Fuji VII은 제 1, 2대구치의 조기보호를 목적으로 소개되었으며 기존의 글래스 아이오노머 시멘트보다 많은 양의 불소를 유리하면서도 점도가 낮아 흐름성이 우수하고 부가적인 산부식 과정이 필요 없는 특징 때문에 치은판개가 일부 덮인 제 1, 2대구치의 교합면이나 일부만 노출된 대구치 협면구의 우식 예방과 재광화에 유용하다고 하였다. 본 연구의 목적은 Fuji VII의 재광화 효과를 알아보고 기존의 글래스 아이오노머 시멘트, 레진 강화형 글래스 아이오노머 시멘트, 콤포머와 복합레진의 재광화 효과를 비교하고자 하였다. 이에 인공적으로 형성한 우식을 가진 42개의 치아에 Fuji VII, Fuji II, Fuji II LC improved F2000, $Filtek^{TM}$ Z250의 재료를 충전하고 편광현미경 상에서 충전 직후와 4주 후 탈회 면적을 측정하였으며 두 면적의 차이를 비교하여 다음의 결과를 얻었다. 1. 재광화 정도는 Fuji VII, Fuji II, Fuji II LC improved, F2000, 대조군, $Filtek^{TM}$ Z250 순으로 크게 나타났으며, 글래스 아이오노머 계통의 Fuji VII, Fuji II, Fuji II LC improved에서 F2000, $Filtek^{TM}$ Z250, 대조군에 비해 유의한 재광화가 나타났다(p<0.05). 2. Fuji VII과 Fuji II Fuji II LC improved 간에는 유의한 재광화 정도의 차이는 관찰되지 않았다(p>0.05).
Purpose: Wnt signaling plays an essential role in the dental epithelium and mesenchyme during tooth morphogenesis. Deletion of the Wntless (Wls) gene in odontoblasts appears to reduce canonical Wnt activity, leading to inhibition of odontoblast maturation. However, it remains unclear if autonomous Wnt ligands are necessary for differentiation of dental pulp cells into odontoblast-like cells to induce reparative dentinogenesis, one of well-known feature of pulp repair to form tertiary dentin. Materials and Methods: To analyze the autonomous role of Wls for differentiation of dental pulp cells into odontoblast-like cells, we used primary dental pulp cells from unerupted molars of Wls-floxed allele mouse after infection with adenovirus for Cre recombinase expression to knockout the floxed Wls gene or control GFP expression. The differentiation of dental pulp cells into odontoblast-like cells was analyzed by quantitative real-time polymerase chain reaction. Result: Proliferation rate was significantly decreased in dental pulp cells with Cre expression for Wls knockout. The expression levels of Osterix (Osx), runt-related transcription factor 2 (Runx2), and nuclear factor I-C (Nfic) were all significantly decreased by 0.3-fold, 0.2-fold, and 0.3-fold respectively in dental pulp cells with Wls knockout. In addition, the expression levels of Bsp, Col1a1, Opn, and Alpl were significantly decreased by 0.7-fold, 0.3-fold, 0.8-fold, and 0.6-fold respectively in dental pulp cells with Wls knockout. Conclusion: Wnt ligands produced autonomously are necessary for proper proliferation and odontoblastic differentiation of mouse dental pulp cells toward further tertiary dentinogenesis.

In the presence of acute pericoronitis of mandilbular third molar, antibiotic therapy and early incision and drainage are the method of choice, followed by definitive surgical extraction of the tooth as soon as it becomes subacute. If excision of the overlying tissues is decided on, it should be done adequately. All overlying tissues must be throughly excised, and the crown portion of the unerupted tooth should be completely exposed. After excision has been completed, the wound should be managed with a surgical dressing. This should be allowed to remain approximately 7 days. And then, surgical extraction of the impacted mandibular third molar can be done usually. In this operation, there are many complications, such as, postoperative bleeding, infection, trismus, dysphasia and paresthesia. The surgeon are discredited and medicolegal problem may be occurred in the presence of many distressed complications. Therefore, the relatively nonsurgical treatment is the method of choice. So, authors selected the conservative treatment methods of incision and drainage, primary endodontic drainage, operculectomy without surgical extraction of the mandibular third molars. The results were more favorable without the postoperative complication in Wonju old offender prison.
본 증례는 열성 이영양성 수포성 표피박리증을 가진 2세 8개월 된 환자로 다발성 우식증 소견을 보여 전신마취하에 치료하였다. 1. 환자는 열성 이영양성 수포성 표피박리증으로 전신에 흉터가 있었으며 심한 빈혈 및 패혈증 증상이 있어 수혈 및 항생제 등으로 먼저 전신상태를 회복하였다. 2. 전신마취하에 모든 절치의 발치 및 구치 부위의 치수치료와 기성금속관 수복을 시행하였다.
Objective: The primary objective of this study was to quantitatively analyze the bone parameters (thickness and density) at four different interdental areas from the distal region of the canine to the mesial region of the second molar in the maxilla and the mandible. The secondary aim was to compare and contrast the bone parameters at these specific locations in terms of sex, growth status, and facial type. Methods: This retrospective cone-beam computed tomography (CBCT) study reviewed 290 CBCT images of patients seeking orthodontic treatment. Cortical bone thickness in millimeters (mm) and density in pixel intensity value were measured for the regions (1) between the canine and first premolar, (2) between the first and second premolars, (3) between the second premolar and first molar, and (4) between the first and second molars. At each location, the bone thickness and density were measured at distances of 2, 6, and 10 mm from the alveolar crest. Results: The sex comparison (male vs. female) in cortical bone thickness showed no significant difference (p > 0.001). The bone density in growing subjects was significantly (p < 0.001) lower than that in non-growing subjects for most locations. There was no significant difference (p > 0.001) in bone parameters in relation to facial pattern in the maxilla and mandible for most sites. Conclusions: There was no significant sex-related difference in cortical bone thickness. The buccal cortical bone density was higher in females than in males. Bone parameters were similar for subjects with hyperdivergent, hypodivergent, and normodivergent facial patterns.
