The effect of oral breast milk on pain response of the neonates during heel lancing (모유경구투여가 발뒤꿈치 천자 시 신생아의 통증반응에 미치는 효과)

  • This study was performed in order to investigate the effect of breast milk on pain relief in newborn during heel lancing. Nonequivalent control group pretest-posttest design by double blinded experiment was used. 50 neonates were randomized to receive breast milk (experimental group, n=25) or no treatment (control group, n=25) before undergoing heel lancing. Informed consent was obtained from parents of 50 neonates. Neonatal infant pain scale (NIPS), respiration rate, heart rate, oxygen saturation, and crying duration were used to assess subjects' pain reaction. Data were analyzed using SPSS 18.0. The experimental and control group showed a statistically significant differences in NIPS score (F=28.89, p<.001), heart rate (F=14.03, p<.001), respiration rate (F=4.79, p=.001), oxygen saturation (F=2.69, p=.027), and crying duration (t=-8.78, p <.001) at each time points (during heel lancing, right after heel lancing, 1, 2, 3 minutes after heel lancing). The result suggest that oral administration of expressed breast milk is safe and natural agents for reducing procedural pain of neonates. Another clinical trial study with more samples is recommended.

Effects of BeHaS Program on Health Behavior, Physiologic Index and Self-Esteem of the Elderly Living Alone with Metabolic Syndrome Based on Community Based Participatory Research (지역사회기반 참여연구를 활용한 베하스(BeHaS) 프로그램이 대사증후군 독거노인의 건강행위, 생리적 지수 및 자아존중감에 미치는 효과)

  • Purpose: This study aimed to determine the effects of a 12-week metabolic syndrome BeHaS (Be Happy and Strong) program in elderly people with metabolic syndrome living alone, based on a community-based participatory research (CBPR). Methods: A nonequivalent control group pre-posttest design was used, and the participants were 43 elderly people living alone (experimental group 24, control group 19). The experimental group received a one-hour program per week and two individual health consultations during 12 weeks. The control group received two sessions about the metabolic syndrome and two individual health consultations. The effects of health behavior, blood pressure, blood sugar levels, abdominal circumference, triglycerides, and self-esteem were evaluated. The data were analyzed using the independent t-test and Mann-Whitney U test. Results: The health behavior with respect to the metabolic syndrome in the experimental group increased significantly (t = - 3.19, p = .002). Both diastolic blood pressure and abdominal circumference decreased in the experimental group (t = 2.00, p = .028 and t = 3.91, p < .001). No significant differences were observed between the groups in systolic blood pressure, fasting blood sugar levels, triglycerides, and self-esteem. Conclusion: The 12-week metabolic syndrome BeHaS program using community resources improves the health of elderly people with metabolic syndrome living alone. Based on these findings, further studies on the effectiveness of the metabolic syndrome BeHaS program and the experiences of those who participated in the CBPR are warranted.

Effects of the Course with Handbook Correcting Life Science Misconceptions for Preservice Elementary School Teachers on Science Teaching Efficacy and Misconceptions (생명과학 오개념 교정 소책자를 이용한 강좌가 초등 예비교사들의 과학 교수효능감과 오개념에 미치는 효과)

  • Misconceptions are one of the most important and long-lasting studied themes on science education. It is because their nature is obstinate and settled as well as they corrupt students' understanding of science concepts. This study examines whether preservice elementary school teachers enrolled in biology teaching material research course changed positively on science teaching efficacy and correcting life science misconceptions when they used the handbook developed for this study. The handbook comprises 203 life science concepts surveyed among preservice teachers holding from advanced researches and arranged by 2007 Revised Science Curriculum. 107 preservice elementary school teachers participated in the study. Forty-six (46) of them were assigned to the experimental group and the other 61 to the control group. ANOVA was conducted for analyzing data. Science Teaching Efficacy Belief Instrument (STEBI) and the life science misconception test which was developed for this study, were administrated as assessing instruments for pretest and posttest. The experimental group, using the handbook, acquired higher scores in both tests on statistical significance level than the control group who were not using the handbook. The results indicate the handbook developed for correcting life science misconceptions for this study is effective in enhancing science teaching efficacy and correcting life science misconceptions.

The Effect of Sensory Integrative Intervention Focused on Proprioceptive-Vestibular Stimuli on the Handwriting and Fine Motor Function in Lower Grade Elementary School Children (고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소운동에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of proprioceptive-vestibular based sensory integrative intervention on handwriting and fine motor function in elementary school students in grades 1 to 3. Methods : In this study, eight students in an elementary school in Busan were enrolled. The intervention was conducted twice a week from May to October, 2016, and a total of 14 intervention sessions were conducted. In order to evaluate the writing ability and the fine-motor exercise ability before and after the intervention, the fine-motor movement items of the Korean alphabet writing test and the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP) evaluation were used. Results : There was a statistically significant difference between the total scores of consonant writing and the Korean alphabet writing assessment after the intervention. In BOTMP-Fine motor, the response speed items showed statistically significant difference. Visual-motor control scores increased during intervention, but the statistical significance was not found. Conclusion : Sensory integration interventions might have positive effects on elementary school students' writing skills and fine motor functions.

