The Role of Genetic Polymorphisms in Nrf2 and P73 in Egyptian Women with Breast Cancer

  • Background: Breast cancer is the commonest cancer in Egyptian females. Nrf2 is involved in oxidative stress while P73 functions in response to DNA damage. This study aimed to assess the role of Nrf2 promoter and P73 G4C14 to A4T14 SNPs in breast cancer in Egypt. Patients: Eighty-five female patients with breast tumours (41 malignant, 44 benign) were included. Nrf2 (rs6721961) and p73 (G4A) SNPs were determined by PCR- CTPP assay. Results: Genotype frequencies of the Nrf2 promoter SNP were 34.2% and 37.9% for AA in benign and malignant groups respectively, and 43.9% and 40.5% for CC and, 21.9 % and 21.6% for CA. Genotype frequencies for the P73 G4A SNP were 52.9% and 44.7% for GA in benign and malignant groups respectively, and 47.1% and 55.3% for GG. Discussion: Nrf2 genotypes in pre - and post-menopausal patients, showed significantly different distributions in the 2 patient groups, the AA genotype being significantly more common in pre-menopausal patients. The P73 G4A SNP showed no relation to age of disease onset. Conclusion: The Nrf2 (rs6721961) AA genotype might be related to early breast cancer onset. In contrast the P73 G4A polymorphism showed no relation to either disease risk or age at presentation.

Diagnostic Yield of Primary Circulating Tumor Cells in Women Suspected of Breast Cancer: the BEST (Breast Early Screening Test) Study

  • Purpose: To determine the diagnostic yield of primary circulating tumor cells in women with suspicion of breast cancer, detected as a result of an abnormal mammography. Materials and Methods: Consecutive women presenting for breast biopsy as a result of a mammogram BiRADs of 3 or more, had an 8ml blood sample taken for primary circulating tumor cell (CTC) detection. Mononuclear cells were obtained using differential gel centrifugation and CTCs identified using standard immunocytochemistry using anti-mammoglobin. A test was determined to be positive if 1 CTC was detected. Results: A total of 144 women with a mean age of $54.7{\pm}15.6$ years participated, 78/144 (53.0%) had breast cancer on biopsy, 65/140 (46.3%) benign pathologies and 1(0.7%) non-Hogkins lymphoma. Increasing BiRADs scores were associated with increased cancer detection (p=0.004, RR 1.00, 4.24, 8.50). CTC mammoglobin positive had a sensitivity of 81.1% and specificity of 90.9%, with positive and negative predictive values of 90.9% and 81.1% respectively. Mammoglobin positive CTCs detected 87% of invasive cancers, while poorly differentiated cancers were negative for mammoglobin. Only 50% of in situ cancers and none of the intraductal cancers had CTCs detected. Menopausal status did not affect the diagnostic yield of the CTC test, which was higher in women with BiRADS 4 mammograms. There was a significant trend (p<0.0001 Chi squared for trends) in CTC detection frequency from intraductal, in situ and invasive (OR 1.00, 8.00, 472.00). Conclusions: The use of primary CTC detection in women suspected of breast cancer has potential uses, especially with invasive cancer, but it failed to detect intra-ductal cancer and 50% of in situ cancer. There was no difference in the diagnostic yield between pre and post menopausal women. To confirm its use in reducing biopsies in women with BIRADs 4a mammagrams and in the detection of interval invasive breast cancer, larger studies are needed.

The Effects of Laughter Therapy Program on Perceived Stress, and Psycho-Neuro-Endocrino-Immuno Responses in Obese Women (웃음치료프로그램이 비만여성의 지각된 스트레스와 심리-신경-내분비-면역 반응에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of the laughter therapy program on perceived stress and psycho-neuro-endocrine-immune responses in obese women. Methods: A nonequivalent control group with a pretest-posttest design was used. The participants (n=60), whose age ranged from 30 to 50 years (pre-menopausal and body mass index of over $25kg/m^2$), were assigned to the experimental group (n=24) or control group (n=26). The experimental group was provided with the laughter therapy program (12 sessions) for 6 weeks. Results: There were significant differences in perceived stress, psychological stress response, fasting blood sugar, interleukin-6, and tumor necrosis factor alpha between the two groups after the program. However, there were no significant differences in normalized low frequency (norm LF), normalized high frequency (norm HF), LF/HF ratio, and cortisol between the two groups after the program. Conclusion: It was found that the laughter therapy program had positive effects on some variables in terms of perceived stress and psycho-neuro-endocrine-immuno responses. It is suggested that the laughter therapy in this study can provide the direction for developing a program for obese women.

