St. Jude Medical 판막에 의한 승모판치환술의 임상성적 (Clinical Results of Mitral Valve Replacement with St. Jude Medical Valve)

  • 저자들은 1986년 8월부터 1996년 5월까지 172례의 환자에서 St. Jude Medical 판막을 사용하여 승모판 치환술을 시행하였다. 병원사망률(hospital mortaliy rate)은 3.5%(6례)였고, 만기사망률(late mortality rate)은 3.3%(5례)였다. 생존한 161명의 환자에서 추적관찰(follw-up)을 시행하였는데 평균기간은 50.23$\pm$0.27개월이었다. 인공판과 관련된 합병증으로는 좌방혈전 및 이로인한 뇌경색으로 사망한 예가 2례있었고, 인공판 심내막염(Prosthetic valve endocarditis)이 발생하여 사망한 예가 1례 있었으며, 항응고요법에 동반된 것으로 추정되는 출혈성 합병증은 2례에서 발생하였다. 전체환자의 10년 생존률은 92.3%이었다. 이상으로 본 흉부외과교실에서는 St. Jude 기계판을 사용하여 승모판 치환술의 중단기 임상성적을 문헌고찰과 함께 보고하는 바이며, 앞으로 계속해서 장기추적관찰에 노력할 생각이다.

이오네스큐 판막의 장기 임상성 (Long-term clinical results of isolated mitral valve replacement with the Ionescu-Shiley bovine pericardial valve)

  • A total of and consecutive 291 patients underwent isolated mitral valve replacement using the Ionescu-Shiley bovine pericardial xenograft valve during the 5-year period between October 1978 and June 1983. Thirty-two patients were the children under 15 years of age. There were 15 deaths within 30 days after surgery [operative mortality, 5.2%]. All early survivors except 6 children were placed on the long-term oral anticoagulation longer than postoperative 3 months. A total follow-up period extended for 398.2 patient-years, and 12 patients died [late mortality, 4.1%, or 3.0%/patient- year]. Ten patients experienced the thromboembolic complication [2.51%/patient-year], occurring in 8 patients within the first 3 postoperative months, and 4 died. Three patients had the late prosthetic valve endocarditis [0.75%/patient-year] and 2 died. The incidence of overall valve failure according to the criteria was 3.01%/patient-year, or 12 patients, and 2 had replacement of the failed bioprostheses [primary tissue failure, 0.5%/patient-year]. The long-term survival rate was 87.8%\ulcorner2.6% at 5 years postoperatively, and 84% of the late survivors were in NYHA Class I at the end of the follow- up. The probability remaining free from thromboembolism and overall valve failure was 89.8%\ulcorner6.3% and 81.2%\ulcorner.8% at 5 years respectively. These clinical results confirm the safety of mitral valve replacement. The only remaining clinical problem is the structural and functional durability of the bovine pericardial xenograft valve, and its use in young patients may be stopped in preference to the mechanical prosthetic valves.

IonescuShiley 조직판막 치환수술후 발생한 판막부전에 관한 임상적 연구 -7년간의 장기성적- (Valve Failure of the lonescu-Shiley Bovine Pericardial xenograft: 7 Year Follow - Up)

  • The result of valve failure with the lonescu-Shiley pericardial xenograft was presented with the review of current knowledge. This study reviewed 557 patients, who underwent total of 683 lonescu-Shiley pericardial valve replacement from 1979 to 1985 at Seoul National University Hospital. There were 357 patients who had mitral valve replacement, 73 with aortic valve and 127 with double valve replacement. There were 35 operative deaths. The survivors were followed at OPD. There were 32 patients who had prosthetic valve failure, whose ages ranged from 11 to 58 years [mean 27.8] and their postop interval was 56 ~ 22 months [range; 6-87] The causes of valve failure are prosthetic valve endocarditis in 14, primary disruption or calcification in 13, paravalvular leakage in 4, and others in 2 patients. Redo valve replacement was done in 12 patients after a mean interval of 50 * 20 months. [range; 6-79 months] Actuarial analysis of late results indicates actuarial freedom from endocarditis at 6 year is 87.9 ~ 6.8%, and actuarial freedom from primary disruption or calcification or paravalvular leakage at 5 year is 84.4 * 2.3%. In this series, however, valve failure due to thrombosis is not included.