The purpose of this study was to investigate the effects of load on root that was applied to edentulous area in three simulated situation, in each case the guiding planes of abutment were right vertical, 95 degrees, or 100 degrees to residual ridge. The 2-dimensional finite element method was used and the finite element model was prepared as fellows. Right mandibular 1st and 2nd molar was lost and the 2nd premolar with distal rest was used as primary abutment which had three different degrees of guiding plane. Then 150N of compressive force was applied to central fossae of the 1st and 2nd molars and von Mises stress and displacement was measured. The results were as follows; 1. Irrespective of slopes of guiding planes, the stress was concentrated on mesial side of root apex and distal side of coronal portion of root, in particular on junction with distal alveolar bone. As slopes of guiding planes were increased. stress on root and compact bone surrounding abutment was increased but no considerable effect was seen on compact bone of residual ridge. 2. Distal side of coronal portion of root limited by periodontal ligament was displaced distally and mesial side of apical portion was mesially. With slope of guiding plane increasing, the pattern of displacement was similar with one another but the quantity was increased. 3. Both abutment & alveolar bone were displaced downward and root of abutment, especially distal side of coronal portion, was displaced severely. As the guiding plane was tiffed more mesially over $90^{\circ}$, the degree of displacement was also increased.
The purpose of this study was to suggest the use of laser energy in the the field of operative dentistry without considerable pulpal damage and significant effects on the dental hard tissue, additionally to find out the methods which could control the temperature rise. The laser beam (CW $CO_2$ laser, output: 6W, beam diameter: 1.5mm) was focused on the center of the occlusal surface of extracted lower molars. A Ge lens (focal length 200mm) was used to focus the primary laser beam. In order to vary the total amount of the same irradiated energy, experimental subjects were devided into three groups: continuously irradiated group, intermittently irradiated group, and water-cooled group after continuous laser irradiation. Temperature changes in the pulp chamber after laser irradiation were measured and recorded by the digital thermometer and recorder. The following results were obtained: 1. Temperatures in the pulp chamber were raised up in the order of the continuously irradiated group, intermittently irradiated group, water-cooled group after continuous laser irradiation. 2. In the continuously irradiated group, the temperature was raised up $1.7^{\circ}C$, $3.8^{\circ}C$, $7.3^{\circ}C$, $17.2^{\circ}C$ after 2, 4, 8, 16 seconds of the irradiation of laser. In the intermittently irradiated group, the changes were $1.2^{\circ}C$, $3.4^{\circ}C$, $6.3^{\circ}C$, $11.1^{\circ}C$, respectively. In the water-cooled group after continuous laser irradiation, the changes were $0.0^{\circ}C$, $0.8^{\circ}C$, $1.6^{\circ}C$, $6.9^{\circ}C$, respectively. 3. The starting time of temperature rise in the pulp chamber had no connection with laser irradiation time.
This study was performed to elucidate the distribution and correlation of immunoglobulin G subclasses with the degree of inflammation in the experimentally induced rat pulp and periapical pathoses. The pulp exposures were made in 108 mandibular 1st molars of 54 rats and the teeth were left open to the oral environment The animals were sacrified at 3, 7, 15, 30, 60 and 90 days after pulp exposure, and examined microscopically and radiographically Seventy one specimens were routinely sectioned at the thickness of 4 - $6{\mu}$ and stained with Hematoxylin - eosin for histologic examination, with toluidine blue for mast cells, and with the primary antibodies against rat IgG subclasses by using the Avidin - Biotin complex method. The following results were obtained: 1. As the degree of inflammation of rat pulp and periapeces intensified, the number of IgG subclass containing cells per unit area, especially IgG2a and IgG2c, decresased. 2. The IgG2c cells were most predominantly found in the lesions with slight inflammation, IgG1 cells in mild or severe inflammation, and IgG2a cells in moderate inflammation. 3. IgG subclass containg cells were more predominantly observed in the periapical granuloma than periapical abscess or cyst(p<0.01). 4. IgG2a containing cells were predominant in pulp inflammation, IgG1 containing cells in periapical granuloma, IgG2a cells and IgG1 cells in periapical abscess, and IgG2a cells were significantly predominant in periapical cyst. 5. The number of IgG subclass containing cells and mast cells in periapical tissue decreased with time lapse after pulp exposure. And correlation index between mast cells and IgG1, IgG2a, IgG2b was stastically high.
The strain gage, holographic and photoelastic analysis etc. have been used for stress analysis of prosthesis, orthodontic or orthopedic appliances and filling materials. But these methods has some limitation in analyzing the internal stress. The Finite Element Analysis has been proved to compensate this defect and widely used in this area. The purpose of this study was to compare the stress distributions of the various temporary filling methods being used in pulpotomy procedure. Three different models were designed according to temporary filling material and method: amalgam filling with ZOE base(Model I), amalgam filling with ZPC sub-base and ZOE(Model II), IRM filling only(Model III). The results of the experiment were as follows: 1. In model I under the load case 6 and 1, the significant stress was shown to be concentrated on the buccal portion of crown. 2. Model II showed the similar pattern of stress distribution to Model I. 3. In model III under load case 2, the stress was mainly distributed on the buccal cusp tip and buccal margin of filling material. In same model under the load case 3, the stress was distributed on the lingual cusp tip. 4. Based on the above data, IRM can be assumed to have advantage over the other tested materials in reducing the incidence of crown fracture by localized the stress within the filling materials.