The Effect of a Multidisciplinary Team Approach on the Rehabilitation of Stroke Survivors (뇌졸중 환자를 위한 팀접근 재활프로그램의 효과)

  • The purpose of this study was to develop a multidisciplinary team approach program for stroke survivors, and to identify its effects on their rehabilitation. The team was composed of 7 members: a rehabilitation nurse, a physician, a physical therapist, an occupational therapist, a therapeutic recreational therapist, a nutritionist, and a researcher. A quasi-experimental study was performed with 36 subjects: 18 in the experimental group and 18 in the control group, using a noneqivalent control group pretest-posttest design. The experimental group participated 4 times in rehabilitation programsfocused on information and emotional support provided by the rehabilation team-and received telephone counseling from the researcher. The control group did not receive any treatment. The selection criteria for the subjects in this study were: (a) patients who were diagnosed as having had an ischemic stroke within the last year, (b) patients free of any communication disorder, (c) and those having a primary caregiver who could assist in filling out the form assessing the level of ADL. The data were collected from patients who had been discharged from a tertiary hospital, between October 1st, 1999 and September 30th, 2000. The data were analysed by X2 test, Fisher's exact test, ANCOVA, Wilcoxon's rank sum test, and Wilcoxon's signed rank test using an SAS program. The results were as follows: 1. In terms of physical variables (blood pressure, grasp power, and ADL) 1) There was a significant difference in blood pressure between the two groups (systolic pressure P= .012, diastolic pressure P= .050). 2) There was also a significant difference in grasp power between the two groups (affected side : P= .012, unaffected side : P= .010). 3) There was no significant change in the level of ADL between the two groups. 2. In terms of psychosocial variables (depression, self-efficacy, self-esteem, and social activities) 1) There were no significant differences between the two groups. However, all four psychosocial variables showed a tendency to improve in the experimental group, while only two variables (depression and self-efficacy) showed a simalar tendency in the control group. 2) The level of social activities in the control group decreased significantly after a month (P= .050). 3. The level of life satisfaction improved in both groups, but no significant difference was found. Stroke has high recurrence rate and requires considerable follow-up care. The program used in this study was developed and designed for stimulting the rehabilitation process of stroke survivors. Through the program period of one month (meetings were held weekly), a positive effect was detected in physical variables, although the psychosocial variables did not improve significantly. In retrospect, a one month period may not be an adequate length of time to improve the psychosocial variables, as the stroke survivors were complicated cases, and most of them were elderly. Further research is therefore recommended by increasing the length of program, so that its effect can be more noticeable.

Effectiveness of Intervention for the Caregivers of Long-term Care Elderly (장기요양보호대상노인 가족부양자에 대한 개입의 효과성)

  • The major purpose of this study is to examine effectiveness of an intervention program which aims at education/training caregivers of the dementia and/or stroke elderly. This program was conducted at group level. Participants of this program were the primary caregivers who have taken care of the elderly with dementia and/or stroke. To test the effectiveness of the caregiver education/training program, this study employed an quasi-experimental design: to compare pretest score with posttest ones for the same participants. Total forty two primary caregivers have completed the program. Seventeen caregivers were in experimental group, whereas twenty five ones belonged to control group. Our data suggest that, for the primary caregivers, subjective quality of life was more important than cargiving burden. Subjective quality of life among the caregivers significantly improved, after completing the program. This result suggests that a short-term intervention program at group level is not effective to reduce caregiving burden because an infra structure of community resources, to which the caregivers and their family access, did not remain at sufficient level. However, participants have showed improved mutual solidarity, and they have exposed to wider spectrum of a variety of information. As a result, they have perceived that the level of subjective quality of life has positively changed. Additional factors have influenced on determining the quality of life among the caregivers. The caregivers, whose demented elderly showed lower level of ADL and IADL, or who have experienced the short period of caregiving, were more likely to belong to higher level of quality of life. The quality of life among the caregivers were even more improved in the following group: being young, and highly educated, man. One of the contributions from this study is that we have found caregiver's characteristics, which need an intervention most. In addition, our study implies that specific contents of the caregivers' education/training program should be conducted based on each family's unique characteristics.

