Cross-sectional Study on Health Status and Symptom Recognition of Adolescents by Grade (학년에 따른 청소년의 건강상태와 증상인식에 대한 단면조사 연구)

  • Lifestyle of adolescents cause a lot of health effects in the future. Therefore, in Korea, school health law was enacted and relevant business such as school education program is being carried out. This study was conducted to recognize symptom according to grade. A survey of youth health status was conducted at 19 middle and high schools in Seongnam city from May 2015 to December 2015. The survey made up of 14 questions which was about the health status satisfaction on the adolescent was conducted to investigate frequency by year and the respective health status of 6 grades. A total of 9,584 students responded to the survey, 58.22% answered that they were not free of constipation. 25.69% of respondents had no symptoms of headache, consequently over 70% of respondents had headache. 57.06% of respondents had no symptoms of low back pain and 34.7% had no symptoms of neck & shoulder pain, therefore over 50% of respondents had muscular skeletal symptoms. In menstrual history, only 17.95% of respondents said their period was regular and painless. In respiratory history, except cold, no nasal drop & obstruction has appeared in the group of 54.02%. And 62.97% of respondents had persistent cough usually with cold and 23.41% had cough with cold breeze even if not catch cold. In the third grade of high school students, there were many complaints of pain in various parts such as headache, back pain and shoulder pain, neck pain and menstrual pain, and there was a high rate of complaints of digestive system symptom and defecation symptom. More than half of respondent had constipation discomfort, headache and musculo-skeletal symptoms, menstrual problems and cough. In the third grade of high school students, the rate of complaints of pain complaints, digestive system symptoms, and bowel symptoms was high. Therefore, there is a need for measures and management for continuous health care and health promotion in accordance with students' symptoms and age at each grade level.

Changes in Quality of Life and Related Factors of Surgical Patients with Thyroid Cancer (갑상선암 수술환자의 삶의 질 변화와 관련요인)

  • This study was conducted 1) to investigate the effects of treatment and other factors on the quality of life of thyroid cancer surgical patients and 2) to provide fundamental data for development of an intervention and symptom management program to improve the quality of life of those patients. A total of 76 patients who were diagnosed with thyroid cancer and underwent thyroidectomy from July 2013 to December 2014 participated in this study. To investigate the factors affecting quality of life, a t-test and ANOVA analyses were conducted, after which multiple regression analysis was performed. The results were statistically significant between preoperative and one month after surgery of sex, cancer history, fatigue, and quality of life until 3 months after surgery of stage, cancer history, anxiety, and pain. Multiple regression analysis showed that the most influential factors affecting the quality of life were depression and fatigue at one month prior to and after surgery and anxiety at three months after surgery, while no factors were found to be influential at six months after surgery. Overall, the results of this study suggested that it is imperative to manage depression and fatigue one month prior to and after surgery to reduce the physical and psychological pain experienced by thyroid cancer patients. Moreover, three months after the surgery, anxiety should be closely monitored and controlled to improve the quality of life of the patients. This approach is expected to reduce the burden on the health care system and social costs, which will positively affect public health.

Effects of Multiple Chronic Diseases on Periodontal Disease in Korean Adults (우리나라 성인에서 복합만성질환이 치주질환에 미치는 영향)

  • Objectives: This study was conducted to identify the relations between multiple chronic diseases and peridontal diseases in Korean adults. Methods: A total of 4,142 cases was set for analysis, who aged 35 and over and finished with the third year health survey and oral health check-up of the fifth Korea National Health and Nutrition Examination Survey(2012). Peridontal disease was defined if community periodontal index(CPI) was 3(formation of paradental cyst of more than 4mm) or 4(formation of paradental cyst of more than 6mm). Results: The subjects consisted of 48.5% male and 51.5% female. The prevalence rate of peridontal disease was found to be 30.1% in total. In peridontal disease the more the age increased, and the lower the education level and income level as well as the more where the residential area was rural, the higher the prevalence rate was(p<0.01). According to the number of multiple chronic conditions the prevalence rate of periodontal disease accounted for 27.8%, 31.9%, 33.1%, and 35.2% when there were 0, 1, 2, and 3 or more chronic diseases respectively. As a result of logistic regression analysis, gender, age, education level, residential area, current smoking, and use of oral hygienic products were found to be significant factors on peridontal disease. Conclusions: This research revealed the prevalence rate of peridontal disease was 30.1% in Korean adults and health behaviors affecting on periodontal disease were more significant.