A Study on Sexual Function of Women with Coronary Artery Disease (관상동맥질환 여성의 성기능에 관한 연구)

  • The purpose of this study was to identify risk factors related to sexual function of women with coronary artery disease, and to determine the predictors of sexual function. The study design, a descriptive correlational study, was done through structural questionnaire and interview. A total of 50 subjects from C University Hospital at Kwang-ju city who have undergone coronary angiography at department of cardiology were observed and interviewed from Feb. 22, 1999 to March. 23, 1999. The number of affected vessels, the level of total serum cholesterol, and the ejection fraction of 2-D echo cardiography were analyzed to evaluate the severity of coronary artery disease. And also type A behavior pattern, health behavior, Brief Index of Sexual Functioning for Women (BISF-W) were measured. The data obtained were analyzed using percentage, mean and standard deviation, t-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression analysis via SPSS PC+. The results of this study were as follows: 1. The mean age of the subjects were 58.1 and 72.0% of those have been married over 30 years. Seventy two percentage were unemployed and monthly family income of 56.6% was less than 1,000,000 won (approximately $ 840). Eighty percent were in their postmenopausal state, and the frequency of sexual intercourse of 84.0% were two to three times per month. 2. The scores of type A behavior pattern were from 16 to 38(mean 24.94) and health behavior ranged from 21 to 43(mean 31.2). Abstinence from smoking, alcohol, and caffeine were best compliant factors and weight control and exercise were least abided ones. The result of 2D-ECHO EF showed that the half of the subjects were abnormal, and 24% had more than 240mg/dl of total serum cholesterol. The coronary angiography showed that 64% of the subjects had more than one affected vessels. 3. The predictors to explain the factor score of 'orgasm' were number of health examination, the pre- or post-menopausal state, protestant, number of coronary vessel affected, level of serum total cholesterol, and comorbid group of hypertension and diabetes, and it's total variance accounted for 52.4%. The predictors to explain the factor score of 'sexual activity' were comorbid group of hypertension and diabetes and type A behavior pattern, which accounted for 22.4% of total variance. The predictors to explain the factor score of 'sexual satisfaction' were type A behavior pattern, no religion, exercise, level of serum total cholesterol, and pre or post menopausal state, which accounted for 52.1%. The predictors to explain the factor score of 'sexual desire' were the period of marriage, type A behavior, employment or unemployment, and weight control, which accounted for 43.2%. The predictors to explain the factor score of 'external force of sexual functioning' were physical overload and exercise, which accounted for 41.1%. The predictors to explain the factor score of 'sexual activity' were family monthly income, catholics, and exercise, and which accounted for 35.4%. Above results lead us to some consensus that sexual function of women with coronary artery disease is related to various factors including vasogenic factors such as total serum cholesterol level, number of coronary vessel affected, an endocrinal factor such as menopausal state, and type A behavior pattern as a sociopshychological factor. And also health behaviors such as fitness care, overwork, weight control, and emotional tension are contributed to sexual function.

Intake Status and Recognition of Health Functional Foods by Pre- and Post-Menopausal Women in Seoul and Gyeonggi Province (서울·경기지역 폐경 전후 여성의 건강기능성식품 섭취 실태 및 인지도 조사 연구)

  • The purpose of this study was to investigate intake status and recognition of health functional foods by pre- and post-menopausal women in Seoul and Gyeonggi province. Survey questions were administered to 400 women around menopausal period, and data analysis was completed using the SPSS window program. Thirty-three percent of women recognized that they are healthy, and 47.1% of respondents are concerned with maintaining their health. However, respondents showed a low level of knowledge about the definition of menopause and health functional foods. Health concerns of respondents were significantly affected by marital status and level of education (P<0.05). Purchasing and intake of health functional foods was also significantly affected by health concerns and menopause symptoms (P<0.05). Twenty-nine percent of participants had taken health functional foods to prevent disease. However, most of them (85.9%) showed no knowledge of the main components of functional foods. In addition, knowledge associated with menopause and functional foods was affected by the level of education. The respondents' health concern, and thus purchase and intake frequencies of health functional foods, were affected by level of education. 'Effect of functional foods' was a top priority when respondents purchased health functional foods. The respondents answered black bean and pomegranate as foods that relieve menopause symptoms. However, they actually showed high intake frequency of black beans than pomegranate due to the high accessibility of black beans. The results of this study show that educational support for dietary guidelines is needed for middle-aged woman to be healthy after menopause.