다중판막 치환술의 임상 성적 (Clinical Experience of Multiple Valve Replacement)

  • Records of 71 consecutive patients who had received multiple valve replacement were reviewed[34male,37female,mean age 40.5$\pm$11.2 <14-63> . The early death rate was 2.8%[2/71 . A completed follow-up rate of 95.7% was accomplished in these 69 patients who left hospital[mean 42.5 $\pm$29.5 patients-years . Five of these patients died. The late death rate was 7.2%. Four patients experienced anticoagulant-related hemorrhage[all were minor . One patient had a thromboembolic episode[permanent ,and 2 had late prosthetic valve endocarditis. There was no clinical evidence of hemolysis and structural failure of valves used. Of those patients who survived,NYHA functional class improved significantly[from 87.2% class III & IV before to 95.8% class I & II after . Linearized rates for thromboembolism and anticoagulant-related hemorrhage,and for prosthetic valve endocarditis were 0.67%/100 patient-years,2.95%/100 patient-years,1.34%/100 patient-years, respectively. The actuarial estimates of incidence free of all complications and valve-related deaths were 92.2%/patient-years.Despites the advanced heart disease involving two or more native valves, the patients who had multiple valve replacement had very good results, over a 9-year period.

승모판대치술후 발생한 Aspergillus 대동맥염의 치험 1예 (Aspergillus aortitis following replacement of mitral valve)

  • Bacterial endocarditis is a well-recognized complication of prosthetic valve replacement. Postoperative fungal endocarditis of aortitis has been reported with increasing frequency. Aspergillus endocarditis or aortitis following open heart surgery has been reported in several patients. The difficulty in the diagnosis of this type of infection, the problems of therapy of prosthetic valve endocarditis0 and the relative ineffectiveness of antifungal agents account for the high mortality. Recently, we have experienced a patient with aspergillus aortitis after replacement of mitral valve. The diagnosis was finally established by histotogical examination of emboli removed from the femoral artery. The patient died after second open heart surgery for replacement of ascending aorta.

Left Coronary Ostial Obstruction by a Dislocated Sutureless Aortic Valve Prosthesis: Redo Aortic Valve Replacement with Hybrid Coronary Revascularization: A Case Report

  • Coronary ostium obstruction due to dislodgement of the prosthetic valve is a rare and life-threatening complication, and particular caution is required for sutureless aortic valve replacement (AVR) with concomitant valvular surgery. In general, coronary artery bypass surgery is performed when coronary ostium obstruction occurs after AVR, but other options may need to be considered in some cases. Herein, we present a case of coronary artery occlusion in an 82-year-old female patient who had undergone AVR and mitral valve replacement for aortic valve stenosis and mitral valve stenosis at the age of 77 years. A hybrid procedure involving redo AVR and percutaneous coronary intervention after left main coronary ostium endarterectomy was performed. To summarize, we present a case of hybrid AVR in a patient with coronary artery obstruction after AVR that was successfully managed using this method.

16세 미만에서의 심장판막치환술 (Cardiac valve replacement in patient less than 16 years of age)

  • During 12 years period from 1974 to 1985, 76 valve replacement procedures were performed at Seoul National University Hospital in 67 pediatric patients aging up to 15 years [mean 12.3 years] and it was 7.3% of total valve replacement procedures in same period. Sixty seven patients were composed of 44 males and 23 females. Of the 67 patients, 43 patients had acquired valve lesions and 24 patients congenital valve lesions. Sixty cases received single valve replacement, 7 cases double or triple valve replacement and 9 cases redo-valve replacement. Sixty seven patients received 84 valves [65 bioprosthetic valves and 19 prosthetic valves by] 76 operations, Sixty three of 65 bioprosthetic valves had been replaced before 1983 and 15 of 19 prosthetic valves in recent 2 years. Eleven patients [14.6%] died within one month post-operatively and 8 patients [10.5%] during the follow-up period with the overall mortality rate of 25.1%. There were early post-operative complication of 23 cases [30.3%] and late post-operative complication of 27 cases [35.3%] with the total complication rate of 65.8%. In 58 patients received bioprosthetic valves, there were 14 cases of restenosis of replaced valves. Of the 14 cases of bioprosthetic valve restenosis, 8 cases received redo-valve replacement procedures without mortality in 1 5/12-5 years after initial operation but 4 cases died without reoperation and 2 cases lost during follow-up. The overall mortality and complication rate were still high but they markedly decreased during recent 2 years. So, in spite of its containing many problems, valve replacement in children is becoming safe and useful procedure in whom valve reconstructive procedures are impossible.

