The Effects of Short Term Comprehensive Life Style Modification Program on Glycemic Metabolism, Lipid Metabolism and Body Composition in Type 2 Diabetes Mellitus (단기간의 포괄적인 생활습관개선 프로그램이 제2형 당뇨병환자의 당대사 및 지질대사와 체구성에 미치는 효과)

  • Purpose: This study was to evaluate the effects of a short term comprehensive life style modification program on glycemic metabolism, lipid metabolism and body composition in type 2 diabetes mellitus patients. Method: A nonequivalent control group with a pre post test was designed. Data collection was done from October 2003 to June, 2004 at a hospital. Glycemic metabolism was measured by a.c., p.c. and $HbA_{1}c$, and lipid metabolism was measured by cholesterol, HDL cholesterol, LDL cholesterol and triglycerides. Body Composition was measured by body weight, body mass index, waist, measurement waist per hip ratio, body fat, muscle weight and abdominal fat tissue(intra abdominal distance). The Experimental group, which was composed of 29 participants, was educated based on a life style modification protocol at a weekly meeting for 12 weeks and carried out exercise, diet along individual parameters and self monitoring, while 24 participants in the control group received only diet education. Result: 1. The experimental group showed a significant lower a.c.(t=2.11, p=.04) and $HbA_{1}c$(t=2.65, p=.01) compared to those of the control group. 2. The experimental group showed a significant lower LDL than the results of the control group(t=2.42, p=.02). 3. The experimental group showed a significant lower weight(t=3.09, p=.00), BMI(t=3.01, p=.00), body fat(t=2.94, p=.01) and abdominal fat tissue(t=3.05, p=.01) than those of the control group. Conclusion: The results provided evidence for the effectiveness of a short term comprehensive life style modification program composed of exercise, diet, support, self efficacy elevation and self monitoring in type 2 diabetes mellitus.

5-Aza-2'-deoxycytidine Inhibits the Maintenance of Cancer Stem Cell in a Mouse Model of Breast Cancer (마우스 유방암 모델에서 5-Aza-2'-deoxycytidine의 암줄기세포 유지 억제 효과)

  • Aberrant DNA methylation plays an important role in the development of cancer. It has been reported recently that DNA hypermethylation is involved in the maintenance of cancer stem cells. The present study was designed to test the hypothesis that the demethylating agent, 5-aza-2'-deoxycytidine (AZA), can inhibit the potential for maintenance of cancer stem cells. To validate this hypothesis, we used 4T1 syngeneic mouse models of breast cancer. The AZA pre-treated 4T1 cells showed a dramatic inhibition of tumorsphere formation, compared to their counterparts in vitro. In addition, the AZA treatment significantly suppressed the expression of stem regulator genes, such as oct-4, nanog and sox2, compared to counterparts in vivo. Therefore, selective inhibition of DNA methylation may be useful for stem-specific cancer therapy.

Evaluation of the Relevance of Nutritional Status and Dietary Inflammation Index to Blood Glucose Levels in Middle-aged Women: in terms of 2013-2018's Korean National Health and Nutrition Survey Data (중년 여성의 혈당수준에 따른 영양상태 및 식이염증지수의 융합적 관련성 평가: 2013-2018 국민건강영양조사 자료 이용)

  • This study targeted 4,572 middle-aged women to examine the relationship between nutritional status and dietary inflammatory index according to blood glucose level using data from the Korean National Health and Nutrition Examination Survey (KNHANES). Data were analyzed using complex survey chi-square, General Linear Model and logisitc regression in SPSS Win 25.0 program. Women with high blood glucose (normal blood sugar→diabetes) had high rates of obesity and blood TG/HDL-cholesterol ratio. On the other hand, the Mean Adequacy Ratio (10 nutrients) and the intake of anti-inflammatory foods: beans, seeds, mushrooms, and fruits, were lower in the diabetic category. When we analysed the association between blood glucose and the Dietary Inflammatory Index, the risk of pre-diabetes and diabetes was significantly higher in the most pro-inflammatory diet category (Q5) compared to the most anti-inflammatory diet category (Q1). These findings suggest that nutritional education emphasizing the intake of various foods should be effectively conducted effectively in order to improve blood glucose among middle-aged women.

