Changes of Cerebral Metabolism and the Related Factors during Cardiac Surgery

  • The effect of cardiopulmonary bypass (CPB) on cerebral physiology during heart surgery remains incompletely understood. This study was carried out to investigate changes of cerebral metabolism and the association between the changes and clinical factors during heart surgery. Seventy adult patients (n=70) scheduled for elective cardiac surgery were participated in the present study. Middle cerebral artery blood flow velocity (V$_{MCA}$), cerebral arteriovenous oxygen content difference (C(a-v)O$_2$), cerebral oxygen extraction (COE), and modified cerebral metabolic rate for oxygen (MCMRO$_2$) were measured during six phases of the operation; Pre-CPB, CPB-10 min, Rewarm-1 (nasopharyngeal temperature 34$^{\circ}C$), Rewarm-2 (nasopharyngeal temperature 37$^{\circ}C$), CPB-off, and Post-OP (at skin closure after CPB-off). Each relationship of age, arterial blood gas parameters, or other variables to V$_{MCA}2$, C(a-v)O$_2$, COE, or MCMRO$_2$ was evaluated. V$_{MCA}$ increased (P<0.0001) whereas C(a-v)O$_2$ decreased (P<0.01) throughout the five phases of the operation compared to Pre-CPB value (control). COE diminished at CPB-10, Rewarm-1, and CPB-off (P<0.05) while MCMRO$_2$ reduced at CPB-10 and Rewarm-1 (P<0.05) compared to Pre-CPB value. Positive correlation was found between age and cerebral metabolic parameters (V$_{MCA}$, C(a-v)O$_2$, COE, or MCMRO$_2$) during CPB (range r=0.24 to 0.38, p<0.05). Four cerebral metabolic parameters had partially negative or positive correlation with arterial blood gas parameters and other variables (arterial blood pH, $O_2$ tension, $O_2$ content, $CO_2$ tension, blood pressure, blood flow, temperature, or hematocrit) during the operation. In conclusion, CPB led to marked alterations of cerebral metabolism and age, pH, and $CO_2$ tension profoundly influenced the changes during cardiac surgery.

급성심근경색 후 발생한 양심실파열로 인한 심장압전에서 체외순환의 사용없이 시행한 수술적 치료 - 1예 보고 - (Operative Treatment for Cardiac Tamponade with Ventricular Rupture of Post Myocardial Infarction without Cardiopulmonary Bypass - A case report -)

  • 허혈성 좌심실벽 파열은 심근경색 후에 발생하는 치명적인 합병증 중의 하나인 심장파열의 한 유형으로 빠른 진단과 수술이 필요하다. 환자는 내원 15분 전 갑작스런 의식소실을 주소로 내원한 75세 여자로 당시 의식은 반 혼수상태로 얼굴과 상하지에 청색증 소견을 보였으며 응급실에서 시행한 심초음파 검사상 심낭에 약 $1.5{\sim}2\;cm$ 두께로 삼출소견이 관찰되었으며 심장박동 수가 35회/분까지 떨어져 심장마사지 시행하면서 응급수술을 시행하였다 우심실 전벽에 1 cm정도의 파열과 좌심실 벽에 괴사성 출혈반흔을 동반한 파열부위를 확인하고 체외순환 없이 사친스키 겸자를 이용해 출혈부위를 잡은 뒤 봉합하였다. 환자는 수술 후 28일째 약간의 호흡곤란은 있으나 일상생활 가능한 상태로 퇴원하였다.

요양보호사 교육생을 위한 기본심폐소생술 교육프로그램 개발 및 효과 (Development and effects of basic cardiopulmonary resuscitation education program for trainee care workers)

  • Purpose: This study set out to use a one-group pretest-posttest experimental design to develop a highly motivating CPR program for help care workers who lack CPR education, and to evaluate its effects on their knowledge about program application, attitude, and self-efficacy. Methods: For this purpose, 140 questionnaires were administered to subjects who underwent training at an educational institution for care workers in S city and G province. The program was developed from April, 25 to May, 15 in 2016, and the evaluation period was from May to July, 15. The collected data were analyzed through SPSS version 23, and a paired t-test was conducted. Results: The program spanned 240 minutes, including 80 minutes on theoretical education and 160 minutes on practical education. Knowledge of CPR in the post-test proved to be higher than that in the pre-test, from 6.4 to 9.2 (t=-15.72, p<.001),; the score on attitude increased from 38.4 to 41.7 (t=-5.89, p<.001),; and self-efficacy increased from 41.0 to 66.7 (t=-21.83, p<.001). Conclusion: The basic CPR education program emphasizing motivation was effective for the care worker trainees and could be used for other curriculum by expanding the scope of the subject.