The Effects of Foot Reflex Zone Massage on Patients Pain and Sleep Satisfaction Following Mastectomy (발반사 마사지가 유방암 환자의 수술 후 통증과 수면만족도에 미치는 효과)

  • This study, a quasi-experimental study using a nonequivalent control group pretest-posttest design. conducted a survey on 30 patients test group and control group of IS patients sampling an adjacent population at random - who are operated on the breast cancer and are in the A University Hospital located in Suwon, to examine into the effects of foot reflex zone massage on their pain and sleep satisfaction through the nursing interventions. It gave them a foot reflex zone massage for the total 30 minutes-basic massage 10 minutes and reflex massage 20 minutes required to alleviate their pain day in day out covering a two-month period from the first of July to the first of September 2004. for the purpose of collecting data. It launched into a two-round foot reflex zone massage at 6 and 24 hours since mastectomy through a direct visit at the hospital room to measure pain intensity, physiological index(pulse and blood pressure) and sleep satisfaction of them. The results are as follows. First, it showed that there is all the difference between pain intensity of test group and that of control group. In result, the first hypothesis, pain of test group. which measure at 6(p=.000) and 24 hours(p=.001) since mastectomies, will be bigger than that of control group, was established. Second, it showed that there is all the difference between physiological index of test group and that of control group. In result. the second hypothesis, pulse(p= .025, p= .002), systolic blood pressure(p= .004, p=.012) and diastolic blood pressure(p=.004. p= .003) of test group, which investigate at 6 and 24 hours since mastectomies, will be bigger than that of control group, was established. Third, it showed that there is a significant difference between sleep satisfaction of test group and that of control group(p=.000). In result, the fourth hypothesis, sleep satisfaction of test group. which examine in the morning after mastectomies, will be bigger than that of control group, was established. In the result. a foot reflex zone massage is seen to be effective in the pain reduction and sleep promotion of patients who are operated on breast cancer, and in providing them with more qualitative care by improving confidence between them and nurses through a physical touch. Also, it can be applied to a clinical examination through an independent nursing intervention.

The Effects of Courseware Instruction Using Scaffolding Strategy on 10th Grade Students' Learning Chemical Reaction Rate (10학년의 화학반응속도 학습에서 스캐폴딩 전략을 적용한 코스웨어 수업의 효과)

  • The purpose of this study were to develop of courseware using scaffolding strategy and to investigate the effect of courseware instruction upon students' conceptual understanding and affective domain including effort and self-checking. 10th grade students in the experimental and control groups were selected from high school at Cheongwon-gun in Chungbuk, and taught about chemical reaction rate for 3 class hours. The students in the experimental group studied individually through courseware using scaffolding strategy and worksheet and teacher-centered expository lesson was used in the control group. Prior to the instructions, test specification for problem-solving assessment (TSPSA) was administered and the scores of the previous science achievement test were obtained. After the instructions, the posttest on conceptual understanding and TSPSA were administered. Results of this study revealed that the mean score of the experimental group in the conceptual understanding test was similar to that of the control group at statistically significant level. This result implies that the courseware using scaffolding strategy could be a tool of the individualized instruction. The experimental group improved significantly better in three components of the TSPSA such as effort, self-checking, and cognitive strategy. From this result, it is considered that learners' self regulation is improved by being taken over responsibility of learning. So, it is suggested that instruction using scaffolding strategy is needed in science class to improve self-leading learning ability.

The needs and effects of convergence-based simulation practice for obstetrical nursing (분만간호에 대한 융복합 기반 시뮬레이션 실습 효과 및 요구도)

  • The purpose of this study was to evaluate the needs and effects of convergence-based simulation practice for obstetrical nursing. The study participants were 47 junior nursing students of a university in Chungnam province. Data were collected by a self-administered questionnaire from February 13 to 25, 2012. Convergence-based simulation practice for obstetrical nursing was applied 8 hours in a day for 2 days and consisted of four sessions. Self-efficacy in obstetrical nursing (t=-14.74, p<.001) and nursing skills related to obstetrical nursing (t=-17.51, p<.001) showed significant increase in the posttest and practice satisfaction was higher than practice stress. The needs of simulation practice was 7.96 out of 10 point and higher as practice satisfaction was higher. The needs of simulation practice before clinical practice was higher than simulation practice after clinical practice. These results indicate that simulation practice for obstetrical nursing can be used as the alternative for clinical practice. In addition to simulation practice should be done before clinical practice and strategies to increase the nursing student's cognition for the need of simulation practice are also needed.

The effects of Peer Mentoring Learnings-based Preclinical OSCE program on Self-Confidence on Core Basic Nursing Skills and Critical Thinking Disposition for Nursing Student (임상실습 전 동료멘토링 학습을 적용한 OSCE 프로그램이 간호학생의 핵심기본간호술 수행자신감과 비판적 사고성향에 미치는 효과)

  • This study was performed to examine the effects of the peer mentoring learning-based program, named preclinical objective structured clinical examination (OSCE), on self-confidence on core basic nursing skills and critical thinking disposition of nursing students. A nonequivalent control group pretest-posttest design was used. The participants were 68 sophomore nursing students of a university in J province. The research period was conducted from February 2016 to February 2017. As a result, OSCE increased self-confidence on core basic nursing skills and critical thinking disposition, compared to the control group. Post self-confidence on core basic nursing skills had positive correlations with post critical thinking. Therefore, we suggest that based on this study findings, further research be done to compare and analyze effectiveness of various peer mentoring learning-based OSCE programs.