심장판막 치환술후 단기 추적성적 (Four Year Experience with Valve Replacement of Valvular Heart Diseases)

  • 96 patients underwent cardiac valve replacement for valvular heart diseases consecutively between February 1986 to February 1990 in the Department of Thoracic and Cardiovascular Surgery of Yeungnam University Hospital. The follow up period was between 6 months and 4.5 years postoperatively[mean 23.4$\pm$13.1 months]. 75 cases got mitral valve replacement, 6 cases, aortic valve replacement, 15 cases, double valve replacement. 30[31.2%] patients were male and 66[68.8%] were female and the age ranged from 14 to 66 years old. Early hospital death within 30 days postoperation were 5 patients[5.2%], consisting of by low cardiac output in 2, infective endocarditis in 1, multiple organ failure with sepsis in 1 patient. There was no late postoperative death. Most common early postoperative complication was wound disruption [8.7%] and then low cardiac output, pneumothorax, pleural effusion in order. Most common late postoperative complications were minor bleeding episodes[8.7%] related to anticoagulant therapy which were consisted of frequent epistaxis in 3, gum bleeding in 2, hemorrhagic gastritis in 1, hypermenorrhea in 1, hematoma in right arm in 1 patient. Valve-related complications included valve thrombosis [1.6%/ patient-year], valve failure due to pannus formation[1.1% /patient-year], prosthetic valve endocarditis[1, 1%o/patient-year] and minor anticoagulant hemorrhage[4.4% /patient-year]. 5 cases of reoperations were performed in 4 patients due to valve failure and all of them were in the mitral positions[2.7% /patient-year]. Cardiothoracic ratios in the chest X-ray decreased at the 6th month and 1st year postoperation in all patients. But in New York Heart Association[NYHA] functional class IV, no change in cardiothoracic ratio was found between 6 months and 1 year postoperation. In the echocardiogram, the size of the cardiac chambers decreased, but ejection fraction increased postoperatively in each functional class. In the electrocardiogram, decreases were found in the incidence of atrial fibrillation, left atrial enlargement, left ventricular hypertrophy with right bundle branch block increasing postoperatively in each functional class. The actuarial survival rate was 98.4% for all patients, 98.7% for mitral valve replacement, 83.8% for aortic valve replacement, and 80% for double valve replacement at the end of a 4.5 year follow up period. Meanwhile the actuarial freedom rate was 91.5% for prosthetic valve endocarditis, 91.6% for thromboembolism, 89.0% for prosthetic valve failure and 83.7% for minor anticoagulant hemorrhage. Preoperative NYHA class III and IV were 75% of all patients, but 95% of all patients were up graded to NYHA class I and II postoperatively.

골육종에 시행한 Kotz prosthesis의 역학적 실패 - 증례 보고 - (Mechanical Failure of Kotz Prosthetic Replacement for Osteosarcoma - A Case Report -)

  • Prosthetic reconstruction of musculoskeletal defects about the knee for tumor has many advantages, particularly the maintenance of motion and immediate functional restoration. But, prosthetic reconstruction has inherent limitations in terms of long-term durability. The authors have reported here a patient who had mechanical failure at 61 months later following use of a modular resection system to reconstruct the segmental defect of proximal tibia in osteosarcoma. In this case, another technique of extensor mechanism reconstruction using Achilles tendon allograft was attempted. Because of the concerns involving durability of tumor prosthesis, increased emphasis has to be placed on innovation in prosthetic design.
