Acoustic differences according to the epileptic focus in benign partial epilepsy with centrotemporal spikes patients (양성 부분 간질 환아에서 간질 발생 위치에 따른 음성언어 분석)

  • Purpose : The aim of this study was to investigate the speech problems in benign rolandic epilepsy (BRE) according to the seizure focus in EEG and semiology. Methods : Twenty three patients [right origin (13 patients) or left side (10 patients)] who met the BRE criteria by International League Against Epilepsy (ILAE) were prospectively enrolled. We excluded the patients who had abnormal MRI or showed both side spikes in EEG. Computerized Speech Lab was used to assess the speech characteristics of the patients. Results : The error pattern of laryngeal articulation in BRE was exclusively substitution of stop consonants, these errors showed more frequent in the left group (16.0% vs 25.5%). Voice onset time (VOT) of stop consonants and Total duration (TD) of word in both groups were prolonged than normal control group, especially in left group (P<0.05). The first formant of vowel /o/ and second formant of /e/ were significantly decreased in left group (P<0.05). The right group scored wider on pitch range ($192.9{\pm}54.0Hz$) and energy range in spontaneous speech ($14.2{\pm}6.4db$) than the left group ($233.3{\pm}12.5Hz$, $19.4{\pm}9.3db$, respectively, P>0.05). Duration of counting (5 to 9) in left group slower than right group ($8.6{\pm}1.7$ vs $7.9{\pm}1.8sec$). Conclusion : Our data suggested that interictal spikes and seizures in either centrotemporal sides, especially left side group, may induce speech problems. We recommend the logopedic and phoniatric evaluations of speech in BRE patients.

  • The purpose of this study was to compare the characteristics of nasalance in speakers with spastic cerebral palsy (CP) according to the types of sentence used for nasalance test. Twenty-eight speakers with spastic CP participated in this study. The experiment was conducted by analyzing nasalance of prolonged vowel utterance using the Sea sentence, the Zoo sentence, and the Mother sentence. The three sentences differ in the ratio of nasal consonants. The results show significant differences among the types of sentence for nasalance test.

The perceptual judgment of sound prolongation: Equal-appearing interval and direct magnitude estimation (연장음 길이에 따른 비유창성 정도 평가: 등간척도와 직접크기평정 비교 연구)

  • This study aimed to propose an appropriate evaluation method for the perceived level of speech disfluency based on sound prolongation (i.e., increased duration of segments). To this end, 34 Korean-speaking adults (9 males, 25 females, average age: 32.9 yrs.) participated as raters in this study. The participants listened to sentences containing a total of 25 stimuli with the Korean voiceless fricative /s/ extended by 80-ms increments up to 2,000 ms (i.e., 285 ms, 365 ms., ..., 2,125 ms, 2,205 ms), and evaluated them using an equal-appearing interval scale (EAI, 1-7 points, where 1 represents "normal" and 7 represents "severe"). Subsequently, based on the interval-scale results, the sentence stimuli with the prolonged voiceless fricative corresponding to the mild-to-moderate level (rated as 4 points) were selected as the reference modulus for direct magnitude estimation (DME). After scatter plots were created for the two evaluation results, the relationship between the two measured mean values was analyzed using a curve estimation method for the observed data with the highest R2-value to determine whether a linear or curvilinear approximation fit the data better. A curvilinear relationship between the two evaluation results was indicated, suggesting that DME is a more appropriate evaluation method than the EAI scale for assessing the perceived level of disfluency based on sound prolongation.

  • Spasmodic dysphonia is an uncommon and poorly understood disorder of motor control of laryngeal speech. We analysed 88 patients with spasmodic dysphonia, using chart review. These patients had historical information evaluated for age of onset(mean 39.2 years), duration of symptoms(mean 8.8 years), sex(4.2 : 1 female to male) family history(positive in 16.7%), and primary(84.8%) and secondary(15.2%) etiology : neurological evaluation for other dystonic involvement(40.7%). Eighty-three patients(94.3%) had adductor type of spasmodic dysphonia and 4 patients were abductor type and 1 patient was mixed type. All patients had normal thyroid and parathyroid functions and among 46cases, 8 patients had abnormal brain MRI finding. seventeen patients were evaluated by auditory brainstem response parameters. four out of the 17 patiemts had prolonged wave Ⅰ-Ⅴ interpeak latency.

