PET/CT 검사에서 호흡에 따른 인공산물을 줄이기 위한 Average CT의 유용성 (Evaluation of Average CT to Reduce the Artifact in PET/CT)

  • 종양학에서 암의 진단, 병기 결정, 재발 판정, 그리고 치료에 대한 반응의 평가에 PET/CT의 유용함은 이미 인정되고 있다. 그런데 크기가 작거나 폐의 기저 부위, 혹은 간의 상부에 위치한 종양일수록 호흡으로 인한 위치 변위와 왜곡의 정도가 크며 PET영상의 SUV에도 영향을 미치게 된다. 따라서 정상 호흡 상태에서 얻어지는 PET 또는 CT영상을 토대로 방사선 치료를 하게 되면 치료해야 할 표적이 치료 범위에서 벗어나거나 정상 조직에 과도한 방사선이 조사될 수 있으므로 치료율이 낮아지거나 방사선에 의한 부작용이 증가할 수 있다. 본 논문의 목적은 호흡에 의한 인공산물을 최소화하고 보다 정확한 SUV 측정을 위해 ACT를 이용한 감쇠 보정 방법을 적용하여 그 유용성을 평가하고자 함이다. 하 흉부에 종양이 있는 13명의 환자를 대상으로 Discovery STE8 PET/CT스캐너를 사용하여 두 가지의 PET/CT영상을 얻었다. HCT를 사용한 감쇠 보정 영상과 ACT를 사용한 감쇠 보정 영상에서 측정한 인공물의 크기와 $SUV_{max}$를 비교 분석하였다. 인공물은 모든 환자의 하 흉부의 백색 음영 영역을 측정하여 평가하였다. $SUV_{max}$는 주요 종양의 $SUV_{max}$를 측정하여 평가하였다. 분석 프로그램은 Advantage Workstation v4.3을 사용하였다. 환자에게 7.4 MBq (0.2 mCi)/kg의 $^{18}F$-FDG를 투여한 1시간 뒤 스캔하였다. 방출 스캔은 3 min/bed로 스캔하였다. HCT 보정 영상과 비교하여 ACT 보정 영상에서 인공산물의 크기가 눈에 띄게 줄어든 것을 관찰할 수 있었다. 하 흉부의 저 보정으로 인한 인공산물 크기는 ACT 보정 영상과 HCT 보정 영상 각각 $1.5{\pm}3.5$ cm과 $13.4{\pm}4.2$ cm를 나타냈다. 주요 병소의 $SUV_{max}$의 변화는 ACT 보정 영상이 HCT 보정 영상보다 눈에 띄게 상승하였다. ACT 보정 영상에서 HCT영상과 비교하여 tumor의 $SUV_{max}$가 평균 $5.3{\pm}3.9%$ 상승하였다. 가장 큰 차이를 보인 종양은 lung의 lower lobe에 있는 종양으로 $SUV_{max}$ 7.7에서 $SUV_{max}$ 8.7로 13% 상승하였다. ACT를 이용한 감쇠 보정 영상은 하 흉부의 인공산물을 눈에 띄게 줄일 수 있어 보다 정확한 병소의 SUV를 측정 및 방사선 치료 범위 설정에 도움이 될 수 있을 것이다. 또한 ACT 기법은 병소가 횡격막 부근에 있는 환자의 경우 폐와 간의 경계를 보다 정확히 구분할 수 있어 판독시 도움이 될 수 있을 것이라고 판단된다. 추가 ACT 촬영에 의한 피폭 선량이 증가하는 점을 고려하여 적용한다면 임상적으로 유용한 효과가 있다고 사료된다.

과로로 인한 업무상 질병의 산재보상 인정기준에 관한 연구 (A Study on the Clauses of the Work-Related Disease due to Overwork in the Workmen's Compensation Law)

