A Study on Cold Damage(傷寒) in the Compendium of Prescription from the Countryside(鄕藥集成方) - Focusing on citation, medical theory, prescription, medicinal herbs - (조선 의서 『향약집성방』 중에 실린 상한(傷寒) 논의 연구 - 인용 문헌, 의론(醫論), 처방, 본초 등을 중심으로 -)

  • The purpose of this paper is to derive the features of cold damage clinical medicine during the early days of the Chosun(朝鮮) period by analyzing discussions on cold damage published in the official medical book of the Chosun period, Compendium of Prescription from the Countryside(鄕藥集成方, CPC). Cold damage was one of the typical diseases in East Asia where there was constant seeking of the utilization of prescriptions, ways of preparations, and awareness regarding cold damage as shown in Zhang, Zhongjing(張仲景)'s Treatise on Cold Damage Disease(傷寒論, TCDD) below. Traditional Korean medicine which possessed the medical universality of East Asia also was no exception and through an analysis of the part on cold damage in CPC, it is expected that medical features of cold damage in Korea passed down from the Koryo(高麗) Dynasty to the early Chosun period will be revealed. For this, first there needs to be an organization of past discussions on cold damage surrounding the existence of infection and after checking the issues, exploring which of the writings related to TCDD and editions are being utilized through an analysis on citing literature of Cold Damage Disease Literature(傷寒門) and Heat Pathogen Disease Literature(熱病門) which have developed discussions on cold damage in CPC. In addition, by comparing Peaceful Holy Benevolent Prescription(太平聖惠方, PHBP) and Complete Record of Sacred Benevolence(聖濟總錄, CRSB), known to have greatly influenced CPC and Cold Damage Literature and Heat Pathogen Disease Literature, features of form and content used by CPC were analyzed. Features of form were examined through pattern of organization and number of citing literature were examined and for features of content, cold damage infection, classification, syndrome differentiation method, and utilization of materia medica among prescriptions were examined. Discussions on cold damage as being uninfectious as stated in Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論) unlike pestilence, epidemic pathogen(時氣), warm pathogen disease(溫病), and heat pathogen disease were excluded in PHBP. PHBP opened the possibility of cold damage infection and later writings, CRSB and CPC also follow this. As a result of analyzing citing literature of the part on cold damage in CPC, it is uncertain which edition of TCDD is being utilized; however, the most distinctive feature was that Classified Emergency Materia Medica(證類本草) and not writings specializing in cold damage are in use. In general, although CPC in terms of form is similar to CRSB, content creation predominantly depended on PHBP. More specifically; first, in terms of the existence of cold damage infection, arguments of PHBP and CRSB are maintained. Second, in terms of cold damage classification, although CRSB is followed, heat pathogen disease is classified separately developing PHBP as is. Third, in terms of method, as Book of Keep Healthy(南陽活人書) and CRSB compiled in later times are cited, it is deemed that arguments were raised to a certain extent regarding six-meridian syndrome differentiation(六經辨證). Fourth, although the majority of utilized materia medica among cold damage prescriptions utilize Materia Medica from the Countryside(鄕藥本草) in CPC and materia medica from Korean Peninsula, this is due to the desire for the compilation performance of CPC to be propagated to ordinary citizens and not the ruling class. CPC as the official medical book compiled in the early days of the Chosun period was greatly influenced by the Song(宋) Dynasty's medical books, PHBP and CRSB shows that cold damage medicine in the early Chosun Period indeed possesses the medical universality of East Asia. Furthermore, the features of published medical theory and prescriptions reveal the existence of the cold damage medical tradition of the Chosun period serving as clues for cold damage research tradition among Korea's medical history.

Researches on Diagnostic Indices of Skin Diseases Based on Concepts of Interstices (주리개념(腠理槪念)에 근거한 피부과 질병의 진단지표 기초연구)

  • In order to establish the diagnostic indices of skin diseases, some physiological and pathological concepts of interstices(腠理) were researched based on , and western dermatology with etymological analysis. As physiological indices for diagnosis of skin diseases, measurement of epidermis and dermis using ultrasonogram in the zhongwan and dachui's location. And for grading looseness and fineness of interstices with 3 unsound groups, measuring numbers and sizes of sweat pores in each point's 1 cm diameter circular area using comparative method and palpation assessment. Another index is superficial temperature. As pathological indices for diagnosis, validating volumes of dead skin cells and grading degrees of atrophy and degeneration of skin lesion. And as supplementary measures, absorptive degrees of cosmetics on face should be recorded according to 3 grades. These diagnostic indices can contribute to establishment of standard pattern identification and prescription of skin diseases through converting anatomical cognizance into classical concepts of interstices objectively.