The Effect of Patient-centered CPR Education for Family Caregivers of Patients with Cardiovascular Diseases

  • Purpose: For cardiovascular patients, family caregivers play a vital role in daily nursing and cardiac emergencies. This study aimed to evaluate the effect of patient-centered CPR education (PCE) for family caregivers of patients with cardiovascular diseases. Methods: Fifty-four participants were randomly assigned to the PCE or control group. The PCE group received tailored counseling on overall cardiovascular disease information and CPR followed by interactive instructor-guided CPR training and re-education follow-up by telephone 2 weeks later. The control group received only video-based CPR self-education and booklets. Cardiovascular disease and CPR knowledge and self-efficacy were measured before (pre-test), immediately after (post-test 1), and 4 weeks after the PCE (post-test 2). CPR skills and performance were measured pre-test and at post-test1. Results: The PCE group demonstrated significant improvements in knowledge (F=91.09, p<.001), self-efficacy (F=15.19, p<.001) and CPR skills and performance (F=8.10, p=.008), as well as significant differences over time (knowledge: F=364.25, p<.001; self-efficacy: F=1162.28, p<.001; CPR skills and performance: F=1798.81, p<.001). There were significant group-by-time interactions for knowledge (F=8.10, p=.001), self-efficacy (F=4.30, p =.019) and CPR skills and performance (F=4.81, p=.036) by repeated measures ANOVA. Conclusion: This is the first study to demonstrate the effects of a patient-centered intervention with CPR education tailored for patients' and family caregivers' preferences, needs, and lifestyles. The results of this study encourage the use of tailored, patient-centered interventions in cardiovascular nursing practice.

심폐소생술에 대한 현장업무 프로토콜 교육 효과 비교 (The Comparison of Effectiveness in Prehospital Protocol Education on CardioPulmonary Resuscitation)

  • 본 연구의 목적은 비외상성 심정지 환자에게 적용하는 심폐소생술에 대한 현장업무 프로토콜의 유용성을 검증하는 것이다. 연구는 2008년 5월 1일부터 동년 6월 27일까지, 전라남도와 전라북도에 소재한 J, K대학에 재학 중인 응급구조과 학생 150명을 대상으로 하였다. 비동등성 대조군 전.후 유사 실험 설계(nonequivalent control group pretest-posttest design)를 사용하여, 실무적합성 검증을 실시하였다. 현장업무 프로토콜을 적용한 실험군과 기존의 심폐소생술을 적용한 대조군으로 나누어, 각 프로토콜 항목들의 소요시간을 비교하여 통계 분석하였다. 연구 결과, 5개항목(의식상태 평가, 기도유지, 인공호,,흡 2회 실시, 순환 확인, 심폐소생술 5주기 실시)을 제외한 나머지 18개 항목에서 3초 이상 수행시간이 단축되었다. 60초 이상의 단축을 보인 항목은 6개로, 기관삽관 튜브 고정(78.70초), 말초정맥로 확보(64.45초), 의식상태 재평가, 동공반사 확인(110.05초), 목동맥 확인(112. 55초), 활력징후 평가(85.7초)였으며, 전체시간은 110.85초가 단축되었다. 실험군과 대조군의 프로토콜 적용에 따른 사전.사후 변화에서 전체시간(t=-6.580, p=.000)은 통계학적으로 유의하게 감소한 것으로 나타났다. 따라서 비외상성 심정지 환자의 심폐소생술에 대한 현장업무 프로토콜은 정확하고 신속한 의사결정을 지원할 수 있는 지침으로 활용될 수 있으며 응급의료 서비스를 향상시킬 것으로 사료된다.

