Aneurysm of the Proximal Subclavian Artery - A case report - (우측 근위부 쇄골하 동맥류의 수술치험 - 1예 보고 -)

  • Subclavian artery aneurysms are quite rare and they are known to be as only 1% of all peripheral aneurysms. Atherosclerotic disease is the most common cause of subclavian artery aneurysms. These aneurysms can cause rupture, thrombosis, embolisms or symptoms by local compression. Surgical operations are generally performed as treatment. We report here on a case of successful surgical treatment for a right proximal subcalvian artery aneurysm, and the surgery was peformed via the medical exclusion method.

Influence of Bradykinin on the tubular function of the dog kidney (신세뇨관기능(腎細尿管機能)에 미치는 Bradykinin의 영향(影響))

  • In this study the influence of Bradykinin, a biogenic polypeptide, on the excretory function of the dog kidney was investigated, utilizing the clearance and stop-flow method. The results are summarized as follows; 1) Bradykinin administered intravenously elicited a marked antidiuresis. 2) When given into the renal artery, however, prompt increase of the urine flow, sodium excretion and free water clearance without significant change in the glomerular filtration rate ensued. 3) It was also effective during an osmotic diuresis with 10% Mannitol infusion, though the response was not so marked. 4) The stop-flow experiment showed that sodium reabsorption in the proximal tubules is inhibited by the infusion of bradykinin into the renal artery. It was thus concluded that the diuretic effect of bradykinin given intra-arterially results from the inhibition of sodium transport in the proximal tubules.

Numerical Study on Aneurysmal Blood Flow After Coil Embolization

  • Aneurysm embolization method using coils has been widely used. When partial blocking of an aneurysm is inevitable, the locations of coils are important since they change the flow patterns inside the aneurysm, which affect the embolization process. We calculated the flow fields inside the partially blocked lateral aneurysm models for different coil locations-proximal neck, distal neck, proximal dome and distal dome. Flow into the aneurysm sac was significantly reduced in the distally blocked models, and coils at the distal neck blocked inflow more effectively comparing to those at the distal dome. This study suggests that the distal neck should be the most effective location for aneurysm embolization.

  • The purpose of this study is to examine the effects of scaffolding instruction on motivated learning strategies and academic achievement. Subjects of this study were 96 middle school first grade students in Busan, who were randomly assigned to two experimental group and one control group. The one experimental group had received scaffolding instruction by teacher, and the other experimental group had received scaffolding instruction by the interaction of peers. Control group were given traditional lessons only by the method of lecture. Students were given English Academic Achievement Test, Motivated Strategies of Learning Questionnaire. T test and ANOVA were used to analyze date. The results of this study showed that scaffolding instruction by ZPD turned out to have a positive influence on motivated learning strategies and academic achievement.

  • We introduce optimization algorithms using Bregman Divergence for solving non-negative matrix factorization (NMF) problems. Bregman divergence is known a generalization of some divergences such as Frobenius norm and KL divergence and etc. Some algorithms can be applicable to not only NMF with Frobenius norm but also NMF with more general Bregman divergence. Matrix Factorization is a popular non-convex optimization problem, for which alternating minimization schemes are mostly used. We develop the Bregman proximal gradient method applicable for all NMF formulated in any Bregman divergences. In the derivation of NMF algorithm for Bregman divergence, we need to use majorization/minimization(MM) for a proper auxiliary function. We present algorithmic aspects of NMF for Bregman divergence by using MM of auxiliary function.

Novel Noncrossing Y-Stent Technique Using Tapered Proximal End of a Solitaire AB Stent for Coil Embolization of Wide-Neck Bifurcation Aneurysms

  • The crossing Y-stent method is one of the indispensable techniques to achieve sufficient neck coverage during coil embolization of bifurcation aneurysms with a wide neck and/or branch incorporation. However, the inevitable hourglass-like expansion of the second stent at the crossing point can result in insufficient vessel wall apposition, reduced aneurysm neck coverage, delayed endothelialization, and subsequent higher risks of acute or delayed thrombosis. It also interferes with engagement of the microcatheter into the aneurysm after stent installation. We expected to be able to reduce these disadvantages by installing a noncrossing type Y-stent using the Solitaire AB stent, which is fully retrievable with a tapered proximal end. Here we report the techniques and two successful cases.