The Effects of Walking on the Physical Health of Residents in Rural Areas (걷기운동 프로그램이 농촌주민의 신체적 건강에 미치는 효과)

  • Purpose: The Purpose of this study was to analyze the effect of walking on physical health such as body composition, blood pressure, blood glucose and blood lipids for residents in rural areas. Method: Data were collected from 109 residents at 4 community health centers and during the 12 weeks' period between May and July, 2007 with quasi-experimental pre/post-test design. The data were processed with SPSS Win 12.0. Result: 69.5% of the subjects had chronic degenerative diseases such as hypertension, arthritis and diabetes mellitus. Also, 52.9% had overweight and 14.7% had excessive obesity. There were significant positive changes in BMI, blood pressure, blood glucose and LDL among obese residents. However, there was no significant difference in total cholesterol, triglycerides and HDL among blood lipids. Conclusion: This study showed that a 3 months' walking program had positive effects on physical health and it should be continued.

  • TGF-${\beta}$ pathway is being extensively evaluated as a potential therapeutic target. The transforming growth factor-${\beta}$ (TGF-${\beta}$) signaling pathway has the dual role in both tumor suppression and tumor promotion. To design cancer therapeutics successfully, it is important to understand TGF-${\beta}$ related functional contexts. This review discusses the molecular mechanism of the TGF-${\beta}$ pathway and describes the different ways of tumor suppression and promotion by TGF-${\beta}$. In the last part of the review, the data on targeting TGF-${\beta}$ pathway for cancer treatment is assessed. The TGF-${\beta}$ inhibitors in pre-clinical studies, and Phase I and II clinical trials are updated.

  • Purpose: To analyze the effects of customized home visiting health services on the health and health behaviors of clients with hypertension (n=107) and diabetes mellitus (DM: n=67). Methods: A one group pre and post-test research design was used. The subjects were registered in a customized 8-week, interventional, home visiting health services available in Daegu. Data was collected from November 17, 2008 to January 23, 2009. Analyses involved descriptive statistics, $x^2$ test and paired t-test. Result: Hypertension control rate was improved 25.2% and DM control rate was improved 3.0%. There were significantly beneficial hypertension-related differences in BP, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise, diet, BP monitoring and medication. Significantly beneficial DM-related changes included glucose, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise and glucose monitoring. But there were no significant hypertension-related differences in health belief (barrier) and health behavior including drinking and exercise length/frequency. Also, no significant DM-related differences were evident in health belief (barrier) and health behaviors including drinking, smoking, exercise length/frequency, diet and medication. Conclusion: Customized home visiting health service can provide effective, but not complete. Whether these benefits are maintained in the longer term is unknown.

Inhibitory Effects of (-) Epigallocatechin Gallate and Quercetin on High Glucose-induced Endothelial Cytotoxicity

  • Functional damage to microvascular endothelial cells by hyperglycemia is thought to be one of the critical risk factor.; in the impaired wound healing seen with diabetes mellitus. It is also thought that oxidative stress plays a significant role in this endothelial cell dysfunction. The present study examined the differential effects of flavonoids on endothelial cell dysfunction under high glucose conditions. Human endothelial cells exposed to 30 mmol/L glucose for 7 d were pre-treated with various flavonoids and pulse-treated with 0.2 mmol/L $H_2O_2$ for 30 min. High glucose markedly decreased cell viability with elevated oxidant generation and nuclear condensation. $H_2O_2$ insult exacerbated endothelial cytotoxicity due to chronic exposure to high glucose. (-)Epigallocatechin gallate and quercetin improved glucose-induced cell damage with the disappearnnce of apoptotic bodies, whereas apigenin intensified the glucose cytotoxicity. In addition, cell viability data revealed that these flavonoids of (-)epigallocatechin gallate and quercetin substantially attenuated both high glucose- and $H_2O_2$- induced dual endothelial damage. These results suggest that (-)epigallocatechin gallate and quercetin may be beneficial agents for improving endothelial cell dysfunction induced by high glucose and may prevent or reduce the development of diabetic vascular complications.

Role of suprascapular nerve block in idiopathic frozen shoulder treatment: a clinical trial survey