  • The work-related diseases due to continuous overwork are mainly cerebro- and cardio-vascular ones, which is commonly called 'Karoshi', death from overwork. Many factors are capable for Karoshi : occupational stress in relation to technological renovation and industrial rationalization, competitive social structure, and accumulated fatigue accured to long time or irregular working. And its occurence is on the rise. The World Labor Report 1993 released by ILO, pointed out the diseases related to overwork and stress as one of the most important occupational health problem. In Korea, social awareness of Karoshi is at an infant stage, and reliable statistics for its occurence are not compiled in a convenient manner. Despite the rising Karoshi, there are no reliable clauses in workmen's compensation enough to settle down the disputes. Therefore, it is not uncommon that the Labour Ministry and Civil Court find difficulties in reaching an agreement. This study was intended to provide proper compensation and prevention program for workers by suggesting reasonable compensation clauses for the death from overwork. This study consists of two comparative reviews on the compensaton clauses for the death from overwork. One is to review legal standards of Karoshi among three countries, such as Korea, Japan and Taiwan. The other is to investigate the cases of Karoshi in Korea, 121 cases identified at the Labor Welfare Corperation and the Labour Ministrial process of examination and reexamination, and 73 leading cases at the High Court of Justice. The main findings of the study are as follows : 1. Comparisons of comperative review on compensation clauses for the death from overwork among three countries. 1) All of three countries have the same kinds of disease for compensation, which were cerebro-and cardiao-vascular diseases, while for cardiac disease group, Korea has the smaller number of diseases for compensation than Japan. 2) As for the definition of overwork, the three countries share equally that overload for one week prior to collapse is considered as an important factor, but accumulated chronic fatigue is disregarded. 3) As the basis of overwork, in Japan, there is a tendency to move from the conditions of an ordinary healthy adult to those of the individual concerned in Japan, whereas there is no such concern yet in Korea. 4) All the three countries use a common standard of medical judgement in demonstrating causal relationship between a job and a disease. However, Korea is progressive in the sense that in the case of CVA at worksite, the worker himself has no obligation to prove the cause. 2. The results of a comparative review on excutive decisions by Labor Ministry and judicial decisions by the Court in Korea : A judicial decision is based on the legalistic probability, but a excutive decision is not. Therefore, excutive decisions have such restrictions that : 1) TIA (transitory ischemic cerebral attack) and myocarditis are excluded from compensation, and there is little consistency of decision in the case of cause-unknown death. 2) There is a tendency not to compensate for the death from overwork since the work terms such as repeated long-time working, shift work or night-shift work are not considered as overloading. 3) There is a tendency to regard the conditions of a ordinary healthy adult rather than those of the individual concerned(age, existing diseases, health state, etc.) as the comparative basis of overload. 4) There remains a tendency not to compensate for the death from overwork in the case of collapse occuring out of workplace, on the ground of 'on the course of working' and 'in the cause of accident'. Through the study, the fact manifests itself that Korea's compensation clauses for work-related diseases due to overwork are very restrictive. So, it is necessary to extend the Labor Ministry's clauses of compensation for the death from overwork following to the recent changes of other countries and internal judicial decisions. This is very important in the perspective of occupational health that aims at health promotion of workers including prevention of the Karoshi.

10주간 스마트머신 순환운동이 비만 중년여성의 체조성, 폐기능, 혈중지질 및 인슐린 저항성에 미치는 영향 (Effect of 10 Weeks Smart Machine Circulation Exercise on Body Composition, Lung Function, Blood Lipids and Insulin Resistance in Obesity Middle-aged Women)

  • 본 연구는 만 40-60세 비만 중년여성을 대상으로 10주간 스마트머신 순환운동이 체조성, 폐기능, 혈중지질 및 인슐린 저항성에 미치는 영향을 구명하기 위하여 운동군(n=8), 대조군(n=6)으로 구분하여 실시하였다. 스마트머신 순환운동은 주 3회, 회당 55분으로 유산소 운동의 강도는 스마트머신과 POLAR T31이 연동 되어 스마트머신에 적용되며, 1-4주차는 40-50%HRR, 5-8주차는 50-60%HRR, 9-10주차는 60-70%HRR을 적용하였고, 저항성 운동의 강도는 스마트머신을 이용하여 등속성 운동 기반으로 대상자들의 1-RM test의 데이터 값을 이용하여 1-4주차는 1-RM의 40%, 5-8주차는 1-RM의 60%, 9-10주차는 1-RM의 80%를 적용하여 실시하였다. 그 결과 체중, 체질량지수, 체지방율, 허리-엉덩이둘레비율에서 그룹×시기 간 상호작용 효과가 나타났다. 폐기능의 FVC는 시기 간 주 효과와 사후 그룹 간 유의한 차이가 나타났으며, FVC 및 FEV1은 그룹×시기 간 상호작용 효과가 나타났다. TC 및 TG는 시기 간 주 효과가 나타났으며, TC, TG 및 HDL-C는 그룹×시기 간 상호작용 효과가 나타났다. Insulin, Glucose 및 HOMA-IR은 운동 전·후 시기 간 차이에서 운동군이 유의하게 감소한 것으로 나타났다. 따라서 10주간 스마트머신 순환운동 프로그램이 비만 중년여성의 체조성, 폐기능, 혈중지질 및 인슐린 저항성에 긍정적인 영향을 미쳤으며 이는 중년여성의 비만을 개선하거나 비만을 예방할 수 있는 운동 프로그램이라고 사료된다.

