Alterations in thyroid function test results are frequently seen in patients with nonthyroidal illness & correlate with the severity of the illness & prognosis. We studied thyroid hormone changes in 14 patients received cardiopulmonary bypass[CPB]. All patients were biochemical euthyroidism preoperatively. TSH[Thyroid Stimulating Hormone] level reached its nadir[0.46 $\pm$ 0.11 ulU/ml, P<0.0005] at 12 hours after the start of CPB & showed elevating pattern to the preoperative level thereafter. FT4[Free Thyroxine] reached to its nadir[10.16$\pm$ 1.17 pmol/L, P<0.01] at POD[Post Operative Day] #4 & reached to the preoperative level at POD #7. Mean serum TSH & FT4 concentration were within normal limits[P>0.25] during CPB & thereafter. TT3[Total Triiodothyronine] reached to its nadir[38.6 $\pm$ 8.4 ng/dl, P<0.001] at 30 minutes after the start of CPB & remained low[P<0.05] throughout the study period. The patients whose recovery was uneventful[Group I] had higher serum TSH, TT3 levels[P<0.05] than who had complications or died[Group II]. Group I showed the elevating pattern of TSH, TT3 at POD #4, but Group II failed to show such elevating pattern. In Group I, FT4 was within normal limits[P>0.5] throughout the study period, and also within normal limits[P>0.1] in Group II.
We will report 6 cases of cardiac tamponade treated surgically at Severance Hospital during the past 9 years from 1964 to 1972 and reviewed literatures on cardiac tamponade. The age of patients was from 13 years to 45 years old. The male was 4 cases and the female 2 cases. The sites of injury were right atrium; 1 case, right ventricle; 2 cases, right ventricle and coronary artery; 1 case, left atrium; 1 case, and left ventricle; 1 case. 2 cases of cardiac tamponade developed following chest injury, 2 cases following pericardiocentesis,1 case due to continuous bleeding from sutured cardiotomy wound of left atrium following open mitral commissurotomy using cardiopulmonary bypass machine, and 1 case due to traumatic penetration of polyethylene catheter through right ventricle to pericardial sac, introduced via right jugular vein in order to monitor the central venous pressure. Central venous pressure was checked preoperatlvely in 5 cases. In all cases, central venous pressure was rised [the range of central venous pressure was 240 to 330 mmHg]. Immediately after operation,central venous pressure lowered to normal [the range was 80-100 mmHg]. Recently serial gas analysis of arterial blood were checked pre- and post-operatively for the evaluation of hemodynamic change of cardiac tamponade, but our data was not enough for evaluation. It should be studied further.
Purpose: This study is a non-equivalent control group pre-post design, which attempted to observe the effects of Basic Life Support Education on undergraduate students' CPR Knowledge and Attitude. Methods: The subjects were 96 undergraduates in Jeonbuk. They were divided into two groups: an experimental group of 46 students and a control group of 50 students. The BLS education was given by three BLS instructors certified by the American Heart Association using a DVD. Data were collected from October 20 to November 15, 2010 and analyzed, using the ${\chi}^2$ test, t-test, and ANCOVA. Results: There were significant differences between the experimental group and the control group in the levels of CPR Knowledge and Attitude. The 1st hypothesis, that the CPR knowledge scores for the experimental group would be higher than those of the control group, was supported (F=157.58, p=.000). The 2nd hypothesis, that the CPR attitude scores for the experimental group would be higher than those of the control group, was supported (F=23.78, p=.000). Conclusion: This study showed that BLS education had a positive influence on undergraduate students' knowledge and attitudes toward CPR. We expect that BLS education will contribute to spreading knowledge of CPR and reducing mortality among heart attack victims, and we can see from this study how CPR training in combination with theory and clinical practice develops knowledge and practical ability.
