Effects of Socheongryongtang-ga-seoggo and Prescription D on Airway Mucin Secretion (소청룡탕가석고(小靑龍湯加石膏) 및 ${\ll}$석실비록${\gg}$소치법방(小治法方)이 호흡기 점액의 분비에 미치는 영향)

  • Objectives : In the present study, the author intended to investigate whether two oriental medical prescriptions named Socheongryongtang-ga-seoggo (SCTS) and Prescription D (P-D) significantly affect mucin release from cultured hamster tracheal surface epithelial (HTSE) cells. Materials and Methods : Confluent HTSE cells were metabolically radiolabeled with 3H-glucosamine for 24 hrs and chased for 30 min in the presence of SCTS or P-D to assess the effect of each agent on 3H-mucin release. Possible cytotoxicities of each agent were assessed by measuring lactate dehydrogenase (LDH) release. Also, the effects of SCTS and P-D on contractility of isolated tracheal smooth muscle were investigated. Results : SCTS did not affect mucin release from cultured HTSE cells, without cytotoxicity. However, P-D significantly increased mucin release from cultured HTSE cells. with significant cytotoxicity. SCTS inhibited Ach-induced contraction of isolated tracheal smooth muscle. P-D also inhibited Ach-induced contraction of isolated tracheal smooth muscle. Conclusions : The author suggests that the effects of SCTS and P-D with their components should be further investigated and it is valuable to find, from oriental medical prescriptions, novel agents which might regulate mucin secretion from airway goblet cells.

Evaluation of Clinical Outcomes by Therapeutic Dosing Interval of Denosumab (Prolia®) and Calcium-Vitamin D Prescriptions (Denosumab (Prolia®)의 투여 간격 및 칼슘·비타민 D 복합제제 처방 여부에 따른 임상적 결과 평가)

  • Background: Denosumab (Prolia®) is administered every 6 months for osteoporosis treatment. Co-administration of calcium and vitamin D is required to minimize hypocalcemia risk. We evaluated clinical outcomes based on the administration interval of denosumab and co-prescription with calcium-vitamin D combination products. Methods: A retrospective study was conducted using electronic medical records from 668 patients who started denosumab therapy between January 1 and December 31, 2018, at Seoul National University Bundang Hospital. Clinical outcomes, as measured by changes in T-score, were evaluated by the intervals and concurrent prescriptions with calcium-vitamin D combination products. Results: Of the 668 patients, 333 patients met the eligibility criteria. These patients were divided into two groups based on appropriateness of the administration interval: "Appropriate" (304 patients, 91.3%) and "Inappropriate" (29 patients, 8.3%). T-score changes were significantly higher in the "Appropriate" than in the "Inappropriate" group (0.30±0.44 vs. 0.13±0.37, p=0.048). At the beginning of the treatment, 221 patients (66.4%) were prescribed calcium-vitamin D combination products, but the changes in T-scores were not significantly different by the prescription status of the product (0.29±0.46 vs. 0.28±0.38, p=0.919). Conclusion: T-scores were significantly improved in patients with appropriate administration intervals. No significant changes in T-scores were observed by the prescription status with calcium-vitamin D combination products. For optimal treatment outcomes, prescribers should encourage adherence to the approved prescription information on dosage and administration, and pharmacists should provide medication counseling for patients.

Assessment of Visual satisfaction & Visual Function with Prescription Swimming goggles In-air and Underwater (도수 수경 착용시 실내와 수중에서의 시각적 만족도 및 시력 평가)

