  • Consumer's demand level for products and goods selection power is considerably improved. The pressure of loss compensation is also being accelerated by products which bring damage on property. The producer prevents the loss from causing poor products and considering the quality cost allotment in these circumstances. That is to say, producer should be concerned with the strategy of product liability prevention instead of passive defence policy. This paper intends to estabilish the optimum quality cost in considering product liability, and present the correlation of quality cost elements and multiple regression analysis. It can utilized in qualitycost and budget planning.

  • Recent heavy rains and typhoons broke down road slopes, which caused a hazard to safety. As a result, interests in the effective investment for disaster prevention is increasing. Measures of investigation method for slope have been taken, but the financial costs have never been analysed. This study applied the cost-benefit analysis theory used in public economics to the evaluation of the financial reasonability of management for slope. By applying cost-benefit analysis to road slope investigation method, the reduction of safety costs and resulting financial benefits can be seen and it also clarifies the reasonability and investment effects of slope management.

우리나라 해운기업의 품질코스트 관리와 활용방안 (A Study on Quality Cost Management in Korean Shipping Company)

  • Quality costs seem to be one of the effective tools to assess quality management activities, but shipping industries have not controlled quality costs because of difficulty in cost itemization and cost data collection. This study has attempted to itemize shipping quality costs in detail and researches the composition of actual quality costs of sampled shipping companies in Korea. Result of research shows that stopping quality costs were composited by 24.3%, 3.8% and 71.8% of prevention costs, appraisal costs and failure costs, respectively and very high relationship between total qualify cost and other quality costs. This research also recommended practical methods for quality management by using indexes of quality costs.

제조산업에서의 품질비용의 구성비에 관한 연구 (A Study on the Ratio Analysis of Quality Costs)

  • The purpose of this article was to understand the individual proportion of quality costs which have been expended by manufacturing industries in Seoul and Kyunggido district. The collected data on 72 firms could be use for an empirical analysis and the ratio test was implementented with the questionnaires. The finding were as following : by conparison with the proportion of quality cost in typical manufacturing industrial of U.S., We concluded that total quality cost was occupied with a small portion of sale. Also as compared with the individual item of quality cost, it was noted that the proportion of prevention cost had increased in a relatively large company.

기술가치평가를 위한 시장대체원가 접근법 (A Market-Based Replacement Cost Approach to Technology Valuation)

  • This paper proposes a new approach to technology valuation, the market-replacement cost approach which integrates the cost-based approach and market-based approach. The proposed approach estimates the market-replacement cost of a target technology using R&D costs of similar R&D projects previously conducted. Similar R&D projects are extracted from project database based on document similarity between project proposals and technology description of the target technology. R&D costs of similar R&D projects are adjusted by mirroring the rate of technological obsolescence and inflation. Market-replacement cost of the technology is then derived by calculating the weighted average of adjusted costs and similarity values of similar R&D projects. A case of "Prevention method and system for the diffusion of mobile malicious code" is presented to illustrate the proposed approach.

  • Common utility tunnel is essential to the daily lives of people underground utilities (electricity, gas and supply facilities such as water, communication facilities, sewer facilities, etc.) to improve the appearance by co-acceptance and disaster prevention, important for the conservation of the city's population was concentrated road construction the city-based facilities. There is recognition of the importance of the various supply treatment facilities in common utility tunnel as infrastructure to accommodate joint according to the city expanded, the demand for infrastructure. In this paper, a cost-benefit analysis using a one-time occurrence, without simply relying on cost or current cost, project manager for the city-dimensional feasibility study conducted, the user level of the maintenance costs and user costs, including social costs items from various angles can be investigated and proposed a mechanism of economic feasibility common utility tunnel. Evaluation of the proposed technique is cost-benefit and cost caused by installing common utility tunnel the existing pipeline area - was investigated by the benefit analysis, extended and repeated common utility tunnel installation depends much affected by the excavation, so users of reducing the number of repeat excavation convenience can be seen that this occurs.

건설사고 예방을 위한 근로자의 불안전한 행동과 휴먼에러와의 관계 분석 (An Analysis of Relationship between Unsafe Acts and Human Errors of Workers for Construction Accident Prevention)

  • Construction industry is becoming more advanced, but safety accidents are not decreasing and unsafe act (UA) and human errors (HE) are the main causes of safety accidents. Therefore, this study aims to analyze the relationships between unsafe acts and human errors for construction accident prevents. Specifically, the Correlation Analysis is used to quantify 24 combinations of the relationship between the UA and HE. Then, the Kano Model, and Timko Satisfied Coefficient was utilized to find 6 combinations for construction accident prevention plans. As the result of Timko Satisfied Coefficient, an interview was conducted with three safety managers and 6 safety prevention plan is proposed. Through these results, it is expected that the combination of 24 accidents will be basic data of safety management. Especially, the proposed safety prevention plans considering the characteristics of 6 combinations with high correlation can contribute to prevention of safety accidents at the construction site.