긴 파이프로 이뤄진 세장형 부이 구조물의 파랑 중 생존성에 관한 모형시험 및 수치해석 연구 (An Experimental and Numerical Study on the Survivability of a Long Pipe-Type Buoy Structure in Waves)

  • 본 논문에서는 긴 파이프 이뤄진 세장형 부이 구조물의 파랑 중 거동특성에 관한 모형시험과 수치해석 연구를 수행하였다. 대상 부이 구조물은 긴 파이프를 기본 뼈대로 하여, 상부구조물, 부력재, 중력식 앵커로 구성된 아티큘레이트(Articulated)형 부이 구조물이다. 대상 해역인 서해에서의 본 부이 구조물의 생존성을 평가하기 위하여, 축척비 1/22의 축소 모형을 제작하여 선박해양플랜트연구소 해양공학수조에서 일련의 모형시험을 진행하였다. 이 때 50년 재현주기의 극한파 조건을 고려하였으며, 또한 조류 및 주기 효과를 검토하기 위하여 추가적인 실험을 수행하였다. 생존성 평가를 위한 주된 평가항목으로는 구조물의 거동, 앵커 지지력, 침수 횟수를 고려하였다. 모형시험 결과와의 상호검증을 수행하기 위하여 상용계류해석 프로그램인 OrcaFlex를 이용하여 수치 시뮬레이션을 병행하였다. 평가결과로써 먼저 조위차에 따른 본 부이 구조물의 거동 특성에 대해 살펴보았다. 고조위와 저조위 조건에서의 종동요 응답, 앵커지지력의 변화를 살펴보았으며, 수치 시뮬레이션 결과와의 직접 비교 검토하였다. 두 번째로는 파도 주기와 조류의 유무에 따른 부이 구조물의 응답 특성 변화에 대해 고찰하였다. 세 번째로는 상부구조물의 침수와 관련하여 비디오 분석을 통한 침수 횟수를 수치해석 결과와 비교 제시하였다. 마지막으로 모형시험에서 직접 계측하지 못한 구조응답과 관련하여 수치 시뮬레이션 결과를 제시하고, 극한파 중 구조적 안전성에 대해서 논하였다. 일련의 생존성 평가 연구를 통하여 본 부이 구조물의 극한파 중 거동 특성에 대해 살펴볼 수 있었으며, 파도, 조류, 조위차에 따른 민감도 특성을 통해 본 부이구조물의 취약점 및 활용성에 대해 고찰해 보고자 하였다.

기공 운동이 비만인의 건강관련 삶의 질(SF-36)과 심리적 요인에 미치는 영향 (The Effect of Qigong Exercise on SF-36 and Psychological Factors of Middle-aged Obese)

  • 많은 비만인들은 사회적인 편견과 차별로 인하여 신체상에 대한 불만족, 우울, 불안 및 정서장애를 경험하고 있으며, 더 나아가 건강과 관련된 삶의 질의 저하를 나타내고 있다. 이러한 비만인들의 정신적 고통을 개선하기 위하여 운동요법은 하나의 긍정적인 치료 방법으로 권장되고 있다. 본 연구는 동양의 전통 운동형태 중 기공 운동이 비만인에게 건강관련 삶의 질(SF-36), 자기효능감, 우울, 불안, 피로의 심리적 요인에 어떠한 효과를 나타내는지를 살펴봄으로써 비만인을 위한 운동처방 지침을 제공하고자 한다. 본 연구의 피검자는 S시 G구에 소재한 S대학교 골든웰빙 운동프로그램에 참가하는 50-60대의 성인으로 통제군 16명, 운동군 17명으로 분류하였다. 기공 운동은 12주 동안 주2회의 빈도로 실시하여, 운동 전 후로 건강관련 삶의 질(SF-36), 신체적 자기효능감, 우울, 불안, 피로도 변인을 측정하였다. 12주간의 기공 운동 전 후 건강관련 삶의 질(SF-36)의 8항목 중 감정역할 제한(F=0.187, p=0.668), 정신건강(F=2.043, p=0.163)은 집단 간의 유의한 차이를 나타내지 못하였으나, 신체기능(F=15.151, p<0.001), 신체역할 제한(F=18.278, p<0.001), 사회적 기능(F=4.957, p=0.033), 활력/피로(F=11.485, p=0.002), 신체통증(F=6.623, p=0.015), 일반적인 건강(F=4.498, p=0.042)의 6가지 변인은 집단 간의 유의한 차이를 나타내었다. 불안(F=0.631, p=0.433)은 집단 간의 유의한 차이를 나타내지 못하였으나, 신체적 자기효능감(F=6.124, p=0.019), 우울(F=5.109, p=0.031), 피로(F=7.998, p=0.008)의 3가지 변인은 집단 간의 유의한 차이를 나타내었다. 비만인에게 있어서 기공 운동은 비록 느린 동작으로 수행되는 낮은 강도의 운동형태일지라도 운동자체가 가져다주는 정신 심리적인 개선효과가 분명하게 나타나는 운동임을 본 연구를 통하여 확인할 수 있었다.