  • This paper deals with the social system from the point of system thinking consisting the fundamental construct of system dynamics. The Bertalanffy's general system theory, having been criticized because of its ambiguity, and the complex science theory, emerging system theory, are integrated by using the system thinking which is characterized with three concepts, 'feedback thinking', 'dynamic thinking', 'operational thinking'. In the integration, system thinking suggests the dynamic pattern of the social system have not only an equilibrium status but also complex status. The science of complexity gives an implication to system dynamics the important of the uncertainty and complexity if we interpret the social system as an open system. To show more concrete description, I simulate the cooperation model based on the iterated prisoner dilemma. The simulation results show the diverse patterns of cooperation and betrayal. Especially the sensitivity of initial payoff will cause the chaotic strategic landscapes as the game gose on. These results mean that we should not give the hasty prescription to control social system artificially. Because social system retains the self-organizing force in itself.

A Study on the Change of Heat Transmission Coefficient According to the Degree of Windows Slope (창의 경사도에 따른 열관류율 변화에 관한 연구)

  • This study investigated the heat transmission coefficient through the experiment that the skylight, slope window of 60 degree and 30 degree consisted of pair glass and the double window of external window and internal window paper were suitable for heat insulation. As the result of experiment, the heat transmission coefficient of slope window was 1.06 times in the 60 degree, 1.18 times in the 30 degree and 1.31 times in the skylight as a standard lateral window. The heat transmission coefficient in the double window of external window and internal window paper was 3.017$\textrm{㎉}$/$\textrm{m}^2$.hr.$^{\cire}C$. The slope window was not suitable for the prescription by the increase of the heat transmission coefficient, so the user must pay attention to the treatment. This study investigated only the slope window of 12mm and 16mm pair glass and the double window of external window and internal window paper, study on the various pattern of window must be achived in a future.

The Analysis of Computational Thinking Practices in STEAM Program and its Implication for Creative Problem Solvers in the 21st Century

  • The purpose of this study was to explore if, what kinds of, how much computational thinking (CT after this) practices could be included in STEAM programs, and what kinds of CT practices could be improved to make STEAM revitalized. The CT analyzing tool with operational definitions and its examples in science education was modified and employed for 5 science-focused and 5 engineering-focused STEAM programs. There was no discerning pattern of CT practices uses between science and engineering STEAM programs but CT practices were displayed depending on their topics. The patterns of CT practices uses from each STEAM program could be used to describe what CT practices were more explored, weakly exposed, or missing. On the basis of these prescription of CT practices from each STEAM program, the researchers could develop the weakly exposed or missing CT practices to be improved for the rich experience in CT practices during STEAM programs.

Korean Medical Treatment Focusing on Cheongsim-tanggamibang for Hypertriglyceridemia Patient : A Case report (고중성지방혈증 환자에 대한 청심탕가미방 중심의 한방치료 : 증례보고)

  • Introduction The purpose of this study was to investigate the effect of Cheongsim-tanggamibang on hypertriglyceridemia in a 37-year-old Taeeum- type male patient. Case presentation A 37-year-old male patient complained of hyperhidrosis and hot sensation was diagnosed with hypertriglyceridemia by blood test and 'Liver-Heat Lung-Dry (Ganyeol-paeJo) severe pattern' of Korean Medicine. Cheongsim-tanggamibang was executed twice everyday during 2 months from July, 23rd to September, 21st of 2019. The effectiveness of Cheongsim-tanggamibang was assessed with blood test. Conclusions After the Korean traditional medicine treatment with Cheongsim-tanggamibang prescription, blood triglyceride and total cholesterol levels decreased, and chief complaining symptoms with hot sensation also improved.

Therapeutic Effect of Nangan-geon for an Elderly Patient with Refractory Abdominal Pain: a Case Report

  • Objectve: To inform a clinical usefulness of Nangan-geon (暖肝煎) decoction to treat a refractory abdominal pain especially in eldery. Methods: This case report presents an 85-year old male patient who had been suffered with a refractory abdominal pain without any abnormality in laboratory test and radiological examination for 5-years. The clinical outcome was observed by numerical rating scale (NRS) of self-reporting method. Results: Based on his previous multiple normal examination results from Western hospitals and clinical features including thin body (19.6 BMI), nocturnal pain around lower abdomen and cold-sensitivity, author diagnosed him as "deficiency and cold of liver-Qi (肝氣虛寒)" similar to intestinal spasm. After prescription with Nangan-geon (暖肝煎) for one month, all symptoms became improved notably (NRS 5), and almost completely recovered as a level of normal condition in 3 months (NRS 1). Conclusion: This study would show the potential of traditional Korean medicine (TKM) therapies for a refractory functional abdominal pain, and especially efficacy of Nangan-geon against cold-pattern of nonspecific abdominal pain in elderly patients.

