Incidence of Paresthesia Related to the Insertion Length of Catheter during the Epidural Catheterization (경막외강 카테터 삽입 시 삽입 길이와 감각이상의 빈도)

  • Background: Continuous epidural catheterization is a popular and effective procedure for postoperative analgesia. However, continuous epidural catheterization has associated complications such as venous puncture, dural puncture, subarachnoid cannulation, suboptimal catheter placement, and paresthesia because the tip of the epidural catheter touches thenerves of the dura in the epidural space. In this study, we compared the incidence of paresthesia in two different lengths of epidural catheter insertion. Methods: One hundred women undergoing gynecologic or orthopedic surgery were enrolled in this prospective, double-blinded, randomized study. All patients were randomly divided into two groups based on the insertion length of the epidural catheter 2 cm (group A) or 4 cm (group B). A Tuohy needle was inserted in the lumbarspinal region with a bevel directed cephalad by use of the median approach, and then the epidural space was confirmed by the loss of resistance technique with air. While the practitioner inserted an epidural catheter into the epidural space, a blind observer checked for paresthesia or withdrawal movement. Results: In 97 included patients, 30.6% of the patients in group A (n = 49) had paresthesia, versus 31.3% in group B (n = 48). Withdrawal movements were represented in 2% and 6% of the patients in group A and group B, respectively. There was no difference in the incidence of paresthesia and withdrawal movement between the two groups. Conclusions: There is no clear relationship for the incidence of catheter-related paresthesia according to the catheter length inserted into the epidural space for epidural analgesia.

COMPARISON OF OPERATIVE TECHNIQUES BETWEEN FEMALE AND MALE DENTISTS IN CLASS 2 AND CLASS 5 RESIN COMPOSITE RESTORATIONS (2급/ 5급 와동 복합레진 수복 술식에 대한 남녀 치과 의사의 비교)

  • This study aimed to assess whether the gender of the dental practitioner affects operative techniques in class 2 and class 5 resin composite restorations. In 2008, a nationwide survey was given to Korean dentists. Total 12,193 e-mails were distributed, 2,632 were opened by recipients, and 840 responses were collected. Of the respondents, 78.9% were male and 21.1% were female. The gender distribution in the age groups between respondents and the total population did not differ (p > 0.05). A chi-square test was used to compare technical differences between female and male dentists. A multiple logistic regression analysis was performed to assess the association between gender and operative techniques in resin composite restoration. For class 2 resin composite restoration, female dentists were 1.87 times more likely than male dentists to do multiple incremental fillings (four layers or more) and 2.72 times more likely than males to spend 30 minutes or more for the treatment (p < 0.05). For class 5 resin composite restoration, female dentists were 2.69 times more likely than their male counterparts to use a cavity base or liner, 1.83 times more likely to do multiple incremental fillings (four layers or more) and 1.63 times more likely to spend 20 minutes or more for the procedure (p < 0.05). The gender factor was influential to individual operative techniques in restorative treatment.

Comparisons of the Performance with Bayes Estimator and MLE for Control Charts Based on Geometric Distribution (기하분포에 기초한 관리도에서 베이즈추정량과 최대우도추정량 사용의 성능 비교)

  • Charts based on geometric distribution are effective to monitor the proportion of nonconforming items in high-quality processes where the in-control proportion nonconforming is low. The implementation of this chart is often based on the assumption that in-control proportion nonconforming is known or accurately estimated. However, accurate parameter estimation is very difficult and may require a larger sample size than that available in practice for high-quality process where the proportion of nonconforming items is very small. An inaccurate estimate of the parameter can result in estimated control limits that cause unreliability in the monitoring process. The maximum likelihood estimator (MLE) is often used to estimate in-control proportion nonconforming. In this paper, we recommend a Bayes estimator for the in-control proportion nonconforming to incorporate practitioner knowledge and avoid estimation issues when no nonconforming items are observed in the Phase I sample. The effects of parameter estimation on the geometric chart and the geometric CUSUM chart are considered when the MLE and the Bayes estimator are used. The results show that chart performance with estimated control limits based on the Bayes estimator is generally better than that based on the MLE.

