic stress disorder in occupational therapy majors to provide the basic data necessary for future occupational therapy. A questionnaire survey was conducted on 545 students in occupational therapy departments of three year and four year universities. The frequency of the questionnaire was calculated by frequency analysis using the SPSS 19.0 win program. A chi-squared test was conducted to verify the analyzed questionnaire data. The reliability of the questionnaire in this study showed a Cronbach' alpha value of 0.891. According to the survey results, approximately 20% of learners who majored in occupational therapy were unaware of the symptoms, developmental mechanism, and diagnostic criteria of post-traumatic stress disorder(PTSD). Knowledge of the underlying causes of psychological symptoms, such as post-traumatic stress disorder as well as physical damage through industrial accidents, was found to be 2.92 on the Likert 5-point scale. To be effective in rehabilitation treatment, the degree of the approach to education from the viewpoint of occupational therapy is important enough to be recognized as 3.90 on the Likert 5-point scale. The Pearson correlation coefficient for the need for education on disasters was higher than the correlation with the awareness of disasters.

Effectiveness of Cognitive Behavioral Therapy Techniques for Control of Pain in Lung Cancer Patients: An Integrated Review

  • Background: Experience of lung cancer includes negative impacts on both physical and psychological health. Pain is one of the negative experiences of lung cancer. Cognitive behavioral therapy techniques are often recommended as treatments for lung cancer pain. The objective of this review was to synthesize the evidence on the effectiveness of cognitive behavioral therapy techniques in treating lung cancer pain. This review considered studies that included lung cancer patients who were required to 1) be at least 18 years old; 2) speak and read English or Thai; 3) have a life expectancy of at least two months; 4) experience daily cancer pain requiring an opioid medication; 5) have a positive response to opioid medication; 6) have "average or usual" pain between 4 and 7 on a scale of 0-10 for the day before the clinic visit or for a typical day; and 7) able to participate in a pain evaluation and treatment program. This review considered studies to examine interventions for use in treatment of pain in lung cancer patients, including: biofeedback, cognitive/attentional distraction, imagery, hypnosis, and meditation. Any randomized controlled trials (RCTs) that examined cognitive behavioral therapy techniques for pain specifically in lung cancer patients were included. In the absence of RCTs, quasi-experimental designs were reviewed for possible conclusion in a narrative summary. Outcome measures were pain intensity before and after cognitive behavioural therapy techniques. The search strategy aimed to find both published and unpublished literature. A three-step search was utilised by using identified keywords and text term. An initial limited search of MEDLINE and CINAHL was undertaken followed by analysis of the text words contained in the title and abstract, and of the index terms used to describe the article. A second search using all the identified keywords and index terms was then undertaken across all included databases. Thirdly, the reference list of all identified reports and articles were searched for additional studies. Searches were conducted during January 1991- March 2014 limited to English and Thai languages with no date restriction. Materials and Methods: All studies that met the inclusion criteria were assessed for methodological quality by three reviewers using a standardized critical appraisal tool from the Joanna Briggs Institute (JBI). Three reviewers extracted data independently, using a standardized data extraction tool from the Joanna Briggs Institute (JBI). Ideally for quantitative data meta-analysis was to be conducted where all results were subject to double data entry. Odds ratios (for categorical data) and weighted mean differences (for continuous data) and their 95% confidence intervals were to be calculated for analysis and heterogeneity was to be assessed using the standard Chi-square. Where statistical pooling was not possible the finding were be presented in narrative form. Results: There were no studies located that met the inclusion requirements of this review. There were also no text and opinion pieces that were specific to cognitive behavioral therapy techniques pain and lung cancer patients.Conclusions: There is currently no evidence available to determine the effectiveness of cognitive behavioural therapy techniques for pain in lung cancer patients.