Effects of KMS on the DNCB induced animal Model of Atopic Dermartitis (가미미후등식장탕이 DNCB 유도 아토피 피부염 동물모델에 미치는 효과)

  • Objectives: The purpose of this study is to examine the effect of water and fermentation extracts of KMS (Kami-Mihudeongsikjang-tang) on AD (atopic dermatitis). Additionally, by applying the fermentation extracts of KMS at the first sensitization and latency period, I investgated whether it could prevent AD. Methods: In this study, to test the effect and preventive efficacy of KMSs on AD. DNCB-induced BALB/c mice of AD model was used. Through histological observation, epidermis and dermis thickness, the infiltration of eoshiphils and mast cells in epidermis and dermis were examined. We measured the serum level of IgE and IL-6, TNF-alpha, and the expressions of $NF-{\kappa}B$ and MAPK protein. In order to examine the effect of KMSs on keratinocyte, HaCaT cells were treated TNF-alpha and IFN-gamma to induce an inflammatory response. The KMSs were applied at various concentration in the experimental group. We investigated TARC expression. Results: The treatment groups were reduced epidermis and dermis thickness, inhibited the infiltration of eosinophils and mast cells, reduced the serum level of IL-6. Moreover, sfKMS group reduced serum level of TNF-alpha, inhibited the protein expressions of $NF-{\kappa}B$ and the phosphoryllation of ERK, JNK and p38. Especially sfKMS-pre group were reduced the serum level of IgE, show significant inhibition on the protein expression of $NF-{\kappa}B$ and the phosphoryllation of ERK, JNK and p38. In the experiment on HaCaT cells, sfKMS group were reduced expression of TARC. Conclusions: These result suggest that wKMS and sfKMS was effective in the treament on AD, and sfKMS would prevent AD.

The Effect of Health Promotion Program on the Frailty of Rural Elderly Women Implemented at Primary Health Care Posts (일부 보건진료소에서 실시한 건강증진프로그램이 농촌여성노인의 노쇠에 미치는 효과)

  • Objective: This study was conducted to examine the effects of a health promotion program on the health condition of rural elderly women implemented at primary health care posts using Korean Frailty Index. Methods: The research was designed a nonequivalent control group pretest-posttest setting. The participants in this study were 50 residents (intervention group: 25, control group: 25) selected from 4 villages 2 primary health care posts in J city Gyeongsangnam-do. The health promotion program was conducted at the primary health care posts twice a week for 12 weeks. This program consisted of basic exercises(Gukseondo + Theraband muscle strength training) and additional activities(including modified Theraband activity, rubber ball exercise, ball massage, nutrition class, singing class). Collected data were analyzed by descriptive statistics, paired t-test, and repeated measures ANOVA with SPSS 21.0. Results: Results of the health promotion program showed that the health conditions(measured by perceived health status, frailty score, upper/lower flexibility, maximum grip strength, dynamic balance test Timed Up and Go) of the experimental group(25) all statistical significantly improved. Conclusion: Study findings indicate that the health promotion program implemented at primary health care posts on rural elderly women is effective and can contribute to a developed health promotion program for local residents in the future.