Moderate diet-induced weight loss is associated with improved insulin sensitivity in middle-aged healthy obese Korean women

  • BACKGROUND/OBJECTIVE: The goal of the present study was to investigate the effects of moderate caloric restriction on ${\beta}$-cell function and insulin sensitivity in middle-aged obese Korean women. SUBJECTS/METHODS: Fifty-seven obese pre-menopausal Korean women participated in a 12-week calorie restriction program. Data on total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), and fasting serum levels of glucose, insulin, C-peptide, blood pressure, leptin and anthropometrics were collected. A dietary intake assessment was based on three days of food recording. Additionally, ${\beta}$-cell function [homeostasis model assessment of ${\beta}$-cell (HOMA-${\beta}$), insulinogenic index (ISI), C-peptide:glucose ratio, and area under curve insulin/glucose ($AUC_{ins/glu}$)] and insulin sensitivity [homeostasis model assessment for insulin resistance (HOMA-IR), Quantitative insulin-sensitivity check index (QUICKI) and Matsuda index (MI)] were recorded. RESULTS: When calories were reduced by an average of 422 kcal/day for 12 weeks, BMI (-2.7%), body fat mass (-10.2%), and waist circumference (-5%) all decreased significantly (P < 0.05). After calorie restriction, weight, body fat percentage, hip circumference, BP, TC, HDL-C, LDL-C, plasma glucose at fasting, insulin at fasting and 120 min, $AUC_{glu}$ and the insulin area under the curve all decreased significantly (all P < 0.05), while insulin sensitivity (HOMA-IR, QUICKI and Matsuda index) measured by OGTT improved significantly (P < 0.01). CONCLUSIONS: Moderate weight loss due to caloric restriction with reduction in insulin resistance improves glucose tolerance and insulin sensitivity in middle-aged obese women and thereby may help prevent the development of type 2 diabetes mellitus.

Relationship between Psychological Factors and Obesity before and after Obesity Treatment in Korean Obese Women (한국 비만 여성에서 비만 치료 전 후의 심리적 상관관계 분석)

  • Objectives : Psychological comorbidities are high in patients with obesity and are associated with a variety of medical and dietary problems. This study aims to examine the association between psychological factors and obesity. Methods : This study was performed in pre-menopausal obese($BMI{\geq}25kg/m^2$, waist circumference ${\geq}85cm$) women in Seoul, in 2008 (n=35). Every patient underwent the obesity treatment program. The program included dietary and exercise education, and abdominal mesotherapy for 6 weeks. Simple anthropometry including weight, BMI and Computed Tomography (CT) including Subcutaneous adipose tissue (SAT), Visceral adipose tissue (VAT) and Visceral adipose tissue/Subcutaneous adipose tissue ratio (VSR) were done. To assess psychological factors, the Rosenberg self-esteem scale (SES) questionnaire, Beck depression inventory (BDI) questionnaire and stress response inventory(SRI) questionnaire were administered. Results : 1. All of the obesity indicators (except VSR) decreased significantly after the obesity treatment program. 2. There was a significant relationship between self esteem (SES score) and visceral obesity (VAT and VSR) measured at the end of the program. 3. During the intervention, the more weight, BMI, and subcutaneous adipose tissue decreased, the more self-esteem (SES) increased. There was no relationship between depression (BDI) and obesity. And the change in stress response (SRI) was associated with the change of deep subcutaneous adipose tissue and total abdominal adipose tissue. Conclusions : This study proves that visceral obesity may contribute to low self-esteem, and there is a possibility that the other psychological factors could also be related with obesity in Korean obese women. Individualised antiobesity therapy may be required depending on the patient's psychological characteristics and weight loss could be helpful in order to treat psychological problem in obese patients.

