A Preliminary Study for Evaluating on Demonstration Project of Community-based Home Health Care Nursing Services by the Seoul Nurses Association (지역사회중심 가정간호 시범사업 성과평가를 위한 기초연구- 서울시 간호사회 주관 -)

  • This study, based on current home nursing services, aims at promoting measures for establishing a community-based home nursing system derived from the pilot home nursing demonstration project conducted by the Seoul Nurses Association. The study was based on an analysis of home nursing records from march 1993 to December 1999. The following is a summary analysis, based on individual characteristics of the patients, the organization, which recommended the service for their patients and personnel services. 1. The service has been used by many elderly people 60years of age or older(66.4%). and married people(60.9%). The average number of visits by service personnel for patients of city government was 23.5. This is 2.5 times as many visits by general patients. General patients(20.2%) had only one visit from service personnel, while 65.5% of patients of city government had 10 or more visits. Particularly, for government recommended patients, 72.7% of the patients were recommended by nurses, while only 21.9% where referred to the services by doctors. The main focus of a home nursing service was to maintain present health status (53.4%), and hospice(11.6%). Also to increase hospital-based home nursing services focused on recovery(55.9%) and maintain present health conditions (19.0%). 2. For general patients, 42.0% of patients were suffering from problems related to CVA, 11.3% from high blood pressure, and for patients referred from city, 21.2% from skeletal muscular disease. Results of home nursing services 29.4% of patients were able to recover or maintain their health status, but 48.9% of the patients died. Another main point of community-based home nursing services is medication(6.7%), other basic nursing services(6.1%), special treatment, instructions on how to use medical devices(5.9%), change of physical posture(4.6%), and training on changing physical positions(4.7%). As mentioned above there were some differences between the characteristics of patients who used the pilot home nursing service conducted by the Seoul Nurses Association and those hospital-based service users. The results are believed to be useful to support a community-based home nursing service model. Particularly, patients under medical supervision and patients recommended by government-run health clinics show a higher frequency and longer use of home nursing services compared to general patients or hospital-based home nursing service users. According to the study, nurses accounted for a large number of recommendations for home nursing services. Many patients with CVA, high blood pressure, skeletal muscular disease and bedsores used community-based home nursing services, while others used the service for minor treatments or maintaining their current health status. Based on the study, the researchers make several suggestions to establish a community- based home nursing service system. First, different ways of setting up a community-based home nursing system have to be mapped out based on the evaluation of the pilot home nursing service conducted by the Seoul Nurses Association. Secondly, a new, community-based, home health care nursing service model, and reimbursement payment system have to be developed. This is based on the outcome of the analysis, and implemented policy. Accordingly, efforts are needed to develop a community- based home nursing system with an intermediary role to promote the visiting nursing services of government-run health centers.

THE CORRELATION BETWEEN CRANIAL BASE SIZE, SHAPE AND HEAD POSTURE, AND THE POSITION OF MAXILLO-FACIAL STRUCTURES (두개저의 크기, 형태 및 두부자세와 악안면구조의 위치적 상관관계)

  • This study was done to evaluate the correlations between the size, the form of the cranial base, head posture and the horizontal and vertical position of craniofacial structures. For this purpose, 100 cephalometric radiographs were taken from the sample composed of 51 male and 49 female, 12 measurement criteria and 37 reference points were established and digitized, then calculation was performed for the values of measurement variables and the horizontal and vertical position of reference points. The correlations be4ween them were analyzed statistically and mean facial diagrams were constructed and compared with the selected groups which were composed of 10 Samples each as large and small group from the measurement value. The following results were obtained: 1. The angles n-s-ba and n-s-ar as variables for the ion of cranial base correlated highly to the horizontal and vertical position of reference points in the cervical column with statistical significance($0.1\%$ level). 2. The angles n-s-ba and n-s-ar as variables for the form of cranial base correlated to the horizontal position of the reference points in the facial structure with statistical significance($1\%$ level), but not to the vertical position of them($5\%$ level). 3. The length n-s, s-ba, and n-ar as variables for the size of cranial base were correlated th the position of craniofacial structures in various ways, but in general, highly correlated to the horizontal and vertical position of midfacial structures around the teeth and alveolar area. 4. the angle NSL/CVT and NSL/OPT as postural variables tot the inclination of cranial base and cervical column were correlated to the horizontal position of the craniofacial structures with statistical significance($1\%$ level), but not to the vortical position of them($5\%$ level). 5. The angle OPT/HOR and CVT/HOR as postural variables lot the inclination of cranial base and true horizontal line were not correlated to the horizontal and vertical position of the craniofacial structures with statistical significance($5\%$ level). 6. The correlation between the measurement variables and horizontal and vortical positions of the reference poits in soft tissue were shown as similar to the related hard tissue points.