  • Background: Several therapeutic methods have been proposed for frozen shoulder syndrome. These include suprascapular nerve block, a simple and cost-effective technique that eliminates the need for nonsteroidal anti-inflammatory drug therapy. Methods: This was a clinical trial that included patients with unilateral shoulder joint stiffness. Patients were divided into three groups: those treated with isolated physiotherapy for 12 weeks (PT group), those treated with a single dose intra-articular injection of corticosteroid together with physiotherapy (IACI group), and those treated with a suprascapular nerve block performed with a single indirect injection of 8-mL lidocaine HCL 1% and 2 mL (80 mg) methylprednisolone acetate together with physiotherapy (SSNB group). The variables assessed were age, sex, side of involvement, dominant limb, presence of diabetes, physical examination findings including erythema, swelling, and muscle wasting; palpation and movement findings; shoulder pain and disability index (SPADI) score; and the visual analog scale (VAS) score pre-intervention and at 2-, 4-, 6-, and 12-week post-intervention. Results: Ninety-seven patients were included in this survey (34 cases in the PT group, 32 cases in the IACI group, and 31 cases in the SSNB group). Mean age was 48.55±11.06 years. Fifty-seven cases were female (58.8%) and 40 were male (41.2%). Sixty-eight patients had a history of diabetes (70.1%). VAS and SPADI scores and range of mototion degrees dramatically improved in all cases (p<0.001). Results were best in the SSNB group (p<0.001), and the IACI group showed better results than the PT group (p<0.001). Conclusions: Suprascapular nerve block is an effective therapy with long-term pain relief and increased mobility of the shoulder joint in patients with adhesive capsulitis.

The effects of exercise therapy applied in an efficacy expectation promoting program on self-efficacy, cardiopulmonary function and metabolism in type 2 diabetes mellitus patients (제 2형 당뇨병 환자에서 효능기대증진 프로그램을 적용한 운동요법이 자기효능과 심폐기능, 당대사 및 지질대사에 미치는 영향)

  • This study was conducted to identify the effects of exercise therapy applied in an efficacy expectation promoting program that was based on Bandura's self efficacy model on self-efficacy, cardiopulmonary function and metabolism in type 2 diabetes mellitus patients. The study design was nonequivalent pre-test post-test control design. 34 type 2 diabetes mellitus patients who received follow-up care regularly through the diabetic out-patient clinic were randomly sampled for this study. Twenty patients were assigned to the experimental group and fourteen patients were assigned to the control group. To the experimental group, exercise therapy applied in an efficacy expectation promoting program that is composed of individualized exercise prescription for 12 weeks was provided. In case of the control group, they were instructed to continue their usual lives. Data collection period was from March 1998 to June 2000 Data were analyzed using SPSS/WINDOW 10.0 program. The results were as follows. In experimental group, The score of self efficacy has increased from 64.20 to 66.65 after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant(t=2.07, p=.04). The anaerobic threshold has increased from $18.20\;m{\ell}$/kg/min to $19.07\;m{\ell}$/kg/min and it was statistically significant(t=2.05, p=.04). Level of fasting blood sugar has decreased from $188.20\;mg/d{\ell}$ to $155.55\;mg/d{\ell}$ after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant.(t=-2.69, p=.01). For the lipid metabolism, percent body fat has decreased from 27.16% to 26.57% after exercise therapy applied in an efficacy expectation promoting program. In conclusion, the exercise therapy applied in an efficacy expectation promoting program showed positive effect of self-efficacy, cardiopulmonary function and glucose and lipid metabolism.

Effect of African Mango (Irvingia gabonesis, IGOB 131TM) Extract on Glucose Regulation in STZ-Induced Diabetes (Streptozotocin으로 유발한 당뇨동물 모델에서 아프리칸 망고 추출물의 혈당 조절 효과)

  • This study investigated the regulatory effects of African mango (Irvingia gabonesis, IGOB $131^{TM}$) extract on blood glucose level in streptozotocin (STZ)-induced diabetic rats. Experimental groups were treated with two different doses of IGOB $131^{TM}$ (1% and 2% in each AIN93G supplement) for 5 weeks [4 weeks pre-treatment and 1 week post-STZ treatment (60 mg/kg body weight)]. STZ-induced diabetic rats showed significantly reduced body weight gain compared to normal control (NC). Oral glucose tolerance test (OGTT) was measured using glucose oxidase-peroxidase reactive strips. The area of under the curve for the glucose response from OGTT in STZ-induced diabetic rats was higher than that of NC rats, and there was a significant difference between the DM and the IGOB $131^{TM}$-treated groups. Serum glucose levels after sacrifice were significantly lower in the IGOB $131^{TM}$ group than the DM group. However, there was no statistical difference between low- and high-dose treatments. Serum insulin levels increased by 234.4% and 175.9%, respectively, upon treatment with IGOB $131^{TM}$. Serum lipid profiles were not significantly different among the experimental groups. The tested samples had no effects on serum levels of lipid profiles (triglyceride, total cholesterol, low density lipoprotein/very low density lipoprotein-cholesterol, high density lipoprotein-cholesterol). These results suggest that IGOB $131^{TM}$ is able to ameliorate diabetes by reducing serum glucose levels that may result from increased insulin levels.