  • Three-dimensional solid model has not been widely used in surgical prediction of orthognathic surgery because frequent artifacts from occlusal restorations or prosthesis limited the usefulness of simulated surgery involving occlusion. We prepared three-dimensional(3D) solid model from CT data and integrated the 3D solid model with dental cast using a face-bow transfer technique combined with skeletal reference measurement and confirmation with cephalometric radiographs. With this simple and easy method, it was possible to predict bony interference between the proximal and distal segment of the mandible so that we can prevent condylar displacement after sagittal split ramus osteotomy of the mandible with prominent asymmetry. The method error was within 2mm and it seemed to be useful in preoperative planning for maxillofacial surgery with maxillo-mandibular occlusal change.

  • This article analyzes mathematics education from dialectical materialism acknowledging the objectivity of knowledge. The thesis that knowledge is objective advances to the recognition that knowledge will be internalized, and an idea of zone of proximal development(ZPD) is established as a practice program of internalization. The lower side of ZPD, i.e. the early stage of internalization takes imitation in a large portion. And in the process of internalization the mediational means play an important role. Hereupon the role of mathematics teacher, the object of imitation, stands out significantly. In this article, treating the contents of study as follows, I make manifest that teaching and learning in mathematics classroom are united dialectically: I hope to findout the method of teaching-learning to mathematical knowledge from the point of view that mathematical knowledge is objective; I look into how analysis into units, as the analytical method of Vygotsky, has been developed from the side of mathematical teaching-learning; I discuss the significance of mediational means to play a key role in attaining the internalization in connection with ZPD and re-illuminate imitation. Based on them, I propose how the role of mathematics teachers, and the principle of organization to mathematics textbook should be.

Fast and Safe Contact Establishment Strategy for Biped Walking Robot (이족 보행 로봇을 위한 빠르고 안전한 접촉 생성 전략)

  • One of the most challenging issues when robots interact with the environment is to establish contact quickly and avoid high impact force at the same time. The proposed method implements the passive suspension system using the redundancy of the torque-controlled robot. Instead of utilizing the actual mechanical compliance, the distal joints near the end-effector are controlled to act as a virtual spring-damper system with low feedback gains. The proximal joints are precisely controlled to push the mid-link, which is defined as the boundary link between the proximal and distal joints, towards the environment with high feedback gains. Compared to the active compliance methods, the contact force measurements or estimates are not required for contact establishment and the control time delay problems do not occur correspondingly. The proposed method was applied to the landing foot control of the 12-DoF biped robot DYROS-RED in the simulations. In the results, the impact force during landing was significantly reduced at the same collision speed.

Morphometry of the patellar tendon using a simple tracing method: a gold standard for anterior cruciate ligament reconstruction

  • The anterior cruciate ligament (ACL) is mostly damaged in sporting activities. To reconstruct the damaged ACL, a patellar tendon (PT) is often the most preferred graft due to its fast healing and bone integration i.e. bone-patellar tendon-bone graft. Suitability of the PT often depends on the morphometric profile of the tendon. This study reported on the harvestable surface area (SA) of the tendon using a simple tracing method. The PT of 79 adult formalin-fixed cadavers of South Africans of European Ancestry were dissected, and the margins of the PT were traced on a wax paper before the tracings were scanned. The SA, straight proximal width (SPW), curved proximal width (CPW), straight distal width (SDW), curved distal width (CDW) and length of tendon (LOT) from the digitized image of the PT was measured. In addition, the length of the lower limbs was measured to normalize the measurements. The results showed no significant side differences, and the measurements were not sexually dimorphic. A strong correlation was reported for SA vs. LOT, SPW vs. CPW and SDW vs. CDW for both sexes and sides. The presented morphological profile provides additional information on the usability of the graft and with respect to healing and recovery.