  • Purpose: To investigate the visual function with prescription swimming goggles. Methods: 15 university students (mean age: $22{\pm}1.54$ years) participated, with a mean distance refractive error of RE: S-1.67 D/C-0.40 D, LE: S-1.70D/C-0.37 D. Inclusion criteria were no ocular pathology, able to wear soft contact lenses to correct their refractive error to emmetropia and able to swim. Participants were fitted with contact lenses to correct all ametropia. Subjective evaluation for satisfaction of visual acuity, asthenopia and balance were also measured using a questionnaire while wearing swimming goggles with cylinder (C+1.50 D, Ax $90^{\circ}$) compared with plano sphere outside the swimming pool area. Visual acuity was assessed using the same ETDRS chart. The prescription swimming goggles powers were assessed in random order and ranged in power from S+3.00 D to S-3.00 D in 0.50 D steps. Results: Subjective evaluation was significantly worse for the swimming goggles with cylinder than for the plano powered goggles for all 3 questions, visual acuity, asthenopia and balance. Visual acuity were significantly affected by the different power of the swimming goggles (p<0.05), but there was no significant difference between the in-air in-clinic and underwater in-swimming pool measures (p=0.173). However, visual acuity measured in the clinic was significantly better than underwater for some swimming goggle powers (+3.00, +1.00, +0.50, 0, -1.00 and -2.00 D). Conclusions: Wearing swimming goggles underwater may degrade the visual acuity compared to within air but as the difference is less than 1 line of Snellen acuity, and it is unlikely to result in significant real-life effects. Having an incorrect cylinder correction was found to be detrimental resulting in lower score of satisfaction. Considering slippery floor of swimming pool area, it can be a potential risk factor. Therefore, it is important to correct any refractive error in addition to astigmatism for swimming goggle.

  • Objectives: Oriental pear was used as traditional remedies for respiratory diseases like cough and asthma etc. The purpose of this study was to observe the effects of preventing asthma by the combination of phenolic compound extracted from pear and herbal drugs. Methods: In order to study the effects of preventing asthma by the combination of phenolic compound extracted from pear and herbal drugs(Platycodon grandiflorum, Prunus armeniaca) on allergic asthma, mice were pre-treated by oral administration of the solution before antigen sensitization for 10 days and 20days. And 2 days later, mice were actively sensitized with a subcutaneous injection of ovalbumin and 13 days later, they were provoked with ovalbumin aerosols. The experimental groups were divided 6groups(10d1P, 10d2P, 10d4P, 20d1P, 20d2P and 20d4P) by meditation quantity and period. We measured isometric contractile responses to acetylcholine(ACh) and KCl in the isolated tracheal smooth muscle(TSM), IL-4, eosinophil and lymphocyte in bronchoalveolar lavage fluid(BALF), IgE in serum, WBC, RBC and hemoglobin in blood. Results: Contractile responses of TSM to ACh, the sensitivity of TSM to Ach and the maximal contractile response of TSM to KCl were decreased by direct proportion of meditation quantity. Eosinophil and IL-4 level in BALF were more significantly decreased, and IgE level in serum was more significantly increased in 10d4P and 20d4P group than the control group. Conclusions: Based on the above results, it is assumed that oral administration of the combination of phenolic compound extracted from pear and herbal drugs(Platycodon grandiflorum, Prunus armeniaca) can help the preventing effects of allergic asthma.

  • This study is designed to investigate the differences of ferritin and hemoglobin levels according to the vitamin D state in 23577 Korean adult subjects by using data from the Korea National Health and Nutrition Examination Survey IV, V. Ferritin and hemoglobin levels are analyzed based on the quintile of vitamin D levels by a generalized linear model. The results of analysis showed that there was a relationship between vitamin D state and hemoglobin levels in females in the reproductive status group (p<0.001). In case of ferritin, there was a relationship with vitamin D state in females in reproductive status group (p<0.001) and males under 65 years old group (p<0.05). Within the male group, high ferritin correlated with low vitamin D state; but in the female group, high ferritin correlated with high vitamin D state. Therefore, it is possible for the ferritin level to be affected by vitamin D state in females in reproductive status group and males under 65 years old group.

