A Model for the Estimation of Progression Adjustment: Factors on a Signal-Controlled Street Network (신호등이 있는 가로망에서의 신호 연동화보정계수 산정모형)

  • The purpose of this paper is to construct a model to compute a progression adjustment factor on a signalized network. In a way to construct the model, a simulation method is introduced and the TRAF-NETSIM is used as a tool of simulation. The structure of the network chooses an urban arterial network so as to measure the effect of progression and compute average stopped delay on each link. A regression model is constructed by using the results of the simulation. The stepwise variable selection in the regression model in used. The findings of this paper are as follows: i)The secondary queue and platoon ratio are sensitive to the values of the progression adjustment factor ii) The continuous model can practically reflect on various situations in the real world. The platoon adjustment factor can be computed by this model and the data required for this model can be easily obtained in the field.

Effect of Ginseng on Renal Function in Patient with Renal Injury (인삼이 신 질환 환자에서 신 기능에 미치는 영향)

  • There are many mechanisms that mediate progression of renal injury, such as abnormalities of nitric oxide (NO) regulation, increased Platelet aggregatlon, and oxygen free radical injury. Ginseng has been known to have NO dependent vasorelaxant effect and antioxidant effect, also inhibit thrombin stimulated platelet aggregation. And these effects of ginseng may have some roles in prevention of renal injury. So we studied 24 patients with mild pathologic proteinuria and hypertension to evaluate the effect of ginseng on progression of renal injury. After 1 month treatment, creatinine clearance was significantly increased especially in the patient group with normal serum creatinine level without specific side effect. The result of this study suggest that ginseng may pre vent or retard the progression of renal injury especially in early stage.

Oral precancerous lesion and oral cancer prevention (구강 전암병소 및 구강암 예방)

  • Oral precancerous lesion is a morphologically altered tissue in which oral cancer is more likely to occur than is apparently normal counterpart. As dentists always do oral examination and dental treatment, with fundamental knowledge and attention of this lesion, it is relatively easy to find one. If followed by proper treatment and management, it is possible to minimize its oral cancer progression, or at least delay it. Even if it were to progress to oral cancer, very early detection is possible. However, no specific biomarkers are present at the moment that could reveal oral precnacerous lesion that is high risk of oral cancer progression. Since early detection of oral cancer followed by treatment could show good prognosis with just a simple ablative surgery. Dentists should also instruct people to avoid risk factor related oral cancer progression and take natural compound having anticancer effect. Hereby, As a primary care givers, dentists play an important role in prevention of oral cancer.

  • The main objective of this study is to evaluate the true fracture energy and monitor the damage progression in steel fibre reinforced concrete (SFRC) specimens using acoustic emission (AE) features. Four point bending test is carried out using pre-notched plain and fibre reinforced (0.5% and 1% volume fraction) - concrete under monotonic loading. AE sensors are affixed at different locations of the specimens and AE parameters such as rise time, AE energy, hits, counts, amplitude and duration etc. are obtained. Using the captured and processed AE event data, fracture process zone is identified and the true fracture energy is evaluated. The AE data is also employed for tracing the damage progression in plain and fibre reinforced concrete, using both parametric- and signal- based techniques. Hilbert - Huang transform (HHT) is used in signal based processing for evaluating instantaneous frequency of the acoustic events. It is found that the appropriately processed and carefully analyzed acoustic data is capable of providing vital information on progression of damage on different types of concrete.

  • Chronic kidney disease (CKD) in children is associated with various complications, including poor growth and development, mineral bone disorder, cardiovascular disease, kidney failure, and mortality. Slowing down the progression of CKD is important since CKD is often not curable. Prospective cohort studies have been conducted to understand the progression and outcomes of CKD in children, and these studies have identified non-modifiable and modifiable risk factors. Recognition of known risk factors and early intervention are important to delay the progression of kidney function decline in children.

A Case of Korean Herbal Medicine Treatment for Normal Tension Glaucoma using Go-Bang (고방을 통한 정상안압 녹내장의 한방치험 1례)

  • Objectives: To provide a case report of effective use of Korean herbal medicine treatment to reduce the development and progression of normal-tension glaucoma(NTG). Methods: This case involved a 52-year-old patient who had a history of excimer laser surgery at the age of 25 and was using prescriptions such as Cosopts2 and Xalatan for eye drops. We treated this patient with Korean herbal medicine treatment for 2 years and assessed the progression of NTG based on the optical coherence tomography test results that he recevied at the ophthalmology clinic every 6 months. Results: During the two years of treatment with Korean herbal medicine, it was observed that the progression of NTG was halted. However, after discontinuing the Korean herbal treatment, the progression resumed. Conclusion: Korean herbal medicine treatment was found to be effective in treating normal tension glaucoma, and a minimum treatment period of two years is necessary for an accurate evaluation of NTG.

