상악 선천성 결손과 하악 골격성 제3급 부정교합 경향성을 보이는 환자에게서 CAD-CAM 기법을 이용한 진단과 고정성 보철 수복 증례 보고 (CAD-CAM technique based digital diagnosis and fixed partial denture treatment on maxillary congenital missing teeth with skeletal class III tendency patient: A case report)

  • 디지털 기술의 발달은 치의학의 큰 변화를 일으키고 있고, 이러한 디지털 워크플로는 보철 치료 영역에서도 다양한 3D 데이터들을 하나로 중첩시켜, 진단과 보철물 제작에 활용되고 있다. 디지털 데이터를 종합하여 형성된 가상 환자에게서 계획된 진단과 보철물의 형성은 기존 통상적인 방법에 비해 보다 더 직관적으로 보철 치료의 결과를 시뮬레이션 할 수 있고, 이로 인해 심미적인 보철 치료의 예측성을 높일 수 있다. 본 증례에서는 디지털 워크플로를 통해 상악 전치부 선천성 결손 부위를 고정성 보철물로 제작하여 기능적, 심미적으로 만족할 만한 임상적 결과를 얻었으므로 이를 보고하는 바이다.

Full Mouth Rehabilitation of a Patient with Bite Collapse in the Molar Area Using Removable Partial Denture and Dental Implant Prosthetics

  • Dental clinicians often encounter cases wherein the patient's lost molar area was neglected and left untreated for an extended period of time, thereby causing the extrusion of opposite molars and occlusal disharmony as well as occlusion in the anterior teeth and consequently resulting in anterior displacement in the area. Clinicians normally carry out prosthetic treatment via occlusal plane lifting when such becomes absolutely necessary due to the lack of sufficient space needed for prosthetic therapy aimed at proper anterior and lateral induction. In this case report, we examined occlusal disharmony and VDO loss in a patient who had lost his molars and had not received prosthetic treatment for an extended period of time. We treated the maxillary area with dental implant prosthetics and Kennedy Class I RPD and the mandibular area with residual natural tooth-based implant placement and dental implant prosthetics. The patient reported treatment outcomes that were deemed satisfactory both functionally and aesthetically.

하지 분절절제 및 회전재접합술자의 의지 보행훈련 증례연구 (A Case Study of Prosthetic Ambulation Training for Rotation-Plasty Client)

  • The purpose of this study was to introduce rotation-plasty procedure and prosthetic ambulation training. The recent development of chemotherapy and diagnostic facility have permitted the orthopaedic surgeons to try limb saving procedures rather than amputations for the treatment of the malignant bone tumors. If the tumors around the knee joint were treated by mid-thigh amputation or hip disarticulation, it would impose the client with a great handicap for rehabilitation. Rotation-plasty procedure was first done by Borggreve, in 1930 for the congenital short femur. Recently this procedure was used a malignant bone tumor at the distal femur by Kotz and Salzer in 1982. In spite of its cosmetic problem of the distal stump, this procedure has the great functional advantage of converting the above-knee amputation to the below-knee amputation. The inverted foot was also good to control the prosthesis as a below-knee stump and heel functioned as a patella to support the body weight. This 15 years old girl case was had rotation-plasty due to osteosarcoma of the distal femur with 3rd postoperative chemotherapy, and admitted to Yonsei rehabilitation hospital for prosthetic ambulation training. Then, the case had excellent functional results of prosthetic ambulation training with rotaion-plasty after 3 months.

Recurrent Prosthetic Mitral Valve Dehiscence due to Infective Endocarditis: Discussion of Possible Causes

  • Prosthetic valves are being widely used in the treatment of heart valve disease. Prosthetic valve endocarditis (PVE) is one of the most catastrophic complications seen in these patients. In particular, prosthetic valve dehiscence can lead to acute decompensation, pulmonary edema, and cardiogenic shock. Here, we discuss the medical management of late PVE in a patient with a prior history of late and redo early PVE and recurrent dehiscence. According to the present case, we can summarize the learning points as follows. A prior history of infective endocarditis increases the risk of relapse or recurrence, and these patients should be evaluated very cautiously to prevent late complications. Adequate debridement of infected material is of paramount importance to prevent relapse. A history of dehiscence is associated with increased risk of relapse and recurrent dehiscence.

The clinical prognosis of implants that are placed against super-erupted opposing dentition

  • Objectives: If teeth are missing, super-eruption of teeth in the opposing arch can occur in the area and can change the occlusal plane. When missing teeth are replaced with implants, the oral surgeon must determine whether or not the super-erupted teeth need to be treated in order to normalize the occlusal plane. In this study, we evaluated the clinical prognosis of dentition after implant placement and prosthetic treatment were completed in an occlusal plane altered by super-erupted teeth in the opposing arch without additional treatment of the super-erupted teeth. Materials and Methods: Twenty-two patients (9 males, 13 females) were treated with implants and prosthetics without addressing the super-erupted opposing dentition from April 2004 to August 2012 at Seoul National University Bundang Hospital. A total of 33 implants were placed. Values of crestal bone loss, survival rates, and surgical and prosthetic complications for an average of 29.6 months after prosthetic loading were recorded. Results: In one case, the cover screw was exposed after implant surgery. The mean crestal bone loss was $0.09{\pm}0.30mm$. Of the 33 implants, 31 survived, a survival rate of 93.94%. A prosthetic complication occurred in one case but functioned well after correction. Conclusion: Favorable clinical results from prosthetic complications, crestal bone loss, and implant survival rates were exhibited in implants next to a super-erupted opposing tooth.