Case of the Anti-Type 2 Diabetes Effects on the Prescription of Red Onion (한약과 자색양파 제재로 호전된 제2형 당뇨병 42례 보고)

  • The purpose of this study is to observe clinically the effects of red onion prescription on type 2 diabetes. The subject of this study were middle-aged people. We measured clinically physiological index, body composition, radial arterial pulse wave before taking medicine and after taking medicine for 4 month (the prescription of red onion). The study results were as follows. In physiological Index, diastolic blood pressure(mmHg)(p<0.001), Fasting blood glucose(mg/dL)(p=0.001), and HbA1c(%)(p<0.001) were decreased significantly after taking medicine. In body Composition, waist-hip ratio(p=0.031), body fat percentage(p=0.006), fat mass(kg)(p=0.005), and muscle mass(kg)(p=0.001) were decreased significantly after taking medicine. Blood Circulation Index, ECO(L/min)(p=0.038) and ECI($L/min/m^2$)(p=0.007) were increased significantly after taking medicine, ECR($dyne^*sec^*cm^{-5}$)(p=0.002), and ECRI($dyne^*sec/cm$)(p=0.001) were decreased significantly after taking medicine for 4 month. The prescription of red onion was showed significant change in physiological index, character of body composition, blood circulation index. Based on this result, it is considered that red onion prescription has the positive effects on type 2 diabetes and blood circulation clinically. In the future, diversified studies on the effects of red onion prescription are needed.

Effect of Overcorrection (-)Lens on Stereo-acuity and Angle of Deviation in Intermittent Exotropia (간헐외사시에서 과교정 (-)렌즈가 입체시 및 사시각에 미치는 영향)

  • Purpose: This study was conducted to evaluate the effect of prescription of overcorrection (-) lens, which is the one of the non-surgical treatments, on stereo-acuity and angle of deviation in intermittent exotropia. Methods: Twenty four children with intermittent exotropia were enrolled from October 2011 to December 2011. The angle of deviation(${\Delta}$), stereo-acuity (arcsec), monocular and binocular visual acuity (BVA, LogMAR), control of exodeviation and fusional ability using Worth 4 dot test were evaluated at near (33 cm) and far (6 m), under the overcorrecting (-)lens of -1.00, -.00, and -.00 D. Results: As a baseline finding, the angle of exodeviation was $20.9{\pm}9.7$ at near and $23.0{\pm}7.5$ at far. The angle of exodeviation at near decreased to $18.5{\pm}10.0$ (p<0.01), $15.8{\pm}9.0$ (p<0.01), $14.0{\pm}9.1$ (p<0.01) compared with baseline angle of exodeviation at near, as increasing diopters of (-) lens from -.00 D, -2.00 D and -.00 D, respectively. The angle of exodeviation at far also decreased to $21.4{\pm}5.2$ (p=0.01), $19.6{\pm}6.3$ (p<0.01) compared with baseline, as increasing minus lens from -2.00 D and -3.00 D, respectively. However, BVA, control of exodeviation, fusional ability and stereo-acuity showed no significant decrease despite of increasing diopters of (-)lens. Conclusions: The prescription of overcorrection (-)lens is an effective therapeutic method in intermittent exotropia which can reduce the near and far angle of exodeviation, and binocular visual acuity and stereo-acuity maintained without significant decrease despite of application of overcorrection (-)lens.

A Study on the Reliability of Corrected Diopter according to Subjective refraction instrument (자각식굴절검사기기에 따른 교정굴절력의 신뢰도에 관한 연구)

  • Purpose: This research provided basic data for refraction by comparing the corrected diopter of trial lens and phoropter. Methods: We compared the corrected diopter of trial lens and phoropter, and analyzed statistical significance and relations of the spherical lens corrected diopter and cylindrical lens corrected diopter according to the types (trial lens and phoropter) of subjective refractive instruments. Also we analyzed statistical significance and relations between cylindrical lens corrected diopter at the astigmatism and the types (trial lens and phoropter) of subjective refractory instruments. Results: When we measured the corrected diopter of simple myopia, the mean value for corrected diopter was S-2.74D using the trial lens and S-2.65D using the phoropter. So the corrected diopter was 0.09D smaller when measured by phoropter. The degree of astigmatism was measured C-0.81D using the trial lens and C-0.77D using the phoropter which showed that the measured value was 0.04D smaller using the phoropter. On correlation analysis between the refractive instruments (trial lens and phoropter) and the corrected diopter, there was significant (p<0.01) strong correlation between refractory machine and corrected spherical diopter (r=0.996) and the correlation between refractory machine and corrected cylindrical diopter was r=0.986 and was also significant (p<0.01). Conclusions: The use of phoropter than trial lens was more desirable when performing refraction on high myopia (simple refractive error, high astigmatism), and when using trial lens, you should consider the vertex distance and the gap between overlapped lenses before prescription.

