  • Objective : The incidence of aneurysmal subarachnoid hemorrhage has been increasing. At the same time, surgical results for elderly patients are unsatisfactory and no guidelines of treatment are available. We carried out a study comparing variable factors and surgical results between young and old age groups to find ways to improve prognosis. Methods : A retrospective study was carried out on 754 patients who were operated on between 1990 and 2004 by the same surgeon in our hospital. The patients were divided into seven groups according to age : 93 patients below 40 years of age [Group I], 419 patients aged $40{\sim}59$ [Group II], 115 patients aged $60{\sim}64$ [Group III], 82 patients aged $65{\sim}69$ [Group IV], 28 patients aged $70{\sim}74$ [Group V], 12 patients aged $75{\sim}79$ [Group VI] and 5 patients over the age of 80 [Group VII]. We then checked their medical history, Fisher's grade, Hunt-Hess grade, postoperative complications, and Glasgow Outcome Scale. Results : Age was not a statistically significant factor among patients below 70 years of age [P $value{\ge}0.05$]. But for those aged 70 and older, the age factor had a statistical value [P $value{\le}0.001$]. In addition, there was a close correlation between Hunt-Hess grade IV and V patients, and those with vasospasm, and hydrocephalus after surgery, with poor prognosis in elderly patients as well as young patients [P $value{\le}0.001$]. Conclusion : Advanced age [under the age of 70] dose not precluded adequate surgical treatment in patient with AN SAH, and we should be also alert to preventable causes of delayed neurological deterioration for improving the outcome in all elderly groups.

  • Background: Cognitive impairment has been reported to be closely associated with poor oral health, and the relationship is bidirectional, as older adults with poor oral health and chewing function are at a higher risk of cognitive decline (CD). This cross-sectional study aimed to determine whether masticatory discomfort in Korean elderly increases the risk of daily living difficulty (DLD) related to CD and whether there is a difference in risk according to gender and age. Methods: The data used were obtained from the Korean Community Health Survey (2019). The final analysis included 22,154 people aged 65 years and older who completed the survey and responded to all items on the variables used in the study. Chi-square test, trend test, and complex sample logistic regression were performed for statistical analysis, to clarify the purpose of this study. Results: As masticatory discomfort increased, the elderly's DLD tended to increase (p-for trend, p<0.001). For logistic regression, adjusting for all covariates, participants who reported masticatory discomfort as "discomfort" (adjusted odds ratio [AOR]=2.45, 95% confidence interval [CI]=2.11~3.50) and "severe discomfort" (AOR=2.95, 95% CI=2.49~3.50) had a more than a two-fold increased risk of CD-related DLD compared to participants who reported "no discomfort at all." In age-stratified analyses, elderly men aged 75~84 years and elderly women aged 65~74 years had the highest risk of developing CD-related DLD. Conclusion: Oral care interventions to improve masticatory function in older adults may slow CD and improve CD-related DLD. We hope that this study will raise awareness among caregivers and clinical professionals regarding the importance of oral care for older adults with CD.

By the way, social welfare services is less available and accessible to the rural elderly. And we have very limited information about community -oriented welfare services for the elderly. The lack of social welfare services in rural area resulted from mainly geographic isolation and economic deprivation. So, the present research aimed at; 1) to explore what the social welfare service is benefited from governmental or local assembly. 2) to explore what kind of community services is provided for the rural village and elderly. 3) to appear the political propose for the rural elderly. In South Korea, The Rural Development Administration currently operate 'villages which is supported special programs for the improvement of QOL of rural elderly' in 110 villages. It is a model governmental welfare service for rural area. For the purpose, the survey data is gathered from community level data per village (107 villages), individual survey data(881people) who live in the village and qualitative data. Two kinds of quantitative data is combined to form a data. The statistical methods used for data analysis are descriptive statistics, t-test and ANOVA. The major findings of this study were as follows : It was founded that the majority family type of the rural elderly is elderly-only households(75%). In case of poor elderly, they have very limited social insurance benefit and they can not get medical services with satisfaction. The result show that the welfare facilities per village and welfare service for the elderly is extremely low while the needs of welfare service is greate high. A distinctive characteristics in rural villages is that they receive a lot of services from private sectors, like as Women Farmers Union, Adult Union. • The purpose of this study was to evaluate and to analyze the spatial composition by block plan of the free elderly facility and to provide basic information for building up the detail architectural guidelines. The results of the study were as follows: Elderly welfare law needed to control the poor physical environments of the free elderly facility such as the residential space for more than 4 individuals. The facility of single building type should strengthen the informal support networks through the dispersion of public spaces or should encourage moving of the residents through the outdoor space, especially in rural area. On the other hand, the linking type needed to focus on the administrative spaces on the first floor for the managerial convenience, and to group and disperse the public/recreational spaces to decrease the unnecessary moving of the residents and staffs. The passageway linking building to building should directly connect each administrative space or public space so that elderly used them without any inconvenience. In the dispersion type, which is suited to rural area, the lounge, dining hall, and the place for the staff on duty should be located in the same as the residence building for the convenience and the safety of the elderly residents.