마모가 심한 환자의 전악 고정성보철물 제작 증례 (Full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis)

  • This case study is aimed at introducing a full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis. This is a case report of a patient with severe wear dentition with changing vertical dimension. In line with using prosthetic treatment, patient adaptation was verified with provisional restorations followed by diagnostic wax-up. Function, esthetics, and occlusal stability were verified during a 4-week follow up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory results after delivery of definitive prosthesis. Prosthetic treatment of severe wear dentition was functionally and esthetically successful. Dentists and dental technicians would be able to develop better treatment approaches using fixed dental prosthesis.

중년층의 구강건강지식, 실천 및 보철치료 만족실태 조사 (A study for middle-aged on oral health knowledge, oral health care and satisfaction with prosthetic treatment)

  • Objectives : The purpose of this study was to examine the oral health knowledge and actual oral health care of the selected subjects, their decision making about prosthetic treatment, the state and characteristics of their prosthetic treatment and their satisfaction with prosthetic treatment in an attempt to provide some information on the improvement of the quality of life related to oral health and the promotion of oral health. Methods : The subjects in this study were 250 people who received education in two different lifelong education institutions in the city of Busan. After a survey was conducted from May 23 to June 15, 2011, the answer sheets from 217 respondents were analyzed. Results : 1. As for the general characteristics of the respondents, the men(52.1%) outnumbered the women. Those who were in their 60 and up(47.5%) made up the largest age group, and the married people(65.4%) outnumbered the unmarried ones. By occupation, the company employees(20.3%) made up the biggest group. By education, the greatest group was high-school graduates(36.1%). By monthly mean income, the biggest group gained an income of 2.01 million won or more(36.9%). As to medical security, community-based insurance was most prevailing (36.9%). In terms of health promotion, the largest group worked out to stay fit(52.4%). 2. Regarding oral health knowledge, they had a good knowledge about the cause of dental caries(56.7%), but they weren't well aware of the right time for regular dental checkup(47.9%). In relation to oral health care, regular scaling wasn't prevailing(16.9%). 3. Regarding connections between the general characteristics and satisfaction with prosthesis, the less-educated respondents expressed significantly more dissatisfaction(p=0.015). By monthly mean income, those who had a smaller income were dissatisfied in general(p=0.028). Conclusions : The findings of the study illustrated that it's required to spread awareness of the importance of oral health among people in general, and that differentiated incremental oral health care should be provided for different age groups. In order to raise the satisfaction of patients with prosthesis, how to relieve their pain and anxiety and how to adjust prosthetic treatment cost properly should carefully be considered. In addition, the government should take measures to offer assistance for the low-income classes in preparation for an increase in the elderly population.

Quality of life, patient preferences, and implant survival and success of tapered implant-retained mandibular overdentures as a function of the attachment system

  • Purpose: A novel attachment system for implant-retained overdentures (IRODs) with novel material combinations for improved mechanical resilience and prosthodontic success (Novaloc) has been recently introduced as an alternative to an existing system (Locator). This study investigated whether differences between the Novaloc and Locator attachment systems translate into differences in implant survival, implant success, and patient-centered outcomes when applied in a real-world in-practice comparative setting in patients restored with mandibular IRODs supported by 2 interforaminal implants (2-IRODs). Methods: This prospective, intra-subject crossover comparison compared 20 patients who received 2 intra-foraminal bone level tapered implants restored with full acrylic overdentures using either the Locator or Novaloc attachment system. After 6 months of function, the attachment in the corresponding dentures was switched, and the definitive attachment system type was delivered based on the patient's preference after 12 months. For the definitive attachment system, implant survival was evaluated after 24 months. The primary outcomes of this study were oral health-related quality of life and patient preferences related to prosthetic and implant survival. Secondary outcomes included implant survival rate and success, prosthetic survival, perceived general health, and patient satisfaction. Results: Patient-centered outcomes and patient preferences between attachment systems were comparable, with relatively high overall patient satisfaction levels for both attachment systems. No difference in the prosthetic survival rate between study groups was detected. The implant survival rate over the follow-up period after 24 months in both groups was 100%. Conclusions: The results of this in-practice comparison indicate that both attachment systems represent comparable candidates for the prosthodontic retention of 2-IRODs. Both systems showed high rates of patient satisfaction and implant survival. The influence of material combinations of the retentive system on treatment outcomes between the tested systems remains inconclusive and requires further investigations.
