Regulation of Blood Glucose Homeostasis during Prolonged Exercise

  • The maintenance of normal blood glucose levels at rest and during exercise is critical. The maintenance of blood glucose homeostasis depends on the coordination and integration of several physiological systems, including the sympathetic nervous system and the endocrine system. During prolonged exercise increased demand for glucose by contracting muscle causes to increase glucose uptake to working skeletal muscle. Increase in glucose uptake by working skeletal muscle during prolonged exercise is due to an increase in the translocation of insulin and contraction sensitive glucose transporter-4 (GLUT4) proteins to the plasma membrane. However, normal blood glucose level can be maintained by the augmentation of glucose production and release through the stimulation of liver glycogen breakdown, and the stimulation of the synthesis of glucose from other substances, and by the mobilization of other fuels that may serve as alternatives. Both feedback and feedforward mechanisms allow glycemia to be controlled during exercise. This review focuses on factors that control blood glucose homeostasis during prolonged exercise.

The Effect of Silk Amino Acid Supplementation on the Level of Blood Energy Substrates and Hormones during Prolonged Exercise

  • The silk amino acid supplementation is unknown to affect the release of several hormones related to energy production and metabolism during prolonged exercise. This study examined the effects of silk amino acid supplementation on the level of blood amino acid, energy substrates and hormones level during prolonged treadmill exercise in college taekwondo player. A prolonged treadmill test was carried out 60 min at 65% of maximal heart rate on 8 athletics. Blood samples were obtained form antecubital vein of subjects at rest bed 30 minute before test, after exercise and rest 1 hour. The subjects were supplemented silk amino acid (6,390 mg/day) fur 4 week. The silk amino acid supplementation did not produce significant changes on the levels of blood lactate, ammonia, amino acid, glucose, triglyceride, total cholesterol, HDL, LDL, seratonin and leptin at rest bed 30 minute before test, after exercise and rest 30 minute. The silk amino acid 4 week supplementation did not affect the levels of blood amino acid, energy substrates and hormones during prolonged treadmill exercise.

장시간운동시 마늘섭취가 항피로 및 피로회복에 미치는 영향 (Effect of Garlic Intake on the Antifatigue and Fatigue Recovery during Prolonged Exercise)

  • 본 연구는 건강한 남자대학생 16명을 대상(대조군=8명, 실험군=8명)으로 하여 14일간 하루 마늘 90g(매끼당 30g)을 섭취시켜 폐기능 중 산소섭취량, 이산화탄소 생성량, 환기량, 호흡상과 혈중 총 콜레스테롤, 트리글리 세라이드, HDL-콜레스테롤, LDL-콜레스테롤, 젖산, 젖산탈수소효소, 유리지방산, 글루코스의 변화를 측정 비교분석하였다. 자전거 에르고메타를 이용하여 20분간 3분 마다 50watt씩 증가하는 점증 운동부하로 최대 운동검사를 실시하여 폐기능을 측정하였고, 운동부하 전과 후에 혈액을 채취하여 분석 검토하였다. 마을섭취군이 HDL-콜레스테롤치는 높게 나타났고, LDL-콜레스테롤치와 총 콜레스테롤치는 낮게 나타났다. 젖산 농도는 마늘섭취군이 낮게 나타났고, 젖산탈수소효소의 활성도는 높았다. 산소섭취량, 이산화탄소 생성량, 환기량과 호흡상은 마늘섭취군과 대조군에서 차이를 나타내지 않았다. 이상의 본 연구를 통하여 마늘섭취군이 혈액 성분 중 HDL-콜레스테롤, LDL-콜레스테롤, 총 콜레스테롤, 젖산, 젖산 탈수소효소에서 대조군에 비해 효과가 있는 것으로 나타났으나, 마늘이 가지고 있는 어느 성분이 어떠한 작용에 의해 이와같은 효과를 가지는지에 대해서 검토가 필요하다고 생각된다.