  • It was identified that how many homebound bedridden elderlies and their primary caregivers were depressed, and which factors affected the bedridden elderly's depression. Method: The subjects were 191 homebound bedridden elderlies and their primary caregivers. The affecting factors were classified into two categories : bedridden elderly and their primary caregiver related factors. Then bedridden elderly's factors were classified demographic and disease-related factors again. The stepwise regression was used to identify significant factors. Result: The prevalence of bedridden elderly's and caregiver's depression was 77.8% and 67.0%, respectively. And the model explained 33.3% of variance of bedridden elderly's depression. Cognitively-impaired female elderlies who had depressed caregivers were found to be more depressed. And caregivers who perceived burden were identified to be more depressed. Conclusion: It is recommended that the health professionals need to identify bedridden elderlies and caregivers at risk of depression. Especially elderlies who is in poor cognition, those who are female, and those whose caregivers were depressed might be considered carefully in all counseling or follow-up. Also the primary caregivers must be helped to access already available formal and informal support.

  • Objectives: The purpose of the study was to investigate the relationship between frailty and oral health among some elderly community residents. Methods: A self-reported questionnaire was completed by 240 elderly in the Gwangju-Jeonnam area from October 1, 2019, to November 30, 2019, based on convenience sampling. The questionnaire consisted of general characteristics of the subjects, frailty level (Kihon checklist), and Geriatric Oral Health Assessment Index. Results: There were 66 (27.5%) frail elderly, and the GOHAI group with a score less than 45 constituted 187 (77.9%), which was higher than the group with a score of 45 or higher. The distribution of the frail elderly was indicated to be higher in the people with older age (p<0.05), lower educational level (p<0.01), current economic inactivity (p<0.05), living alone (p<0.01), more chronic diseases (p<0.01), and GOHAI score below 45 (p<0.01). Compared to those with a good oral health-related quality of life, those with a poor quality of life showed a 3.03 times higher risk of frailty (95% CI=1.291-7.107)(p<0.05). Conclusions: By recognizing the need for oral health care of the elderly through these results and by identifying the relationship between frailty and oral health, it is possible to consider oral health as a predictor of frailty.

  • Objectives: Sarcopenia is a common disease in the elderly population that causes disability, poor quality of life, and a high risk of death. In the current study, we conducted a meta-analysis to report basic knowledge about the prevalence of sarcopenia in the elderly in Korea. Methods: We searched for articles in the MEDLINE, Cochrane Library, Embase, and Scopus databases published until December 28, 2020. Studies investigating the prevalence of sarcopenia in elderly Koreans aged ≥65 years were included. The methodological quality of the studies was evaluated using the Newcastle-Ottawa scale. Publication bias was evaluated using the Egger test and funnel plots. Results: In total, 3 studies and 2922 patients were included in the meta-analysis. All 3 studies used the European Working Group on Sarcopenia in Older People criteria for the diagnosis of sarcopenia. The total prevalence of sarcopenia was 13.1-14.9% in elderly men and 11.4% in elderly women. Conclusions: This meta-analysis is the first to estimate the pooled prevalence of sarcopenia in elderly Koreans, and its findings suggest that sarcopenia is common in this population. Therefore, attention should be paid to the prevention and control of sarcopenia.