몸통 안정화 운동이 단기 금연자의 허파기능, 최대 산소 섭취량, 심장박동수에 미치는 영향 (Effects of trunk stabilization exercise on pulmonary function, maximal oxygen uptake, and heart rate of short-term non-smokers)

  • 본 연구에서는 흡연자와 비흡연자를 대상으로 단기 금연 동안 실시하는 몸통 안정화 운동이 단기 금연자의 허파기능, 최대 산소 섭취량, 심장박동수, 회복 시간에 미치는 영향에 대해 알아보고자 하였다. 본 연구는 충남 소재 N대학교의 20대 남성 26명으로 흡연자 13명과 비흡연자 13명을 선정하였다. 대상자의 신체적 특성 파악 후 사전 측정 시 허파기능(FVC, SVC, MVV)과 최대 산소 섭취량를 측정하였으며 3일 간 심장박동수와 회복 시간을 측정하였다. 측정 후 3일 금연과 함께 몸통 안정화 운동 프로그램을 실시하였다. 사후 측정으로 허파기능과 최대 산소 섭취량을 측정하였고, 당일 운동 후 즉시 1일차, 2일차 3일차의 심장박동수, 회복 시간을 측정하였다. 본 연구의 자료는 SPSS(Statistical Package for the Social Sciences) Ver. 23.0 for windows를 이용하여 분석되었다. 몸통 안정화 운동이 흡연자와 비흡연자의 심장허파기능에 미치는 영향을 분석한 결과, 흡연자 집단과 비흡연자 집단 모두 운동 전후 허파기능(FVC, SVC, MVV), 최대산소섭취량, 회복 시간에 유의한 차이가 나타났다. 이는 몸통 안정화 운동이 짧은 기간이라도 비흡연자뿐만 아니라 단기 금연자의 심장허파기능에 긍정적인 영향을 미칠 수 있는 것으로 생각되며, 금연 활동을 더한 몸통 안정화 운동이 심장허파기능 감소 예방 및 향상을 위한 더욱 안전하고 효과적인 방법이 될 수 있을 것으로 사료된다.

IT 기업 사무직 근로자의 대사증후군 예방을 위한 맞춤형 운동프로그램의 효과 (Effect of Individualized Exercise Program for Preventing Metabolic Syndrome among IT Company Office Workers)

  • Objectives: Interventions promoting physical exercise and healthy habits in workplaces have been shown to be effective in reducing risk factors for metabolic syndrome. This study was conducted to examine the effects of an individualized conditioning exercise program of IT company office workers with or at higher risk of metabolic syndrome. Methods: A total of 444 IT company office workers with or at higher risk of metabolic syndrome participated in a 3-month conditioning exercise program. Body composition data using bioelectrical impedance analysis and cardiopulmonary data using cardiopulmonary exercise testing from 53 individuals (mean age: 34.8 ± 7.1 years, sex : 21% female, height : 170.4 ± 6.8 cm, weight : 75.2±12.2 kg, body mass index : 25.8±3.3 kg/m2) who have successfully completed pre-test, intervention, and post-test were analyzed. The 12 weeks intervention encompassed: (1) health counseling (2) supervised exercise(endurance-based, aerobic exercise, or circuit training once a week for 50 minutes at heart rate reserve(HRR) of 77-95%) (3) self-directed exercise and biweekly health screening checks. Results: The results indicated a significant decrease in body weight, body fat mass and body mass index, respectively. Moreover, VO2peak, AT VO2 and AT Time significantly improved, respectively. Resting blood pressure(SBP/DBP) showed positive changes but were not statistically significant. We observed the correlation between characteristics of participants and rate of changes in cardiopulmonary outcomes of participants, there are no significant correlation. These results indicate positive changes in body composition and cardiorespiratory fitness parameters following individualized conditioning exercise program. Conclusions: Individualized workplace exercise program for preventing metabolic syndrome can lead to improvements in body composition and cardiorespiratory fitness.