A Study on Low Pitch Accent Produced in Different Locations in English Sentences (영어 문장 내 상이한 위치에 나타난 저성조 피치 액센트 연구)

  • Recent studies on English $L^*$ (low pitch accent) have revealed the difference of changes in acoustic manifestation between utterances produced by Koreans and those produced by native speakers of English. However, not much effort has been made to compare $L^*$ focused constituents and non-focused constituents. At the same time, most previous works on focus realization are lacking in terms of normalization of acoustic measurement. Therefore, this research is dedicated to comparing the $L^*$ focused items and non-focused items realized by Koreans and Americans and to examining the realization of English $L^*$ produced by the two language groups with improved normalization of the acoustic features (F0, intensity and duration). Within-group analysis comparing focused words and non-focused words showed both Americans and Koreans prolonged the $L^*$ focused syllables but the effect size of syllable lengthening made by Koreans was far less than that made by Americans. Furthermore, significant F0 lowering was found in Americans but not in Koreans. However, the effect of intensity change caused by $L^*$ focus was not significant within each group. The effect of focused words was tested between the two groups revealing that Koreans implemented English $L^*$ focus with higher F0, lower intensity and shorter duration than Americans. In the instances in which a significant Group x Focus Location (initial, middle and final of a sentence) interaction was found, further analysis testing the effect of Group on each Focus Location was conducted. The testing showed that the Koreans produced shorter syllables at initial and middle of a sentence and higher F0 at initial of a sentence than Americans. Implications for the intonation training were also discussed.

The Correlation of Voice Characteristics and Depression Index Analysis in Accordance with Menstrual Cycle (월경주기에 따른 우울지수 정도와 음성특성과의 상관관계 분석)

  • This study investigated the differences between emotional parameters BDI, VHI, STAI-X-I and STAI-X-II according to the menstrual cycles of the female and the relation between changes of the depression index and voice characteristics (jitter, shimmer, CPP, HNR, $pF0{\cdot}F1{\cdot}F2{\cdot}F3$, sF0, sF4, sB1, $H1_{c/u}$, $A1_u$, $A3_c$, $H1A3_{c/u}$, $H1A1_u$). Twenty three females ($30{\pm}4.4$ years old) living in Seoul and Gyeonggi Province were participated in this study to answer the questionnaires and record their voice. The participants prolonged /a/ vowel for 5 seconds in a natural condition for their voice recording. Voice data were analyzed using the Matlab and Praat program. A t-test and a correlation analysis were conducted by using SPSS for the statistical analysis. The results are as follows. First, the BDI is significantly higher in group I (lurear phase contrast the menstrual period) and group II (follicular phase against the menstrual period) than group III (luteal phase for follicular phase) (p<.05). Second, shimmer, CPP, pF0 showed a statistically high correlation regarding the BDI in group I (lurear phase contrast the menstrual period). Voice parameters may be useful as supplement in evaluating the emotional change in the phase of menstrual cycle.

A Characteristic EEG Pattern of Angelman Syndrome

  • The two new female cases of Angelman syndrome (AS) were described, which diagnosed on the basis of clinical features (dysmorphic facial features, severe mental retardation with absent speech, peculiar jerky movements, ataxic gait and paroxysms of inappropriate laughter) and neurophysiological findings. Failure to detect the deletion of the long arm of chromosome 15 or the absence of epileptic seizure were not considered sufficient to exclude a diagnosis of AS. Feeding problems, developmental delay and early signs of ataxia, especially tremor on handling objects and unstable posture when seated, proved effective as the clinical markers for early diagnosis of AS. Most of the authors agreed about the existence of three main EEG patterns in AS which may appear in isolation or in various combinations in the same patient. The most frequently observed pattern in children has prolonged runs of high amplitude rhythmic 2-3 Hz activity predominantly over the frontal region with superimposed interictal epileptiform discharges. High amplitude rhythmic 4-6 Hz activity, prominent in the occipital regions, with spikes, which can be facilitated by eye closure, is often seen in children under the age of 12 years. The EEG findings are characteristic of AS when seen in the appropriate clinical context and can be helpful to identify AS patients at an early age when genetic counselling may be particularly important.