Changes in atlas position with Class ll activator treatment in Class II malocclusion patients (II급 부정교합자에서 액티베이터 사용에 따른 atlas의 위치 변화에 관한 연구)

  • Objective: Previous studies have reported that morphological features of the first cervical vertebra (atlas) have been associated with mandibular growth direction. The purpose of this study was to show the possible positional and morphological changes of the Atlas from activator treatment in Class II malocclusion patients. Methods: Lateral cephalometric radiograph tracings were made at initial, middle and final stages of treatment. Angular and linear measurements of skeletal and morphological features were measured on the anatomical landmarks and reference planes. Results: The skeletal effects of activator treatment on Class II malocclusion patients were evident on ramal height, body length, effective body length, ANB, and overjet. Clockwise rotation of the long axis of the Atlas was found in Group 1, but there was no inclination change of the Atlas in groups 2 and 3. There was no significant correlation between anterior and posterior positions of the atlas or morphological change in all groups. - except for posterior movements of the Atlas found in group 1. Conclusion: Clockwise rotation of the atlas axis resulted from activator treatment in Cl II malocclusion patients. Change in atlas axis can be thought of as an indicator for success of activator treatment.

Association between Genotypes of the Isocitrate Dehydrogenase 3, beta subunit (IDH3B) Gene and Carcass Traits in an F2 Crossbred Population of Landrace × Jeju (Korea) Black Pigs (제주흑돼지와 랜드레이스 교배 2세대의 도체형질과 IDH3B 유전자형의 상관관계)

  • This study tested the association between genetic polymorphisms of the isocitrate dehydrogenase 3, beta subunit (IDH3B) gene and economic traits in an F2 crossbred population of Landrace × Jeju (South Korea) Black pigs. A 304-bp insertion/deletion mutation in promoter region was screened for determining genotypes of the IDH3B gene in a total of 1,105 F2 pigs. Three genotypes (AA, AB, and BB) were identified in the founder, F1, and F2 populations. Association analysis showed significant differences in carcass weights (CW), backfat thicknesses in three positions of the body (4th-5th ribs, BF5; 11th-12th ribs, BF12; 13th rib-1st lumbar, BFL), and carcass lengths (CL) (p<0.05), but not in meat color (MC), eye muscle area (EMA), or marbling scores (MARB) (p>0.05). The F2 IDH3B BB homozygotes showed heavier CW (80.790±0.725 kg) and shorter CL (101.875±0.336 cm) than the other genotypes (p<0.05). In addition, the BF levels between the 4th - 5th and 11th - 12th vertebrae were thicker in the carcasses of pigs with the IDH3B BB genotype than with the other genotypes (p<0.05). These results suggested that genetic variations in the IDH3B gene may serve as molecular genetic markers for improving the Landrace × Jeju Black pig crossbreeding systems.