Risk associated with Adverse Events of Folk Medicine Reported in the Internet News Articles (인터넷 신문기사로 본 민간요법 유해사례의 위험성)

  • Folk medicine is traditionally passed down to cure disease, and adverse events (AEs) of folk medicine are any unfavorable and unintended discomforts temporally associated with the use of folk medicine. The aim of this study was to analyze AEs types and risks of folk medicine through the internet news articles. Included in this analysis are all articles on the topic of folk medicine and AE reported in the top 3 online news websites (NAVER, DAUM and NATE) determined by InternetTrend$^{TM}$(www.internettrend.co.kr). It was searched in the last five years (between 1 January 2009 and 28 February 2014). In total, 18 AEs articles of 973 news articles met our inclusion criteria. A total of 27 people were experienced AEs associated with use of folk medicine. Age was from 4 months to 76 years old, and it was occurred in both men and women. Folk medicine that caused AEs in twice or more was therapy that patient taking the dictamnus or aconitum of toxic herbal medicines, vinegar therapy of external use to topical skin, and cupping or bee sting therapy by practitioners. Death as a kind of serious AEs was 11 people, and 10 people were died after treatment by unqualified practitioner. Folk medicine that is popular and widely used in Korea is actively interacted with information on the internet, so it apt to misuse and abuse without guidance of health professionals. Aspects of health care system, we point out that the need for government and medical society establish not only correct health information plan and promotion of risk but also system as reporting and monitoring of AEs by folk medicine.

  • Since the year of mid 2000, the EPC industry in Korea has the highest amount of orders received due to tremendous need form middle east asia region. However, the quality requirement from the owner is getting critical, which lead to the more competitive engineering capacity equipped. In order to do so, the development of objective and quantitative evaluation model of engineering process is essential. In order to develop the engineering checklist model for case-based evaluation of EPC project, first, the general description of worldwide EPC industry as well as the literature review of prior study have been performed. Then, the survey interview with experts in the engineering area of EPC project was performed In order to Identify the engineering work tasks and the critical check items, which should be checked during engineering process. Based on these checklist, the weights of check items as well as engineering work tasks also have been defined. The developed checklist model evaluated the two EPC project cases of S company in Thailand in order to explore its applicability. From the analysis of evaluation results, it is expected that the continuous implementation of evaluation based on checklist model can help the EPC practitioner check the critical items in engineering process more efficiently and take an appropriate action for better engineering performance, which finally can lead to the Korea EPC industry being more competitive.

Comparison of meridians electric response property for laser and acupuncture stimulation (레이저자극과 수기자극에 대한 경락전기반응 특성비교)

  • Laser stimulation has been widely studied and used in clinic. However, electrical response by laser stimulation on meridians has not been investigated. In this study, we compared electric potential of laser and acupuncture stimulation on meridians. We measured electric potential variation at acupoints(Samgan(LI3) and Hapgok(LI4)) on Large Intestine Meridian. In laser stimulation results, average peak electric potential is very low($7.53{\pm}3.44{\mu}V$) for before and after stimulation. However, acupuncture stimulation was performed in ground connection condition and resulted in huge variation of average peak electric potential($2.65{\pm}1.53mV$). That is, the intensity and pattern of electric potential were dependent on the ground connection condition and individual. Also, the electric potential pattern was very similar to the pattern of electric charge and discharge of capacitor. The acupuncture stimulation using a insulating needle resulted in lower average peak electric potential variation($0.25{\pm}0.16mV$) than that of acupuncture stimulation. It might present little electrical response of acupuncture stimulation using insulating needles. In point of electrical response, the laser stimulation was determined to be no acupuncture effect at meridian. Acupuncture stimulation seems to be most effective method to induce electrical response at meridians. The procedure and effect of acupuncture might be considered as energy consensus phenomenon by transportation of bio-ion charge between a practitioner and patient.

A study on the development of family resilience enhancement program for couples in early stage of marriage (결혼초기부부의 가족레질리언스 강화 프로그램 개발 연구)

  • The purpose of this study was to develop a program for couples in early stage of marriage for enhancing family resilience based on the concept of 'Family Resilience' by Walsh. Based on the literature review of couples in early stage of marriage, marital enrichment, and Family Resilience, a program focused on belief system, organizational pattern, and communication process was designed and implemented. The program was applied to seven couples in early stage of marriage for four (3hours/session/week) sessions. The Wilcoxon Signed Ranks Test was used to compare the pre and post effects of the program. By operating a focus group composed of the program participants, a qualitative analysis of the participant's evaluation was performed. The pre and post comparative analysis showed a significant enhancement in belief system, organizational pattern, communication process, and dyadic adjustment. The results of the qualitative study in the focus group also reinforced the effectiveness of the program. The evaluation showed that the 3 hour/ 4 session program was appropriate in terms program format. The significances of this study are the concretion of the concept of Family Resilience and providing practice tools for the daily practitioner.