Prescribing Pattern and Safety Analysis of Nonsteroidal Anti-inflammatory Drug and Gastro- Protective Agent following Reimbursement Guidelines Relaxation (요양급여심사기준 완화에 따른 비스테로이드성 항염제 및 위장관 보호제 처방 변화 및 안정성 분석)

  • Objective: The prevalence rate of osteoarthritis in Koreans aged 50 years or older is 14.3%, and the total amount of medical costs is more than KRW 1 trillion. Recently, the reimbursement guidelines for osteoarthritis treatment have changed. Methods: In this study, we sought to describe prescription patterns of nonsteroidal anti-inflammatory drugs (NSAIDs) and gastro-protective agent (GPA) and analyze the clinical and economic impacts of the new policy using the national health insurance claims data. The incidence of upper gastrointestinal adverse event by policy change was identified through the odds ratio, and changes in medicine and medical costs related to osteoarthritis through mean and median. Results: There were 204,552 patients before the reimbursement guidelines relaxation and 239,710 after it, a 17.2% rise. The prescription ratio was 3.3% for the patients prescribed with COX-2 selective NSAIDs alone and 1.3% for those with both COX-2 selective NSAIDs and GPA combination before the reimbursement guidelines relaxation. The reimbursement guidelines relaxation significantly increased their ratios to 6.9% and 2.8%, respectively. Gastrointestinal adverse events significantly reduced by 1.21%p after reimbursement guidelines relaxation. The average medicine cost per person increased significantly to KRW 140,291 from KRW 137,323 after the reimbursement guidelines relaxation, while the average medical cost per person slightly decreased from KRW 311,605 to KRW 310,755 after the relaxation, showing no meaningful difference. Conclusion: The reimbursement guidelines relaxation may influence on decreasing the upper gastrointestinal adverse event, increasing the medicine costs and maintaining the medical costs for osteoarthritis.

  • Objective : This study was carried out with focus on written by Joh, Taek-seung (曺澤承) and Joh, Byeong-who(曺秉矦) in the relation of father and son in 1933. This book is a medical book including rare data, which has never been reported to academic circles all this while. Method : First, this study looked into the authors of this book and its history of publication. Further, this study analyzed the composition and contents of this book. Lastly, this study summed up the meaning of this book from the standpoint of medical history. Result : The authors were Confucian doctors who were active in the latter era of the Joseon Dynasty and also in the period of Japanese colonial rule. They lived in Haenam district of Jeonlanam-do, and cured its neighboring local residents while studying. They published the book of by putting together their own medical experiences. The authors suggested their remedial prescription according to gender and age whereas Zhang Zhongjing(張仲景) suggested the remedial prescription according to Six-Meridian Pattern Identification & Syndrome Differentiation(六經辨證). In addition, the authors of gave weight to the relationship with internal damage. Additionally, the authors not only thought much of the relationship between internal damage and external damage but also thought of the weakness and strength of the healthy qi, and the new and the old of a disease as an important clue to medical treatment. It seems that such contents was influenced by (東醫寶鑑). Conclusion : shows the results of the research on which was spontaneously conducted in Joseon.

Analysis of Changes in Electrical Signals and Ground Reaction Force in Muscles According to Exercise Method

  • With an increasing aging population and improved standards of living, more attention has been paid to health. Although walking exercise is known as an aerobic exercise, it imposes repeated and continuous impacts on the joints of the lower extremities. Therefore, when overweight gives a burden to the lower extremity or there is a joint disease, exercise limit occurs. The articular cartilage, weakened with age, also makes it difficult for the elderly to perform walking exercises. Accordingly, this study conducted a comparative analysis between regular walking using only the lower extremity and Nordic walking, which has been known as stable. For analysis, electromyography (EMG) was performed and the ground reaction force of the upper and lower extremities were measured in the same exercise. Integrated EMG (iEMG) revealed that the upper extremity muscles were more active in Nordic walking than in regular walking, where lower extremity muscles were relatively more inactive. In addition, when EMG measurements were performed at each measurement point during walking exercise, the pattern was different. Nevertheless, the result was the same as in iEMG. The load that occurs in each exercise was measured using the ground reaction force system. As a result, Nordic walking had a lower load than regular walking. Therefore, it was found that Nordic walking minimized the load on the lower extremities owing to the effect of whole-body exercise and was a safer and more efficient exercise method.