Influence of Multidimensional Deprivation on the Latent Class of Changing Trajectories: Comparison by Gender Differences (다차원적 박탈이 문제음주 변화궤적의 잠재집단에 미치는 영향: 성별 차이 비교)

  • This study performed a longitudinal research on the causal relationship between multidimensionality of problem drinking and poverty, and multidimensional deprivation meaning the inequality, focusing on gender difference. For this, this study examined the latent group of problem drinking change trajectory through the latent class growth analysis targeting total 3,770 men and 5,632 women by using the 6th-year Korea Welfare Panel Study data from 2013 to 2018, and then conducted the multinominal logistic regression analysis to verify the influence of multidimensional deprivation factors on this latent group. The main results of this study are as follows. First, the latent group of problem drinking change trajectory according to gender was classified into three latent groups in both men and women while the development aspect was different from each other. The male latent group with 'moderate level' or higher showed higher level of problem drinking than women. However, in case of 'drinking group with high level' according to gender, as time passed, the men tended to maintain it while the women tended to increase it. Second, in the results of examining the effects of multidimensional deprivation on the latent group of problem drinking change trajectory, the men with more experiences of social deprivation and the women with more experiences of social security deprivation showed the higher possibility to belong to the 'drinking group with high level' compared to the 'drinking group with low level'. Based on such results above, this study discussed the preventive/intervention measures for problem drinking according to gender.

Pneumococcal Vaccination Rate and Related Factors in Older Adults in Republic of Korea (한국 노인의 폐렴구균 백신 접종률 및 관련 요인)

  • Objectives: This study aimed to identify the vaccination rate for pneumococcus among aged 65 or older, and at the same time to determine the reasons for vaccination or non-vaccination. Methods: The population of this study was aged 65 or older, with a total of 1,150 subjects to be analyzed and a computer assisted telephone interviewing was used. The survey included pneumococcal vaccination status, reasons of vaccination, sources of information on vaccination, reasons for not vaccination, and other related factors. Results: The vaccination rate for pneumococcus among aged 65 or older was 56.2 percent, lower than 69.0 percent in the U.S. (2017), and 69.2 percent in the U.K. (2019). Among the factors related to the pneumococcal vaccination, the groups with the high vaccination rate were women, low age groups, residents of urban areas, people without a job, people with high education, medical insurance subscribers, married people, and people who have family members. In addition, the groups with high vaccination rates were those with high awareness, those who received recommendations from doctors, those who had vaccination records, those who believed in vaccination effects, and those who saw public service advertisements. Conclusions: In the future, it is necessary to develop alternatives to accurately manage vaccination records for adults who are not eligible for state support, and regular adult vaccination rates should be calculated so that they can be used as evidence for the country's infection control policy.

Preventive Effect of Garlic Administration on Respiratory Toxicity Induced through Intratracheal Instillation of Fine Dust (PM10) in Rats (실험동물 랫드를 이용한 미세먼지 기도노출에 따른 호흡기계 독성에 대한 마늘의 예방효과 탐색)

  • Objectives: Exposure to fine dust (PM10) could contribute to the occurrence of cardiovascular disease or respiratory abnormalities. Since garlic is known to possess an anti-oxidative stress effect, the present study was performed to evaluate the effect of garlic intake on fine dust-mediated pulmonary toxicity. Methods: Rats were intratracheally instilled with fine dust at 15 mg/kg body weight (BW)/day for five days following five-day intragastric intubation of garlic at 0.7 or 1.4 g/kgBW/day, or 13.1 mg/kgBW/day S-allyl-cysteine (SAC) as a reference component in garlic. Blood and bronchoalveolar lavage fluid (BALF) were collected. Results: Deposit of fine dust was visually and histopathologically observed in the lungs. Body weight gain during the instillation period was significantly lowered in all the groups instilled with fine dust. Neutrophil numbers in blood were significantly elevated in the fine dust alone group, but this alteration was diminished in the groups administered with garlic. Levels of serum glutathione were lower in the rats instilled with fine dust alone, and this decrease in the glutathione level seems dose-dependently compensated among the groups administered with garlic. Similar findings were observed in the BALF with statistical significance. Typical pulmonary histopathological observation related with inflammation was demonstrated in the lungs of the rats exposed to fine dust alone, whereas such histopathologic findings were not improved in the groups administered with garlic. Conclusion: The present study suggests that garlic intake could alleviate fine dust-mediated pulmonary or systemic toxicities. Further investigation is necessary to delineate the mechanism of garlic-mediated effects on pulmonary function.