The Effects of Dental Hygienists' LMX (Leader-Member Exchange) and Empowerment on Organizational Performance (치과위생사의 LMX (Leader-Member Exchange)와 임파워먼트가 조직성과에 미치는 영향)

  • The purpose of this study was to determine the effects of dental hygienists' leader-member exchange (LMX) and empowerment on organizational performance, improve human resource management in their organization and their job satisfaction and organizational commitment, and provide basic data that could help improve organizational performance. A survey was conducted in 324 dental hygienists at dental care institutions in Ulsan, Busan, and Daegu from March 2 to 31, 2015 and the following results were obtained: Dental hygienists at dental hospitals had higher levels of LMX than those at dental clinics; those who were at higher positions, who were older, and who were more educated showed higher levels of empowerment. There was significant, positive correlation between LMX and empowerment and job satisfaction and organizational commitment: dental hygienists with better LMX and higher levels of empowerment showed higher levels of job satisfaction and organizational commitment. For dental hygienists, significance (${\beta}=0.325$, p<0.001) exerted the greatest impact on job satisfaction, followed by organizational commitment (${\beta}=0.264$, p<0.001) and competence (${\beta}=0.164$, p<0.01) and LMX (${\beta}=0.321$, p<0.001) had the greatest impact on organizational commitment, followed by job satisfaction (${\beta}=0.275$, p<0.001) and significance (${\beta}=0.210$, p<0.001).To put the results together, dental hygienists in dental care practice had their job satisfaction and organizational commitment affected by LMX and empowerment; therefore, it is necessary to develop diverse education programs with the objective of promoting mutual exchange between a manager of dental hygienists' team and members and improving empowerment.

60 Years since the Armistice Treaty, the NLL and the North-Western Islands (정전협정 60년, NLL과 서북 도서)

  • The United Nations Command (UNC) and the communist North failed to reach an agreement on where the maritime demarcation line should be drawn in the process of signing a truce after the Korean War because of the starkly different positions on the boundary of their territorial waters. As a result, the Armistice Treaty was signed on July 1953 without clarification about the maritime border. In the following month, Commander of the UNC unilaterally declared the Northern Limit Line (NLL) as a complementing measure to the Armistice. Referring to this, North Korea and its followers in South Korea wrongfully argue that the NLL is a "ghost line" that was established not based on the international law. However, one should note that the waters south of the NLL has always been under South Korea's jurisdiction since Korea's independence from Japan on August 15, 1945. There is no need to ask North Korea's approval for declaring the territorial waters that had already been under our sovereign jurisdiction. We do not need North Korea's approval just as we do not need Japan's approval with regard to our sovereign right over Dokdo. The legal status of the NLL may be explained with the following three characteristics. First, the NLL is a de facto maritime borderline that defines the territorial waters under the respective jurisdiction of the two divided countries. Second, the NLL in the West Sea also serves as a de facto military demarcation line at sea that can be likened to the border on the ground. Third, as a contacting line where the sea areas controlled by the two Koreas meet, the NLL is a maritime non-aggression line that was established on the legal basis of the 'acquiescence' element stipulated by the Inter-Korea Basic Agreement (article 11) and the Supplement on the Non-aggression principle (article 10). Particularly from the perspective of the domestic law, the NLL should be understood as a boundary defining areas controlled by temporarily divided states (not two different states) because the problem exists between a legitimate central government (South Korea) and an anti-government group (North Korea). In this sense, the NLL problem should be viewed not in terms of territorial preservation or expansion. Rather, it should be understood as a matter of national identity related to territorial sovereignty and national pride. North Korea's continuous efforts to problematize the NLL may be part of its strategy to nullify the Armistice Treaty. In other words, North Korea tries to take away the basis of the NLL by abrogating the Armistice Treaty and creating a condition in which the United Nations Command can be dissolved. By doing so, North Korea may be able to start the process for the peace treaty with the United States and reestablish a maritime line of its interest. So, North Korea's rationale behind making the NLL a disputed line is to deny the effectiveness of the NLL and ask for the establishment of a new legal boundary. Such an effort should be understood as part of a strategy to make the NLL question a political and military dispute (the similar motivation can be found in Japan's effort to make Dokdo a disputed Island). Therefore, the South Korean government should not accommodate such hidden intentions and strategy of North Korea. The NLL has been the de facto maritime border (that defines our territorial waters) and military demarcation line at sea that we have defended with a lot of sacrifice for the last sixty years. This is the line that our government and the military must defend in the future as we have done so far. Our commitment to the defense of the NLL is not only a matter of national policy protecting territorial sovereignty and jurisdiction; it is also our responsibility for those who were fallen while defending the North-Western Islands and the NLL.