일주일간의 간헐적 좌식차단의 혈관기능 개선 효과 (Seven Days Breaking Up Prolonged Sitting Improves Systemic Endothelial Function in Sedentary Men)

  • PURPOSE: To examine the cumulative (7 days) effect of breaking up prolonged sitting on systemic endothelial function in sedentary men. METHODS: Thirty sedentary men ($33.93{\pm}5.72years$) participated in two randomized 7 days sitting trial (Sit group (control) vs. Breaks group). The protocol of Breaks group is as follows: 4-minute of moderate-intensity marching in place (walking) every 1 hour during business hour (total: 8 breaks/day). Assessment of brachial artery endothelial function using flow-mediated dilation (FMD) and arterial stiffness indices (augmentation index, arterial pressure and pulse wave velocity) were measured before and after 7 days treatment. RESULTS: Brachial artery FMD significantly increased after 7 days breaking up prolonged sitting treatment (Breaks groups, $9.65{\pm}2.61$ to $9.62{\pm}2.6%$) compared with 7 days prolonged sitting (Sit group, $8.37{\pm}3.41$ to $10.11{\pm}3.75%$) (interaction effect, p=.004). Arterial pressure (AP) significantly increased after treatment (Breaks group, $2.75{\pm}2.19$ to $2.38{\pm}1.63mmHg$, p=.002) in Sit group but there was no change (Sit group, $1.00{\pm}3.18$ to $2.50{\pm}9.23mmHg$) in Breaks groups (interaction effect, p=.008). CONCLUSIONS: These finding show that 7 days regular breaking up prolonged sitting improve in FMD, compared with prolonged sitting. Therefore, regular breaking up prolonged sitting may improve systemic endothelial function in sedentary men.

운동과 면역반응에 대한 고찰 (Immunological Aspects of Contemporary Exercise)

  • 면역학이 발점함에 따라서 인간의 면역체계에 영향을 미치는 많은 요인들에 대한 수많은 연구들이 진행되고 있다. 연령, 영야상태, 스트레스, 질병, 비만, 고지혈증, 장애 등 무수히 많은 변인들이 면역방응에 직접적인 영향을 주고 있으나 운동 또한 상당한 영향력이 있어, 운동시 나타나는 면역 반응에 대하여 고찰하게 되었다. 규칙적인 운동은 초기면역과 그 이후의 적응면역을 증진시키며 특히 적응면역 중 세포 매개 면역반응과 항체매개 면역반응에 영향을 미치는 것으로 알려져 있다. 하지만 이러한 운동도 몸의 상태가 건강하거나 몸에 맞는 운동을 규칙적으로 실시해야 효과적이며, 운동의 경험이 없거나 스트레스나 질병이 있는 상태에서의 운 동은 오히려 신체에 부담을 주어 면역력을 악화하는 결과를 초래하게 된다. 또한 일회성의 운동도 체력수준에 맞게 수행되어야 하는데, 장시간 너무 무리하게 수행하면 에너지 고갈과 함께 면역력의 감소를 초래하여 상기도 감염과 같은 질환을 야기하게 된다. 따라서 무리한 운동 후 'open window' 시기에 적절한 이온음료의 섭취, 영양분 공급 및 휴식은 건강 관리에 필수적이라 할 수 있다. 또한 수행한 운동이 규칙적이라 할지라도 운동은 운동 환경이나 선천적인 요소에 따라 운동 유발성 천식을 포함하는 운동 유발성 알레르기 질환을 야기하는 환경을 조성할 수 있으므로 본론에서 제시한 지식을 바탕으로 마라톤과 같은 장기간 운동 시 특히 주의해야 할 것으로 여겨진다.

Effects of Heat Treatment and Rest-inserted Exercise on Muscle Activity

  • Prolonged immobilization leads to significant weakness and atrophy of the skeletal muscle and can also impair the recovery of muscle strength following injury. Therefore, it is important to minimize the period under immobilization and accelerate the return to normal activity. This study examined the effects of combined heat treatment and rest-inserted exercise on the muscle activity of the lower limb during knee flexion/extension. Twelve healthy subjects were assigned to 4 groups that included: (1) heat treatment + rest-inserted exercise; (2) heat treatment + continuous exercise; (3) no heat treatment + rest-inserted exercise; and (4) no heat treatment + continuous exercise. Heat treatment was applied for 15 mins prior to exercise. Continuous exercise groups performed knee flexion/extension at 0.5 Hz for 300 cycles without rest whereas rest-inserted exercise groups performed the same exercise but with 2 mins rest inserted every 60 cycles of continuous exercise. Changes in the rectus femoris and hamstring muscle activities were assessed at 0 and 2 weeks of treatment by measuring the electromyography signals of isokinetic maximum voluntary contraction. Significant increases in both the rectus femoris and hamstring muscles were observed after only 2 weeks of treatment when both heat treatment and rest-inserted exercise were performed. These results suggest that combination of various treatment techniques, such as heat treatment and rest-inserted exercise, may accelerate the recovery of muscle strength following injury or immobilization.

