The Effect of Ankle Balance Taping on Gait and Balance in Stroke Patients

  • Objective: Kinesio tape has been applied to the ankle to improve balance and gait. Stroke patients show abnormal gait patterns due to foot drop. This study aimed to determine the effects of ankle balance taping which to support the ankle joint on balance and gait in patients with chronic stroke. Design: A randomized controlled trial. Methods: Twenty-four chronic stroke patients were selected and randomized into experimental group (n=12) and control group (n=12). The experimental group applied kinesio taping three times a week for three weeks, and the control group applied placebo taping for the same amount of time. To evaluate the effectiveness of the treatment, the subjects' walking ability, static balance, and dynamic balance were assessed before and after the experiment. Gait speed and spatiotemporal gait ability were measured to examine walking ability, postural sway velocity and velocity moment for static balance, and Timed-Up and Go test and Berg Balance Scale were conducted to check dynamic balance. Results: The experimental group showed a significant increase in walking ability, static balance, and dynamic balance in the within-group pre-post difference (p<0.05). In the between-group comparison, the experimental group had a significant difference in walking ability than the control group (p<0.05). Conclusions: Ankle balance taping can help improve gait, and this study can be used as a basis for future studies of ankle balance taping.

만성 뇌졸중 환자의 마비측 중간볼기근 테이핑이 균형과 보행에 미치는 영향 (The Effect of Gluteus Medius Taping on Balance and Walking in Chronic Stroke Patients)

  • Purpose : Kinesio taping is a therapeutic method used in the treatment of various musculoskeletal and neuromuscular deficits limited evidence the effects of gluteus medius kinesio taping in neurologic patients. Therefore, this study aimed to determine the effects of gluteus medius kinesio taping on balance ability and gait function in after a stroke. Methods : Twenty-four post-stroke patients were included in this study. Gluteus medius taping group and sham gluteus medius taping group were divided into intervention. Kinesio tape was applied the gluteus medius muscles. In all the subjects, the balance ability was measured using the force plate and timed up & go test (TUG) and gait function was assessed using the 10-meter walking test at time points of both before and after the taping. Result : There was a significant difference in balance ability and gait function between the two groups before and after gluteus medius taping group (p<.05). The gluteus medius taping group showed a significant difference between the groups (p<.05). Conclusion : The results suggest that taping may be a useful method during rehabilitation programs for stroke patients. Application of Kinesio taping to the gluteus medius muscles was found to be useful especially in improving balance ability.

닫힌 사슬 하지 저항운동이 뇌졸중 환자의 보행에 미치는 영향 (Effects of Close Kinetic Chain Resistant Exercise of Lower Extremity on the Gait with Stroke)

  • PURPOSE: The purpose of the study was to determine the effects of close kinetic chain resistant exercise of lower extremity on the gait with stroke patients. METHODS: The subjects were 50 patients who were diagnosed with cerebrovascular accident. They were randomly assigned either to a close kinetic chain resistant exercise of lower extremity group (study group)(n=25) or open kinetic chain resistant exercise of lower extremity exercise group (control group)(n=25). Gait abilities were measured by using Timed Up & Go (TUG) test, Functional Gait Assessment (FGA) and spatio-temporal gait variable that were velocity, cadence, stride length, double limb support by 3 axises wireless accelerometer and sway angle of center of mass by same instrument. RESULTS: Study group and control group before and after the intervention there were significantly difference in TUG, FGA, spatio-temporal gait variables and sway angle of center of mass (p<.05). There were significantly different between study group and control group for all variables at post-exercise. CONCLUSION: When all is said and done it is expected to be used as a method for the treatment and prevention in the process of rehabilitation of patients with stroke. In its final analysis when applying resistant exercise of lower extremity to stroke patients' gait, close kinetic chain is more effective than open kinetic chain.

이중운동과제 훈련이 만성 뇌졸중 환자의 균형 및 보행에 미치는 효과 (The Effects of Two Motor Dual Task Training on Balance and Gait in Patients with Chronic Stroke)

  • Purpose: The goal of this study was to investigate the effect of balance and gait ability through two motor dual task training in chronic stroke subjects. Methods: A group of twenty-five subjects who were six months post stroke participated in this study, where they were designated into pretest-posttest control The subjects were randomly allocated into two groups: experimental (n=13) and control (n=12). Both groups received physical therapy for 5 session 30 minutes per week during 6 weeks. Experimental group practiced additional two motor dual task training programs for thirty minutes a day, three days a week during six weeks. Evaluation of results was obtained through analyzing static balance, dynamic balance and gait function. Results: There was significant improvement among the group that practiced the additional two motor dual task training in that the postural sway area with open eye and close eye on the foam surface, the dynamic balance (p<0.05), and the gait function (p<0.05). Conclusion: Two motor dual task training improved static balance on the foam, dynamic balance, gait function. These results suggest that two motor dual task training is a feasible and suitable treatment for individuals with chronic stroke.