The Effect of a Potential Antiobesity-Supplement on Weight Loss and Visceral Fat Accumulation in Overweight Women (과체중인 여성에서 체중 감소 보조제를 이용한 체중 및 내장지방의 감소 효과)

  • Chitosan, hydroxycitrate and L-carnitine have been known to be antiobesity components. The purpose of this study was to evaluate the combined effects of chitosan, hydroxycitrate and L-carnitine mixture as a potential antiobesity supplement in overweight women. Pre-menopausal healthy females who were overweight (percent ideal body weight > 110) were included in this study. Forty-nine subjects randomly received a placebo (n = 25) or antiobesity-supplement (n = 24), which was a mixture of chitosan, hydroxycitrate, and L-carnitine. Before and after the eight-week experimental period, anthropometric parameters, blood components and computerized tomography were measured. At baseline, the two groups were well matched in terms of age, body mass index and lipid profile. After the eight weeks of potential antiobesity supplementation, the subjects' body fat percent had decreased significantly (p < 0.001) by 5.6% (39.1 $\pm$ 1 vs 36.9 $\pm$ 1%) while lean body mass increased (p < 0.01). Vsceral fat area at the L4 vertebra decreased significantly (p < 0.01) by 8.6% in the supplemented group and the total fat area at the L4 vertebra showed a tendency to decrease (p = 0.051) by 2.4%. Also, in the group given the antiobesity-supplement rather than the placebo, the fasting triglyceride level decreased significantly (p < 0.05) by 10.0%. In addition, serum total cholesterol levels in the antiobesity-supplement group showed a tendency to decrease (p=0.159) by 2.7% (194 $\pm$ 6 vs 189 $\pm$ 6 mg/dl). No side effects were found in either group during the intervention. In conclusion, the present study demonstrated that taking a mixture of chitosan, hydroxycitrate, and L-carnitine as a potential antiobesity supplement for eight weeks produced advantageous changes in the weight and visceral fat accumulation of overweight women without any side effects. Urinary Profiles of the Endogenous Steroids in Pre-Menopausal Women with Uterine Leiomyoma (자궁근종 성장에 있어서 내인성 성호르몬의 역할)

  • 목 적: 본 연구의 목적은 내인성 성호르몬이 자궁근종의 성장에 미치는 영향을 알아보고자 연구를 시행하였다. 연구대상 및 방법: 폐경전 자궁근종 환자 27명과 같은 연령대의 정상여성 25명을 대상으로 하였다. 모든 대상군의 여성에서 24시간 소변을 모아서 소변내 estrogen, androgen의 대사체들을 GC-MS를 이용하여 측정하였으며 두 군에서의 차이를 비교분석 하였다. 결 과: 소변내 $17{\beta}$-estradiol, 5-androstene-$3{\beta}$, $16{\beta}$, $17{\beta}$-triol, 11-keto-ethiocholanolone, $11{\beta}$-hydroxy-androsterone, THS, THA, THE, a-cortolone, a-cortol 및 $\beta$-cortol가 환자군에서 의의있게 증가하였으며 $17{\beta}$estradiol/estrone 및 $11{\beta}$-hydroxy-ethiocholanolone/$11{\beta}$-hydroxy-androsterone도 환자군에서 의의있게 증가하였다. 결 론: 자궁근종의 성장은 요중 estrogen과 androgen의 농도와 밀접한 관련이 있으며 이는 환자의 스테로이드 호르몬 대사 감소에 기인한 것으로 사료된다.

Knowledge, Attitudes and Behavior of Bangkok Metropolitan Women Regarding Cervical Cancer Screening

  • Purpose: To assess knowledge, attitudes and cervical cancer screening behavior of Bangkok Metropolitan women. Materials and Methods: Thai women, aged 25-to-65 years old, having lived in Bangkok for 5 years or more were invited to participate in the study. After signing informed consent, all women were asked to complete a self-questionnaire (Thai language) with literate assistance if needed. The questionnaire was divided into 3 parts: (I) demographic data; (II) knowledge about cervical cancer screening; and (III) behavior and attitudes, towards cervical cancer screening. Adequate screening was defined as women who had ${\geq}$two cervical cancer screening tests except women aged 25-30 years who may have only one screening, and the last screen was within 5 year or had had regular screening. Results: Of 4,339 women, there were 1,857 (42.8%) with adequate screening and 2,482 (57.2%) with inadequate screening. Significant factors associated with inadequate screening included age < 45 years, pre-menopausal status, family monthly income <625 USD, no reported sexual intercourse, nulliparous, no knowledge, lack of awareness and poor attitudes. Three major reasons provided by women for inadequate screening were no symptoms (54.4%), fear of pain (33.2%), and embarrassment (34.6%). Conclusions: Personal features, knowledge, and attitudes influence screening behavior of Bangkok Metropolitan women. The three most common reasons of women for not undergoinging screening are no symptoms, fear of pain, and embarrassment. These factors should be the focus of attention to improve coverage of cervical cancer screening in Bangkok.