Effect of Green Tea and Coffee Drinking after Meal on the Postprandial Fall of Blood Pressure in Elderly Person with Postprandial Hypotension (식후저혈압 노인에서 식후의 녹차와 커피 섭취가 혈압하강에 미치는 효과)

  • The purpose of this study was to compare the preventive effects of green tea and coffee drinking on postprandial hypotension in the elderly. A total of 30 women ($81.2{\pm}6.8years$) who had experience of postprandial hypotension participated 3 interventions by random order allocation. During the experimental interventions, each participant had a cup of instant coffee (60mg caffeine), or green tea (15.8mg caffeine) after lunch, and control group had only their lunch. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) were measured with 15 minute interval from 30 minutes before the meal to 120 minutes after the meal. The preprandial SBP and DBP were not significantly different between three groups. Postprandial SBP reduction measured at the 1 hour after meal was significantly smaller in coffee group compared to the control group, whereas green tea group was not ($-15.1{\pm}17.9mmHg$ vs $-13.4{\pm}22.4mmHg$ vs. $-4.1{\pm}18.6mmHg$ for control, green tea, and coffee group, respectively, p=.032). Likewise, DBP decrement at the 1 hour after meal was significantly smaller in coffee than control group ($-12.8{\pm}13.0mmHg$ vs $-6.9{\pm}16.6mmHg$ vs $-0.8{\pm}13.4mmHg$ respectively, p=.033). The HR response of the three groups were not significantly different throughout the intervention. Coffee drinking was potentially beneficial for preventing postprandial hypotension and resultant falling event.

A Scientific Critique of a Korean Court's Acquittal for Involuntary Manslaughter Related to 5-chloro-2-methylisothiazol-3(2H)-one/2-methylisothiazol-3(2H)-one (CMIT/MIT), a Humidifier Disinfectant (HD) Part II: Animal experiments, criteria for HD lung injury, and causality on individual levels (CMIT/MIT 함유 가습기 살균제 제품의 제조 및 판매기업 형사판결 1심 재판 판결문에 대한 과학적 고찰 (II) - 동물실험, 폐 손상 판정기준, 개인 인과)

  • Objectives: In January 2021, the former heads of the manufacturer SK Chemical and the vendor Aekyung were acquitted for manufacturing and selling humidifier disinfectant (HD) containing 5-chloro-2-methylisothiazol-3(2H)-one/2-methylisothiazol-3(2H)-one (CMIT/MIT). In this article, we analyzed the rationale used in this judgement in the light of scientific consideration. Methods: The sentencing document for the judgements was obtained from the Korea Supreme Court Service. In particular, the judgements made by the court related to the toxicological and individual association with HD perspectives were discussed based on scientific evidence. Results: The ruling stated that the necessary conditions for causality between CMIT/MIT and such diseases were not met based on the fact that asthma and lung damage were not found in the inhalation exposure animal experiments. The judgment overlooked the inevitable limitations of using animal experiments for verifying health effects in humans, which are often inconsistent with the observations in animals. Among 11 governmentaffirmed lung injury cases with CMIT/MIT usage, three patients' humidifier disinfectant-associated lung injury (HDLI) pathology proved that CMIT/MIT could cause lung injury similar to that caused by PHMG and PGH. In addition, five children showed decreased lung function related to damage caused by humidifier disinfectant exposure. Conclusions: We conclude that there is sufficient evidence supporting the assertion that HDs containing CMIT/MIT cause lung injuries, including asthma, contrary to the court's decision.