수지경혈 마사지가 뇌졸중환자의 일상생활동작 수행 능력과 악력반응에 미치는 효과 (The Effects of Hand Acupoint Massages on ADL Ability and Grip Strength of Stroke Patients)

  • Purpose: The purpose of this study was to explore the effects of hand acupoint massages on ADL ability and grip strength of stroke patients. Method: This study was designed as a quasi-experimental nonequivalent control group for pre and post-test. Twenty-eight patients were assigned to the experimental group and thirty to the control group. The hand acupoint massage was applied to both hands of the experimental group for sixteen minutes each time, a total of 15 times during five weeks. The patient's ADL ability and grip strength were measured before and after the program. The data were analyzed with mean. percent, chi-square, a paired t-test and t-test. Result: The level of ADL ability was significantly higher in the experimental group than in the control group. Also, the level of grip strength was significantly higher in the experimental group of the plegic side than in the control group, but not in the normal side. Conclusion: We have found that the hand acupoint massage helps the stroke patients improve their ADL ability and grip strength. Through this result, the hand acupoint massage could be considered as an effective application for rehabilitation of hemiplegic patients after stroke.

The Effect of Treadmill Training Applied Simultaneously with Action Observation on Walking Ability in Chronic Stroke Patients

  • Purpose: To investigate the effect of treadmill training applied simultaneously with gait related action observation on walking ability in chronic stroke patients. Methods: Sixteen chronic stroke patients participated in this study. Participants were randomly allocated into either the treadmill applied simultaneously with action observation training group (TAG) or treadmill applied simultaneously with landscape observation training group (TLG). The participants in both group underwent treadmill training for four weeks (a total of twelve minute, once a day, three times weekly for a four week period). All participants were measured to gait speed (10 m walking test, 10 MWT), gait endurance (6 minute walk distance, 6 MWD), dynamic gait index (DGI). In order to assure the statistical significance of the results, we used for SPSS 15.0 for windows. The Wilcoxon signed ranks test was used to compare pre-test and post-test result, and the Mann-Whitney U-test was employed for comparison between groups. Results: The 10 MWT, 6 MWD, DGI was significantly different between the TAG and the TLG group. Conclusion: According the results of this study, treadmill applied simultaneously with action observation (TAG) is effective intervention for improvement of walking ability in chronic stroke patient.

중뇌 경색 이후 발생한 병적 웃음과 울음 환자: 증례 보고 및 문헌 고찰 (Pathological Laughing and Crying following Midbrain Infarction: Case Report and Literature Review)

  • Pathological laughing and crying (PLC) is a condition defined by relatively uncontrollable episodes of laughter, crying or both. PLC is an uncommon symptom usually caused by cerebral lesions. Midbrain involvement causing PLC is extremely unusual and the exact mechanism by which this condition develops is poorly understood. We recently experienced a 51-year-old woman who were diagnosed as PLC after midbrain infarction. She was treated by acupuncture, pulsed electromagnetic therapy (PEMT). After 6 weeks treatment, Pathological Laughter and Crying Scale (PLACS), Korean version of the Scale for the Assessment and Rating of Ataxia (K-SARA) are decreased and Korean version of Modified Barthel Index (K-MBI) is increased. Treatment of traditional Korean Medicine could be effective for stoke rehabilitation including post-stroke PLC. And we have considered mechanism of PLC associated with midbrain lesion, dysfunction of cortex-thalamus-hypothalamus-basal ganglia-mesencephalon and faciorespiratory nuclei pathways, cerebro-ponto-cerebellar pathways and damaged serotonergic neurotransmission can cause this based on recent neurobiology of emotion. To define exact mechanism and find effective treatment, further studies are needed.