완전 순환 정지 없이 시행한 총 폐정맥 환류 이상의 수술 교정 (Surgical Correction of Total Anomalous Pulmonary Venous Connection without Total Circulatory Arrest)

  • 배경: 초 저체온 하 순환 정지는 총 폐정맥 환류 이상을 수술하는데 있어 중요한 보조 수단이다. 그러나 초 저체온 하 순환 정지 하에 심장 수술을 하는 것은 순환 정지 후 신경계 이상의 위험을 동반하고 있다. 이에 완전 순환 정지 없이 총 폐정맥 환류 이상을 수술하여 그 결과를 평가하고자 한다. 대상 및 방법: 2000년 4월에서 2004년 10월까지 10명의 환자의 의무 기록을 후향적으로 분석하였다. 결과: 해부학적 연결 이상의 위치는 심장 상부형이 7예,심장형이 1예, 심장 하부형이 2예였다. 평균 심폐기 가동 시간과 대동맥 차단 시간은 각각 116.8 $\pm$40.7분, 69.5$\pm$24.1분이었다. 수술로 인한 사망 예는 없었고, 합병증은 술 후 폐정맥 협착이 1예, 폐렴, 기흉, 창상 감염, 횡격막 마비가 각각 1예씩 있었다. 평균 16.6개월의 추적 관찰 기간동안 페동맥 협착이 없었던 모든 환아는 NYHA class I으로 지내고 있다. 결론: 총 폐정맥 환류 이상을 완전 순환 정지 없이 수술하여 아주 만족할 만한 결과를 얻을 수 있었다.

황견에서 우측폐 이식수술기에 관한 실험적 연구 (Surgical Techniques of Right Lung Transplantation in Dogs)

  • We have performed eight, single transplantations of right lung in dogs from September, 1988 to March 1989 at the Thoracic & Cardiovascular Surgical department, Yonsei University, College of Medicine, Seoul, Korea. We wrapped bronchial anastomosis site with great omentum and used cyclosporin in preoperative and postoperative periods in seven cases except one. The one without wrapping the bronchial anastomotic site with omentum and using cyclosporin died due to bronchial anastomotic site rupture in postoperative fourth day. If there is no reason to choose one side over the other, we would generally choose to do left-sided transplant as this is technically somewhat easier because of the long length of recipient bronchus and the ease of clamping the left atrium proximal to the pulmonary veins. The right atrium limits the amount of left atrium that can have incorporated into the clamp proximal to the pulmonary veins on the right side. But we had chosen to do right-sided transplant of lung because we must take variable technical experiences on right sided lung transplant in dogs. We have to anastomose one of pulmonary vein and left atrial wall on right-sided transplant easily only with double ligation of one pulmonary vein because right atrium limited the clamp of left atrium proximal to pulmonary veins with decreased venous return and cardiac output in some dogs. All seven dogs with right-sided lung transplant had survived more than one day with good condition except one. The one dog have to be sacrificed to evaluate the difference between the gas analysis in pulmonary venous and arterial blood in post-operative eight hours. We found hemorrhagic pulmonary edematous changes of contralateral left lung in this dog. And also all dogs have to be sacrificed for the evaluation of surgical problems, anytime in post-operative periods without any cardiopulmonary resuscitative efforts when the general condition would be worse progressively. We found no any surgical technical errors in seven dogs except one with thrombi in suture site of left atrium. There were hemorrhagic pulmonary edematous changes of transplanted right lung in one, of contralateral left lung in one, of contralateral left lung with double ligation of its pulmonary artery in one, thrombi around left atrial sutures sites in one, multiple air leakage in one bronchial rupture in one due to rejection or infection. There were accidental extubation and delayed intubation in one and unknown cause of death in one.

Pulmonary Thromboembolectomy for Acute Pulmonary Thromboembolism

  • Background: Acute pulmonary thromboembolism is fatal because of abruptly occurring hypoxemia and right ventricular failure. There are several treatment modalities, including anticoagulation, thrombolytics, ECMO (extracorporeal membrane oxygenator), and thromboembolectomy, for managing acute pulmonary thromboembolism. Materials and Methods: Medical records from January 1999 to December 2004 at our institution were retrospectively reviewed for pulmonary thromboembolectomy. There were 7 patients (4 men and 3 women), who underwent a total of 8 operations because one patient had post-operative recurrent emboli and underwent reoperation. Surgery was indicated for mild hypoxemia and performed with CPB (cardiopulmonary bypass) in a beating heart state. Results: The patients had several symptoms, such as dyspnea, chest discomfort, and palpitation. Four patients had deep vein thromboembolisms and 3 had psychotic problems, specifically schizophrenia. Post-operative complications included hemothorax, pleural effusion, and pericardial effusion. There were two hospital deaths, one each by brain death and right heart failure. Conclusion: Emergency operation should be performed when medical treatments are no longer effective.