Fermentation of Waste Woody Biomass for the Production of Bioenergy (바이오에너지생산을 위한 목질계 폐바이오매스의 발효)

  • In this study, fermentation characteristics of waste agricultural and forest biomass for production of heat energy were focused to be used in agricultural farm households. The purpose of this study was focused on seeking practical utilization of agricultural and forest biomass wastes in agricultural farm households in the form of thermal energy by means of simple fermentation process. Fermentation process was performed in terms of different raw-materials and their mixture with different ratios. Urea, lime, and bioaids were added as fermenting aids. Moisture contents of fermenting substrates were adjusted to 55~65%. In order to optimize the fermentation process various factors, such as raw-materials, moisture contents, amount of fermenting aids, and practical measurement of hot-water temperature during fermentation were carefully investigated. The optimum condition of fermenting process were obtained from hardwood only and hardwood: softwood (50 : 50) beds. In case of hardwood only the highest temperature was recorded between 60 to $90^{\circ}C$ the lowest temperature was determined to more or less $40^{\circ}C$ and the average temperature was ranged to $50{\sim}60^{\circ}C$ and this temperature ranges were maintained up to 20~30 days. The optimum amount of additives were estimated to ca. 15 kg of urea, 20 kg of bioaids, and 10 kg of lime for 1 ton of substrate. To reach the highest temperature the optimum moisture content of fermenting substrate was proved to 55% among three moisture content treatments of 45%, 55% and 65%. The temperature of hot-water tank installed in fermenting bed of hardwood : grass (50 : 50) showed very different patterns according to measuring positions. In general, temperatures in the mid- and upper-parts of substrate piling were relative higher than lower and surface parts during 45-day fermentation process. The maximum temperature of fermenting stage was determined to $65^{\circ}C$, minimum temperature, more or less $40^{\circ}C$, and average temperature was $60^{\circ}C$. The water temperature of tank exit was ranged to $33{\sim}48^{\circ}C$ during whole measuring periods. It could be concluded that fermentation process of waste agricultural and forest biomass produces a considerable amounts of heat, averaging about $50{\sim}60^{\circ}C$ for maximum 3 months by using the heat exchanger (HX-helical type).

Independent Verification Program for High-Dose-Rate Brachytherapy Treatment Plans (고선량률 근접치료계획의 정도보증 프로그램)

  • Purpose: The Planning of High-Dose-Rate (HDR) brachytherapy treatments are becoming individualized and more dependent on the treatment planning system. Therefore, computer software has been developed to perform independent point dose calculations with the integration of an isodose distribution curve display into the patient anatomy images. Meterials and Methods: As primary input data, the program takes patients'planning data including the source dwell positions, dwell times and the doses at reference points, computed by an HDR treatment planning system (TPS). Dosimetric calculations were peformed in a $10\times12\times10\;Cm^3$ grid space using the Interstitial Collaborative Working Group (ICWG) formalism and an anisotropy table for the HDR Iridium-192 source. The computed doses at the reference points were automatically compared with the relevant results of the TPS. The MR and simulation film images were then imported and the isodose distributions on the axial, sagittal and coronal planes intersecting the point selected by a user were superimposed on the imported images and then displayed. The accuracy of the software was tested in three benchmark plans peformed by Gamma-Med 12i TPS (MDS Nordion, Germany). Nine patients'plans generated by Plato (Nucletron Corporation, The Netherlands) were verified by the developed software. Results: The absolute doses computed by the developed software agreed with the commercial TPS results within an accuracy of $2.8\%$ in the benchmark plans. The isodose distribution plots showed excellent agreements with the exception of the tip legion of the source's longitudinal axis where a slight deviation was observed. In clinical plans, the secondary dose calculations had, on average, about a $3.4\%$ deviation from the TPS plans. Conclusion: The accurate validation of complicate treatment plans is possible with the developed software and the qualify of the HDR treatment plan can be improved with the isodose display integrated into the patient anatomy information.