장시간의 고강도 트레드밀 운동이 기관지 폐포계에 미치는 유해한 효과 (The Harmful Effects of Prolonged Strenuous Treadmill Exercise on Bronchoalveolar System in Rats)

  • 본 연구는 장시간의 고강도 트레드밀 운동이 호흡기계의 생리를 가장 잘 반영하는 기관지 폐포계에 미치는 생리학적 효과를 규명하기 위해 실시되었다. 랫트를 대상으로 매일 8주간 운동시킨 결과 60분 및 120분 운동 그룹군은 대조군(비운동군)과 30분 운동군보다 체중이 유의하게 낮았으며 운동전보다 체중증가율도 높지 않았다. 기관지 폐포세척액 내 총 백혈구수는 세 운동군(30분, 60분, 120분) 모두 대조군보다 유의하게 높았다. 호중구수는 대조군에 비해 30분군과 120분군은 유의하게 증가하였고 60분군은 감소하였으며 30분군이 가장 높았다. 림프구수는 세 운동군 모두 대조군에 비해 유의하게 높았고 120분군이 가장 높았다. 대식세포(단구) 수는 세군 모두 대조군보다 높았고 120군이 가장 높았다. 기관지폐포액 내 인터루킨-6 농도는 대조군보다 30분군은 낮았고 60분과 120분군은 높았다. 그러나 인터루킨-10과 인터페론 감마 농도는 세 운동군 모두 대조군에 비해 유의하게 낮았다. 산화질소(nitric oxide) 농도는 세 운동군 모두 대조군보다 유의하게 높았다. 본 연구의 결과들은 1시간 이상의 고강도 트레드밀 운동은 기관지 폐포 내 백혈구 분포도에 변화를 유도하고 염증성 사이토카인 및 산화성질소 농도의 증가와 항염증성 사이토카인 농도의 감소와 같은 유해한 효과를 일으킬 수 있음을 시사하고 있다.

일개 시 지역사회 거주 독거여성노인의 요실금예방 운동프로그램 중재효과 (The Effect of an Exercise Program for Preventing Urinary Incontinence among Community-Dwelling Elderly Females Living Alone)

  • Purpose: The purpose of this study was to evaluate the effects of a comprehensive exercise program for the prevention of urinary incontinence (UI), frailty, and depression, as well as the promotion of physical function in community-dwelling elderly female living alone. Methods: A nonequivalent control group pretest-posttest design was employed. The participants were 76 vulnerable elderly female with a mild-to-moderate urinary incontinence. The exercise group completed 12 weeks of a moderate intensity, comprehensive exercise program. Descriptive statistics, independent t-tests and ANCOVA were used for data analysis with SPSS. Results: Upon the completion of the 12-week exercise program, significant improvements for UI symptoms (p=.001), timed up and go (p<.001), frailty (p=.006), and depression (p<.001) were observed, but not for hand grip strength (p=.053). Conclusion: The findings of this study indicate that the 12-week comprehensive exercise program for the prevention of urinary incontinence had positive effects on improving UI symptoms, physical function, levels of frailty and depression in elderly women living alone. More prolonged exercise programs with other types of exercise should be developed for these vulnerable elderly women, and future studies are encouraged to confirm the effect of the comprehensive exercise program in other settings.

Effects of partial sleep deprivation after prolonged exercise on metabolic responses and exercise performance on the following day

  • [Purpose] We determined the effect of partial sleep deprivation (PSD) after an exercise session on exercise performance on the following morning. [Methods] Eleven male athletes performed either a normal sleep trial (CON) or a PSD trial. On the first day (day 1), all subjects performed an exercise session consisting of 90 min of running (at 75% ${\dot{V}}O_{2max}$) followed by 100 drop jumps. Maximal strength (MVC) was evaluated before and after exercise. In the CON trial, the sleep duration was 23:00-7:00, while in the PSD trial, the sleep duration was shortened to 40% of the regular sleep duration. On the following morning (day 2), MVC, the metabolic responses during 20 min of running (at 75% ${\dot{V}}O_{2max}$), and time to exhaustion (TTE) at 85% ${\dot{V}}O_{2max}$ were evaluated. [Results] On day 2, neither the MVC nor ${\dot{V}}O_2$ during 20 min of running differed significantly between the two trials. However, the respiratory exchange ratio was significantly lower in the PSD trial than in the CON trial (p = 0.01). Moreover, the TTE was significantly shorter in the PSD trial than in the CON trial (p = 0.01). [Conclusion] A single night of PSD after an exercise session significantly decreased endurance performance without significantly changing muscle strength or cardiopulmonary response.
