A Human Case of Zoonotic Dog Tapeworm, Dipylidium caninum (Eucestoda: Dilepidiidae), in China

  • We described a human case of zoonotic dog tapeworm, Dipylidium caninum (Eucestoda: Dilepidiidae), rarely occurring in China. The mother of a 17 month-old boy noted the appearance of small white and active worms over a month period in her son's feces, but the boy was asymptomatic except mild diarrhea. We observed 3 tapeworm proglottids resembling cucumber seeds in his stool sample. Microscopically, each proglottid had 2 genital pores, 1 on each lateral edge, and numerous egg capsules in the uterus. The patient was successfully treated with a single oral dose of praziquantel. Adult worms were recovered in the diarrheic stool after praziquantel treatment and purgation. His family had household pet dogs for several years, and he might have acquired the infection by ingestion of infected fleas of his pet dogs. A history of dog or cat pets and flea bites may be important clues to diagnosis of D. caninum infection. The infected pets should also be treated.

  • Isolated brainstem invlovement of neurocysticercosis is rare. We present the clinical and radiological features of a patient with a solitary cysticercus granuloma in the brainstem confirmed by MRI and CSF ELISA test. Good response was obtained from conservative treatment, using praziquantel. We believe that patients from regions where cysticercosis is endemic, who have no systemic disease, who present with nonprogressive symptoms and signs of brainstem involvement, and display a solitary ring-enhancing mass in the brainstem measuring less than 20mm on neuroimaging may be managed conservatively.

A Case of Natural Human Infection by Echinostomu cinetorchis (Echinestoma cinetorchis의 인체감염 1예)

  • A human case of Echinostoma cinetorchii infection was found in Seoul Paik Hospital on August, 1987. Large trematode eggs, $98.9{\times}62.7{\mu\textrm{m}}$ in average size, were detected in stool examination. The eggs had a small.sired operculum and abopercular wrinkling. After treatment with praziquantel (single dose of 16.2mg/kg) and purgation with magnesium citrate, an adult fluke was collected. It was 8.0 mm long, equipped with 37 collar spines around head crown, and had only one testis just behind the Mehlis' gland. It was identified as E. cinetorchis Ando et Ozaki, 1923. The patient is an 18-year old man residing in Seoul, and was hospitalized due to nephritis. He had eaten raw meat juice of fresh water fishes such as top minnows (Orygias Satipes) and loaches, or tadpoles, which are considered possible source of this fluke infection. This is the 4th human case of E, cineterchis infection in Korea.

  • A paragonimiasis infestation is caused by the paragonimus species. Paragonimiasis mainly occurs by ingestion of raw or undercooked freshwater crabs or crayfish. In our country, the prevalence of paragonimiasis was high until late 1960s due to eating habits, but after the 1970s the prevalence of the disease has markedly decreased and now the disease is rarely seen. The diagnosis of tuberculosis by Chest X-ray is often confused with pulmonary carcinoma, bacillary and parasitic infections, and chronic mycosis. Pulmonary paragonimiasis must be considered in the differential diagnosis of lung cancer especially in the appropriate clinical setting because effective treatment with praziquantel can be rewarding. We report a case of a 58-year-old woman with pulmonary paragonimiasis that was suspicious for lung cancer, as detected by biopsy.

  • The third human case of natural infection by Heterophvopsis continuo in Korea was found in Chinju, Kyongsangnam-do. The case was a 53-year-old mn. He used to eat raw brackish and fresh water fish. After praziquantel treatment and purgation, H. continua were collected from the diarrheal stool together with Metononimw vokognwai, Heterophves nocens and Echinostona hokense. His clinical complaints were indigestion, epigastric discomport, poor appetite and fatigue. The complaints were considered rather due to heavy M. Wokoscwci infection.

  • In this study, we intended to describe a human case of lumbosacral canal sparganosis in People's Republic of China (China). A 56-year-old man was admitted to Xiangya Hospital Central South University in Changsha, Hunan province, China after having an experience of perianal pain for a week. An enhancing mass, a tumor clinically suggested, was showed at the S1-S2 level of the lumbosacral spine by the examination of magnetic resonance imaging (MRI) with gadolinium contrast. The patient was received the laminectomy from S1 to S2, and an ivory-white living worm was detected in inferior margin of L5. In ELISA-test with cerebrospinal fluid (CSF) and serum samples, anti-sparganum antibodies were detected. He had a ingesting history of undercooked frog meat in his youth. By the present study, a human case of spinal sparganosis invaded in lumbosacral canal at the S1-S2 level was diagnosed in China. Although the surgical removal of larvae is known to be the best way of treatment for sparganosis, we administered the high-dosage of praziquantel, albendazole and dexamethasone to prevent the occurrence of another remain worms in this study.