  • The tracheocutaneous fistula(TCF) may develop infrequently as a complication after tracheostomy. Prolonged tracheostomy tube dependence increases the risk of TCF developing, and in growth of stratified squamous epithelium lines the furrow connecting the tracheal mucosa and the skin, accounting for persistence of the fistulous tract. Such fistulas are a nuisance and create nursing and social problems including poor hygiene, aspiration, difficulty with speech, and depletion of pulmonary reserve. Surgical closure has generally been successful by primary closure, fistulectomy with primary closure, and closure by secondary intention following excision of the tracheocutaneous fistula. No large series compares the efficacy of these techniques and each has its own merits. Recent literature has purposed to minimizing complications. For ten years, from January 1985 to December 1994, the authors experienced 25 cases of TCF which were analyzed in respect to incidence and interval of cannulation, duration between decanulation and fistular closure, precedent disease, closure methods, and complications of TCF repair.

COVID-19-related Korean Fake News Detection Using Occurrence Frequencies of Parts of Speech (품사별 출현 빈도를 활용한 코로나19 관련 한국어 가짜뉴스 탐지)

  • The COVID-19 pandemic, which began in December 2019 and continues to this day, has left the public needing information to help them cope with the pandemic. However, COVID-19-related fake news on social media seriously threatens the public's health. In particular, if fake news related to COVID-19 is massively spread with similar content, the time required for verification to determine whether it is genuine or fake will be prolonged, posing a severe threat to our society. In response, academics have been actively researching intelligent models that can quickly detect COVID-19-related fake news. Still, the data used in most of the existing studies are in English, and studies on Korean fake news detection are scarce. In this study, we collect data on COVID-19-related fake news written in Korean that is spread on social media and propose an intelligent fake news detection model using it. The proposed model utilizes the frequency information of parts of speech, one of the linguistic characteristics, to improve the prediction performance of the fake news detection model based on Doc2Vec, a document embedding technique mainly used in prior studies. The empirical analysis shows that the proposed model can more accurately identify Korean COVID-19-related fake news by increasing the recall and F1 score compared to the comparison model.

Risk Factors for Hearing Loss in Very Low Birth Weight Infants: Results of Hearing Test in Infants <1,500 g (극소 저체중 출생아에서 청력 손상에 영향을 미치는 요인: 1,500 g미만의 청력 검사 결과)

  • Purpose: An association between very low birth weight infants(VLBWI) and hearing loss has long been recognized. Early identification and intervention for hearing loss benefits language and speech/cognitive development. We investigated the risk factors and clinical outcomes of hearing loss among VLBWI. Methods: We analyzed the results of auditory brainstem response (ABR) testing of VLBWI. These infants were admitted to the neonatal intensive care unit (NICU) of Pusan National University Yangsan Hospital between December 2008 and February 2011. A follow-up was conducted subsequently. Results: ABR evaluations were performed on 65 infants, and 31 showed abnormal results (47.7%). Among the 31 infants, 10 were classified with moderate/severe/profound hearing loss (15.4%). The infants with abnormal ABR had a higher incidence of low birth weight, prolonged ventilator care, cumulative dose of furosemide, and the lowest $PaO_2$ (P<0.05). Those with moderate/severe/profound hearing loss had a higher incidence of low Apgar scores at 5 minutes (odds ratio[OR],0.34; 95% confidence interval[CI],0.13-0.89), prolonged ventilator care (OR,1.06; 95% CI,1.01-1.12), and mild hearing loss compared to those without profound hearing loss. Follow-up evaluations on eight infants with ABR reveled improvements 5.6${\pm}$3.9 months later. One infant, who had profound hearing loss in both ears, used a hearing aid. Conclusion: Factors influencing hearing loss at the first VLBWI hearing screening test included lower Apgar scores at 5 min and prolonged use of a ventilator. Most VLBWI with hearing losses were expected to recover after several months of follow-up.