Dentoalveolar Compensation of Hyperdivergent Skeletal Pattern in Children and Adults (개방교합성 골격 형태를 가진 아동과 성인의 치성보상 양상)

  • Although there is a severe underlying skeletal deformity, the dentition has often maintained some occlusal contact and interdigitation by the teeth compensating in their positions for the skeletal problem, and these dental compensations are manifested in all three planes of space. The purposes of present investigation were 1) to study the pattern of dentoalveolar compensation of hyperdivergent skeletal pattern , and 2) to compare the dentoalveolar compensations of hyperdivergent skeletal pattern in children with adults. The samples selected for this study were consisted of 60 subjects in normal group, 60 subjects in hyperdivergent group. Each was divided into two subgroups by age ; child groups($8\~12yr$ old) and adult groups(17yr old over). The findings of this study were as follows; 1. In child, hyperdivergent subjects had smaller posterior lower facial height(p<0.01) and slightly longer anterior lower facial height than normal ones. In adults, they still expressed smaller posterior lower facial height and much longer anterior lower facial height than normal ones(p<0.01). 2. Hyperdivergent subjects had larger amount of upper and lower incisor relative eruption to their basal bone length than normal ones(p<0.05). In adult, relative eruption of upper incisor was increasing(p<0.05), although relative eruption of lower incisor remained the same as the child. 3. In child, there was no difference between hyperdivergent group and nomal one in the upper and lower molar relative eruption to their basal bone length. In adult, hyperdivergent group had target amount of upper molar relative eruption than normal ones(p<0.01), but relative eruption of lower molar was similar to normal ones. 4. Hyperdivergent group had larger angle between lower occlusal plane and mandibular plane than normal group(p<0.01). Upper occlusal plane of adult groups rotated more antero-superiorly than child groups, and adult hyperdivergent group had sleeper upper occlusal plane than normal group(p<0.05).

A Study on the Postoperative Stability of Hard Tissue in Orthognathic Surgery Patients Depending on the Difference of Occlusal Plane (악교정 수술시 교합평면의 차이에 따른 술후 경조직의 안정성에 관한 연구)

  • In orthognathic surgery to obtain proper functional and esthetic form after skeletal discrepancy treatment, precise diagnosis and treatment plan are essential. Especially in two jaw surgeries that have serious upper and lower jaw problems, maxilla and mandible are arranged in three dimensions. Based on the maxillary rearrangement, mandibular sagittal and transverse positions are determined, and thus new occlusal plane is established. The object of this study is to evaluate the stability of the indiviual ideal occlusal plane based on the architectural and structural craniofacial analysis of Delaires. The subjects of this study were 48 patients who underwent two jaw surgeries, and they were equally divided into two groups, A and B. A group was operated with ideal occlusal plane and B group was not. Two groups were compared at the preoperative, immediate postoperative (average 4.3days), and long-term postoperative (average 1.3years) lateral cephalometric radiographs. The following results were obtained: 1. ANS was lower than that of PNS for both A and B after the surgery. That is, maxilla and mandible are rotated in posterior and superior direction. 2. Significances were found between $T_2$ and $T_3$ for both A and B are HRP-Me at vortical measurements, articular angle(p<0.01), gonial angle(p<0.01), and Mn. plane angle(p<0.05) at angular measurement. Mn. plane angle is increased at HRP-Me is decreased for both A and B. 3. There is no significance in skeletal stability aster the surgery between group A and B. 4. Horizontal movements of B and Pog by surgery have statistically significant inverse correlations with horizontal relapse of B and Pog, and vertical relapse of PNS, as well as Mn. Plane angle, and gonial angle after the surgery.