Objective: The purpose of this study was to changes in respiratory function due to differences in mask blocking grade, and effects on cardiovascular function during aerobic exercise. Design: Comparative study using paired t-test and analysis of variance measures. Methods: Male and female subjects were randomly divided into 4 groups according to mask blocking grade and measured respiratory functions before and after wearing the mask using spirometers, and their cardiovascular capabilities were measured using pulse oxygen meters and blood pressure meters before and after running for 10 minutes wearing mask. Used paired t-test, one-way ANOVA and Bonferroni post-test to compare the differences according to the mask blocking grade. Results: The experimental results verified that three groups except for the Non-mask group had a significant reduction in all values of FVC, FEV1, MVV, and PEF after wearing a mask compared to those before wearing a mask(p<0.05). Also a significant difference in cardiovascular functions was also observed after aerobic exercises wearing mask in all groups and there was a significant difference between all four groups(p<0.05). Conclusions: Lung patients with poor breathing, elderly and children with poor lung function, and professional groups who need to wear masks and take high-intensity work or aerobic movements are encouraged to take frequent breaks in advance, wear masks and social distancing at moderate and low levels.
This study analyzed the physical changes in 119 paramedics transporting equipment at the emergency site and performing post-cardiopulmonary resuscitation through experiments. First, the average heart rate increased by about 25 times comparing CPR was performed without physical load and with personal protective equipment after moving equipment. In the third quartile, it increased to about 27 times. Second, when CPR was performed without physical load, and CPR was performed after moving the equipment with personal protective equipment, both the body temperature was raised and the rising body temperature was measured within normal body temperature. Third, the change in respiration rate increased by 7 times on average comparing CPR was performed without physical load and CPR was performed after moving the equipment while wearing personal protective equipment. In the third quartile, it increased to about 11 times. Finally, the change in blood pressure increased by 26.6 mmHg on average comparing CPR was performed without physical load and with wearing personal protective equipment after moving the equipment, and increased by 31.2 mmHg on average in the third quartile.

Objective: This study aimed to identify the effects of serum potassium and lactate on neurologic outcomes in out-of-hospital post-cardiac arrest adult patients. Methods: This study was a single center, retrospective observational study. We recruited out-of-hospital post-cardiac arrest adult patients admitted to an intensive care unit from 2011 to 2017. Primary outcome was good neurologic outcome at discharge. To evaluate the prognostic impact of serum potassium and lactate, univariate and multivariate logistic regression analyses were performed. Results: A total of 57 patients were included in this study. The number of patients with good neurologic outcome was 19 (33.3%). In the univariate analysis, good neurologic outcome patients showed a higher smoking rate, shorter pre-hospital transportation time, higher rate of percutaneous coronary intervention, and lower severity score (all P<0.05). The good neurologic outcome patients also presented higher pH, lower partial pressure of carbon dioxide, and lower potassium regarding laboratory findings on the first hospital day (all P<0.05). In the multivariate analysis, the independent factors favoring good neurologic outcome were pre-hospital transportation time (adjusted odds ratio [aOR], 0.82; 95% confidence interval [CI], 0.69-0.97; P=0.019) and lower partial pressure of carbon dioxide on the first hospital day (aOR, 0.95; 95% CI, 0.91-0.99; P=0.034). Conclusion: Serum potassium and lactate were not significantly associated with good neurologic outcome in out-of-hospital post-cardiac arrest adult patients. The prognostic factors for good neurologic outcome were pre-hospital transportation time and initial partial pressure of carbon dioxide.
Background: We tested the technical feasibility of fetal cardiac bypass and collected baseline data on the fetal hemodynamics and placental functions related to the cardiopulmonary bypass in the fetal lamb model. Material and Method: Eleven fetuses at 120 to 150 days of gestation were subjected to bypass via trans-sternal approach with a 12 G pulmonary arterial cannula and 14 to 18 F venous cannula for 30 minutes. All ewes received general anesthesia with ketamine. In all the fetuses, no anesthetic agents were used except muscle relaxant. Eight served as a group in which placenta was excluded from the extracorporeal circulation by clamping the umbilical cord during the bypass(the oxygenator group) and in the remaining three, the placenta worked as the only source of oxygen supply(the placenta group). Observations were made every 10 minute during a 30-minute bypass and 30-minute post bypass period. No prostaglandin inhibitors were used both in ewes and in fetuses. Result: Weights of the fetuses ranged from 1.9 to 5.2 kg. In the oxygenator group, means of arterial pressure, PaO2, atrial pressure, heart rate, and bypass flow rate ranged 69.8 to 82.6 mmHg, 201.7 to 220.9 mmHg, 4.1 to 4.3 mmHg, 169 to 182/min, and 140.3 to 164.0 ml/kg/min, respectively during bypass, but rapid deterioration of the fetal cardiac functions and the placental gas exchange was observed after the cessation of bypass. In the placenta group, means of arterial pressure decreased from 44.7 to 14.4 mmHg and means of PaCO2 increased from 61.9 to 129.6 mmHg during bypass. Flow rate was suboptimal(74.3 to 97.0 ml/kg/min) during bypass. All hearts fibrillated immediately after the discontinuation of bypass. Conclusion: In this study, the technical feasibility of fetal cardiopulmonary bypass was confirmed in the fetal lamb model. However, further studies with modifications of the bypass including an addition of prostaglandin inhibitor, an application of the total spinal anesthesia on the fetus, a creation of more concise bypass circuit, and a use of active pump are mandatory to improve the outcome.