만성 뇌졸중 환자에 대한 PNF 집단 운동프로그램의 효과 (Efficacy of PNF Group Exercise Program in Chronic Stroke)

  • Objective : The purpose of this study was to evaluate the influence of a community based group exercise intervention on motor functional capacity. To evaluate the immediate(post-treatment) effects after 6-weeks exercise program on the group exercise intervention(PNF and Circuit exercise). Methods : Subjects-Included persons with stroke who were living in the community. Thirty-seven subjects were randomly assigned to the PNF, Circuit exercise and control group participated in a repeated measures design that evaluated the subjects with pre-treatment, post-treatment(6 weeks). Functional ability outcome measures assessed the motor assessment scale(MAS) and EMG. Both treatment groups participated in exercise cJass three times a week for 6 weeks. Group programs focused on balance, functional motor capacity and walking ability. The PNF program was modified PNF pattem and techniques with emphasis on functional tasks when possible, as well as stretching of the more affected limb particularly in the more affected shoulder. The Circuit program with subjects completing practice at a selies of work station as well as participating in walking races and relay with other members of the group. Results : Compared with the control group, the treatment group had larger improvements in the motor function ability after 6 weeks treatment and Post-treatment test scores were more significant than the pre-treatment score. 1. Motor function were assessed by using MAS, sit to stand, walking and upper function were assessed pre-treatment versus post-treatment measures revealed a statically significant(p<.05). There were significant differences between the groups. Compared with the control group, the treatment group had larger improvements. 2. In the treatment groups, demonstrated difference in the electromyographic activation of biceps, triceps, quadriceps and tibialis anterior muscles on the paretic side in the response to the reaching arm movement and stepping motion in stance. The difference in muscle activation improvement were not statically significant. Conclusion : The results of this study showed that the PNF and Circuit group exercise intervention can improve motor functional ability. This study suggests that the PNF and Circuit exercise programs is appropriate for community-based group exercise principles. It leads to gain and maintain potential function for disabled persons after stroke in the community.

The Effect of Action Observation Training on Sit to Walk with Chronic Stroke Patients

  • Purpose: This study was performed to examine the effect of action observation training on sit to-walk times in chronic stroke patients. Methods: Twelve stroke patients were randomly allocated to two groups, as follows: an action observation training (AOT) group and a scenery picture observation (SPO) group. The AOT group was engaged in video observation for 2 minutes 30 seconds with 12 ADL functional activities related to the sit-to-walk task: physical training was carried out in two repetitions lasting 2 minutes 30 seconds. The landscape imagery control group observed landscape picture for 2 minutes 30 seconds and then carried out the same physical training. The intervention involved a total of 12 sessions over 4 weeks, and each session lasted 30 minutes. The pre and post-tests measured the timed up and go test (TUG), Sit to stand test (STS), and Dynamic gait index (DGI). The Wilcoxon signed ranks test was used to compare pre-test and post-test result, and the Mann-Whitney U test was employed for comparison between groups. Results: The TUG time was significantly different between the AOT group and the SPO group. According to the findings, the experimental group's TUG was significantly decreased (p<0.05). In comparison of the two groups, there was no significant in STS or DGI between the AOT group and SPO group. Conclusion: This study showed that the intervention can be used to decrease TUG time. Our research suggests that action observation training has a positive effect on sit-to-walk times in patients with chronic stroke.

시간사용중재가 뇌졸중 환자의 생활만족도 및 자아존중감에 미치는 영향 (Effects of Time-Use Intervention on Life Satisfaction and Self-Esteem in Stroke Patients)

  • 목적 : 본 연구는 뇌졸중 환자를 대상으로 8주간 실시된 시간사용중재가 뇌졸중 환자의 생활만족도 및 자아존중감에 미치는 영향을 알아보았다. 연구방법 : 본 연구는 2019년 7월부터 10월까지 뇌졸중 환자 21명을 대상으로 실험군과 대조군으로 나누어 실시하였다. 생활만족도의 변화는 생활만족도 척도(Life satisfaction scale)를 사용하였고, 자아존중감의 변화는 자아존중감 척도(Self-esteem scale)를 사용하였다. 결과 : 첫째, 실험군과 대조군 각각 중재 전과 후를 비교한 결과, 생활만족도와 자아존중감 모두 통계학적으로 유의한 차이를 보였다(p<.05). 둘째, 중재 후 실험군과 대조군의 생활만족도와 자아존중감을 비교한 결과, 통계학적으로 두 집단 간 유의한 차이를 보였다(p<.05). 결론 : 본 연구의 결과 시간사용중재가 뇌졸중환자의 생활만족도와 자아존중감에 긍정적인 영향을 미쳤으며, 따라서 의미있는 작업 수행의 참여가 뇌졸중 환자의 정신건강과 생활의 질을 향상시킬 수 있을 것이다.