A Qualitative Case Study on the Low-Income Divorced Single Parents Self-help Group: Focusing on the Case of 'Jinggomdari' at Wolgye Social Welfare Center (저소득 이혼 한부모 자조집단에 관한 질적 사례연구 - 월계종합사회복지관 '징검다리' 사례를 중심으로 -)

  • This is a case study with the objectives of performing an in-depth investigation of the development process, outcome, and the factors related to the development process of Jinggomdari-the self-help group for divorced single parents at Wolgye social welfare center. Document review, in-depth interviews of the member/practitioner/middle manager, and participatory observations were performed and qualitatively analyzed. The results showed that the development process of Jinggomdari consisted of 4 phases including initiation, formation of basis, growth, and expansion/independence. Two categories including 'nurturing abilities and growth as an individual or within families or communities' and 'beyond Jinggomdari, into society', 6 subcategories, and 31 concepts were derived as the outcome. In terms of the factors related to the development process and outcome, 6 categories including 'members participating with sympathy despite various issues', 'outstanding and stabilized leadership', 'commitment of the practitioners showing utmost efforts despite shortcomings', 'effective functions as the hub supporting the low-income divorced single parents families based on the institution's firm determination', 'mixed group showing cautious but synergistic effects', 'dilemma of managing the low-income divorced single parents self-help group and lack of manual for management', 20 subcategories, and 61 concepts were derived. Management issues concerning low-income divorced single parents' self-help group was discussed based on these results.

Senior Center Based Diabetes Self-management Program: An Action Research Approach (노인복지관 당뇨병 자기관리 프로그램의 과정과 평가: 실행연구방법)

  • The purpose of this study examined the feasibility and efficacy of a senior center-based diabetes self-management program applying action research approach. The cyclical action research method was applied for this study: plan, act, evaluate, and reflect in delivering three waves of the intervention program. Three waves of a 12 weeks-length small group diabetes self-management program were offered during the period of 15 months in a senior center in Seoul. Planning of $2^{nd}$ and $3^{rd}$ wave program were based on reflection of $1^{st}$ and $2^{nd}$ program evaluation respectively. Among the 46 participants, 93.48% (N=43) completed the program. The quantitative evaluation showed statistically significant improvement in HbA1C(p<.001), fasting plasma glucose(p<.001), BMI(p=.016), waist circumference(p=.001), systolic blood pressure(p=.036), diabetes self-management behavior(p<.001) and health knowledge(p=.008). Qualitative data revealed that individual management was very helpful in empowering and adhering for own diabetes care for the participants. Participants reported high satisfaction towards the program with mean satisfaction score of 65.12. Application of the Diabetes Self-management program with action study strategy was successful in community setting for improving participants' subjective and objective outcomes. Action research method guides the practitioner to tailor the program to respond for the participants and field needs.

A Study on the Legal Nature of the Duty to Arrange Spill Clean-up Equipment and the Issue on the Justification of Its Privatization (방제선 등 배치의무의 법적 성질 및 민간개방의 정당성에 관한 연구)

  • Under the Korean Marine Environment Management Act (hereunder, the KMEMA), the duty to arrange spill clean-up equipments, including spill clean-up vessels, required by the article 67 of the KMEMA is in essence the provision of public goods since it is a precautionary or preventive measure for the efficient performance of oil spill cleaning up. Also the obligation to control marine pollution and the duty to arrange of anti-pollution measures imposed on polluters is the obligation imposed by the public law in accordance with "the Polluter-Pays Principle". Therefore, the execution of such obligations shall be accompanied by the minimum legal and institutional arrangements. On the other hand, judging whether to form the roles of the public and private sectors in providing public goods is basically a matter of policy decision. However, even if the private sector is allowed to participate in the provision of public goods, it is imperative that a minimum requirement be provided to secure the public interest. Although major countries allows polluters to conclude a preliminary contract with a civil anti-pollution management company, these civilian institutions are in principle constituted by the owners of oil storage facilities. Additionally it is worth noting that it operates as a non-profit organization. In particular, if the practitioner performs pollution control for commercial purposes, their profitability may depend on the size of the pollution, the period spent on pollution control, the size of the equipment and manpower mobilized in the pollution control, and so on. Considering the above problems, caution should be taken to allow marine environmental management companies to be un-limitedly entrusted with the responsibility of arranging measures such as pollution control. In order to allow the marine environmental management contractor to be entrusted for the assignment of duty to protect the marine environment, the marine environment management business should be expanded so that the marine pollutant management capacity satisfies the statutory control capacity. For this purpose, it is necessary to manage and supervise the maintenance and improvement of the control capability of the marine environment management business. It is also necessary to discuss the introduction of the grading system for the control ability of the civil control companies alike in major countries.