Background: Recently, percutaneous cardiopulmonary support (PCPS) has been widely used to rescue patients in cardiogenic shock or cardiac arrest. However, patients with cardiopulmonary bypass (CPB) weaning failure during open heart surgery still have very poor outcomes after PCPS. We investigated clinical results and prognostic factors for patients who underwent PCPS during open heart surgery. Material and Method: From January 2005 to December 2008, 10 patients with CPB weaning failure during open heart surgery underwent PCPS using the CAPIOX emergency bypass system ($EBS^{(R)}$, Terumo Inc, Tokyo, Japan). We retrospectively reviewed the medical records of those 10 patients. Result: The average age of the patients was $60.2{\pm}16.5$ years (range, $19{\sim}77$ years). The mean supporting time was $48.7{\pm}64.7$ hours (range, $4{\sim}210$ hours). Of the 10 patients, 6(60%) were successfully weaned from the PCPS While 5 (50%) were able to be discharged from the hospital. Complications were noted in 5 patients (50%). In univariate analysis, long aortic cross clamp time during surgery, mediastinal bleeding during PCPS and high level of Troponin-I before PCPS were significant risk factors. All of the discharged patients are still surviving $34{\pm}8.6$ months (range, $23{\sim}48$ months) post-operatively. Conclusion: The use of PCPS for CPB weaning failure during open heart surgery can improve the prognosis. More experience and additional clinical studies are necessary to improve survival and decrease complications.

Observations were made every 10 minutes during a 30-minute bypass and 30-minute post bypass period. Result: Weights of the fetuses ranged from 2.2 to 5.2 kg. In Control group, means of arterial pressure decreased from 44.7 to 14.4 mmHg and means of Pa$CO_2$ increased from 61.9 to 129.6 mmHg at each time points during bypass. Flow rate was suboptimal (74.3 to 97.0 $m\ell$/kg/min) during bypass. All hearts fibrillated immediately after the discontinuation of bypass. On the contrary, in Experimental group, means of arterial pressure reamined higher (45.8 to 30 mmHg) during bypass (p<0.05). Means of Pa$CO_2$ were less ranging from 59.8 to 79.4 mmHg during bypass (P<0.05). Flow rates were higher (78.8 to 120.2 $m\ell$/kg/min) during bypass (p<0.05). There were slower deterioration of cardiac function after cessation of bypass. Conclusion: In this study, we demonstrated that the placental flow was increased during fetal cardiopulmonary bypass in the group pretreated with indomethacine and total spinal anesthesia. However, further studies with modifications of the bypass including a creation of more concise bypass circuit, and a use of axial pump are mandatory for the clinical application.
The purpose of this study is to investigate the CPR within ambulance. The Subjects were 890 emergency medical technicians in fire department in Gyeonggido. After continuing education program, the emergency medical technicians completed the questionnaires from January to February, 2012. Questionnaire consisted of demographic characteristics, method of chest compression (one handed or two handed), chest compression - ventilation ratio, and hands only CPR. Data were analyzed by frequency analysis and Chi-square test, t-test. One-way analysis of variance (ANOVA) was followed by Scheffe post-hoc test to analyze changes in all parameters between all groups. One handed chest compression accounted for 14.0% and two handed chest compression accounted for 86.0%. Hands only CPR accounted for 71.7% and standard CPR (30:2) accounted for 28.3%. In order to perform the high quality CPR, as least three persons must be dispatched in the ambulance. The flexible management of manpower may improve the survival from the cardiac arrest. A systemic approach for educating and training EMT's in CPR is needed.