수목 도복의 3D 충격량 산출 기법 개발 (Development of 3D Impulse Calculation Technique for Falling Down of Trees)

  • 수목의 파손은 주변 재산과 생명에 치명적인 위험을 초래한다. 특히, 도복으로 인한 위험은 피해범위가 넓고, 충격의 강도가 높기 때문에 선제적으로 대응하는 것이 중요하다. 이에 도복 시 예상되는 잠재적 위험범위를 산정하거나 위험등급을 평가한 시도가 있었지만, 구체적인 물리량으로 위험을 정량화하지 못하였다. 또한, 수목과 피해대상의 형상을 입체적으로 반영하지 못했다는 아쉬움이 있다. 본 연구는 수목의 도복 시 발생할 수 있는 위험범위와 충격량을 정량적, 입체적으로 예측하기 위한 기법의 개발을 목적으로 하였다. 주요 결과는 다음과 같다. 첫째, 수목의 도복 시 발생하는 위험범위와 충격량 산출식을 정립하였다. 위험범위 산출 시 수목이 쓰러져 미끄러지는 가능성을 반영해 수고의 1.5배를 반경으로 하는 원의 내부 범위로 확대하여 계산하였다. 수목의 기울기에 따라 직립으로 생육하는 수목은 근원부를 중심으로 360° 범위, 기울어져 생육하는 수목은 기운 방향에서 180° 범위로 설정하였다. 충격량은 충돌이 발생하기 직전 수목의 나중운동량이 피해대상에 전달되는 현상을 이용해 산출하였다. 또한, 수목이 쓰러질 때 근원부를 기점으로 회전운동 하는 것을 반영해 각운동량을 산출하였으며, 이를 선운동량으로 전환하여 충격량을 계산하였다. 둘째, Rhino3D와 Grasshopper를 이용하여 위험범위, 충격량 산출 알고리즘을 개발하였다. 알고리즘은 3차원 모델 제작, 산출, 조회 기능으로 구성하였다. 3차원 모델은 Rhino3D를 이용하여 지형, 건물, 수목의 형상을 입체적 모델로 제작하였고, 이를 Grasshopper에 연결하여 공간정보를 구축하였다. 산출 기능에서는 산출식을 활용하여 알고리즘을 코딩하였다. 산출 시 수고, 기울기, 중량 등 수목의 생육 정보와 인접수목, 피해대상, 분석범위 등 주변 환경을 고려하였다. 조회 기능에서는 산출 결과를 종합하여 3차원 모델에 가시화하였다. 산출값에 따라 색상으로 구분하여 위험수목과 위험구역을 효율적으로 판단하였다. 본 연구는 수목의 도복 시 발생하는 잠재적 위험범위와 충격량을 정량적으로 산출하고, 이를 가시화하여 우선관리가 필요한 대상을 효율적으로 판단하는 방법을 제시하였다. 이는 도복 발생 시, 주변 건물과 관람객의 안전을 위한 대책 수립 및 재난 예방의 기초자료로 활용될 수 있을 것이다. 또한, 도심지와 공원, 문화재구역에서 명확한 기준 없이 진행되는 수목의 제거를 방지하는데 기여할 것이다.

