Purpose: The purpose of this study was to evaluate whether the effect of the Internet diabetic education on plasma glucose in people with hyperglycemic diabetes. Method: A randomized design with control and experimental groups being assessed pre- and post-intervention was used. Twenty-two patients were randomly assigned to an intervention group and 21 to a control group. Participants were requested to input the blood glucose level weekly in http://www.biodang.com by cellular phone or wire Internet for 3 months. The researcher sends optimal recommendations to each patient using short message service(SMS) of cellular phone and wire Internet weekly. Results: Patients in the intervention group had a mean decrease of 1.3% in glycosylated haemoglobin ($HbA_1c$) levels and those in the control group had no significant difference. There was a significant mean change in 2-hour postprandial blood glucose(2HPPG) for the intervention group, with a mean change of-75.2mg/dl. The mean change in the control group was, however, not significant. Conclusion: These findings indicated that a web-based intervention using SMS of cellular phone improved $HbA_1c$ and 2HPPG.

- Abstract - Objectives: We examined the effects of diabetes mellitus (DM) case management program developed by National Health Insurance Corporation in Korea. Methods: One trained nurse performed the case management for 30 DM patients recruited through the reimbursement information of National Health Insurance in a rural community. The case manager had four home visits and two telephone calls for 12 weeks. We evaluated the effectiveness of the program in terms of self-efficacy, self-care behaviors and glyco-metabolic control. Results: The self-efficacy score was significantly increased from 70.0 of pre-intervention to 80.5 of post-intervention (p<0.001). The self-care score was also significantly increased from 2.86 to 3.60 (p<0.001). The scores of its components such as nutritional care, self-test of sugar level, foot care, oral care, and aerobic exercise were significantly improved (p<0.001). However, the improvement of score in medication management was statistically marginal (p=0.071). The fasting blood sugar level and HbA1c level was significantly decreased from 128.6 mg/dl to 114.5 mg/dl (p<0.001) and from 6.78 % to 6.54 % (p<0.001) respectively. Conclusions: The case management program was effective to improve the self-efficacy and self-care behaviors, and to control blood sugar level in DM patients.
This study was conducted to identify the effects of a planned exercise program based on Bandura's self efficacy model on metabolism, and the exercise compliance in type 2 diabetes mellitus patients. The study design was a nonequivalent pre-test post-test control design. Thirty four type 2 diabetes mellitus patients, who received follow-up care regularly through the diabetic out-patient clinic, were randomly sampled for this study. Twenty patients were assigned to the experimental group and fourteen patients were assigned to the control group. In the experimental group, a planned exercise program is composed of an individualized exercise prescription for 12 weeks, an individual education, and even a telephone coach program. In the case of the control group, they were instructed to continue with their usual schedules. The data collection period was from March 1999 to February 2000 Data were analyzed using SPSS/WINDOW 10.0 program. The results were as follows. 1. In the experimental group, the level of fasting blood sugar has significantly decreased from 188.20 mg/㎗ to 155.55 mg/㎗ after planned exercise program (F= 16.86, p=.000). For lipid metabolism, body fat per cutaneous decreased from 27.16% to 26.57% after planned exercise program. The score of self efficacy has increased from 64.20 to 66.65 after planned exercise program and it was statistically significant (F=4.850, p=.040) The functional vital capacity has increased from 3.28$\ell$ to 3.37$\ell$and it was statistically significant(F=7.300, p=.020). 2. In an after effect of a planned exercise program, 35 percent of the subjects who participated in a planned exercise program continued to exercise for another six months. In conclusion, the planned exercise program can improve cardiopulmonary function, glucose, and lipid metabolism. This program was show a positive effect on the self efficacy and exercise compliance.
Pirpose: The aim of this study to analyze the association between history of diabetes mellitus (DM) with risk of prostate cancer (PCa) and cancer grade among men undergoing radical prostatectomy for PCa. Materials and Methods: 50 patients with DM and 50 patients without DM who undervent radical prostatectomy (RP) were included in the study. Age at biopsy, height, weight, digital rectal examination (DRE), pre-biopsy PSA levels, prostate volume, histopathologic diagnosis after surgery and gleason scores were collected data from all patients. Histologic material obtained at biopsy was given a Gleason score; tumours with a Gleason score ${\geq}7$ were considered high grade and <7 were considered low grade. Results: The mean age at the time of biopsy was 63.7 in patients with DM and 61.6 in patients without DM. Diabetic men had significantly lower PSA levels (p=0.01). Mean PSA level $7.04{\pm}2.85$ in patients with DM and $8.7{\pm}2.86$ in patients without DM, respectively. Also, diabetic men had higher RP tumor grade than men without DM (p=0.04). We found that HbA1c levels were higher in patients who have high grade prostate cancer (p<0.05). Conclusions: Diabetic men undergoing RP have lower PSA levels and have significantly higher grade PCa. We must be careful for screening PCa in patients with DM. Although the patients had lower PSA levels, they might have high grade disease.