Patterns of failure and prognostic factors in resected extrahepatic bile duct cancer: implication for adjuvant radiotherapy

  • Purpose: To find the applicability of adjuvant radiotherapy for extrahepatic bile duct cancer (EBDC), we analyzed the pattern of failure and evaluate prognostic factors of locoregional failure after curative resection without adjuvant treatment. Materials and Methods: In 97 patients with resected EBDC, the location of tumor was classified as proximal (n = 26) and distal (n = 71), using the junction of the cystic duct and common hepatic duct as the dividing point. Locoregional failure sites were categorized as follows: the hepatoduodenal ligament and tumor bed, the celiac artery and superior mesenteric artery, and other sites. Results: The median follow-up time was 29 months for surviving patients. Three-year locoregional progression-free survival, progression-free survival, and overall survival rates were 50%, 42%, and 52%, respectively. Regarding initial failures, 79% and 81% were locoregional failures in proximal and distal EBDC patients, respectively. The most common site was the hepatoduodenal ligament and tumor bed. In the multivariate analysis, perineural invasion was associated with poor locoregional progression-free survival (p = 0.023) and progression-free survival (p = 0.012); and elevated postoperative CA19-9 (${\geq}37U/mL$) did with poor locoregional progression-free survival (p = 0.002), progression-free survival (p < 0.001) and overall survival (p < 0.001). Conclusion: Both proximal and distal EBDC showed remarkable proportion of locoregional failure. Perineural invasion and elevated postoperative CA19-9 were risk factors of locoregional failure. In these patients with high risk of locoregional failure, adjuvant radiotherapy could be considered to improve locoregional control.

Exploring 6th Graders Learning Progression for Lunar Phase Change: Focusing on Astronomical Systems Thinking (달의 위상 변화에 대한 초등학교 6학년 학생들의 학습 발달과정 탐색: 천문학적 시스템 사고를 중심으로)

  • The purpose of this study was to explore $6^{th}$ graders learning progression for lunar phase change focusing astronomical systems thinking. By analyzing the results of previous studies, we developed the constructed-response items, set up the hypothetical learning progressions, and developed the item analysis framework based on the hypothetical learning progressions. Before and after the instruction on the lunar phase change, we collected test data using the constructed-response items. The results of the assessment were used to validate the hypothetical learning progression. Through this, we were able to explore the learning progression of the earth-moon system in a bottom-up. As a result of the study, elementary students seemed to have difficulty in the transformation between the earth-based perspective and the space-based perspective. In addition, based on the elementary school students' learning progression on lunar phase change, we concluded that the concept of the lunar phase change was a bit difficult for elementary students to learn in elementary science curriculum.

Survival Outcomes of Recurrent Epithelial Ovarian Cancer: Experience from a Thailand Northern Tertiary Care Center

  • To assess survival outcomes in a retrospective study, recurrent epithelial ovarian cancer patients were divided into three groups according to the platinum free interval as follows: platinum refractory that included the patients with tumor progression during treatment; platinum resistant and platinum sensitive that included the patients with tumor progression less than or more than six months, respectively. Clinical data for tumor progression in epithelial ovarian cancer patients treated at Chiang Mai University Hospital between January, 2006 and December, 2010 were reviewed. Thirty-nine patients were in the platinum refractory group while 27 were in the platinum resistant group and 75 in the platinum sensitive group. The mean age, the parity, the administration of neoadjuvant chemotherapy and the serous type did not significantly different across groups while the mean total number of chemotherapy regimens, the early stage patients, the patients with complete surgery and the surviving patients were significant more frequent in the platinum sensitive group. Regarding subsequent treatment after tumor recurrence, 87.2% underwent chemotherapy. With the median follow up time at 29 months, the median overall survival rates were 20 months, 14 months and 42 months in platinum refractory, platinum resistant and platinum sensitive groups, respectively (p<0.001). In addition, when the platinum sensitive patients developed the next episode of tumor progression, the median progression free interval time was only three to four months. In conclusion, the outcomes for platinum refractory the and platinum resistant groups was poorer than the platinum sensitive group. However, subsequent progression in the platinum sensitive group was also associated with a poor outcome.

Slowing the Progression of Chronic Kidney Disease in Children and Adolescents (소아 청소년 만성 콩팥병의 진행 억제)

  • Slowing the progression of chronic kidney disease is much more important in children and adolescents with a relatively longer remaining life span. A practical way to assess the rate of progression of chronic kidney disease is to measure the change of GFR estimated by formulae. To slow the progression, hypertension and proteinuria have to be controlled strictly, and hypoplastic anemia must be treated with erythropoietin. If not contraindicated, ACE inhibitor or angiotensin receptor blocker is recommended with monitoring of the side effects. Trials to slow the progression should be commenced as soon as the chronic kidney disease is confirmed and needs to be continued until renal transplantation as long as residual renal function remains. An online system, the Korean Pediatric Chronic Kidney Disease Registry (http://pedcrf.or.kr/), provides tools that are useful in evaluation and management of the children and adolescents with chronic kidney diseases.