IN-VIVO DOSE RECONSTRUCT10N USING A TRANSMISION FACTOR AND AN EFFECTIVE FIELD CONCEPT (팬텀투과계수와 유효조사면 개념을 이용한 종양선량 확인에 관한 연구)

  • The aim of this study Is to develop a simple and fast method which computes in-vivo doses from transmission doses measured doting patient treatment using an ionization chamber. Energy fluence and the dose that reach the chamber positioned behind the patient is modified by three factors: patient attenuation, inverse square attenuation. and scattering. We adopted a straightforward empirical approach using a phantom transmission factor (PTF) which accounts for the contribution from all three factors. It was done as follows. First of all, the phantom transmission factor was measured as a simple ratio of the chamber reading measured with and without a homogeneous phantom in the radiation beam according to various field sizes($r_p$), phantom to chamber distance($d_g$) and phantom thickness($T_p$). Secondly, we used the concept of effective field to the cases with inhomogeneous phantom (patients) and irregular fields. The effective field size is calculated by finding the field size that produces the same value of PTF to that for the irregular field and/or inhomogeneous phantom. The hypothesis is that the presence of inhomogeneity and irregular field can be accommodated to a certain extent by altering the field size. Thirdly, the center dose at the prescription depth can be computed using the new TMR($r_{p,eff}$) and Sp($r_{p,eff}$) from the effective field size. After that, when TMR(d, $r_{p,eff}$) and SP($r_{p,eff}$) are acquired. the tumor dose is as follows. $$D_{center}=D_t/PTF(d_g,\;T_p){\times}(\frac{SCD}{SAD})^2{\times}BSF(r_o){\times}S_p(r_{p,eff}){\times}TMR(d,\;r_{p,eff})$$ To make certain the accuracy of this method, we checked the accuracy for the following four cases; in cases of regular or irregular field size, inhomogeneous material included, any errors made and clinical situation. The errors were within 2.3% for regular field size, 3.0% irregular field size, 2.4% when inhomogeneous material was included in the phantom, 3.8% for 6 MV when the error was made purposely, 4.7% for 10 MV and 1.8% for the measurement of a patient in clinic. It is considered that this methode can make the quality control for dose at the time of radiation therapy because it is non-invasive that makes possible to measure the doses whenever a patient is given a therapy as well as eliminates the problem for entrance or exit dose measurement.

  • The pharmacotherapy of schizophrenia exhibits wide inter-individual variabilities in clinical efficacy and adverse effects. Recently, human genetic diversity has been known as one of the essential factors to the variation in human drug response. This suggests that drug therapy should be tailored to the genetic characteristics of the individual. Pharmacogenetics is the field of investigation that attempts to elucidate genetic basis of an individual's responses to pharmacotherapy, considering drug effects divided into two categories as pharmacokinetics and pharmacodynamics. The emerging field of pharmacogenomics, which focuses on genetic determinants of drug response at the level of the entire human genome, is important for development and prescription of safer and more effective individually tailored drugs and will aid in understanding how genetics influence drug response. In schizophrenia, pharmacogenetic studies have shown the role of genetic variants of the cytochrome P450 enzymes such as CYP2D6, CYP2C19, and CYP2A1 in the metabolism of antipsychotic drugs. At the level of drug targets, variants of the dopamine $D_2$, $D_3$ and $D_4$, and 5-$HT_{2A}$ and 5-$HT_{2C}$ receptors have been examined. The pharmacogenetic studies in schizophrenia presently shows controversial findings which may be related to the multiple involvement of genes with relatively small effects and to the lack of standardized phenotypes. For further development in the pharmacogenomics of schizophrenia, there would be required the extensive outcome measures and definitions, and the powerful new tools of genomics, proteomics and so on.