Objective: Postoperative delirium (POD) is characterized by an acute change in cognitive function and can result in longer hospital stays, higher morbidity rates, and more frequent discharges to long-term care facilities. In this study, we investigated the incidence and risk factors of POD in 224 patients older than 70 years of age, who had undergone a neurosurgical operation in the last two years. Methods: Data related to preoperative factors (male gender, >70 years, previous dementia or delirium, alcohol abuse, serum levels of sodium, potassium and glucose, and co-morbidities), perioperative factors (type of surgery and anesthesia, and duration of surgery) and postoperative data (length of stay in recovery room, severity of pain and use of opioid analgesics) were retrospectively collected and statistically analyzed. Results: POD appeared in 48 patients (21.4%) by postoperative day 3. When we excluded 26 patients with previous dementia or delirium, 17 spontaneously recovered by postoperative day 14, while 5 patients recovered by postoperative 2 months with medication, among 22 patients with newly developed POD. The univariate risk factors for POD included previously dementic or delirious patients, abnormal preoperative serum glucose level, pre-existent diabetes, the use of local anesthesia for the operation, longer operation time (>3.2 hr) or recovery room stay (>90 mini, and severe pain (VAS>6.8) requiring opioid treatment (p<0.05). Backward regression analysis revealed that previously dementic patients with diabetes, the operation being performed under local anesthesia, and severe postoperative pain treated with opioids were independent risk factors for POD. Conclusion: Our study shows that control of blood glucose levels and management of pain during local anesthesia and in the immediate postoperative period can reduce unexpected POD and help preventing unexpected medicolegal problems and economic burdens.
The different polymorphisms of the transcription factor 7-like 2 (TCF7L2) gene promote variances in diabetes susceptibility in humans. We investigated whether these genotypes also promote differences in diabetic susceptibility in commercial pigs. Growing pigs (Landrace, both sex, 50-60 kg) with the C/C (n=4) and T/T (n=5) TCF7L2 genotypes were identified and intravenously injected with streptozotocin (STZ, 40 mg/kg) twice in weekly intervals, then a high-energy diet was offered. Oral glucose tolerance tests, blood analyses and the homeostasis model assessment-insulin resistance (HOMA-IR) index calculations were performed. The animals were sacrificed at the end of 12 weeks of treatment to reveal the pancreas histomorphometry. The results showed that all of the treated pigs grew normally despite exhibiting hyperglycemia at two weeks after the induction. The glycemic level of the fasting or postprandial pigs gradually returned to normal. The fasting insulin concentration was significantly decreased for the T/T carriers but not for the C/C carriers, and the resulting HOMA-IR index was significantly increased for the C/C genotype, indicating that the models of insulin dependence and resistance were respectively developed by T/T and C/C carriers. The histopathological results illustrated a significant reduction in the pancreas mass and insulin active sites, which suggested increased damage. The results obtained here could not be compared with previous studies because the TCF7L2 background has not been reported. Growing pigs may be an excellent model for diabetic in children if the animals are genetically pre-selected.
Purpose : The purpose of this study was to investigate the effects of physical activity on impaired fasting glucose in adults aged 20 years or older. Methods : This study utilized raw data from the 8th National Health and Nutrition Examination survey (2019~2021). The subjects of this study were 5,344 adults aged 20 years or older who were confirmed to be free of diabetes. The control variables in this study model are health behavior characteristics (subjective health status, smoking, drinking), anthropometric characteristics (body mass index), and personal background characteristics (gender, age, income level, education level, marital status). As for the analysis method, the degree of physical activity was made into a dummy variable, and a probit model was used. Results : As a result of this study, compared to quartile 1 of the relative grip strength value obtained by dividing the grip strength by the body mass index (body mass index, kg, m2), fasting blood glucose levels were significantly higher in quartile 2 (.05, p<.01), quartile 3 (.04, p<.01), and quartile 4 (.04, p<.01). It was found that the probability of belonging to the normal category was higher than that of impaired fasting glucose. In addition, in the group of adults aged 20 or older who had a lot of aerobic and anaerobic physical activity, fasting blood sugar was more likely to be in the normal category. Conclusion : Based on the results of this study, it was suggested that diabetes should be managed through physical activity in the pre-diabetic stage, as prevention is important as well as treatment. From a practical point of view, muscle strength, such as grip strength, can be identified as a reliable indicator for identifying impaired fasting glucose.
Purpose: This study was conducted to examine and compare clinical manifestations and predicting factors for treatment-seeking delay among patients <65 and ${\geq}65\;yr$ with first-time acute coronary syndrome (ACS). Methods: A total of 288 patients who were diagnosed with ACS were individually interviewed at C university hospital in G-city from November 2007 to December 2008. Results: Median pre-hospital delays for younger and older patients were 5 and 12 hr, respectively. Younger patients were more likely to be current smokers, heavy drinkers, obese, stressed, and have an unhealthy diet and family history, and to complain of chest pain, left shoulder and arm pain, perspiration, and nausea. Older patients were more likely to have hypertension and diabetes, and to complain syncope and dyspnea. Logistic regression analyses showed that after adjustment for age, gender and education, progressive onset of symptom and no attribution to cardiac problem significantly predicted pre-hospital delay >3 hr in both younger and older patients. Low perceived health status was a significant independent predictor in older patients only. Conclusion: Health care providers should be concerned with different manifestations between younger and older adults, and educate people at risk for heart attack about symptoms and actions to get immediate help.
Purpose: The purpose of this study was to evaluate the effects of Internet education on plasma glucose and serum lipids in female type 2 diabetic patients. Method: Control and experimental groups were assessed by a pre-test and at 3 months, 6 months, 9 months, and 12 months. Fourteen patients were assigned randomly to an intervention group and 15 to a control group. Patients in the intervention group were requested to input their blood glucose levels everyday by cellular phone or wire Internet for 1 year. The goal of the intervention was to keep blood glucose concentrations close to the normal range (glycosylated hemoglobin, HbA1c<7%). An intervention was applied to the intervention group weekly for 1 year. Optimal recommendations were sent weekly by a short message service from a cellular phone and wire Internet. Results: Patients in the intervention group had a significant mean change in HbA1c, from 7.6% at pre-test to 6.9%, at 3 months and equalized at 6.7% at 12 months. There was no significant time and group differences observed in the serum lipids. Conclusion: These findings indicate that Internet education would improve and maintain the normal range of HbA1c in female type 2 diabetic patients.