Therapeutic evaluation of sustained-releasing praziquantel (SRP) for clonorchiasis: Phase 1 and 2 clinical studies

  • Sustained-releasing praziquantel (SRP) tablet was designed for single dose treatment regimen of clonorchiasis. A previous pre-clinical study confirmed its sustained-releasing characteristics and a better cure rate than conventional praziquantel (PZQ). In this clinical study, the pharmacokinetics of this SRP tablet were investigated in human volunteers (phase 1; 12 volunteers), and its curative efficacy was examined in clonorchiasis patients (phase 2; 20 volunteers). In the phase 1 clinical study, blood concentrations of both tablets showed wide individual variation. The $AUC_{last}$ of SRP was $497.9{\pm}519.0ng{\cdot}hr/ml\;(mean{\pm}SD)$ and PZQ of $628.6{\pm}695.5\;ng{\cdot}hr/ml$, and the $AUC_{inf}$ of SRP was $776.0{\pm}538.5\;ng{\cdot}hr/ml$ and of PZQ $658.6{\pm}709.9\;ng{\cdot}hr/ml$. $C_{max}$ values of SRP and PZQ were $90.7{\pm}82.2ng/ml\;and\;214.9{\pm}251.9\;ng/ml$, and $T_{max}$ values were $3.42{\pm}1.43\;hr\;and\;1.96{\pm}1.23\;hr$, respectively. SRP tablets showed similar AUC values, but lower $C_{max}$ and longer $T_{max}$ values than PZQ. In the phase 2 study, SRP at 30 mg/kg (single dose) achieved a 60% cure rate and a 95.5% egg reduction rate. The cure rate of a single dose SRP was unsatisfactory compared with that of the conventional PZQ dose, but much better than that achieved by a single dose PZQ.

  • A total of 69 patients of confirmed neurocysticercosis was followed serologically by ELISA up to 22 months after praziquantel treatment. The intervals and numbers of follow-up were variable by patient. Serially collected samples of serum and CSF were examined simultaneously for their specific IgG antibody levels by ELISA, using cystic fluid, saline extracts of bladder wall and scolex as antigen. Within 4 months after praziquantel treatment, the antibody levels were elevated temporarily in both serum and CSF in most patients. In some cases antibody levels exhibited steady declining tendency after the treatment. Concomitant administration of dexamethasone appeared to suppress the elevation of antibody levels. The rate of mean absorbance of antibody changed more in serum than in CSF. The rate of elevation was greater in antibodies to parenchymal antigens than that to cystic fluid, but absolute difference of antibody levels was greater in antibody to cystic fluid. Previously negative samples for IgG antibody may become positive after the praziquantel treatment, which could be used as a complementary tool (provocation test) in serodiagnosis. One month was considered to be sufficient interval for the follow-up test for that purpose. In the follow-up of up to 22 months, only few cases of chronic neurocysticercosis showed declining tendency of IgG antibody levels below negative range. During acute encephalitic attacks in chronic patients, IgG antibody to parenchymal antigen were elevated in CSF temporarily. These results indicated that serologic follow-up of every year was recommendable to differentiate the cured patients from chronic patients with slowly calcifying lesions.

Reinfection by Opisthorchis Viverrini after Treatment with Praziquantel

  • Background: The prevalence of infection by the liver fluke, Opisthorchis viverrini (O. viverrini), has remained high in Northeast Thailand where it is a major risk factor for the eventual development of cholangiocarcinoma (CCA). The infection is acquired by the consumption of dishes containing unsafely prepared freshwater fish, a dietary tradition which has proved resistant to change. Since many people are aware that dosing with praziquantel (PZQ) is a successful treatment for an episode of the infection, there is a risk that, to avoid the long term consequences, they will engage in a cycle of infection, dosing and reinfection. Objectives: There is a dearth of studies of reinfection by O. viverrini, and the aims of this study were to assess re-infection rates in a typical province of Northeastern Thailand where O. viverrini infection is likely and to investigate factors associated with reinfection. Materials and Methods: A total of infected 607 villagers were treated with PZQ, and those found to be no longer infected were followed up at six-monthly intervals over 12 months. Results: At the end of this period data on 457 subjects were available for analysis using descriptive statistics and logistic regression, and 50 were found to have become reinfected, giving a cumulative reinfection rate of 10.9%. The results of a multiple logistic regression analysis showed that the only factor found to be associated with reinfection was past use of PZQ. Conclusions: Recommendations are made for future larger scale and better designed reinfection studies in the light of limitations of the current study. Further efforts are needed to discourage people from eating fish dishes likely to contain viable metacariae.

Two Human Cases of Diphyllobothrium latum Infection through Eating Raw Freshwater Trout, Salmo gairdneri (송어를 감염원으로한 광절열두조충 인체감염)

  • A total of 31 human diphyllobothriasis cases have been reported in Korea. Authors experienced two more human cases of Diphyllobothrium latum infection, especially due to eating raw freshwater trout. Two cases were husband and wife in a famil residing in Chunchon city, Kangwon-do, who raw fish in October 1994. A 43-year old male(husband) was 69kg in body weight and healthy. A worm (6.65m in length; 8-l3mm in width) was expelled from him after, anthelmintic treatment. The second case(wife) was a 39-year old female who weighted 56kg. She complained about gastrointestinal trouble and abdominal discomfort. A worm (5.50m in length; 9-l3mm in width) was Obtained her after anthelmintic treatment. No scolex was collected from the two worms. Two patients were administrated with praziquantel(15mg/kg of body weight) and 15gm of magnesium sulfate as a purgative. Two worms identified as D. latum, .based on the following biological characters: extermal morphologies, coiling of uterus, the number of uterine loops, position of genital opening, morphologies of cirrus and cirrus sac and seminal vesicle on the histological sections, positions of vagina and uterine pore, distribution patterns of vitellaria, and microscopical and SEM morphologies of the egg.