미끼식물을 이용한 화상병 감염 기주 매몰지 내 화상병균 제거 효율 검증 및 병 재발 모니터링 (Monitoring the Reoccurrence of Fire Blight and the Eradication Efficiency of Erwinia amylovora in Burial Sites of Infected Host Plants Using Sentinel Plants)

  • Erwinia amylovora에 의해 발생하는 화상병은 2015년에 국내에서 처음으로 보고된 이후 2021년 기준으로 전국 22개 지역으로 전파되었다. 우리나라는 식물방역법에 따라 화상병이 발생한 사과 및 배 과원은 발생주율을 기준으로 모든 기주식물들을 완전히 제거하여 구덩이에 매몰처리한다. 이후, 3년간 화상병균 전파를 막기 위해 매몰지 위에 기주식물 식재를 금하고 있다. 매몰처리법에 의한 화상병균 박멸 효과를 확인하기 위하여, 화상병 감수성 식물을 미끼식물로 이용하여 매몰지에 식재 후 화상병 재발 유무를 확인하였다. 이를 위해, 2019년부터 2021년에 매몰처리한 경기도 안성시와 충북 충주시 소재 매몰지 3곳을 선정하여 미끼식물 감시시설을 설치하였다. 화상병 감수성 식물인 사과(부사)를 미끼식물로 선정하고, 각 감시시설당 5주를 식재하였다. 감시시설은 울타리와 그물로 격리하였다. 또한, 실시간 모니터링을 위한 CCTV와 동작감지기, 그리고 현지 기상상황을 기록하는 센서를 설치하였다. 감시시설을 주기적으로 방문하여 육안으로 화상병 발병 유무를 확인하였다. 미끼식물로부터 화상병 의심 증상을 나타내는 표본을 채취하고 화상병균 특이적 프라이머를 이용하여 loop-mediated isothermal amplification polymerase chain reaction (PCR)과 conventional PCR을 통해 화상병균 감염 유무를 확인하였다. 그 결과, 현재까지 어떠한 미끼식물에서도 화상병균은 검출되지 않았다. 따라서, 현재 시행되고 있는 매몰 후 화상병 기주식물 3년 식재 금지 조항을 완화하는 근거로 본 연구 결과를 제시하고자 하며, 이를 통해 국내 과수산업과 농가의 피해를 최소화하는 데 기여할 수 있을 것이라 생각된다.

남북한 초등 물질 영역의 교육 내용 비교 분석 (Comparative Analysis of Educational Content in the Elementary Material Area: North and South Korea)

  • 이 연구의 목적은 남북한의 초등 과학과 교육과정에서 물질 영역의 교육 내용을 비교·분석하는 것이다. 연구 대상은 남한 2022 개정 과학과 교육과정의 '물질'과 '운동과 에너지(일부)' 영역, 북한 2013 자연과 교육강령에서 '우리 주위의 물질'과 '생활 속의 과학(일부)' 영역이다. 본 연구에서는 남북한 물질 영역의 교육 내용 요소를 학년군별로 비교하였고, TIMSS 2023 초등 4학년 물질 내용 영역의 평가틀을 이용하여 남북한의 국제 수준의 물질영역 목표 반영도를 분석하였다. 남북한 물질 영역 분석에는 초등과학교육을 전공한 교사 4명과 과학교육전문가 1명이 참여하였다. 연구 결과는 다음과 같다. 첫째, 물질의 성질 주제에서 남북한의 교육과정에서 다루는 내용은 학년별로 적용 시기가 달랐고 내용의 범위와 수준에도 차이가 있었다. 둘째, 물질의 변화 주제에서 북한은 '산과 염기'를 다루지 않았지만, 용해를 빨리하는 방법은 다루고 있었다. 셋째, 북한이 남한보다 TIMSS 2023 '물질의 성질' 주제의 목표 반영도가 더 높았다. 넷째, '물질의 성질' 분석 결과와 유사하게 북한이 남한에 비해 TIMSS 2023 '물질의 변화' 주제의 목표 반영도가 높았다. 결론적으로 남북한의 물질 내용 요소와 이를 적용하는 시기가 달랐고, 국제 수준의 목표 반영도는 북한이 더 높은 것으로 나타났다. 앞으로 남북의 이념적 대립을 넘어 통일을 준비하는 관점에서 남북한 교육 교류의 활성화와 더불어 통합 과학과 교육과정의 개발에 관한 협력과 연구가 이루어지기를 기대한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay.