Cost Effectiveness Analysis of Different Management Strategies between Best Supportive Care and Second-line Chemotherapy for Platinum-resistant or Refractory Ovarian Cancer

  • Background: There is no standard treatment for patients with platinum-resistant or refractory epithelial ovarian cancer. Single agent chemotherapies have evidence of more efficacy and less toxicity than combination therapy. Most are very expensive, with appreciable toxicity and minimal survival. Since it is difficult to make comparison between outcomes, economic analysis of single-agent chemotherapy regimens and best supportive care may help to make decisions about an appropriate management for the affected patients. Objective: To evaluate the cost effectiveness of second-line chemotherapy compared with best supportive care for patients with platinum-resistant or refractory epithelial ovarian cancer. Materials and Methods: A Markov model was used to estimate the effectiveness and total costs associated with treatments. The hypothetical patient population comprised women aged 55 with platinum-resistant or refractory epithelial ovarian cancer. Four types of alternative treatment options were evaluated: 1) gemcitabine followed by BSC; 2) pegylated liposomal doxorubicin (PLD) followed by BSC; 3) gemcitabine followed by topotecan; and 4) PLD followed by topotecan. Baseline comparator of alternative treatments was BSC. Time horizon of the analysis was 2 years. Health care provider perspective and 3% discount rate were used to determine the costs of medical treatment in this study. Quality-adjusted life-years (QALY) were used to measure the treatment effectiveness. Treatment effectiveness data were derived from the literature. Costs were calculated from unit cost treatment of epithelial ovarian cancer patients at various stages of disease in King Chulalongkorn Memorial Hospital (KCMH) in the year 2011. Parameter uncertainty was tested in probabilistic sensitivity analysis by using Monte Carlo simulation. One-way sensitivity analysis was used to explore each variable's impact on the uncertainty of the results. Results: Approximated life expectancy of best supportive care was 0.182 years and its total cost was 26,862 Baht. All four alternative treatments increased life expectancy. Life expectancy of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 0.510, 0.513, 0.566, and 0.570 years, respectively. The total cost of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 113,000, 124,302, 139,788 and 151,135 Baht, respectively. PLD followed by topotecan had the highest expected quality-adjusted life-years but was the most expensive of all the above strategies. The incremental cost-effectiveness ratios (ICER) of gemcitabine followed by BSC, PLD followed by BSC, gemcitabine followed by topotecan and PLD followed by topotecan was 344,643, 385,322, 385,856, and 420,299 Baht, respectively. Conclusions: All of the second-line chemotherapy strategies showed certain benefits due to an increased life-year gained compared with best supportive care. Moreover, gemcitabine as second-line chemotherapy followed by best supportive care in progressive disease case was likely to be more effective strategy with less cost from health care provider perspective. Gemcitabine was the most cost-effective treatment among all four alternative treatments. ICER is only an economic factor. Treatment decisions should be based on the patient benefit.

The National Cancer Screening Program for Breast Cancer in the Republic of Korea: Is it Cost-Effective?

  • This goal of this research was to evaluate the cost-effectiveness of the National Cancer Screening Program (NCSP) for breast cancer in the Republic of Korea from a government expenditure perspective. In 2002-2003 (baseline), a total of 8,724,860 women aged 40 years or over were invited to attend breast cancer screening by the NCSP. Those who attended were identified using the NCSP database, and women were divided into two groups, women who attended screening at baseline (screened group) and those who did not (non-screened group). Breast cancer diagnosis in both groups at baseline, and during 5-year follow-up was identified using the Korean Central Cancer Registry. The effectiveness of the NCSP for breast cancer was estimated by comparing 5-year survival and life years saved (LYS) between the screened and the unscreened groups, measured using mortality data from the Korean National Health Insurance Corporation and the National Health Statistical Office. Direct screening costs, indirect screening costs, and productivity costs were considered in different combinations in the model. When all three of these costs were considered together, the incremental cost to save one life year of a breast cancer patient was 42,305,000 Korean Won (KW) (1 USD=1,088 KW) for the screened group compared to the non-screened group. In sensitivity analyses, reducing the false-positive rate of the screening program by half was the most cost-effective (incremental cost-effectiveness ratio, ICER=30,110,852 KW/LYS) strategy. When the upper age limit for screening was set at 70 years, it became more cost-effective (ICER=39,641,823 KW/LYS) than when no upper age limit was set. The NCSP for breast cancer in Korea seems to be accepted as cost-effective as ICER estimates were around the Gross Domestic Product. However, cost-effectiveness could be further improved by increasing the sensitivity of breast cancer screening and by setting appropriate age limits.

밴드형 첨족예방기구의 효과와 유용성 (The Effects of the Band Type Foot Drop Device)

  • A long-term bed rest results in an inevitable foot drop. Yet preventive device such as a foot board and high-neck tennis shoes, are not widely used. This study was conducted to compare the effects of a pillow-type foot board with that of the band-type device. The band-type device of a resonable price is designed for an effective prevention of foot drop, and the convenience for it use. Among the non-equivalent control group design, foot drop preventive effects, usefulness of the device, as well as its cost effectiveness were examined. The study was carried out for four months from December 1999, and its subject had been patients and nurses of two intensive care units at K medical center in Seoul. According to the study purpose, the data collected were analyzed by $x^2-test$ and t-test. The results are as follow : 1. The ROM of ankle joint is promoted in the experimental group with a band-type device comparing the control group with a pillow type one. Dorsiflexion is particularly increased in the experimental group as compared with the control group, while planterflexion hardly shows any change. 2. The usefulness score of the experimental group nurses using the band-type device (23.97) is higher than that of the control group nurses (18.88). 3. In an aspect of the unit production cost, band-type devices are low-priced compare with pillow-type or other foot board (wooden) or high-neck tennis shoes. In summary, the newly developed band-type device is both useful and preventive for a foot drop. It is also desirable regarding to the production cost and the cost effectiveness. This fact proves that the new anti-foot drop device could be practical both for clinical care and home care. Yet the results were obtained by some subjects, it should be used after a further examination. More effective nursing intervention could be observed throughout general studies, including proper positioning and foot and ankle exercises which are the important variables of the foot drop prevention.
